Findings: This cohort study including 63 children and 118 adults found no significant difference between the number of children and the number of adults testing positive for SARS-CoV-2 infection during the study period; children were asymptomatic significantly more often compared with adults (46% vs 13%). In addition, a reconstruction of the outbreak showed that most transmission events originated from within the school.
Meaning: These results suggest that children may play a larger role in the transmission of SARS-CoV-2 than previously assumed.

Results:
Political and policy actors at international, national, and regional level aligned — predominantly though not invariably — with medical scientific orthodoxy which promoted the droplet theory of transmission and considered aerosol transmission unproven or of doubtful relevance. This dominant scientific sub-field centred around the clinical discipline of infectious disease control, in which leading actors were hospital clinicians aligned with the evidence-based medicine movement. Aerosol scientists — typically, chemists, and engineers — representing the heterodoxy were systematically excluded from key decision-making networks and committees. Dominant discourses defined these scientists’ ideas and methodologies as weak, their empirical findings as untrustworthy or insignificant, and their contributions to debate as unhelpful.

jmCases of SARS-CoV-2 infection in Manaus, Brazil, resurged in late 2020, despite previously high levels of infection. Genome sequencing of viruses sampled in Manaus between November 2020 and January 2021 revealed the emergence and circulation of a novel SARS-CoV-2 variant of concern. Lineage P.1, acquired 17 mutations, including a trio in the spike protein (K417T, E484K and N501Y) associated with increased binding to the human ACE2 receptor. Molecular clock analysis shows that P.1 emergence occurred around mid-November 2020 and was preceded by a period of faster molecular evolution. Using a two-category dynamical model that integrates genomic and mortality data, we estimate that P.1 may be 1.7–2.4-fold more transmissible, and that previous (non-P.1) infection provides 54–79% of the protection against infection with P.1 that it provides against non-P.1 lineages. Enhanced global genomic surveillance of variants of concern, which may exhibit increased transmissibility and/or immune evasion, is critical to accelerate pandemic responsiveness.

Not only do we get a clear and consistent picture of many things that have happened since, we also get the mitigation strategy. Further, all three dimensions support each other: transmission from people not (yet) coughing or sneezing very much argues in favor of aerosol transmission, which explains how large clusters can be driving the epidemic and how transmission in a situation like that ship can occur. And the mitigation and other strategies become clear: pay attention to clusters and ventilation, universal masks, different policies for indoors and outdoors, etc.

jmOn 28 March 2020, two months after the WHO had declared COVID-19 a global health emergency, the agency broadcast a public-health message on Twitter and Facebook. “FACT: #COVID19 is NOT airborne,” it said, labelling claims to the contrary as misinformation. But evidence quickly established that the virus is transmitted by air, and researchers roundly criticized the agency.
The WHO updated its advice on SARS-CoV-2 transmission three months later, acknowledging the possibility that airborne transmission might occur in some community settings. Airborne transmission in “crowded and inadequately ventilated spaces over a prolonged period of time with infected persons cannot be ruled out”, the updated advice says.
Yuguo Li, a building environment engineer at the University of Hong Kong, says that he is disappointed it took the WHO and other health authorities so long. “We would have saved a lot of people” if airborne transmission was recognized earlier, he says.

jmA team of public health specialists in the midlands traced how a case of household transmission led to 26 cases of Covid-19 in a manufacturing plant and a further 20 cases in other households, a nursing home and a school.
The first case, or “index case” they became aware of was a woman who worked in a manufacturing plant. Household transmission of the virus occurred when a person she was living with, who had acquired Covid in a pub, passed the virus to her.
So in other words -- the true index case was the person in the pub, or at the very least, her housemate who picked up COVID-19 in the pub, and this was a case where the pub was the initial cluster location, leading to 47 further cases. But for some reason, the article chooses "household transmission" as the headline...

jmWe examined the distribution of transmission events with respect to exposure and onset of symptoms. We show that for symptomatic individuals, the timing of transmission of SARS-CoV-2 is more strongly linked to the onset of clinical symptoms of COVID-19 than to the time since infection. We found that it was approximately centered and symmetric around the onset of symptoms, with three quarters of events occurring in the window from 2-3 days before to 2-3 days after. However, we caution against overinterpretation of the right tail of the distribution, due to its dependence on behavioural factors and interventions. We also found that the pre-symptomatic infectious period extended further back in time for individuals with longer incubation periods. This strongly suggests that information about when a case was infected should be collected where possible, in order to assess how far into the past their contacts should be traced. Overall, the fraction of transmission from strictly pre-symptomatic infections was high (41%; 95%CI 31-50%), which limits the efficacy of symptom-based interventions, and the large fraction of transmissions (35%; 95%CI 26-45%) that occur on the same day or the day after onset of symptoms underlines the critical importance of individuals distancing themselves from others as soon as they notice any symptoms, even if they are mild. Rapid or at-home testing and contextual risk information would greatly facilitate efficient early isolation.

jmLarge-scale school closures have been implemented worldwide to curb the spread of COVID-19. However, the impact of school closures and re-opening on epidemic dynamics remains unclear.
Methods: We simulated COVID-19 transmission dynamics using an individual-based stochastic model, incorporating social-contact data of school-aged children during shelter-in-place orders derived from Bay Area (California) household surveys. We simulated transmission under observed conditions and counterfactual intervention scenarios between March 17-June 1, and evaluated various fall 2020 K-12 reopening strategies.
Findings: Between March 17-June 1, assuming children <10 were half as susceptible to infection as older children and adults, we estimated school closures averted a similar number of infections (13,842 cases; 95% CI: 6,290, 23,040) as workplace closures (15,813; 95% CI: 9,963, 22,617) and social distancing measures (7,030; 95% CI: 3,118, 11,676). School closure effects were driven by high school and middle school closures. Under assumptions of moderate community transmission, we estimate that fall 2020 school reopenings will increase symptomatic illness among high school teachers (an additional 40.7% expected to experience symptomatic infection, 95% CI: 1.9, 61.1), middle school teachers (37.2%, 95% CI: 4.6, 58.1), and elementary school teachers (4.1%, 95% CI: -1.7, 12.0). Results are highly dependent on uncertain parameters, notably the relative susceptibility and infectiousness of children, and extent of community transmission amid re-opening. The school-based interventions needed to reduce the risk to fewer than an additional 1% of teachers infected varies by grade level. A hybrid-learning approach with halved class sizes of 10 students may be needed in high schools, while maintaining small cohorts of 20 students may be needed for elementary schools.
Interpretation: Multiple in-school intervention strategies and community transmission reductions, beyond the extent achieved to date, will be necessary to avoid undue excess risk associated with school reopening. Policymakers must urgently enact policies that curb community transmission and implement within-school control measures to simultaneously address the tandem health crises posed by COVID-19 and adverse child health and development consequences of long-term school closures.

But what if this is all just a huge waste of time?
In May, the Centers for Disease Control and Prevention updated its guidelines to clarify that while COVID-19 spreads easily among speakers and sneezers in close encounters, touching a surface “isn’t thought to be the main way the virus spreads.” Other scientists have reached a more forceful conclusion. “Surface transmission of COVID-19 is not justified at all by the science,” Emanuel Goldman, a microbiology professor at Rutgers New Jersey Medical School, told me. He also emphasized the primacy of airborne person-to-person transmission.

jmAccording to research on the SARS epidemic, face masks were the most consistently effective intervention for reducing the contraction and spread of SARS. In a Cochrane Review on the subject, 6 out of 7 studies showed that face masks (surgical and N95) offered significant protection against SARS. Hand washing was also very effective, supported by 4 out of 7 studies in a multivariate analysis.
Although most of the studies in the Cochrane Review were on medical workers in a hospital setting, one study followed community transmission of SARS in Beijing. It found that consistently wearing a mask in public was associated with a 70% reduction in risk of catching SARS. Additionally, the authors of the paper noted that most people in the community wore simple surgical masks, not N95 respirators.
