According to a Facebook post from Boise Fire Department, the Region 4 team was requested to respond to a propane trailer that was involved in a wreck. When crews arrived, liquid propane was leaking from the rear of the trailer, and a 5-mile evacuation radius was established.

After staying on location for 2.5 days and flaring for 24 hours, Boise Fire HazMat was able to flare off the propane from the trailer.

Ottawa Fire Services responded to a call from the Dynacare medical services lab on Peter Morand Crescent, east of the The Ottawa Hospital General Campus, after a report of a propane smell in the building.

An individual from the lab called Ottawa Fires Services (OFS) around 7:53 p.m. on Friday, Dec. 16, reporting the five people in the lab could smell propane and were experiencing symptoms of "watering eyes." The building was evacuated and the OFS Hazadous Materials (HazMat) team dispatched.

The HazMat team discovered elevated levels of carbon monoxide in the building, peaking at 30 part per million. An acceptable level of carbon monoxide is considered to be less than 25 parts per million as an average over an eight hour period, according to the Ontario regulation for occupational exposure limits.

A fire crew inspecting the roof found a faulty heating system to be the cause. The system was shut down and tagged for inspection.

SAN ANTONIO (AP) — A drug lab and explosive devices have been ruled out as the possible causes of a deadly explosion that killed four people in San Antonio last week, authorities said Tuesday. They said they are still investigating other possible causes, including potential problems with a propane tank.

The explosion just before midnight Friday destroyed K-Bar Services, which San Antonio Fire Department officials described on Tuesday as an underground residence and construction business located on 75 acres (30 hectares) of property. About 100 vehicles, including construction machinery, cars and recreational vehicles, are also located on the property, the officials said at a news conference.

Wet weather, muddy conditions, structural issues and a large radius of evidence produced by the magnitude of the blast have slowed the investigation, they said.

"It was absolutely unsettling as far as the devastation that we witnessed," said San Antonio Fire Department Chief Charles Hood. "As far as first responders, we don't see things like this type of explosion inside of a building like this."

Witnesses who said they had been on the property prior to the explosion told officials that they could smell gas, and a propane tank on the property had been worked on recently, the fire officials said. They added that foul play has not been ruled out and the investigation is ongoing.

SAN JUAN, Puerto Rico -- The Environmental Protection Agency on Monday ordered the owners of an oil refinery in the U.S. Virgin Islands to remove thousands of pounds of chemicals from the site, warning that its pipes and valves are severely corroded and in disrepair.

The EPA estimates that more than 40,000 pounds (18,000 kilograms) of anhydrous ammonia and more than 37,000 pounds (16,000 kilograms) of liquefied petroleum gas are stored at the refinery in St. Croix. It also ordered the removal of hydrogen sulfide.

The order comes after the EPA inspected the refinery in September and found serious deficiencies that could lead to a fire or chemical release, including piping and valves on LPG units that are severely corroded and in disrepair. Last month, the EPA announced the oil refinery could not resume operations without obtaining a clean air permit.

The refinery was forced to pause operations in May 2021 following an EPA emergency order, with the agency noting that the facility is located near marginalized communities where people have been sickened and schools forced to close.

Thick smoke billowed from the Nox-Crete facility just southwest of downtown Omaha that could be seen as far away Monday evening as the Elkhorn River, about 20 miles (32 kilometers) to the west.
Battalion Chief Scott Fitzpatrick said the first call for help came shortly before 7 p.m. Monday, and firefighters who initially entered the building found a much bigger fire than they had anticipated, forcing them to retreat.
Fire crews then fought the fire from outside the building, noting the presence of chemicals and propane bottles inside. Explosions could be seen and heard as propane tanks ruptured.

The fire first was reported in a company producing lab equipment due to a blast in an Liquified Petroleum Gas (LPG) cylinder which then spread to 12 other shops and small scale companies in the vicinity. The authorities confirmed that no one was injured in the fire.
The Regional Disaster Management Cell (RDMC) informed that nine blasts were heard in the area and the investigation as to the source of these blasts is still in progress.
Avinash Sawant, chief of RDMC said, “The fire was reported at 10.09 pm in Ambika Nagar area of Wagle Estate. There was a blast in an LPG cylinder in Silica Scientific, which produces raw materials for laboratories. We doused the fire in an hour and the cooling operation is in progress. Over 13 shops were gutted in the fire and we are yet to establish the damage. Once the cooling operation is completed, we will investigate further.”

MESA, AZ (3TV/CBS 5) - Garbage trucks in Mesa are catching fire, creating dangerous conditions for truck drivers and the community. City officials said there have been six garbage truck fires in the last two months because people are throwing away hazardous materials in the trash.

“We want residents to know that hazardous materials never should go in a trash or container,” said Mariano Reyes with the City of Mesa Environmental Management and Sustainability.

Items like batteries, aerosol cans, pool chemicals and propane gas tanks should be dropped off at the Mesa Hazardous Materials Facility. Reyes said even if they are empty, those items can catch fire or explode. “They can still contain enough liquid material in them that when they are compacted in our garbage trucks, it can actually cause an explosion,” said Reyes.

The facility even has a “swap shop” where people can drop off used hazardous items like paint, cleaning products and other chemicals that Mesa residents can pick up for free. The facility is open Wednesday through Saturday from 7 a.m. to noon. For a full list of items it accepts, click here.

What started as an innocent backyard fire in Pleasant Grove backyard yesterday quickly spiraled out of control and brought down power lines in the area.

According to Andrew Engemann, Pleasant Grove fire chief, the fire began when a man started burning unsuitable materials in his backyard.

“A gentleman in one of the homes was in the backyard burning some stuff that probably shouldn’t have been burning,” Engemann said. “Just too close to fence lines and the sheds. Just a reckless type of burning; he probably shouldn’t have been doing it but he had nowhere else to do it.”

The man had left the fire unattended to go inside his home when it quickly spread along the fence line to two nearby dry wooden sheds. Residents of the home were evacuated, and American Fork Fire & Rescue responded to the scene.

As the fire progressed, flammable chemicals that were stored inside one of the sheds exploded.

“He had a number of chemicals and stuff in one of the sheds that had some minor explosions,” Engemann said. “We did have one propane tank that actually exploded, which made a pretty good noise and actually got everyone’s attention.”

In the end, a small shed and its adjacent two-story garage burned black inside and out, though walls remained standing.

Following the approximate 1:15 p.m., May 5, 2022, call to 911, a Lincolnville firefighter who happened to live nearby was quick to the scene, according to Lincolnville Fire Chief Don Fullington. The shed was fully engulfed in flames despite garden hose water being pumped inside its door. Yet, when one of the homeowners spoke of oxyacetylene, propane, and welding supplies, and possibly some propane tanks in the back, the initial attempts with the garden hose ceased and relinquished to the interior-equipped firefighters still en route from Lincolnville Fire, with more personnel coming from Hope, Camden, and Searsmont.

“When I got here and started attacking the fire, there were several explosions going off,” said Fullington.

Five people were injured Wednesday morning in an explosion and a major fire at a hub for industrial food preparation and processing in Sherbrooke, Que.

Fire crews and paramedics responded to the Centre de valorisation de l'aliment de l'Estrie (CVA) after reports of an explosion around 8:30 a.m.

"They're dealing with burns, but we still don't know the extent of the injuries," Stéphane Simoneau, director of the Sherbrooke fire department, told Radio-Canada.

It's not clear if any of the injuries are life-threatening.

Dr. Marie-Maud Couture, chief of the emergency department at the Sherbrooke University Hospital Centre (CHUS), said five patients were treated at the ER. Some, with more serious injuries were transferred to other specialized health centres.

The building, with nearly the footprint of a CFL football field, includes several industrial kitchens, food processing warehouses, workshops, event spaces and other food-related businesses.

Around 40 firefighters from six different stations are on site, and Simoneau expects crews to be there all day.

"There was a lot of combustible material inside," he said, adding that the explosion appears to have damaged the building's sprinkler system and prevented it from activating.

It's still too early to determine the origin of the explosion but "it seems obvious" hazardous materials were in play, Simoneau said.

"It was likely caused by propane or natural gas, " said Simoneau. "That's what the investigation will show us."

Firefighters were called Full Circle Ag around 11:30 a.m. for a report of a fire, Day County Emergency Management Director Bryan Anderson said.

No injuries have been reported, he said, and the cause is unknown as firefighters from five departments work to extinguish flames. The fire has spread from one of two storage buildings to the office building, he said.

Fire departments from Pierpont, Webster, Andover, Bristol and Langford are on scene, and the Britton fire department is on standby in case additional water is needed, Anderson said.

State Highway 27 is blocked, and traffic is being directed around the area, Anderson said.

The fire originated in the chemical shed, and smoke is rising up about 1,000 feet before moving to the southeast, he said. That means toxic fumes from the chemicals are not a concern at this time, he said.

Materials in storage are not combustible, Anderson said, and the nearby propane tank has been turned off and is being monitored.

At about 23:00 local time on 2 December, systems aboard the ship detected smoke in an electrical utility area, triggering automatic fire detection and management. The fire was extinguished, preventing further spread.

Production at the facility – which operates off the coast of Australia – is suspended until the main power is restored. Shell did not say when resumption is expected.

This news represents the latest setback in a series for the 488 m long ship, which has production capacity of 3.6m t/y of LNG, 1.3m t/y of condensate, and 0.4m t/y of LPG. This includes a previous electrical issue in February 2020, which halted production until January 2021. Operations started in December 2018.

According to police, the propane truck overturned in the 14000 block of Tylers Beach Road, killing the driver. The crash caused a hazmat spill, which closed the road in both directions.

The preliminary investigation by state police indicated the 2005 International 4400 truck (Southern States Propane) was traveling on Tylers Beach Road when the driver lost control of the truck.

The truck ran off the roadway, the driver then overcorrected and struck a tree, causing the truck to overturn.

Ferndale, MI – Homes and businesses near 8 Mile and Livernois in Ferndale shook as the thunderous sound of an explosion inside a shipping container pierced the otherwise quiet evening of Jan. 2, 2021.

The explosion had enough force to buckle out the walls and roof of the 4,000 lb, thick-steel-walled container, leaving it with a ballooned out appearance. The force also blew out windows of Olive Labs, a laboratory currently specializing in COVID-19 testing.

The container was one of several placed on the site without the owner having gotten a permit. And the cause of the explosion was because a contracted worker had set up a propane space heater in the enclosed space to help dry glue on insulation panels he was adding to the inside walls.

“It was not a good decision,” said Ferndale Fire Marshall Dennis Barr who was tasked with investigating the explosion. “First off, you shouldn’t use propane space heaters in enclosed spaces, and you should never leave space heaters unattended,” he said.

Maj. Bernie Faldas, chief of Norala municipal police station, said Monday the incident happened past 9 a.m. at the residence of the Rapista family in Sub-Purok Bliss, Purok Reloquemas in Barangay Poblacion.

He said a balloon vendor identified as Neilfren Cabuhay, 40, and several family members had just finished inflating balloons using what looked like an LPG tank when it suddenly exploded.

Citing accounts from witnesses, Faldas said one of the victims reportedly mixed unknown chemicals and filled the tank with them before the incident.

“The investigation is still ongoing and we’re mainly determining the chemicals that caused the explosion,” he said in a report.

The Chicago Fire Department responded to the extra-alarm fire near 119th Street and Avenue O in the Hegewisch neighborhood about 8:40 p.m.

The Corpus Christi Inner Harbor is closed from the Chemical Turning basin to the Viola Turning basin, disrupting some shipping traffic at the port which is a key outlet for exporting US crude and other commodities. The port on 21 August was closed from the Harbor Bridge inward.

A dredging barge struck a submerged propane pipeline at around 9am ET on 21 August causing the explosion and fire. The Coast Guard yesterday recovered the bodies of two crew members and two others are still missing. Six other crew members were taken to area hospitals last week, according to the Corpus Christi fire department.

A fire on the barge reignited late on 21 August and was extinguished before the vessel broke apart and sank. The Coast Guard was planning to recover the barge, but inclement weather could hamper the process as tropical storm Laura appeared headed to the Texas or Louisiana coast later this week. Firefighters were called around 4:30 p.m. Friday for a leaking tank on Ferndale Drive. Chief Ken Kelly arrived at the address and found a leak in a 20-pound propane barbecue tank. The owner said the tank was full.

The department's HazMat team was sent to the address to burn off the gas, which is the "safest and most efficient" way to eliminate the danger, Kevin Regan, a spokesman for the department, said in a news release. The HazMat team connected a hose to the leaking tank to allow the gas to flow a safe distance away through a water-filled barrel. When the propane passes through the hose, the gas rises through the water in the barrel and is ignited by a flare at the top of the barrel. The propane burned for about 45 minutes until the tank was empty.

Crews determined the tank had a lot of rust and a small crack on the bottom.

Federal workplace safety regulators have proposed fining a Waukegan plant just under $1.6 million following a May explosion that killed four people.

AB Specialty Silicones was cited for 12 willful federal safety violations, according to a statement from the Occupational Safety & Health Administration. OSHA, a branch of the U.S. Department of Labor, placed the manufacturer of silicon products in its enforcement program for “severe” violators.

Michelle Penman, an account manager at AB Specialty Silicones, said in an email that the company had cooperated with the investigation and was now reviewing the citations.

"It is important to recognize the scope of the OSHA report goes beyond the elements that contributed to the tragic explosion," she wrote. "Our internal investigation, as well as the fire marshal’s report, confirmed the incident was in fact an accident, and there was no willful intent involved."

The plant’s electrical equipment didn’t meet OSHA standards, according to the OSHA statement. AB Specialty Silicones also moved flammable liquids using forklifts powered by liquid propane, driving them around areas where employees processed volatile liquids and gases, “creating the potential for ignition.”

The FDNY descended on the T & T Supply Co. on Willets Point Blvd. in Flushing after an Oct. 9 inspection by the Bureau of Fire Prevention Salvage Yard Task Force uncovered the plethora of tanks, most used to hold propane or acetylene, authorities said.

The business was immediately shuttered as the owners of the company were ordered to hire outside contractors for removal of row after row of the dangerous canisters.

“That’s a disaster waiting to happen,"said a retired city explosives expert with more than 30 years of experience. “I really don’t believe it. Oh my God. How the hell did he get away with it? You know the shrapnel that would have been flying around if God forbid that was a fire? ... If it blew, the tanks would take off like friggin’ torpedoes."

SJCFR HazMat teams responded and attempted to mitigate the leak, but were unsuccessful due to the damaged condition of the tank.

According to SJCFR, in order to make the tank safe, the gas had to be burned off using a process called "flaring."

In the Facebook post SJCFR explained that "flaring" involves crews re-plumbing the tank to attach a piece of equipment called "the red dragon" that raises the burn off flare high above the ground to control the amount of liquid and vapor burned off.

CHICAGO — Firefighters say they're exhausted after battling a large fire that destroyed multiple self-storage units inside a Garfield Ridge facility around 8 a.m. Sunday morning.

The fire started in a handful of units inside a Cubesmart storage facility at 62nd Street and Harlem Avenue on the Far Southwest Side, but spread to about 20 storage units by the time it was extinguished.

Several storage cubes contained chemicals, moving firefighters to treat the response as a potential hazmat situation. A few propane tanks exploded, but nobody was injured. The fire took a few hours to put out, and there were a lot of firefighters on scene.

Those units are completely destroyed. Firefighters say they had some issues with water because the fire hydrant outside of the facility was not working.

Chicago fire says that is common especially in places like 62nd and harlem which is right at the edge of the city of chicago and water is scarce. The fire spread quickly from unit to unit and that made it difficult to put out quickly.

(BIVN) – A dangerous, floating cloud of gas resulted in the temporary closure of Shipman Industrial Park in Keaʻau today, and forced companies operating in the area to shelter in place.

No one was injured by the cloud of propane gas, which was created when a valve was reportedly torn off a Liquefied Petroleum Gas, or LPG, container, fire officials say.

The fire department responded to Kipimana Street, between Melekahiwa Place and Kalapa Place, just after 11 a.m., and found a “large vapor cloud” in area moving with light winds. Traffic was stopped to Shipman Industrial Area, and the source was identified as “a torn off fill valve located at the bottom of a 20’ LPG shipping container (24 sq meter/ ~6,300 gallons),” a fire dispatch stated.

The liquid LPG “formed a large ice flow of LPG under the container, which released a large cloud of visible Propane gas,” the fire department said.

The cloud propane cloud, which the fire department says can be highly flammable and is heavier than air, moved over the surrounding area in the variable winds and raining conditions. “Neighboring properties were instructed to shelter in place and extinguish any outside flame/industrial sources, vehicles included,” the dispatch said.
