Sulphur, LA (KPLC) - State police are investigating after a man died in a hazardous chemical accident at Holeshot Tank Wash.

State police say Alvin Lively, 48, of Lake Charles, was inside a tank while working at Holeshot Tank Wash off Hwy 108 in Carlyss.

Police say Lively was washing the tank when he was found unresponsive Tuesday afternoon. Police say troopers received a call to investigate the incident around 2 p.m.

Trooper First Class Derek Senegal could not confirm whether Lively was wearing protective equipment.

“He was found inside of the tank unresponsive. Have to make sure that whatever type of, if he had some type of whether he had some type of PPE, personal protective equipment on, that he had the right stuff that’s on. I can’t say that he did, but that’s all part of the investigation,” said Senegal.

Senegal says the truck, owned by Republic Services, was dropped off for cleaning on Nov. 18. It contained pyrolysis gas, or “pygas,” which displaces oxygen.

Senegal says Lively was dead at the scene after apparently becoming overcome by fumes.

TAMPA, Fla. — Crews on a ship were evacuated after a fire erupted within the vessel early Friday morning, Tampa Fire Rescue said.
At around 5:30 a.m., firefighters responded to a reported ship fire located at 1616 Penny St., which is the address for International Ship Repair & Marine Services Inc., according to a news release.
Engine 4 was first on the scene of a vessel called "The Kelly Alafia," which is overseen by Savage Marine Services. The ship was at berth and undergoing repairs.
Fire rescue says crews were cleaning an area inside the cargo hold when oncoming crew smelled smoke during a shift change. After smelling the smoke within the hull, they called 911.
Ship staff and the marine chemist on scene were able to identify a byproduct of residual sulfur the barge transports caught on fire, according to fire rescue. The fire then became a hazardous materials operation because of a chemical product being burnt.
Investigators reportedly determined the cause of the fire came from welding.
All crew members were able to evacuate the ship, fire leaders report.

SUNNYSIDE — Although a chemical fire in Sunnyside quickly consumed an industrial building and emitted sulfur-laden smoke throughout the afternoon and evening, Monday’s blaze was caught early enough to avoid injuries and could have been much worse, officials said.

A smoldering pile of sulfur quickly became a raging chemical fire just after 1 p.m. Monday at Nutrien Ag Solutions, 1101 Midvale Road, Sunnyside Fire Chief Ken Anderson said.

The fire destroyed one storage building on the southeast corner of the fertilizer storage facility in the Port of Sunnyside and damaged others, but adjacent Nutrien buildings and storage tanks containing hazardous chemicals were spared, Anderson said Monday evening.

“It was caught early on — it could have been much worse,” he added.

Nearby industrial facilities in the Port of Sunnyside, including Ostrom Mushroom Farms located immediately south on Midvale, were evacuated, and 18 families living in homes near Interstate 82 also were evacuated as a precaution, Sunnyside Police Chief Al Escalera said.

"There were some chemicals (in a concrete bin) that caught fire, but it was basically contained to the pile," said Dennie Koberczky, assistant fire chief for Pocahontas-Old Ripley Fire Protection District. "It didn't burn any of the buildings."

Fire departments from throughout the region showed up to help, as well as hazmat (hazardous materials) units from St. Clair and Madison county emergency management agencies. Crews worked from about 1 to 5:30 p.m. Saturday. No injuries were reported.

"We did have a major road shut down because smoke from (sulfur-containing material) was going right across the road, and it could cause breathing problems if people inhaled it," Koberczky said.

Widespread flooding and power outages have affected over 1 million residents in Louisiana alone after the Category 4 hurricane passed through the state.

In Norco, Shell Oil’s refinery has spewed black smoke and sulfur – a sight common to some facilities where companies hope to burn off toxic chemicals rather than risk continued spills into the surrounding area.

CLEANUP BOATS ON SCENE OF LARGE GULF OIL SPILL FOLLOWING IDA

The extent of pollution in the area remains unknown partially due to the lack of communications, preventing state police from deploying hazmat units to handle toxic emissions. So far, the U.S. Coast Guard has received 17 calls about air releases, including multiple reports of ammonia released into the air when flares were blown out by the storm, allowing the chemicals to release, The Guardian reported.

Some companies took precautions ahead of the storm by shutting production down rather than risk damage to facilities that could result in significant spillage and pollution to the surrounding area.

The fire was the result of a malfunction of the power unit on a sulphur scrubber, a large piece of machinery that processes sulphur for industrial uses, Trooper Janssen Herb said.

The fire itself was relatively small and physical damage was minimal compared to the economic impact on the plant at 1227 Valley Road, which shut down for most of the afternoon, Herb said. He couldn’t provide a damage estimate.

Plant officials couldn’t be reached for comment, and it was unclear if the plant resumed full operations.

MINSK, Belarus (AP) — About 4,000 children have been evacuated from a town in Russia-annexed Crimea where a noxious chemical leak has been blamed on a local titanium factory.
Residents of Armyansk complained in late August that a sour smell was pervading the community and a substance resembling rust had fallen on roofs, trees and vehicles.
Authorities this week decided to take children away from the town of 20,000 people for two weeks. They say the children are being housed at health resorts elsewhere in Crimea.
Russian media have not reported widespread illness or symptoms in Armyansk. The head of Ukraine's adjacent Kherson region said Friday that 37 border guards have been hospitalized due to negative effects from the leak.
Crimea's leader, Sergei Aksenov, identified the chemical as sulfur trioxide from the titanium plant.

Two officers were injured after a truck exploded in Batesville, Mississippi.

According to Sheriff Dennis Darby, a truck carrying sulfur crashed into a car on Interstate 55 near Pine Lodge Road. As officers attempted to pull the driver out of the truck, an explosion occurred.

Darby said the truck was carrying about 25,000 pounds of sulfur, which had officers waiting for hazmat officials to assist, as sulfur is hazardous if inhaled.

Darby said the deputies were burned on their arms while pulling the driver out of the truck. Both were taken to the ICU at Baptist Memorial Hospital in Oxford, and both are expected to be OK.

The driver, however, was severely burned. He was flown to the hospital in Memphis in critical condition.

Northbound traffic remains shut down in the area, and hazmat crews are still cleaning hours later. Southbound lanes are now open.

The employee was taking a pipe apart when a “small amount” of sulfur dichloride spilled out of the pipe, said Greg Coffey, a spokesman for Bayer. The employee went into a safety shower and rinsed the chemical off, Coffey said.

The employee was taken to Thomas Memorial Hospital, in South Charleston, and released Thursday afternoon. He wasn’t sure exactly how much of the liquid spilled, but a spill report from the Department of Environmental Protection stated the liquid weighed less than one pound.

The leak happened about 1:10 p.m., said C.W. Sigman, director of emergency management for Kanawha County.

Bayer CropScience is a tenant on the Union Carbide Corporation site, Coffey said. Union Carbide Corporation is a subsidiary of the Dow Chemical Company.

UPDATE: Twenty-four students and a teacher were treated by paramedics after a chemical incident at a Darling Downs school.

Emergency services were called to Millmerran P -10 State School just after 10am to reports of the chemical incident as a result of a science experiment involving the use of sulphur gas.

Queensland Fire and Emergency Services entered the school in breathing apparatus and conducted atmospheric testing before ventilating the area.

A Department of Education and Training spokesperson said the incident took place in a science laboratory and Queensland Fire and Emergency Services was called as a precaution.

"Firefighters conducted atmospheric testing and did not detect any dangerous chemicals in the classroom," the spokesperson said.

They said the parents of all students were contacted and the affected area was ventilated with large fans to avoid further safety concerns or disruptions.

Queensland Ambulance Service senior operations supervisor Glen Barron said the teenagers were all taken to Millmerran Health Service in a stable condition, with many returning to school the same day.

"A couple of the children had been affected by some fumes, so off-gasses from one of their science experiments," he said.

"So we attended to that and as it progressed we gradually had more students who presented with some minor symptoms."

Mr Barron said symptoms included nausea and light headedness and one girl was treated for pre-existing asthma.

HOUSTON - The strange odor that swept through the heart of Houston on Monday morning is no longer believed to be related to an incident at a facility in Channelview, according to officials.

The sulfur-like smell was first reported about 10 a.m. in the downtown area of Houston and spread west along the Interstate 10 corridor.

"We never really had a concern because our air quality monitoring never showed it was a public health issue, but we are still monitoring cause it was definitely a cause for concern, and we're still trying to identify the source," said Houston Fire Department Captain Ruy Lozano.

Officials at the Houston Fire Department originally reported that the odor was related to a chemical release the LyondellBasell facility, 15 miles east of the city.

However, a statement from LyondellBasell reported there was a release of oil from a piece of equipment used to control temperature. That oil was contained and never traveled offsite, according to the statement.

“We understand there are reports of an odor around downtown Houston,” the company said in a statement. “At this time, it does not appear that the events are connected.”

The smell, most pungent on the south side of the Torrance Refining Co. complex along Del Amo Boulevard and Prairie Avenue, was reported on the city and Fire Department’s Facebook pages at about 9:30 a.m.

Jennifer Chiang of Torrance said she noticed the smell just before 9 a.m. as she was returning home from dropping her two children off at Victory Elementary School.

The odor, she said, was “like rotten eggs, like sulfur.”

When she arrived at her condominium near Anza Avenue and Spencer Street, the smell was especially strong in her garage.

“I called Torrance refinery at around 9 and they said there was no unusual activity going on,” she said. “But then someone called me back to say they were looking into it.”

Refinery officials later pinned the odor on “tank operations” but did not elaborate other than to say “personnel took immediate actions to mitigate the odor.”

Newcastle Fire Communications shift supervisor Paul Randall said NSW Fire and Rescue Casino brigade was called to Sextonville Rd at 12.30pm today after reports a truck had caught fire.

When fire fighters arrived at the scene they found it was no ordinary truck fire, as the burning eight-tonne rig was carrying three-and-a-half tonnes of sulphur, Mr Randall said.

Reinforcements were called from across the Northern Rivers, with eight tankers called to the scene from Lismore, Goonellabah, Alstonville and Kyogle, and Hazmat units from Goonellabah and Tweed Heads.

“It was very fortunate that when fire fighters arrived the truck driver had managed to get out of the vehicle and to safety,” he said.

The Gavin Creighton’s Fertiliser Spreading truck spilled part of the sulphur, Mr Randall said, and burned for more than three hours as fire fighters battled the blaze and shifting wind conditions.

THE HAGUE, Netherlands (AP) — The U.S. envoy to the international chemical weapons watchdog warned Monday that the use of such toxic arms is "becoming routine in the Syrian civil war."

Rafael Foley was speaking at a closed meeting of the Organization for the Prohibition of Chemical Weapons' Executive Council, which was called to discuss recent reports by a fact-finding mission, including that a "non-state actor" likely used the chemical agent sulfur mustard in August during fighting in the Syrian town of Marea, killing a baby.

Foley said Syrian opposition forces were fighting the Islamic State group in the town close to the Turkish border. The text of his speech was posted on The Hague-based watchdog's website.

The fact-finding mission, which took and tested samples and interviewed witnesses, also said that chlorine likely was used as a weapon between March and May in Idlib, leaving six people dead.

After the meeting, attended by representatives of 38 member states, the executive council issued a statement "expressing grave concern" about the fact-finding mission's conclusions that "chemical weapons have once again been used in" Syria and saying that those responsible should be held accountable.

A special investigation team has been set up by the United Nations and OPCW to identify who is responsible for chemical attacks in Syria.

Now progress is finally being made rounding up and destroying all these deadly weapons that are still maiming and even killing Chinese people today.
More than 50,000 projectiles, mortars, aerial bombs, liquid-filled drums and gas-filled pots containing chemical agents such as sulfur mustards, lewisite and phosgene have been found scattered over 90 sites in China so far. Many of the discoveries were made during construction work. Chemical weapons have been found dumped in densely populated areas, in nature reserves and in rivers and lakes, as well as off shore. Due to their age, these munitions are rusty, damaged and often leaking. Some also contain explosives. City officials were hopeful the odor issue, which began at the Tyson Foods plant, would dissipate in the near future.

The problem started Friday at the Tyson Foods feed mill in Aurora. A holding tank for the liquid feed product Alimet, added to help chickens absorb protein more effectively, developed a leak. Tyson personnel sealed the leak, mopped up the spill and decided to ship the spilled liquid to Monett for disposal at the wastewater pre-treatment plant.

In the first miscommunication, the feed mill failed to alert the Tyson pre-treatment plant of the shipment. Then, according to sources with the city, the truck driver delivering the Alimet misrepresented what he was hauling, telling the the pre-treatment plant operator that he had animal fat, a typical material for the plant to process. After about 20 minutes of unloading, the operator halted the process, realizing the material was not animal fat. He called Becky Thomas, Tyson's pretreatment operator, who ordered the rest of the load to go into storage for handling on Monday. By then, an unknown quantity of Alimet had entered the system.

The tank is owned by Gulf Sulphur. Now, firefighters and the fire marshal are closely watching as company workers offload the sulfur so the tank can be thoroughly inspected. The process is expected to take a minimum of three days. Once empty, the tank must pass a complete inspection and must submit a certificate to the fire marshal's office. Last week, the fire at the tank prompted the department to issues warnings about potentially harmful fumes.

Residents in south Tampa and Harbour Island were even told to stay indoors if possible because the fumes could cause eye, skin and throat irritations. Tonight, only one business south of the fire was evacuated. However, a precaution was issued to people in south Tampa.

Officials said there was no imminent threat as the wind is heading in a southeast direction, so the fumes are moving away from highly populated areas. Ladder trucks sprayed water into the tank. The sulfur was molten as the the sulfur is kept warm by a system of heated coils that are installed around the tank. As of 7 p.m., the cause of the fire had not been determined. Tampa Fire Rescue did contain the fire in less than an hour.

PROVO -- A chemical reaction went wrong at a Provo business Wednesday afternoon, sending smoke through the roof and hazmat teams inside.

Battalion Chief Tom Augustus with the Provo Fire Department said that employees at Wiltec at 488 S. 500 West, a company that tests chemical reactions to determine how much heat is given off, were testing sulfur trioxide when a tube being used failed. That caused an unexpected chemical reaction that involved a significant amount of smoke. As the smoke cleared the building through the roof vents, neighbors saw it and called in a structure fire.

Firefighters arrived shortly before 4 p.m. and found it wasn't a fire but still cause for concern; the building was evacuated and hazmat crews waited for the chemicals to finish reacting. The smoke was quickly dispersed by the wind, and no one was in danger, Augustus said. After about three hours the materials were dissolved, the hazmat team checked the building out and the employees and owner could go back in.

The Oppau explosion occurred on September 21, 1921 when a tower silo storing 4,500 tonnes of a mixture of ammonium sulfate and ammonium nitrate fertilizer exploded at a BASF plant in Oppau, now part of Ludwigshafen, Germany, killing 500–600 people and injuring about 2,000 more. The plant began producing ammonium sulfate in 1911, but during World War I when Germany was unable to obtain the necessary sulfur, it began to produce ammonium nitrate as well...

Northern suburbs of the city on both sides of the River Derwent have been told to close windows and doors and ensure air-conditioners are switched to re-circulate, The Mercury reports.

The Tasmania Fire Service issued the warning following a "hazardous materials incident" - believed to be a spill of sulphur trioxode at the Nystar zinc smelter at Lutana - saying exposure to the material may be dangerous.

Sulphur trioxode, the primary agent in acid rain, is highly corrosive and can cause serious burns if it is inhaled or ingested.

A collision Wednesday night between a semi truck hauling molten sulphur and a pickup truck resulted in a hazmat situation and the closure of Safford/Bryce Road for nearly 13 hours.

According to a press release from the Graham County Sheriff's Office, the County Dispatch received a call regarding the collision at about 6:21 p.m. The incident occurred as both vehicles were westbound on Safford/Bryce Road just west of Branding Iron Road.

According to Sheriff P.J. Allred, the driver of the semi truck, Dwayne Elders of Thatcher, was attempting to pass the pickup truck when the collision occurred. Apparently, the driver of the pickup truck, Dana Carrasco of Safford, attempted to make a left turn into a private driveway when she was sideswiped by the semi. The semi rolled onto its side and stretched across both lanes of the road. Allred told the Courier that Elders said the pickup truck didn't have its turn signal on, but Carrasco insisted that she did.

Elders was transported to the Mt. Graham Regional Medical Center by Southwest Ambulance where he was treated and released. He suffered a head laceration and fractured collar and rib bones, according to Allred.

Carrasco had her 4-year-old daughter with her in a child safety car seat, and neither was injured.

Molten sulphur began leaking out of the top cap of the transport tanker but was quickly contained and handled by the Safford Fire Department. Hazmat teams from the Department of Public Safety traveled from Globe and Tucson and assisted with the leak as well. According to the United States government Material Safety Data Sheet, molten sulphur gives off an odor of rotten eggs and is flammable.

It can cause eye and skin irritation and should only be used with adequate ventilation.

Some villagers were sickened Wednesday evening by gas leak after a chemical plant fire in north China's Hebei Province, but exact number of casualties remains unknown, local officials said Thursday.

The accident occurred Wednesday evening when a benzenesulfonic hydroxide workshop of Hebei Jianxin Chemical Co., Ltd. caught fire at Lingang Economic and Technological Development Zone in Cangzhou City of the province, according to the management committee of the zone.

No deaths or injuries was reported at the site. But some residents at a downstream village named Xinzhuangzi developed such symptoms as chest tension and nauseating after they inhaled acid smog from the plant. Some villagers have been hospitalized for treatment and observation, but exact number of them is yet to be known.

Experts said that the fire, which was put out at around 9:00 p.m. Wednesday, was ignited by nitrobenzene created after sulfur trioxide leaked from the chemical workshop met humid air.

A Coffs Harbour fire crew and hazardous materials unit were called to the Country Energy Field Service Station on Thompsons Road yesterday to clean up a sulphur spill.

They responded at 1pm after two 24 kilogram bags of common sulphur were inadvertently punctured by a forklift.

Coffs Harbour Fire Station senior officer Mark Rose said no one was evacuated because the spill occurred in an isolated area, with no one but the forklift operator around.

“The forklift worker was lightly contaminated but we decontaminated him and cordoned the area off and staff were instructed to stay clear until it was cleaned up,” Mr Rose said.

At about 12:38 a.m. Wednesday, the Kern County Fire Department received a call for a possible explosion at the Hondo Chemical facility in the 20800 block of Stockdale Highway.

The approximately 25 firefighters who responded to the incident were faced with burning sulfur in a cement pit surrounded by a 2-foot wall with exposures to nearby products. Crews used a water tank along with assistance from the facility's water tender to extinguish the fire.

Decontamination was provided for the crews involved with extinguishing the fire because of the presence of potentially hazardous chemicals.

No hazard was present to nearby residents or businesses, no loss was noted to the area and no injuries have been reported.

The fire was caused by the friction from mixing during an operation earlier in the day.

A chemical fire closed a city block and sent a police officer and a firefighter to the hospital Wednesday in Durango.

According to the Durango Herald, the fire was reported just after 1 p.m. in a garage adjacent to an office building on Main Avenue.

Nearby offices were evacuated as light smoke, white in color, could be seen pouring from the garage.

A firefighter and a police officer were sent to Mercy Regional Medical Center with respiratory irritation. The two were evaluated and expected to be alright.

Durango fire officials later announced the fire was a hazardous chemical accident involving bags of sulfur that were stored in the garage. The chemical ignited forming sulfur dioxide, a gas known to produce respiratory difficulties.
