SPANISH FORK, Utah, Feb. 9, 2023 (Gephardt Daily) — A fabrication plant was evacuated and a large hazmat response generated when a resin cloud formed inside the factory late Wednesday night.

All 22 workers evacuated as emergency crews were dispatched at 10:37 p.m. to the Klune Industries plant at 1863 N 300 W, Spanish Fork Fire Chief Eddie Hales said. The Utah Valley Special Response Team was summoned, sending some 15 hazardous materials-trained firefighters to the scene to assist his crew of 16, he said.

A resin production unit had exceeded the 150-degree temperature maximum for the process, causing a visible and potentially toxic cloud to form inside the facility, Hales said.

Units of the Utah Valley SRT sending two responders each included fire departments in Saratoga Springs, American Fork, Pleasant Grove, Lehi, and seven from Provo. But, Hales said, most weren’t needed, and were canceled as soon as they arrived. “It wasn’t as hazardous as we thought,” he said.

Hales’ crews were able to measure the air inside the plant, aka PCC Aerostructures, with gas monitors showing the air was not toxic after the area was well ventilated. The 22 employees returned to finish their shift, Hales said. “There were no injuries.”

A chemical firm has been fined £800,000 after an explosion left a worker fighting for his life.
The 49-year-old man was scarred, partially blinded and also suffered hearing damage in the blast at Gateshead's International Paint plant.
He had been making paint in a large mixing vessel, which involved the use of flammable liquids.
The firm admitted breaching health and safety laws and was fined at Newcastle Magistrates' Court.
The explosion happened as the employee was emptying resin pellets from a large bulk bag into the vessel which generated an electrostatic spark and ignited flammable vapour.
The man, from South Shields, spent eight days on life support.
An investigation by the Health and Safety Executive (HSE) found the company failed to put sufficient measures in place to control the risk, including a correctly working extraction system.

The deputy was transported to the hospital after the crash. The driver of the truck hauling the chemicals was not injured. The crash caused a chemical spill which lead to the road closure during the cleanup.

According to the South Trail Fire Protection & Rescue District, the trailer of the utility vehicle flipped on its side. The front of the LCSO cruiser hit the side of the truck.

The crash flipped the Koala Insulation trailer on its side and toppled barrels of chemicals onto the road. The trailer spilled isocyanide and resin, which is used to create SprayFoam insulation on the roads. Hazmat is cleaning up the chemicals.

There’s a foul smell in Covington. Latonia homeowners say it’s their next-door neighbor — not another homeowner, but a nearby chemical plant.

Interplastic Corporation uses styrene and other chemicals to make polyester resins, which are then used to make plastic parts for boats, trucks, pools, spas and bridges. It’s a process that can emit an odor.

In January, an explosion at the facility occurred when 31,000 pounds of liquid resin began to harden inside a storage tank, according to fire officials. Since then, Covington leaders said they’ve received documents from Fort Wright highlighting nine homeowner complaints.

Homeowners in the Latonia neighborhood of Covington have also filed complaints. David Davidson, Covington’s city solicitor, said during a May 10 commission meeting a consent decree was entered in 2004.

“That requires Interplastic to make certain reports. Requires it to make available the facility for the fire departments of City of Fort Wright, and the City of Covington,” Davidson said. “That was entered in 2004. As far as I know, not much has been done with that.”

An explosion at a chemical factory in Slovenia killed five and left six injured, officials said on Thursday.

A cistern exploded early Thursday, sparking a fire at the factory of resin supplier Melamin, according to fire brigade chief Leon Behin.

The factory is located in Kocevje municipality, some 60 kilometres south of Ljubljana.

"Unfortunately our fears have been confirmed," Melamin head Srecko Stefanic told reporters. The strength of the explosion "did not leave them any chance of survival", he said, in visible shock.

Officials earlier said four people were missing.

The victims were a worker and "four outside workers" at the company.

In addition, six workers were injured, including two who were hospitalised with serious burns, according to the hospital in Ljubljana treating them.

One of them is in a critical situation, Stefanic said.

The tragedy was "caused by a human error", he said, declining to give more information until the investigation has been completed.

THE US Chemical Safety Board (CSB) has released a new safety video about the 2020 fatal incident at the Evergreen Packaging paper mill in Canton, North Carolina, US.

On 21 September 2020, a fire erupted in a confined space when a heat gun fell into a bucket of flammable epoxy vinyl ester resin, killing two contract workers. Evergreen Packaging is a packaging and containers company, making paper and paperboard products. The incident occurred during a planned shutdown of its Canton paper mill, while maintenance and repair activities were ongoing throughout the facility.

CSB’s new safety video includes an animation of the events leading up to the incident. While completing repairs in an upflow tower, Universal Blastco contractors began using a heat gun in the confined space. A fire erupted when the gun fell into a bucket of flammable resin and though the Blastco contractors were able to evacuate, two Rimcor contractors in the connected downflow tower were unable to escape.

The video explains that in its investigation, CSB identified four safety issues that led to the incident, including hot work and confined space safety. CSB Chair Katherine Lemos said that these factors are common safety issues in many major incidents investigated by CSB. The video included commentary from both Lemos and Lead Investigator Drew Sahli.

On Sept. 21, 2020, a fire was ignited in a bucket of flammable resin being used to re-line a fiber-reinforced plastic (FRP) tower at a paper mill (Figure 1). Smoke and fumes from the fire killed two contractors. The incident had many contributing factors and lessons learned, but this Beacon focuses on the uncontrolled hot work.

▲ A fire occurred during hot work at a paper mill. Read U.S. Chemical Safety and Hazard Investigation Report No. 2020‑07‑I‑NC‑3 for more information.
The paper mill was shut down for a turnaround, which included internal repairs to the upflow and downflow bleaching columns (Figure 2). The column repairs were managed under two confined space entry (CSE) permits. Hot work was not planned nor authorized for either job. Initially, no flammable materials were present inside the towers, but the FRP walls in the upflow tower were combustible.

On the day of the fire, the crew working in the upflow tower could not cure their resin properly due to cool temperatures. When the crew could not locate drum heaters, they decided to use a heat gun (Figure 3) to warm the bucket in their working area (Figure 2, red). The crew accidentally dropped the heat gun into the resin bucket, igniting the flammable contents. The crew did not have a fire extinguisher, and the fire spread, eventually igniting the FRP walls. Two contractors working in the connected downflow tower (Figure 2, green) were overcome by the gases before they could escape.

Staff Sergeant Svend Nielson says an employee of the CO-OP Cardlock observed the tractor trailer leaking what appeared to be a purple coloured substance.

Nielson says it turns out that it was the same tractor trailer that was involved in an accident earlier in the week.

“The tractor trailer from the incident from the 11th was moved away from the scene by a tow company that isn’t from 100 Mile, and it was dropped off for a reason we don’t know yet, into the Co-op cardlock without their permission ,and obviously without proper safety precautions in place. The vehicle is covered in essentially wood glue, the resin that was involved in the accident.”

Wheeling fire responders were called on Tuesday at 1:15 p.m. to a factory on the 100 block of Messner Drive in Wheeling for a potentially volatile situation involving chemicals. There were no injuries.

The Aug.10 hazardous material leak incident took place at a factory that makes dental applications such as bridges and retainers. The warehouse is located in an industrial park just west of Chicago Executive Airport near S. Wolf Road.

Crews responded when two 55 gallon drums were apparently having some sort of chain reaction and were leaking, said Wheeling Fire Chief Mike McGreal.

On arrival, responders found the building to be evacuated and located the two drums leaking a chemical, methyl methacrylate, which is a resin used in dentistry to make and repair dentures.

One of the drums was misshapen and responders were worried the barrels could explode.

The chemicals in the barrels were reacting to the humidity and became a fire hazard, McGreal said.

The enraged family kept the bodies of both Sunder Pal (35) and Akash Kumar (19) in a stationary mini-truck on the roadside of Ram Park Colony main gate and staged a protest demanding the owner''s arrest.

On Thursday night, a fire had broken out due to a short circuit at the vehicle filter manufacturing unit that caused a blast in the boiler.

The resin chemical drums, which are used as adhesive to manufacture the vehicle filters also caught fire and spread on the floor.

Sunder Pal and Akash Kumar could not escape as their feet got stuck on the floor and got charred to death, Superintendent of Police (Rural) Neeraj Kumar Jadaun said.

Coffee cups made from resin containing bamboo fibre – often sold as bamboo mugs – leach so much harmful formaldehyde and melamine into drinks that the German Federal Institute for Risk Assessment (BfR) has warned that they shouldn’t be used for hot liquids.

Bamboo mugs are made from a melamine resin reinforced with bamboo fibres. While they are reusable, they are often marketed as eco-friendly, even though they aren’t biodegradable. Melamine resin is a polymer and in itself not dangerous, but its monomers – 1,3,5-triazine-2,4,6-triamine and formaldehyde – are toxic and can cause cancer.

This summer, a German consumer group discovered worrying levels of harmful compounds being released by bamboo mugs. The BfR’s assessment found bamboo mugs release ‘alarming’ amounts of the toxins into 70°C drinks, up to 30 times the EU’s safe limit for adults and up to 120 times the limit for children. Although one-off exposure to large amounts of these chemicals isn’t necessarily problematic, the BfR estimates that regularly using a bamboo mug poses a serious health risk.

Earlier this year Nicole Davis arrived at one of the San Antonio, Tex., offices of the audiology practice she co-owns, ready to see the day’s patients. But upon entering her office, Davis says she quickly noticed a noxious odor that smelled like paint thinner. Her eyes started burning. By noon, she felt nauseated and dizzy, with the burning sensation spreading to her nose and throat. Her mouth went numb. Co-workers in the building told Davis that they felt ill, too. By the evening, she says, she was vomiting.

Two days later, Davis received an e-mail from an employee for a construction firm that was doing work that week on municipal pipes below street-level near the building. The employee apologized in the e-mail for Davis’s “recent experience,” and attached a technical document describing the hazards and health risks associated with materials used to make plastic in the pipe project. The e-mail and attachment do not state that the work caused the odor or Davis’s reaction.

The company was renovating an underground sewer pipe with a widely and increasingly used technique called cured-in-place pipes. A felt or composite sleeve is saturated, typically with a polyester or vinyl ester resin. Workers thread the sleeve through an underground pipe and then inflate and heat it, often with steam or hot water. The sleeve hardens to form a continuous plastic liner along the old pipe’s inner walls. The technique is less expensive and takes less time than fully replacing old sewer-system pipes and stormwater culverts.

DURBAN – A chemical fire in the industrial basin of Durban late on Friday afternoon left residents coughing and battling to breathe, according to community members.

The fire occurred at Hosaf, a fibre plant based in Mobeni, south of Durban, that manufactures polyethylene terephthalate resin and polyester fibre.

Rescue Care’s Garrith Jamieson said paramedics were called to the factory fire at about 4pm. “Fortunately no injuries were sustained, however the fire department and all necessary role players were in attendance.”

By 6.30pm, residents from the Bluff and Umbilo areas had reported "a strong plastic smell", he said, as a strong wind carried the fumes over a wide area.

HOUSTON - Harris County Attorney Vince Ryan has filed a lawsuit against Kuraray America for violations of the Texas Clean Air Act. The violations allegedly occurred as a result of an explosion and flash fire that occurred at the company’s Pasadena manufacturing plant in May 2018.

Through the lawsuit, the county attorney seeks a court order for Kuraray to engage a third party auditor to review its practices and equipment at the Pasadena plant to ensure there are no future violations. Ryan believes that unless Kuraray is ordered to improve manufacturing operations and procedures at its Pasadena facility, poor safety, operations and management practices at the site may cause future releases that will endanger the public and employees.

Kuraray America is an American subsidiary of a Japanese chemical manufacturing company. It specializes in chemical, fiber and resin production and began operations in Houston in 1986.

On May 19, 2018, a reactor at the plant at 11500 Bay Area Boulevard was pressured to the point that a safety valve lifted, which caused ethylene vapor in the reactor to be released into the atmosphere. The ethylene vapor encountered a contractor’s welding equipment, which acted as an ignition source and ignited, causing an explosion and flash fire.

21 workers were injured and transported to area hospitals, three by medical helicopter. The fire is currently under investigation by the Chemical Safety Board and OSHA.

A 15-mile stretch of Interstate 75 in Lee and Charlotte counties was reopened Monday night after being shut down for nearly 12 hours due to a truck leaking a poisonous chemical.

Florida Highway Patrol closed the southbound lanes at Tuckers Grade (Mile Marker 158) and the northbound lanes at Bayshore Road (Mile Marker 143) at around 10 a.m. Southbound lanes of I-75 at Tuckers Grade reopened shortly before 2 p.m.

The northbound lanes of I-75 remained closed until 9:55 p.m., due to containers having to be inspected and offloaded onto a second refrigeration truck, the FHP said.

During a traffic stop, troopers smelled an odor coming from the truck and saw
something leaking from the right rear door.

It was determined that the truck was transporting over 15,000 pounds of a highly flammable resin solution.

The FHP said the carrier involved was Insituform Technologies LLC out of Chesterfield, Montana. The company was cited for a violation of 49 Hazmat regulations covering securing packages in a motor vehicle.

According to information provided by the Cheshire Fire Department, the overpowering odor that emanated from the site was the result of phosphoric pentoxide anhydryde, with polymer resin, being incorrectly mixed with water. A “vapor cloud” was created as well as the powerful odor.

The CFD, along with the City of Waterbury Hazmat team and the state Department of Energy and Environmental Protection metered the surrounding area and determined that “at no time were any levels of any chemicals detected in the air.” The area was evacuated for precautionary purposes and employees at Dalton were medically evaluated but all refused further treatment, according to the CFD.

Portions of Willow/Spring Street remain closed off, however the CFD states that, while there may be a residual smell in the neighborhood and surrounding area for some time, it poses no health risk to anyone.

UPDATE: (3:02 p.m.) The northbound lanes are now open, but the highway remains closed southbound.

Emergency Environmental Response Officers (EERO) have been deployed to stem the damage from a tractor trailer fire which forced the closure of the Coquihalla Highway this morning.

An update from the Ministry of Environment at 11:43 a.m. noted that two response officers were en route to the fire, which is burning near the Carolin Mines Road exit.

As of 1:00 p.m., the Coquihalla remained closed in both directions between Merritt and Hope due to the ongoing incident.

A chemical called phenol formaldehyde resin was leaking from the trailer onto the highway and into an adjacent ditch. The chemical is corrosive, which prompted the ministry to deploy the EEROs.

“Sand is currently being used to dam the ditch and prevent further spread of the product,” stated an update from the Ministry of Environment.

Firefighters could spend several days fighting a massive industrial fire that broke out early Saturday morning on the outskirts of Parkersburg, according to a news release from the Wood County Unified Command.

Firefighters worked all day to fight a blazing fire at what was formerly the Ames Tool Plant, the release said. Black smoke filled the sky, blanketing some of the surrounding areas in a layer of ash.

The Wood County Commission declared the scene a disaster area and strongly advised people to stay indoors to avoid inhaling the smoke, especially people with respiratory issues.

The fire was initially called in to local dispatchers as an arson, according to a copy of the initial spill report provided by the Department of Environmental Protection.

A later news release from the county’s Unified Command said officials from the DEP were testing the quality of the air and the fallout from the smoke.

“Initial air monitoring testing completed in the immediate and surrounding area of the fire scene shows air to be within acceptable quality limits,” the release said.

The fallout particles could be harmful, the release said, and such particles were found 15 miles up the road in Williamstown.

Jake Glance, a DEP spokesman, said the DEP has a “good idea” of what materials were involved in the fire — plastic pellets and a material called acrylic resin. Parkersburg Police Supervisor Jill Murphy said the building was not an active plant, and nothing hazardous was stored there.

HANNIBAL, Mo. -- A semi-truck traveling on Mo. 168 just north of Hannibal went off the road and overturned Monday, spilling the chemical it was carrying.

The Missouri State Highway Patrol received the call at about 10:45 a.m.

"The driver said he felt his weight shift. He then traveled off the road," Trooper James Johnson said.

The semi driver, Anid Turan, 25, of St. Louis, was the only occupant in the vehicle and was uninjured, Johnson said.

The chemical that the driver was hauling to BASF Agricultural Products Group in Palmyra was lupranate mio isocyanate, a resin-based iron product, said John Hark, director of Hannibal/Marion County Emergency Management. It is an inhalant that can cause mild eye and nose irritation.

"The best thing about it -- even though it is considered hazardous -- is that the materials do not have to be placarded," Hark said, referring to the diamond-shaped hazardous material sign affixed to vehicles hauling hazardous materials. "They're not that dangerous, and under these circumstances, we do not have to do an evacuation, which I'm very thankful for."

The hazardous materials team assessed the extent and status of the spill before tow trucks pulled the semi upright and removed it.

Glassers are the surf industry’s version of Clark Kent. While shapers receive praise and name recognition for designing top-selling surfboards, glassers go unnoticed and underappreciated, as they humbly labor away with carcinogenic materials. On Friday, a bastion of those unsung heroes – Global Glassing in Oceanside, CA – was set ablaze from a chemical fire. Reports say the fire caused half-a-million dollars in damage. One employee was in the building at the time, attempting to extinguish the flames, and suffering burns to his face, but opting to drive himself to the hospital.

“Fire crews determined that there was no way to fight the increasing volume of fire inside the involved unit and pulled all personnel outside to begin defensive operations designed to limit the spread to the other businesses in the L-shaped building,” Battalion Chief Pete Lawrence told Fox 5 San Diego.

Surfboard glassing involves using many hazardous and flammable materials, including resin and acetone – which is reported to be the culprit of Friday’s fire. And Global Glassing was a major player in the Southern California surfboard industry, being the last stop for many local shapers and big brands before hitting the store racks. From Channel Islands to Firewire to Slater Designs, the Oceanside glass house had a wide array of popular industry clients.

“I watched from across the street as my place of occupation burned to the ground in what seemed to be an instant,” Global Glassing’s Arin McKinney wrote via Instagram. “It's a horrifying feeling to watch a fire of that magnitude burn beyond our control. Relieved to say that everybody made it out mostly unscathed from what I understand and fortunately I was not directly in harms [sic] way on this go around.”

NORTH ANDOVER – A chemical reaction Wednesday afternoon at Atlee electronics hardware manufacturing company in North Andover caused four employees to evacuate and prompted a response from a regional hazmat team.

At 12:39 p.m., the North Andover fire received a call for a chemical reaction causing smoke inside 10 Bayfield Drive, the building that houses Atlee, part of the business park off of Route 114.

Fire chief William McCarthy said the reaction was caused by a common epoxy resin hardening process gone wrong.

"They mixed larger quantities than normal and it started to overheat and react, and they developed a smoke condition in the building," he said.

The epoxy resin is normally used as a liquid that is poured over a circuit board or other similar electronic hardware and then hardens.

When the fire department arrived, McCarthy said the four people who were inside the building at the time of the incident had already exited, with no injuries.

SPRINGFIELD -- Thursday marks the 17th anniversary of an explosion at the Jahn Foundry plant in Springfield.

On Thursday, Feb. 25, 1999 at around 3 p.m., a small explosion occurred in the Jahn Foundry shell molding department. The explosion sparked a fire and caused resin powder to fill the department. A second explosion then occurred, one investigator said in court had the power of "three severe hurricanes."

The second explosion blew out concrete walls, including an exterior one to the plant, and threw cement blocks 300 feet.

Nine workers were severely burned in the blast and three died due to injuries.

Over $35 million was paid out to victims in years following the explosion as lawyers for the corporation and a chemical supply fought over whose insurance company would pay the higher amount.

Borden Chemical Inc. was the manufacturer of a resin powder used in the plant, a company lawyers for Jahn Foundry argued did not detail explosive the powder could be.

Unable to recover from the blast, parent company Atchison Casting Corp. of Topeka, Kansas, filed for bankruptcy in 2003.

FORT EDWARD  A manufacturer of “phenolic balls” used in the fracking process for extracting oil and gas from rock formations is facing 48 serious violations of federal workplace safety laws.
The firm, A. Hyatt Ball Co., located at 1 School St., faces fines of up to $105,200 for the violations, which stemmed from an inspection launched after a complaint was filed with the U.S. Department of Labor’s Occupational Safety and Health Administration, according to a prepared statement from the federal agency.

“Our inspection identified a disturbing number of violations,” said Kim Castillon, OSHA’s area director in Albany. “A. Hyatt Ball lacks basic safety and health programs to prevent fires, explosions or an uncontrolled release of highly hazardous chemicals, including formaldehyde, which has been linked to cancer and can cause allergic reactions to the skin, eyes and respiratory tract.”

Among the problems found were the presence of combustible resin dust, flammable liquids improperly stored and transferred, the lack of an audible fire alarm and fire-suppression system and locked and obstructed exit routes.

“The company must correct not only the chemical, fire and explosion hazards, but also the wide range of violations that expose its employees to falls, crushing injuries, chemical burns, lacerations and eye and face injuries,” Castillon said.

The company was given 15 business days to comply with the OSHA findings, meet with Castillon or contest the findings before the independent Occupational Safety and Health Review Commission.

Soldiers from the People’s Liberation Army (PLA) were also called in.
The accident happened on April 6 at a plant operated by Xianglu Tenglong Group, a company that media reports say is owned by unspecified Taiwanese investors. The same facility had already experienced a less serious blast in July 2013.
Television reports from the April 6 explosion showed flames shooting tens of feet into the air. China’s state agency Xinhua reports that no one died but 14 people were injured.
Zhangzhou’s fire department posted on Weibo—the Chinese equivalent of Twitter—that 610 firefighters and 122 fire engines from the cities of Zhangzhou, Xiamen, Longyan, and Quanzhou worked together to extinguish the blaze. The fire department also noted that soldiers from PLA’s chemical warfare unit participated in firefighting efforts, although the nature of their contribution was not described.
p-Xylene is a highly controversial chemical in China. Large protests have erupted in recent years in the cities of Xiamen, Maoming, Dalian, and Kunming to demand that p-xylene facilities be either shut down or not built at all. Elsewhere in the world the material, used in the production of polyester fiber and resin, is produced without creating a great deal of controversy.

Strongwell Marketing Manager Barry Myers said the company is still investigating to determine if a freezer malfunctioned or if a catalyst that cause a prescribed chemical reaction began its process too early, but 14 gallons of resin vaporized into the air.
Three workers in that area were exposed and immediately put through a decontamination process and other accident protocols, Myers said.
"There was no explosion or anything like that, just a chemical reaction that emitted a vapor that some workers got into close contact with," he said.
A male worker was taken to Bristol Regional Medical Center for further treatment and was being held for observation for 24 hours, the company official said.

Investigators from the Piqua Fire Department arrived at the scene and contacted the EPA.

It is believed that possibly 20-25 gallons of the chemical P500 (Permacel) Foaming Agent Concentrate spilled onto the concrete at the gas station.

According to the material safety data sheets for the products, P500 is used as a foaming and curing agent for Core-fill 500 resin. It is classified as a corrosive material.

Firefighters monitored the venting of the mixture in the 15000 block of Commerce Lane at about 4 p.m. and evacuated the surrounding businesses as a precaution, fire Capt. Bob Culhane said.

Authorities identified the two chemicals as a resin and triethylamine, which when combined produced heat, carbon monoxide and carbon dioxide.

The container where the substances were "mixed in error," Culhane said, was taken outside. The fire captain added that the fire crew monitored the reaction and allowed it to "run its course."

No injuries were reported.

Huntington Beach police posted on its Facebook page that the reaction is expected to dissipate by 9 p.m.

No one was harmed after a highly toxic chemical spilled at a Salt Lake City warehouse Wednesday morning.

Salt Lake City Fire received a call just before 9 a.m. of a chemical spill near 5178 W. 150 South, according to spokesman Jasen Asay.

Asay said when hazmat crews arrived at RinChem, a chemical distribution company, they found about 10 gallons of highly toxic, flammable chemicals on the floor. The liquid was a mix of a substance called TCIR-ZR8690PB and a second chemical. Asay did not immediately know the name of the second chemical.

According to Asay, 20 glass bottles filled with the first chemical — a resin commonly used to make flash drives — were sitting on a shelf in the warehouse when the shelf broke and the bottles spilled on the ground. As the bottles fell, they knocked down other bottles containing the second chemical.

Asay said the two chemicals were "compatible" and did not produce a reaction.

One of the Capital Region's largest chemical plants has been hit with its third state pollution fine in less than two years, after air pollution control equipment exploded last fall due to improper operation.

SI Group, which operates its plant at Main Street in Rotterdam Junction, agreed to pay a $112,500 fine under the federal Clean Air Act, according to a consent order signed with the state Department of Environmental Conservation this month.

It was the fifth largest air pollution fine levied in the nine-county DEC Region 4 in the last 10 years.

SI Group also agreed to DEC oversight in obtaining and installing new pollution control equipment for its chemical resin processing operation, and to have a system acceptable to the state picked out by mid-May. If those conditions are not met, SI faces another $165,000 in fines.

An Oct. 10 explosion at the plant that destroyed an air pollution unit was preventable, according to DEC, and was caused because SI workers "lacked knowledge of the full range" of the explosive nature of the chemical reaction, and had "improperly engineered" a release of flammable vapor.

After a four-month investigation into the destruction of the equipment, called a regenerative thermal oxidizer, DEC "concluded that neither a malfunction nor an emergency existed" despite what the company had first claimed, according to a Feb. 21 report.

Nearby residents were advised to continue to remain inside, with doors and windows closed and air conditioners switched off, however, because while the smoke was not hazardous it could cause irritation.

A warning was initially issued for people on Wednesday afternoon after a chemical fire broke out about 3.30pm at a factory at Coulson Way in Canning Vale.

While the chemicals in the resin factory were hazardous the smoke is not dangerous.

The Department of Fire and Emergency Services confirmed the chemicals inside the chemical resin factory were hazardous, but emphasised the smoke itself was not dangerous.

DETROIT -- The potential shortage of a key component used to make fuel lines and brake lines could force automakers in the U.S. and around the world to close car and truck plants as they run short of parts.

Auto industry executives have scheduled an unprecedented meeting on Tuesday in suburban Detroit to talk about the problem. Officials from as many as 10 automakers and dozens of parts supply companies are set to attend.

A March 31 explosion at Evonik Industries in western Germany killed two workers and damaged a factory that makes CDT. That chemical is a key component in a nylon resin called PA12, which is used to make a specialized plastic. The plastic is used in auto fuel lines and brake lines. It is also a component in solar cells, pipelines, sporting goods and household items.

Any auto plant closures that might result from the shortage would come at a crucial time for the industry. U.S. sales are the highest they have been in recent years as the industry recovers from the recession. March was the best month for the industry in nearly five years.

Automakers aren't pushing the panic button yet, but they are concerned.

The company has a 55 gallon drum of plastic resin that was overheating. The resin is kept at 110 degrees and when employees arrived to Cryovac Tuesday at 7:30 a.m., they noticed that smoke was coming out of the drum and pulled the fire alarm.

Gates fire chief Jim Harrington said the drum had brought the temperature to over 200 degrees. When the material inside overheats, a chemical reaction causes the material to increase in heat, Harrington said. Firefighters were worried the drum was going to over-pressurize and explode so they evacuated nearby employees and cooled the drum off with a fire hose.

EL PASO, Texas -- Thirty fire units were called to El Paso's Lower Valley Friday afternoon.
About 3,000 gallons of the chemical carbolite resin was spilled.
A drum holding the chemical, most commonly used on auto parts and products, was tipped over at the Bosch Industries Warehouse in the 9600 block of Pan American Drive.
Deputy Chief Keith Burch of the El Paso Fire Department said, "The spill is not enlarging, so we're going to seal up the building and deny entry and now it will be up to the building owner to hire a contractor to come and clean it up."
