The combustion happened around 1:21 p.m. at the building, according to Kendra Loney, a spokesperson with Nashville Fire Department.

When fire crews arrived, they found a box of chemicals believed to contain organic peroxide had combusted and was releasing vapors.

Everyone was evacuated from the building. NFD HAZMAT, according to Loney.

Loney said in a statement the organic peroxide was shipped inside shrink-wrapped carboard boxes on a pallet left of a bay dock. The chemicals inside needed to be stored at minus 4 degrees, otherwise it can combust.

As temperatures began to rise, the chemicals combusted. Loney said the threat was minimal as it was contained within the shrinkwrap.

Everyone was able to evacuate the building without injury. NFD remained on the scene to monitor emitting gases.]]></description>

"There were a lot of questions about the delay in some of that," said Mayor Rodger Lingerfelt.

Wright said once they found out what they were dealing with and the DeKalb County Emergency Management Agency was notified, then EMA began communicating with the trucking company to line up a spill clean-up.

"There was a delay with the chemical company. They had people come that didn't have the manpower, and they had people turn it down," he said. "We were eight to nine hours into the event before anybody arrived to start mitigating, offloading the chemical, and cleanup."

The company associated with the 18-wheeler deployed their hazmat teams out of Nashville and Atlanta,

Wright said the chemical spilled was peroxide acidic acid. It has a lot of hazards associated with it and can cause several health issues, including inhalation hazards, affixation, and flammability.

“It had a lot of chemical reactions to it, including metals and different things. We had 43,000 pounds of this stuff, most of it contained inside,” he said. “As we know, we had another leak the next day. The stuff off-gasses as it heats up or pressurizes.”]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[We're outraged by the news of a deadly blast that occurred at the BM Inland Container Depot in Sitakunda, Chattogram, killing at least 49 people (as of Sunday 7pm) and injuring hundreds. The sheer ferocity of the fire and subsequent explosions that swept through the facility since Saturday night can only be rivalled by the sheer meaninglessness of the tragedy that followed. The fire, which couldn't be fully doused even after about a day, is the latest in an ever-growing list of tragedies that put Bangladesh's appalling industrial safety record once again under the spotlight.  

The death toll will no doubt rise as many of the injured are said to be in a critical condition. Horrifying scenes of panic and devastation have been reported both on the spot and at the hospitals. Some of the explosions – there were many – were so large that their sound could be heard several kilometres away. One eyewitness described seeing "fireballs falling like rain". The challenge for the firefighters and military rescuers, who joined them later, has thus been quite daunting as they struggled to bring the fire under control and pull charred bodies from the debris. At the hospitals, the air is thick with grief and anxiety.

What caused the fire? We don't know yet. But there were reportedly containers with chemicals in the depot, including a huge quantity of hydrogen peroxide – an oxidising chemical – which can intensify fire and is a major fire hazard when combined with other chemicals and substances. Apparently, the fire service wasn't informed of the unauthorised storage of chemicals (leading to at least nine firefighters being killed). This—in a depot containing millions of dollars of garments—will likely be a central focus of the investigation. Whatever caused the fire and subsequent explosions, we hope that investigators will find out soon and that those responsible will be held accountable.

A deadly blaze and several massive explosions have left 48 people dead and nearly 450 injured in Sitakund, near the port city of Chittagong, Bangladesh. The fire began on the evening of 4 June, and took 86 hours to douse with the army’s assistance. The explosions killed ten fire service personnel, with a further three among those still missing. DNA tests have been carried out to identify most of the bodies.

Explosions caused by stored hydrogen peroxide threw multiple containers into the air and released deadly fumes
The incident took place at BM Container Depot, a private facility storing clothing and other items for export. The exact cause is being investigated, but hydrogen peroxide appears to have played a significant role. The chemical was being stored in containers – allegedly without authorisation – destined for use in the textile industry. As the fire spread, eyewitnesses said that multiple containers exploded and were launched into the air. Clouds of toxic fumes formed, causing burns, eye injuries and deaths.

‘The massive explosions have traumatised the population the scale of which was hardly ever seen in Bangladesh,’ says Muhammad Ali Shahin, an environmental activist with the non-profit Young Power in Social Action based in Chittagong. ‘Nearly 200 people got critically injured and deaths are still rising, especially among badly burned people.’

Civil society organisations have called the fire a ‘structural murder’ and blamed the incident on Bangladesh’s apalling industrial safety record, poor infrastructure and institutional preparedness at odds with international safety standards. Activists say that no lessons have been learned from numerous previous accidents in Bangladesh. They expressed anger that eight BM employees were arrested by the police for negligence rather than the facility’s owners.

    <dc:creator>dchas</dc:creator><description><![CDATA[Dhaka, June 5 (PTI) A massive fire and a series of subsequent explosions at a private chemical container depot in southeastern Bangladesh killed at least 49 people, including nine firefighters, and injured more than 450 others, officials said on Sunday, as firefighters were still struggling to put out the blaze.

According to doctors, the toll could rise further as many of the injured people were being treated with severe burns. The firefighters backed by army troops were yet to enter deep down the blast site to douse the fire nearly 23 hours after the fire broke out on Saturday.

"The fire was caused by chemicals including hydrogen peroxide, making the situation difficult, fire service chief Brigadier General Mohammad Mainuddin told reporters at the scene where the firefighters were trying to extinguish the blaze amid intermittent explosions since Saturday evening.

The official expected the firefighters to put out the blaze and halt the explosions by tonight.

On Tuesday morning, just after 5:30 am, a tractor-trailer traveling northbound just south of Route 141 in Newport, Delaware was hit by a pick-up truck carrying a trailer full of hazardous materials.

The pick-up truck, a 2017 GMC Sierra with a trailer attached, had 1,500 pounds of organic peroxide. The contents spilled onto the highway.

A witness tells Action News that the driver suffered two broken legs.

Throughout the day, traffic was a mess around the area with many drivers simply stuck in place. "It increased my travel time by probably 100% at least," said Matt Mackeprang of West Chester. "If it normally takes me 20 minutes to get from point A to point B I'm looking at two hours."

DelDOT diverted traffic and the Delaware Department of Natural Resources and Environmental Control (DNREC) focused on the clean-up. Ultimately, DNREC opted late Tuesday afternoon to burn the organic peroxide off the roadway. DNREC issued a shelter-in-place order for the half-mile radius around the spill while the burn continued for about 45 minutes.

Arkema Inc. and former plant manager Leslie Comardelle were found not guilty in a directed verdict by Judge Belinda Hill on Thursday. On Wednesday, she dismissed an endangerment charge against Arkema's U.S. chief executive officer, Richard Rowe, saying prosecutors had not presented enough evidence to support the charge.

Prosecutors last month separately dropped a criminal assault charge against Arkema and former logistics executive Michael Keough over smoke inhalation injuries suffered by public safety workers assigned to guard the plant.

The judge in the case has not yet ruled on the motion.

"A prosecutor felt that there was enough evidence for a criminal charge, but he could not prove that charge beyond a reasonable doubt," said Dane Schiller, a spokesman for the Harris County District Attorney's office, explaining the motion.

The Fox Crossing Fire Department responded to a business at 2100 Holly Road for a report that a 300-gallon tote of organic peroxide had been punctured and was leaking onto the loading dock floor. R and L Carriers, a freight shipping and logistics company, is listed at that address, according to the company's website. 

Firefighters helped get employees out of the building and to a safe location. Several employees were checked for chemical exposure at the scene, according to the Fox Crossing Fire Department.

The Harris County District Attorney’s Office charged the company and its then-vice president for logistics Michael Keough last April, for allegedly telling county emergency officials that Arkema had real-time monitoring data of all of the trailers holding hazardous organic peroxides at its Crosby plant when that wasn’t the case. About nine months earlier, the state had brought another felony reckless charge against the company, its CEO Richard Rowe and its plant manager Leslie Comardelle for releasing toxic chemicals.

At the center of the state’s case is why Arkema didn’t move its organic peroxides, which are used to make plastics and must often be kept at temperatures at or near freezing to prevent the chemicals from self-combusting, as it prepared for the landfall of Hurricane Harvey. Also, the DA’s office says that the company misrepresented how closely it could monitor the temperature all of the hazardous material remotely to be able to warn emergency responders to safely evacuate.

URI’s Beaupre Center for Chemical and Forensic Sciences was evacuated around 8:15 a.m. Wednesday after an unvented container was discovered containing a potentially volatile hydrogen sulfide and peroxide.

The Kingston Fire Department, URI Office of Emergency Management, including its Environmental Health and Safety unit, and URI Police responded to the scene, followed by regional HazMat units from nearby towns.

The container is being safely vented before being disposed of.

The building was cleared for re-entry around 10:00 a.m.

Lab workers at the Los Alamos National Laboratory destroyed an “unstable” bottle of hazardous waste hours after its discovery May 2.
The 3.5 ounce bottle of lithium borohydride in tetrahydrofuran was discovered in the SM40 Room at Tech Area 3, which is also known as the Physics Building. By 6:05 p.m. May 2, the waste was destroyed in a contained explosion, according to Enrique Torres, division leader of Triad National Security’s Environmental Protection and Compliance Division.

“The material was placed in a total containment vessel and wrapped in sheet explosives,” Torres said in a letter to John Kieling of NMED’s Hazardous Waste Bureau. “Sufficient quantity and configuration of explosives was utilized to ensure the complete consumption of material in question.”

The bottle of lithium borohydride was discovered during an inspection of the building. The bottle was determined to be 15 years old. It had crystallization around the cap, which prompted  lab workers destroyed the bottle that day.

“The crystallization prohibited opening of the bottle for testing and any attempts to sample the visible crystallization may have caused an unintentional initiation of the suspected peroxide crystals,” Torres also said in the same letter to Kieling. “This condition posed an imminent and substantial endangerment to human health and the environment.”

Emergency crews were called to the 800 block of Hoff just after 8 a.m. for a reported organic peroxide leak. St. Charles County officials said the chemical started to produce a gas, causing a concern.

More than 12 people were treated for possible respiratory issues at the scene but it is unknown where the leak started.

The leak was contained within 30 minutes and there is no current threat to the public at this time, officials say.

    <dc:creator>dchas</dc:creator><description><![CDATA[BRAINTREE, MA — A hazmat team will be at Braintree High School Wednesday getting rid of small quantities of hazardous waste.

School officials said benzoyl peroxide was found during a routine disposal of chemicals in the science department this fall. Benzoyl peroxide is a shock sensitive material and a common, regulated hazardous waste. Although the chemical is common in acne medication and other cosmetics, larger amounts can be explosive.

The state's Department of Environmental Protection gave the school an emergency license to stabilize and remove the chemical. Clean Harbors Environmental Services, a Norwell based waste management company, has been tasked with stabilizing the benzoyl peroxide.

After stabilization, the waste will be transported away from the school and properly disposed of at a hazardous waste management facility.

Four people were taken to Northside Hospital Cherokee with breathing difficulty.

The situation was contained and Univeter Road reopened about 7:15 p.m.

Hazardous materials personnel are evaluating how to clean up the leaking chemical, sheriff's office spokesman Jay Baker said just after 5:30 p.m. The spill occurred at approximately 2 p.m. as FedEx delivered a shipment of organic peroxide type F to the Pilgrim's Pride chicken plant at 654 Univeter Road.

The truck contains six, 300-gallon containers of the liquid, which when highly diluted is used for cleaning in the plant. Hazmat workers are concerned other containers may begin to leak as well and are evaluating how to attack the problem and effect the cleanup.

    <dc:creator>dchas</dc:creator><description><![CDATA[In the early hours of August 31, explosions erupted at the Arkema chemical plant in Crosby, Texas, where floodwaters from Hurricane Harvey had cut off the power supply to refrigerated containers containing organic peroxide. Residences in a 1.5-mile radius had been evacuated, and deputies manning barricades began falling ill in the middle of the road one by one. Medics were called, but no further warning was given as columns of black smoke filled the air.

Arkema knew the fires were coming -- organic peroxides burst into flames unless they are kept cool -- but company officials had insisted in a press conference prior to the explosions that the chemicals were not toxic or harmful to people, according to a lawsuit filed in September by emergency workers injured at the scene.

The lawsuit describes the scene near the plant as "nothing less than chaos," with police officers doubled over vomiting and medics gasping for air on their way to assist them. At least 15 people were hospitalized. Arkema initially told authorities the victims had inhaled a "non-toxic irritant," but residues obtained from nearby residences tested positive for dangerous toxins, such as dioxins and heavy metals, according to a separate lawsuit filed by people living nearby.

What else is Arkema hiding? For answers to that question, the public is turning to the US Chemical Safety Board, where an investigation of the Arkema incident is ongoing. However, the federal agency has failed to implement a rule requiring chemical plant operators to report dangerous releases during accidents to its investigators. Congress mandated this provision back in 1990.

Had Arkema been required to report the looming chemical fires to the Chemical Safety Board, the government and emergency workers would have had more to go on than the "vague" disclosure offered by the company during the storm, according to Adam Carlesco, a staff attorney at Public Employees for Environmental Responsibility. The watchdog group filed a lawsuit on Thursday challenging the chemical board's inaction on the reporting rule. Other plaintiffs include the Memorial for Workplace Fatalities and two Gulf South environmental groups.

"America's sole industrial safety monitor is currently flying blind and placing the health of the public at risk," Carlesco said.

The Helena Daily World reported the explosion occurred Wednesday morning at United Initiators, which makes peroxide. The plant is located in an industrial park in Helena-West Helena, 3 miles from the Mississippi River.

Jon Cummins, the plant's vice president of manufacturing, said the chemical involved was benzoyl peroxide. Cummins said there were no chemical releases caused by the explosion. It wasn't immediately clear how serious the workers' injuries were.

WREG-TV of Memphis reported that no fire occurred.

The company stopped production to begin an investigation.

Germany-based United Initiators acquired its Helena-West Helena plant in 2012, according to the company's website.

“Harvey shows that companies can’t rely on past experience” when it comes to emergency planning, says CSB investigator Mark Wingard. “More severe weather events are possible, and we need to be thinking about what can happen and how to prepare for it. Companies need to test past assumptions.” 

Arkema manufactures and uses organic peroxides, which must be refrigerated for stability, at the Crosby site. The plant lost primary electricity and its generators as it flooded because of Harvey. As its cold-storage warehouses lost power, the company shifted the peroxides to nine refrigerator truck trailers. But soon, three trailers caught fire and eventually Arkema officials deliberately burned the remaining trailers.

The company considered neutralizing the peroxides, but it had 158,757 kg held in 15,000 individual containers, making such actions difficult, CSB investigator Wingard says.

CSB officials say floods, high winds, and hurricanes are becoming more frequent and industry and regulators must be prepared. CSB will look at the adequacy of assumptions in current emergency preparedness requirements as well as approaches of different U.S. federal agencies and those of other countries as it prepares its report on Arkema, Wingard says.

Located in Crosby, just outside of Houston, the Arkema Inc. facility was officially placed in the 100-year flood plain in 2007. However, the plant's emergency response plan, which was revised as recently as last year, contained minimal instruction for containing floodwaters.

A copy of the plan was obtained and reviewed by the Associated Press, only said "care shall be taken to be sure water is kept out" of buildings at the facility. Arkema kept a log of workers' efforts to make the plant safer, but there was no mention of moving its organic peroxides, used in a variety of products from plastics to paints.

In fact, the plant's flood emergency plan was just one paragraph long, according to an investigation performed by the Houston Chronicle, and the facility's backup generators were too low to the ground.

The company did not answer questions Wednesday about whether it discussed or tried relocating the chemicals.

"The facility was not prepared for such heavy rainfall," U.S. Chemical Safety Board Chairwoman Vanessa Allen Sutherland said during a news conference Wednesday. She mentioned relocating chemicals as a safety measure companies might use.

Vanessa Allen Sutherland, the Safety Board chairwoman, at one point stated that one of the key takeaways of the investigations thus far is simply, "They did planning and the question is, why wasn't it enough?”

The CSB released a highly produced animation illustrating exactly what took place when Harvey was at its peak, including how power was cut off by man and Mother Nature.  

The animation stated at one point, "Without power those warehouses were at risk of not staying cool enough to prevent decomposition of the peroxide."

This was pivotal since the decomposition could result in explosions. In the days that followed, residents within a mile and a half of the plant were evacuated, and more than a dozen sheriff's deputies were hospitalized responding to the explosions.

The fires and explosions at the Arkema peroxide plant in Crosby, which sickened first responders and terrified the surrounding community, illustrate what happens when industry is allowed to operate for decades without effective safety oversight.

Arkema capitalized on the weakness of the current regulatory system, even as the company lobbied the Environmental Protection Agency to keep those rules unchanged. The current rules largely omit coverage for reactive chemicals like Arkema's peroxides - just as those rules still do not cover the fertilizer ammonium nitrate that detonated and leveled much of West, Texas in April 2013, killing 15 people.

Arkema filed its federally required risk management plans but did far too little to reduce the actual danger from an obvious and acknowledged hazard - the loss of power to the refrigeration systems needed to keep its highly reactive organic peroxides from decomposing explosively.

This highlights a fatal design weakness in current rules. Companies can substantively comply with the rules by generating paperwork showing that they have analyzed their chemical risks and possible worst-case scenarios. But these companies are under no obligation to actually reduce these risks to the lowest practicable levels by applying safer technologies and utilizing the most up-to-date industry best practices.

The findings were disclosed Monday afternoon in a letter the residents' lawyers mailed to the chief executive of the plant's owner, France-based Arkema Inc., and several regulatory agencies, giving notice that they planned to sue.

The letter accused Arkema of violating multiple environmental protection laws because of unauthorized chemical spills and releases that happened when the company failed to properly store and contain its organic peroxides before and after six feet of water overwhelmed the facility.

Arkema representatives could not immediately be reached Monday evening. The company has refused to say which chemicals were released during the fire and has repeatedly declined to discuss how large its inventory quantities were prior to the incident. It provided only the names of its chemicals on its website last month after days of public pressure.

The Houston Chronicle reported Friday that the Harris County district attorney's office confirmed it had opened a criminal investigation into Arkema's actions before the explosions that sent fire and plumes of black smoke shooting into the air. Authorities in late August evacuated a 1.5-mile radius around the plant in Crosby, about 25 miles (40 kilometers) northeast of Houston.

First responders are suing the company, claiming they were left vomiting and gasping for breath because of the fumes from the explosion of volatile organic peroxides. Their lawsuit says Arkema did not have proper procedures to keep the chemicals cooled in case of a flood or emergency.

Harris County commissioners have also approved filing a lawsuit against the company.

EPA Administrator Scott Pruitt told the Washington Examiner on Monday he authorized a request for information to Arkema under Section 114 of the Clean Air Act about whether the company complied with a risk management plan filed with the federal government.

Depending on the information obtained by the EPA, the agency can issue administrative, civil or criminal action against Arkema, a multinational company based in France.

"I issued something called a 114 letter, which was a communication to Arkema to say you need to advise and inform me about your risk management plan [RMP] and whether you followed the specifics of the RMP and were you adequately prepared," Pruitt told the Washington Examiner in an interview. "There is some question about whether the RMP that was in place there was actually complied with."

The EPA, in a letter issued Sept. 7 and obtained by Washington Examiner, has ordered Arkema to respond within 10 days to questions about the handling of chemicals known as organic peroxides, which are combustible if not kept refrigerated. The EPA also wants to know the amount of chemical materials kept at the plant, and the measures taken in advance to guard against flooding and loss of electricity.

The Sept. 7 suit alleges that Arkema was negligent in failing to properly prepare for a power outage, which created dangerous conditions for storing flammable organic peroxides. Lack of refrigeration at the site, which was inundated with nearly 2 meters of water from Tropical Storm Harvey, caused the chemicals to degrade and explode. The suit alleges the first responders were exposed to unspecified “toxic fumes” early on Thursday, Aug. 31.
According to the Houston Chronicle, 15 Harris County deputies and eight emergency medical services personnel were hospitalized due to exposure to fumes. One of the plaintiffs in the case, Christy Graves, is the director of Harris County Emergency Services District 5, based in Crosby.
The first responders were guarding a 2.4 km perimeter around the Crosby site by taking up a position on a nearby road to prevent traffic from entering an evacuation zone set up by local safety officials, according to Kimberley M. Spurlock, the attorney representing the seven responders.
At roughly 2 AM, Spurlock tells C&EN, the responders became overwhelmed by fumes. They used emergency radios to ask about the status of the chemical plant but were told that no incident was reported. According to the suit, “one by one, the police officers and first responders began to fall ill in the middle of the road.” Medical personnel who responded to the scene, the suit continues, also became overwhelmed and began to vomit and gasp for air.

Issued by the Obama Administration in January and blocked by the Trump Environmental Protection Agency in March, that regulation would require chemical safety improvements at industrial plants.
If fully implemented, the new regulation would require greater public disclosure from companies that use large amounts of certain chemicals, notes Yvette Arellano, spokesperson with TEJAS, the Texas Environmental Justice Advocacy Services, a community-based nonprofit. The rule would also call for firms to conduct root-cause analysis after an accident, she adds.
When it postponed the regulation, EPA reopened discussions of what the final rule should require. This reexamination won’t be complete until February 2019, EPA Administrator Scott Pruitt has said.
Pruitt attributed the delay to opposition to the rule from companies. Among them is Arkema, which filed comments against the regulation. Several states, labor unions, and community groups are challenging EPA’s move in court. Chemical companies and some other states, on the other hand, are backing Pruitt.
Advocates say the rule, if implemented, would have a made a difference when Tropical Storm Harvey brought heavy rains that flooded the Crosby plant and killed backup electricity generators used to maintain refrigeration of some 225 metric tons of organic peroxides at the facility. The reactive peroxides decompose with heat, and at Arkema, they began to burn. Government officials arranged for the remaining stock of these chemicals to be set on fire.

Amid last week's flooding, everybody living within a 1.5 mile radius of Crosby's Arkema chemical plant was forced to evacuate just before at least two tons of volatile organic peroxides exploded and caught fire. The whole world watched on television as flames shot up from the floodwaters that inundated Crosby. That calamity was bad enough, but company officials ignited their own firestorm by cloaking their plant's inventory in secrecy. And the blame for keeping the public in the dark is shared by our state's top elected leader.

Arkema - whose poor public relations last week became a case study in botched media crisis management - initially refused to tell the people of Crosby what chemicals were stored in their community. Days after the first explosions, Arkema finally released a list of its inventory, but it still wouldn't reveal important details such as how much was stored on its site or where the chemicals were located. Then officials deliberately set fire to some containers of chemicals without any advance notice, detonating more explosions that frightened anxious neighbors living outside the evacuation zone.

"You don't know what's in there," Crosby resident John Rull told a Chronicle reporter. "You don't know what's in the air. Their time for keeping secrets is up."

    <dc:creator>dchas</dc:creator><description><![CDATA[Residents living near the flooded Arkema chemical plant in Crosby, Texas, were allowed to return to their homes early Monday, a day after federal, state, and local safety officials coordinated with Arkema to ignite flammable containers of organic peroxides at the site that posed explosion and fire dangers.
Evacuation orders were first issued by local officials on Tuesday, Aug. 29 in response to danger posed by the organic peroxides after Tropical Storm Harvey flooded the plant with close to two meters of water.
The facility, 43 km northeast of Houston, lost primary and backup power systems used to keep the peroxides cool. As the chemicals warmed up they began to degrade. Three trailers of stored chemicals exploded on Aug. 31 and Sept. 1, and six remaining ones were expected to explode as well.
Arkema did not specify exactly how officials ignited the six containers, but after being ignited, they burned themselves out.
Later on Monday, Rich Rowe, the CEO of Arkema’s U.S. operations, apologized to the facility’s residential neighbors. “I hope that they can know how profoundly sorry we are for the impact we had on their lives. And how committed we are to working with them to bring their lives back to normal,” Rowe said at a press conference.
Residents were not given advance warning that the six trailers would be purposely set on fire. But Rich Rennard, Arkema U.S.’s president, said that aerial surveillance showed that peroxides were leaking from the containers. “At that point we made the decision that, in order to maintain the safety and security of the site and the area for the residents, a more aggressive approach was necessary.”

The plant had 19.5 tons of organic peroxides of various strengths, all of them requiring refrigeration to prevent ignition.

But the power went out, and then the floodwaters came and knocked out the plant’s generators. A liquid nitrogen system faltered. In a last-ditch move, the workers transferred the chemicals to nine huge refrigerated trucks, each with its own generator, and moved the vehicles to a remote section of the plant.

That was doomed to fail, too. Six feet of water swamped the trucks, and the final 11 workers gave up. At 2 a.m. Tuesday, they called for a water evacuation and left the plant to its fate.

Early Thursday, two loud pops signaled an explosive combustion in one of the trucks, and a black plume of smoke spread from the plant, sending 15 police officers and paramedics to the hospital. All eight remaining vehicles are now likely to burn, said Robert W. Royall Jr., assistant chief of emergency operations for the Harris County Fire Marshal’s Office.

As flood water recedes, residents, state and federal officials, and refinery and chemical plant managers are assessing the damage left in Harvey’s wake.
Dozens of major manufacturing facilities were taken off-line before and during the storm. Experts say it is too early to clearly assess the amount of damage sustained by industry in the region and forecast it will take several weeks to restart the plants.
“The combination of Harvey’s path, duration, and rainfall total is wreaking havoc with the supply side of the U.S. chemicals industry on an unprecedented scale,” says Kevin W. McCarthy, analyst at Vertical Research Partners, an equities research firm.
One chemical plant that has sustained significant damage is Arkema’s polymer additives site in Crosby, Texas, 43 km northeast of Houston. On Aug. 31, storage facilities holding highly flammable organic peroxides began to explode, sending black smoke into the air. The facility and the rural neighborhood around it had been evacuated and no serious injuries were reported. However, several first responders were treated for symptoms of respiratory irritation. Two senators have asked the U.S. Chemical Safety Board to investigate the circumstances that led up to the explosions at the plant.
The U.S. Environmental Protection Agency reviewed data from aircraft shortly after the explosion at the plant. In an Aug. 31 statement, EPA Administrator Scott Pruitt said, “This information indicates that there are no concentrations of concern for toxic materials reported at this time.”

The incidents, which also included a shelter-in-place Monday evening in La Porte from a pipeline leak, reveal how dangerous it can be when a storm of Harvey's magnitude collides with the nation's petrochemical capital. Even the controlled, Harvey-related shutdowns of refineries and plants are resulting in the release of millions of pounds of carbon monoxide and other chemicals into the region's atmosphere - primarily through a process called flaring.
"This is an unprecedented storm, and we have taken every effort to minimize emissions and safely shut down equipment," said Exxon spokeswoman Charlotte Huffaker. "Flaring is an environmentally approved measure to safely burn hydrocarbons that cannot be processed during a unit shutdown. This is necessary so personnel can safely work on the equipment."
Several companies, including Exxon, have reported problems with storage tanks in recent days. In Exxon's case, a roof collapse triggered the release of more than 12,000 pounds of potentially toxic chemical compounds, according to a filing with the Texas Commission on Environmental Quality.
However, Exxon Mobil emphasized the collapse was partial and quickly brought under control. "The tank is now stable, and there are no containment issues," Huffaker added.
Northeast of Houston in Crosby, officials at an Arkema chemical plant are bracing themselves after multiple systems failed due to flooding.
Late Monday night, the facility lost power from both its primary supply and its backup generators. Employees moved highly volatile organic peroxides into back-up containers to keep them cool. If that class of chemical gets too hot, it can cause fires or explosions.
"At this time, while we do not believe there is any imminent danger, the potential for a chemical reaction leading to a fire and/or explosion within the site confines is real," Arkema spokeswoman Janet Smith said on Tuesday.

Salina Fire Marshal Roger Williams said a 15-foot by 20-foot tank that was about one-quarter full of benzoyl peroxide began to heat up, creating a white plume that vented out of the smoke stack at Research Products, a division of McShares, 1835 E. North.

“The community’s safe,” Williams said. “All the necessary evacuations have taken place. We have the product contained. We’re still assessing the situation, and we’re working on remediation of the product.”

Williams said the incident occurred during normal operations at the Salina firm, which makes food additives and mixes of vitamins and minerals, shipping them worldwide to be added to wheat, rice and corn flours and cereals. The Occupational Safety and Health Administration doesn’t inspect most facilities; when it does, it’s often due to an accident or complaint. The Environmental Protection Agency ignores entire classes of hazardous chemicals when looking at which sites to scrutinize. Texas and other states have made accessing information about chemical stockpiles more difficult. Local emergency planning groups are in many cases unfunded and, in the most extreme instances, staff lists include people who have died.]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[Discussion: In 2013, six bottles of Tetrahydrofuran (THF), 100 mL per bottle, were purchased and placed in a flammable storage cabinet in accordance with Y73-939 Hazardous Chemical Storage. THF is both a "time sensitive chemical" and a "peroxide former" and is required to be tested annually for the presence of peroxides. The formation of peroxides can cause the chemical/material to become shock sensitive. Initial peroxide testing was performed in August of 2013 on three opened bottles (only opened bottles are required to be tested). In September 2015, the bottles were being considered for excess and the excess material personnel and materials manager noted that the bottles were not in-date for annual testing. A cursory visual inspection indicated there was no evidence of peroxide formation on the external surface of the bottles. Facility Management was informed of the missed annual testing and access to the lab where the storage cabinet was located was administratively controlled with a "Do Not Enter" sign. Facility Management conducted a Hazard Analysis for the testing of THF bottles and an Operational Safety Board (OSB) review was conducted to evaluate the work scope and hazard controls. With approval from the OSB, the THF bottles were removed from the flammable storage cabinet, carefully inspected, placed in an adjacent hood and tested. Testing demonstrated the absence of peroxides. New labels with current inspection dates were applied to the bottles and the lab/area was returned to normal operations.

Analysis: Required continual annual testing was not performed to verity the condition of the bottles and observe for signs of peroxide formation. Although the THF bottles were listed in the facilitys inventory of hazardous materials (Comprehensive Tracking System or CTS), and were barcode scanned during periodic inventories, personnel performing the inventory did not recognize the items were on the "time sensitive" list. The lack of annual inspection was recognized when the bottles were being evaluated for excess material disposition.]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[CAIRO, Ga. -- On Friday October 2, 2015 at approximately 8:30 a.m. the City of Cairo Fire Department responded to a call received by landline for a strong chemical smell in front of the high school.

Personnel responded to the area and determined that the odor was coming from Big Bend Agri-Services located North East of the school. Units responded to the scene and city police were called to evacuate the High School and the housing development for 2 blocks around the area.

Fortunately, students were not at school as it was a teacher workday.

Contact was made to employees of Big Bend and they advised they did have a spill inside the warehouse. The warehouse was evacuated and power was shut down for the building.

Grady County Fire rescue responded with 3 personnel and an air trailer to support operations as well as standing by to respond to any other incidents that may have occurred.

A team of fire department hazardous materials technicians made entry and determined the product spilled was liquid organic peroxide. The team estimated that a spill of roughly 75-100 gallons had been spilled when an automatic filling machine in the warehouse malfunctioned.

Fire personnel coordinated with officials from Big Bend and their contacts with OSHA to determine the best course of action. The decision was made to attempt to flush the chemical out of the warehouse with a large volume of water and neutralize it with soda ash supplied by big bend. A berm was constructed by city utilities to contain the runoff until it could be neutralized and made safe by approved personnel.

Approximately 5,000 gallons of water was used to dilute the product and flush it out of the warehouse. Approximately 24 bags of soda ash were then distributed through the warehouse to neutralize the chemical and make it safe for the cleanup to proceed.

Power was restored to the building at this point and the warehouse was ventilated to remove the trapped fumes. People were allowed to return to their homes at approximately 1 p.m. The fire department turned the scene over to Big Bend officials at approximately 1:45 p.m. and the fire department returned to service.]]></description>

Firefighters were called to the chemical leak about 11:40 p.m. and found that the 30 or so employees working in the facility in the 6200 block of South 228th Street had left the building and were waiting in the parking lot.

The driver of the forklift was evaluated at the scene.

Crews determined the peroxide posed no danger and found that most of the spill was confined to the trailer in which it was stored.

Employees re-entered the building and a company that specialized in chemical cleanup was called to handle the leak.]]></description>

Crews were dispatched to Insituform at 1:55 p.m. after a possible fire was reported. Firefighters arrived to find light smoke and a hazardous materials spill. Workers at the plant told firefighters that a malfunction during a routine mixing caused two chemicals to ignite.

The fire was quickly put out, according to the Indianapolis Fire Department. Ten employees were nearby when the malfunction occurred, but no one was injured.

One pound of a styrene and peroxide mixture spilled.

“The mixing machine is a static 15 gallon round aluminum bowl that is approximately 3 feet tall and sits on a stand approx. 3 feet off the ground,” IFD public information officer Rita Reith explained in a media release. “The product when mixed, flows out of the bowl and into tubes. It is unclear what caused the ignition to occur. At the time it contained 1 lb of mixed product.”]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[When Huntsville truck driver James Moore pulled his rig and the 40-foot container he was pulling into the lot at Container Solutions in Athens Monday afternoon, the day seemed pretty routine.
Then he got a surprise.
“When I went back and opened up the container doors, the smell about knocked me down,” he said. “It smelled like strong vinegar, and I could see something on the floor that it was bubbling up.”
Both Moore and employees at Container Solutions attempted to go into the container but the fumes drove them back out, said Moore, a driver for Volunteer Express trucking company. Athens Fire Department was called to the scene.
Emergency radio traffic at about 1:15 p.m. indicated a possible synthetic chemical spill that was burning a hole through the floor of its container.
In fear for his tractor as well as his life, Moore said he “dropped the container,” meaning he unhooked the tractor from the box it was hauling to move it to safety.
When firefighters arrived and were able to determine what was inside a 5-gallon container that had leaked onto the container floor, tensions eased. The chemical was not some metal-munching acid but, rather, a type of organic peroxide used in the cleaning of breweries.
The chemical was headed to a brewery in Madison but no one was there when Moore arrived, so he continued on his route. At some point during his journey, the 5-gallon container of the organic peroxide spilled, triggering the smell and the bubbling, which some described as smoke.]]></description>

The “clear liquid” that made a dozen people feel nauseated was determined to be organic peroxide, Monterey Fire Chief Gaundenz Panholzer said. The chemical spilled on an Apple Store package being delivered to Del Monte Center from Apple headquarters, Panholzer said.

Panholzer said the package was contaminated at a FedEx distribution center and was then put on a FedEx truck. FedEx spokeswoman Connie Avery said an event like this is “very unusual.”

“Our immediate concern is for the well-being of those involved,” FedEx said in a statement. “We are cooperating with authorities investigating the cause of the incident.”]]></description>

According to Austin-Travis County EMS, medics received reports of people possibly exposed to sulfuric acid and peroxide at the Samsung building at 12100 Samsung Blvd.

A Samsung spokesperson said two employees of Gruene Environmental, a company contracted by Samsung, are being treated for exposure to a chemical mix believed to contain sulfuric acid at University Medical Center Brackenridge. Their injuries are not believed to be serious.

Samsung spokesperson Catherine Q. Morse said between 10:30 and 10:45 a.m. Thursday, the contractor's truck was trying to hook up to Samsung's tank to deliver the chemicals when there was a malfunction in the line, causing pressure to build up and the chemical to spill.]]></description>

Next week, the Ventura County Board of Supervisors will be asked to ratify the action, a move that would allow the county to seek reimbursement for its costs from state disaster relief funds.

Close to 50 people were treated at local hospitals after a vacuum truck exploded about 3:45 a.m. Nov. 18 at Santa Clara Waste Water Co., leaking chemicals and starting fires at 815 Mission Rock Road.

About 1,000 gallons of a chemical mixture that contained sulfuric acid and an organic peroxide spilled and burned after the rear of the truck exploded. When it dried, the substance spontaneously ignited, sparking small fires. A plume of chemical smoke drifted over the area.

In the days after the explosion, an area within a half-mile of the site was evacuated. The evacuation was lifted Sunday.

Only a handful of smoldering hot spots remained at the site in an agricultural area near Santa Paula, northwest of Los Angeles, according to Ventura County fire Capt. Mike Lindbery.

A vacuum truck exploded early Tuesday at Santa Clara Waste Water Co., spreading about 1,200 gallons of a chemical mixture that contained sulfuric acid and an organic peroxide, fire officials said.

The resulting substance is highly toxic and combustible, Lindbery said.

"We need to determine exactly what this material is," he said. "How do we neutralize it, and how do we clean it up?"

A private contractor will be in charge of the cleanup, under the supervision of county health officials and the federal Environmental Protection Agency.

No burn injuries were reported, but two drivers on the vacuum truck, three firefighters, hospital medical staff and a few nearby residents were washed down or treated for complaints such as breathing problems, red eyes and skin rashes, said Lori Ross, a spokeswoman for the Ventura County Fire Department.

Officials said 37 people were treated at hospitals, including one from the initial blast who had non-life-threatening trauma injuries.

At about 3:45 a.m., two workers were removing 1,200 gallons of a chemical, possibly an organic peroxide, with a vacuum truck at Santa Clara Waste Water Co. in the 800 block of Mission Rock Road when it suddenly exploded, fire officials said.

The chemical, when it hit the air, crystallized and became combustible, fire spokeswoman Lori Ross said.

“It can spontaneously ignite, and that’s what it did,” she said.

The explosion caused the chemical to shoot out at least 300 feet, creating smaller flare-ups, said Capt. Mike Lindbery of the Ventura County Fire Department.

Authorities ordered all businesses and agricultural ranches within a mile of the spill to evacuate because the risk of more explosions was high, fire officials said. A school also was closed.

Firefighters pulled back because their shoes were sparking explosions as they walked on the dry chemical, Lindbery said.

Because firefighters were not sure what type of chemical was spilled, they were allowing the fiery explosion to burn out without water. Meanwhile, they're keeping their distance.

The truck's occupant was taken to an hospital, as were three firefighters who experienced problems after exposure to the chemical, fire officials said. A fifth person drove himself to the hospital.

And mixing piranha begs the question: Add the acid to the peroxide, or the other way around? Everyone hopefully learned in chemistry labs to “never cover an acid”–that is, when diluting, always add acid to water, not the other way around. For piranha, however, best practice is to add the peroxide to the acid.

    <dc:creator>dchas</dc:creator><description><![CDATA[An accident Monday morning at a Dalton chemical plant injured one worker and caused officials to briefly close streets around the plant.

The accident happened around 6 a.m. at MFG Chemical on Kimberly Park Drive as workers were mixing chemicals in a reactor.

“We don’t know exactly what happened but the lid did not shut completely or did not shut properly,” said Dalton Fire Chief Bruce Satterfield.

Chemical fumes escaped from the reactor, overcoming one worker.

“It’s our understanding he was right on top of the reactor,” Satterfield said.

The man was taken by ambulance to WellStar Cobb Hospital in Austell. Satterfield did not know the extent of the man’s injuries.

Satterfield said four chemicals were being mixed: maleic anhydride molten low acid grade, alfa olefin 20-24, di-tert-butyl peroxide and aromatic 100 fluid. Officials said the mixture was not poisonous but an irritant.

Satterfield said the chemical reaction did not last long.

Dalton Police and Dalton Fire responded to a chemical reaction that happened about 6:00am at the MFG Chemical plant on Kimberly Park Drive.

Originally the incident was dispatched as an explosion, but as more information was gathered by the Dalton Police and Fire Departments, it was re-classified as a 'reaction."

According to Dalton Fire Department, workers were dumping chemicals into a reactor. One worker did not close lid fast enough, and the ensuing reaction injured two people. The chemical reaction was from Malaic Anhydride Long Chain Alcohol.

"Organic peroxide was the last chemical to be put in," explained Fire Chief Bruce Satterfield. "It's stated that the worker did not get the lid closed fast enough. When the peroxide is being put in, it's going to react to the other chemicals. But at that point, I don't think he got it closed soon enough therefore the reaction sort of came out and unfortunately his head and face was still near the reactor."

The high explosive acetone peroxide was found on campus after being used in a science class a few weeks ago. The chemical didn’t pose an immediate threat, Dallas Fire-Rescue Lt. Joel Lavender said, but officials evacuated the school as a precaution. The school currently hosts various summer camp programs.

Members of the Dallas bomb squad detonated the chemical in an empty field nearby, although according to Lavender, they didn’t need to blow it up.

    <dc:creator>dchas</dc:creator><description><![CDATA[COLUMBUS — Columbus Fire Department’s hazardous materials team responded to a chemical spill Monday afternoon at a local manufacturing plant.

According to Fire Chief Dean Hefti, the hazmat team was activated around 4:30 p.m. Monday after a chemical spill was reported at FLEXcon, 2021 E. 23rd St.

Hefti said organic hydrogen peroxide was leaking from a semitrailer parked at the plant.

The hazmat team took measures to prevent the chemical from flowing into the stormwater system and contain the fumes, according to Hefti, who estimates about 140 gallons spilled from a container that was being delivered to a different company.

The spill did not cause any health problems for FLEXcon employees or hazmat team members, he said.

The release happened at Catalyst Refiners, but the leak was reported by Bayer CropScience, a neighboring facility.

Kanawha County Commission President Kent Carper said county emergency officials weren't notified of the leak until roughly two hours after it happened, which is unacceptable.

"I spoke to the plant manager and he told me how busy they were they didn't have time," said Carper. "That's a violation in my judgment of county policy and I believe it's also a violation of state law."

According to a release from the Kanawha County Emergency Management, the chemicals involved were Hydrogen Peroxide and Nitric Acid. It was also reported that approximately 950 pounds of Nitric Acid were released in the incident.

However, officials with Catalyst Refiners claim that
only 50 gallons were released and it was contained inside the plant.

Carper said regardless of the amount released, the company did not make the right decision in this case.

"The failure to follow state law, the failure to follow county policy and not to mention commonsense, I think that poses unreasonable risk to the public," he said.

No injuries were reported in the incident.

Carper plans to meet with company officials on Wednesday to further discuss the incident.

"The thing here to do is to see to it that the rules are obeyed," Carper said. "People have individual responsibility and so do the companies."

Carper also didn't rule out a possible meeting in the near future involving all the chemical companies in the Kanawha Valley.]]></description>

As of 10 p.m., a team was being assembled to go inside the building where the chemical was leaking and take pictures before officials figured out their next move of how to dispose of the dangerous items.

The incident began just after 9:30 a.m., when Salt Lake City fire crews were called to a small fire at Quality Distribution Inc., 421 N. John Glenn Road (6070 West). The fire was extinguished quickly, but as an investigator was going through the building a few hours later, he found that Trigonox was leaking.

By 6 p.m., a full assignment was recalled to the warehouse as well as additional help from other agencies including the U.S. Army and the Salt Lake International Airport.

Salt Lake City fire spokesman Jasen Asay said Trigonox is an organic peroxide that can become unstable at 68 degrees, and when the temperature reaches 77 degree the process cannot be reversed at all and the chemical cannot be stabilized. Because of the earlier fire, the refrigeration in the building was not working, he said. Asay said by Sunday night, the temperature of the chemical had surpassed 77 degrees.

For the chemical to explode, it needs to be triggered by a "shock," Asay said. That could be just static electricity or even a piece of metal falling to the floor and creating a small spark.

Approximately 2,400 gallons of Trigonox are believed to be stored in the building in five gallon drums. Asay said five of those drums were believed to be leaking Sunday.]]></description>
