    <dc:creator>dchas</dc:creator><description><![CDATA[An explosion at a Seqens chemical plant in Newburyport, Massachusetts, early on Thursday, May 4, killed one worker. The cause of the explosion has not been determined.
The incident is the third serious accident at the plant, which manufactures pharmaceutical chemicals under contract, since 2020 when an explosion damaged a building. A chemical fire occurred at the site in 2021.
The body of Jack O’Keefe, an employee at the company for more than 25 years according to a local news report, was removed from the site late Thursday afternoon. Four other workers on site when the explosion occurred were hospitalized for precautionary reasons, but were uninjured, according to Barry Salt, Newburyport deputy fire chief.
“They had just wrapped up a typical manufacturing procedure that they’d done numerous times daily,” Salt says. “Obviously there was some sort of accident to cause the explosion, but what that was or how that happened is still in the beginning stages of investigation.”
The plant manufactures both active ingredients and intermediates of pharmaceutical chemicals. Chemicals on site at the time of the accident, according to Salt, included acetone, isopropyl alcohol, and methanol.
Salt says air quality has been monitored by the Massachusetts Department of Fire Services beginning 1.5 h after the explosion. Local residents have been informed that there is no risk in the area.
The area of the plant damaged in the explosion has been in operation for more than 20 years. The facility was built in the 1980s. Novacap, a French firm now called Seqens, became the site’s owner when it bought PCI Synthesis in 2018.

One of the students was hospitalized for 2 weeks. The incident involved a classroom demonstration using methanol, according to the US Chemical Safety and Hazard Investigation Board (CSB), which is again promoting its classroom safety guidance.
The fire was far from the first caused by methanol in school science classes. Typically, such fires stem from demonstrations that involve burning a flammable liquid, usually methanol, according to the CSB. In some cases, after a demonstration has started, methanol from bulk containers is poured directly onto flames, then a flashback into the container ignites the rest of the fuel. The resulting fire can injure people as far as 4.5 m away.
When three serious accidents occurred over 8 weeks in 2014, the CSB issued a safety bulletin.
The bulletin provides four key lessons: do not use bulk containers of flammable chemicals when small quantities are sufficient; implement strict safety controls when handling hazardous chemicals; conduct a comprehensive hazard review of demonstration protocols; and provide a safety barrier between the demonstration and audience.
While demonstrations involving flammable chemicals can be important teaching tools, “they must be done safely,” CSB interim head Steve Owens says in a statement. “We urge school administrators and teachers to review and follow the CSB’s safety lessons for these kinds of demonstrations so that no one is harmed by these preventable accidents.”

Chief Dennis Hale said a bulk-sized container of methanol was used, and there was no shield in place at the time of the demonstration, which both go against the CSB guidance.

Superintendent Dr. Kari Weston said at a press conference on Wednesday that students told investigators personal protective equipment was not used during the demonstration, which is also not in line with CSB guidance.

Weston said it is not standard protocol not to use protective equipment.

She also said Dinwiddie follows recommendations from the Virginia Department of Education for safety during labs.

She said all educators and students go through annual training, and the teachers are supposed to follow a self-assessment checklist prior to demonstrations.

However, she said they do not need to get the demonstrations approved by an administrator.

"Currently there is no formal process where they would have to come before administration or central office to have that be approved. That is something we will be looking at going forward," Weston said.

According to Lehigh County Coroner Daniel A. Buglio, Jeremy J. Lanzo, 39, was pronounced dead at 2:13 p.m. on Tuesday at Lehigh Valley Hospital following a chemical explosion at approximately 5 a.m. Tuesday at the plant.

An autopsy, which was completed Thursday, revealed the cause of death to be chemical and thermal burns, Buglio explained. He added that Lanzo’s death was ruled an accident.

In addition to the coroner’s office, the death is being investigated by OSHA, Buglio noted.

Attempts to reach Eureka Resources for comment on the explosion were unsuccessful Thursday.

It happened at the Avantor building on Inducon Drive East, off of Lockport Road. Shortly before 2 a.m., a caller told first responders that approximately 400 gallons of methanol, a toxic and highly flammable chemical, was spilled.

Other than the three people taken to Mount St. Mary’s Hospital, all other employees there were able to get out safely, and the spill was contained to the building. No one in the nearby area is in danger.

Around 5:30 P.M. on Wednesday, fire departments across Midland County, and mutual aid from Martin County, responded to a fire call at a business.

“They were mixing or off-loading some chemical and somehow, it ignited,” said Midland County Fire Marshal Justin Bunch. “We got two flammable liquids involved: toluene and methanol… What they were trying to mix them for, I don’t know. We’re still trying to investigate that.”

Thankfully, Bunch said no structures were damaged. Only the 55-barrel drums holding the chemicals were destroyed. They got hot and exploded, Bunch said.]]></description>

He, however, said no one is stated to be trapped inside the factory complex. “About 50 fire tenders, many among them summoned from Sonipat and neighbouring Haryana districts as well as from Delhi are trying to bring the flames under control,” he said.
The cause of the fire is yet to be ascertained. Details of casualties, if any, are awaited.

According to a report from the Defiance Fire Department, at approximately 5:57 p.m. Thursday, Engine 222 responded to 736 Summit St. for a report of a “white, foamy substance” in the river.

Upon arrival firefighters were met by a local resident who pointed them to a creek where the substance was draining from a pipe. The crew placed a boom in the water in attempt to stop the flow into the river.

After stopping the flow, firefighters walked along the bank to determine the distance of the spillage. It was determined at that time to dispatch the Hazmat team and the EMA director, Julie Rittenhouse. The engine cleared the scene and returned to service. The crew then returned with a utility truck.

The Hazmat team placed a boom in the creek where it meets the river and samplings were taken. It was discovered that the substance was coming from a methanol processing business at 815 Greenler St. An employee at the company stated that 20-50 gallons of “clean oil” had entered the drain at the facility. Neither Charlie Weber, Noble Township Fire Chief who oversees Hazmat in Defiance, nor Chief Bill Wilkins, of the Defiance Fire Department could positively identify the oil. Wilkins did say that the product is from the facility.]]></description>

The injured workers are being treated at a private hospital near the factory in Anekal taluk and are out of danger, the report said.

The Times of India quoted officials saying that the fire was triggered due to a technical snag with the boiler unit. PM Nagesh, District Fire Officer, told the Hindu that the vessel had chemicals like toluene, methanol, hydrochloric acid. He said that they could not ascertain the exact cause of the blast.

The Bengaluru rural police have registered a case against the factory owners and are investigating the case, mentioned a Hindustan Times report.

Residents in the area reportedly started a flash protest in front of the unit. Vinod, a resident who lives near the factory told Hindustan Times that this was not the first time such an incident occurred at the unit. The resident said that they had told the management of the chemical factory that residents were facing problems due to their output. The resident added that the area was covered with smoke after the blast which concerned them.

It was learnt that the incident occurred at about 10.15pm after two women from the village had trouble breathing and fainted due to the strong odour.

Following that, many other residents noticed the pungent chemical odour and immediately contacted the Fire and Rescue Department for assistance.

Johor Fire and Rescue Department’s Hazardous Materials Unit (Hazmat) chief Saiful Bahri Safar confirmed receiving a distress call at 10.22pm and said a team was immediately despatched to the location.

He said the first team arrived at Kampung Sri Aman within 15 minutes.

“Two female victims in their 20s and 40s were rushed to the Sultanah Aminah Hospital and Sultan Ismail Hospital by the department’s Emergency Medical Response Service team after they had difficulty breathing.

Saiful Bahri said the Hazmat team then detected the suspected source of fumes at the nearby river.

“The personnel isolated and used damping technique in the river after they found spillage of a solid liquid upstream,” he said.

A total of 24 firemen and five fire engines from the Kempas, Larkin and Johor Jaya fire stations have been dispatched to the village.

“At the time of the required report, sodium dichloroisocyanurate dihydrate was not used or processed at the facility,” reads a sheet of frequently asked questions about the facility and its use of the chemical leading up to last week’s explosion. “In fact, it would have been inappropriate to include CDB on the Tier II report since, at the time of the report, sodium dichloroisocyanurate dihydrate was not used at the facility.”

‘It was a massive explosion – it blew metal debris out onto the interstate and across the river … hundreds of feet away,’ Kent Carper, president of the local county commission, told a local radio programme the morning after the accident. He said the Optima facility makes a chlorine-based product to clean spas. ‘It was a dry bleach type of chemical, and somehow it interacted with methanol,’ Carper explained.

Carper said it appears that pipework leading down to the site’s river barge terminal blew up. An Optima statement suggests a 4500 litre metal dryer become over-pressurised during operation.

It has been since been confirmed that there was no associated chemical leak, and no danger to the local community. The state Occupational Safety and Health Administration is reportedly investigating the underlying cause, and the federal Chemical Safety and Hazard Investigation Board is expected to initiate its own probe shortly.

Three people were taken to Charleston Area Medical Center and the other person drove themselves to Montgomery General, according to Kanawha Commission President Kent Carper. The extent of their injuries was not known.

Carper said the incident involved chlorinated dry bleach and methanol, the latter of which was "most likely" the origin of the explosion and acted as an accelerant for the flames.

"It's a very bad explosion and working fire and we're ordering a shelter-in-place in that area," Carper said. "It is an evolving situation."

"Short circuit triggered the fire. As there was a huge stock of chemicals, including that of methanol, at the unit, the fire spread quickly. After being alerted, the personnel of fire brigades from Navi Mumbai, CIDCO, MIDC reached the spot," he said.

The fire brigade staff from Ambarnath also came to the site to help, the official added.

"However, during the firefighting operation Deshmukh inhaled fumes of a hazardous chemical, after which he complained of dizziness. He was then rushed to a private hospital in nearby Kamothe locality, where he died during treatment around 4.30 am," he said.

Many of the cosmetics we use today are made with ethanol, from facial astringents, to lotion. Since it is so effective at killing germs like bacteria, viruses, and fungi, ethanol or ethyl alcohol, it is found in many hand sanitizers, which are widely used when soap and water aren't immediately handy (via Chemical Safety Facts). 

Methanol, on the other hand, is primarily used as a solvent to make inks, resins, adhesives, and antifreeze (via Chemical Safety Facts). The other big difference: Methanol is poisonous. When it enters our body, it can cause nausea, difficulty breathing, and blindness (via USA Today). Unfortunately, some hand sanitizers, mostly from Mexico, may contain this very hazardous ingredient. Methanol's toxicity when it comes into contact with our bodies, either when it is absorbed by our skin or when it is ingested, is the reason the FDA has put hand sanitizers found to have been contaminated with this chemical on a "Do not use" list of dangerous products.

The fire extinguishing system in the pharma unit manufacturing pharmaceutical ingredients was immediately activated. The fire, which triggered a blast, was brought under control in about 30 minutes, said District Fire Officer K. Srikanth Reddy, who rushed to the spot.

The sudden increase in pressure while methanol was being loaded into the reactor caused the fire. “'The reactor has not suffered any damage,” the DFO said. The injured were rushed to a private hospital here. Later three of them in a serious condition were shifted to a hospital in Chennai for better treatment.

    <dc:creator>dchas</dc:creator><description><![CDATA[The Amarillo Fire Department said firefighters were called to the scene of a hazardous materials leak on Saturday morning that resulted in no injuries or environmental damage.

Per an AFD media release, at 8:14 a.m. firefighters were dispatched to the 7600 block of River Road regarding a leaking, 55-gallon barrel. Authorities said the first unit arrived at 8:21 a.m. and notified the department’s Hazardous Materials Team at 8:23 a.m.

A Level 2 HazMat was declared at 8:38 a.m., per the release, six AFD units responded and were able to identify the leaking material as methanol. Crews successfully contained the leak and placed the barrel in an over pack drum to prevent further release.

Per the media release, Industrial Waste personnel arrived and collected the barrel for disposal at 10:30 a.m.

According to the Routt County Sheriff’s Office, a vehicle crash resulted in a chemical spill on the highway between mileposts 140 and 141, close to the base of the pass south of Steamboat Springs. A west-bound semi with a box trailer crossed into the east-bound lane of traffic and rolled approximately 75 feet down the embankment landing on its roof against some trees, according to county officials.


The driver, who was later transported to UCHealth Yampa Valley Medical Center, identified the chemical as methanol, a type of alcohol that is mostly used to create fuel, solvents and antifreeze. It is a colorless liquid, flammable and poisonous for human consumption. The truck was carrying about 38,000 pounds of the substance.

Karen Green of Locust Gap on behalf of her daughter has sued the school district and chemistry teacher Tammy Michaels in Northumberland County court seeking in excess of $50,000 for negligence and infliction of emotional distress.

They were claims U.S. Middle District Judge Matthew W. Brann last April chose not to accept jurisdiction when he dismissed Green’s federal suit.

The daughter, identified as M.G., suffered second-degree burns in an Aug. 30, 2018, explosion that caused flaming liquid methanol to spill onto her leg and desk.

In dismissing the suit, Brann ruled Green failed to allege facts that infer the defendants were aware of the risks of the demonstration, noting Michaels had performed it for 23 years without incident.

Kutch district collector Praveena D K told TOI: “We are awaiting the for final report but according to preliminary assessment some welding work was being done on the upper surface of the tank which could have triggered the fire.” Usually, the maintenance work is done after the tank is emptied but it was not regular kind of maintenance work.

“The message this [verdict] should send is safety first,” says lawyer Ben Rubinowitz, who represented student Alonzo Yanes and his family. “I hope that no child is ever injured again” by this experiment.

Yanes was a 16-year-old sophomore at Beacon High School, a public school in New York City, when his chemistry teacher, Anna Poole, successfully performed the procedure. Then she attempted to restart it by adding more methanol from a 1 gal container, which ignited. Yanes was standing 75 cm away and sustained burns to 30% of his body, including disfiguring scars to his face, ears, neck, arms, and hands, Rubinowitz says. Another student was also injured in the fire and settled with the city.

Just a month before the incident, the Chemical Safety & Hazard Investigation Board warned about the hazards of the rainbow experiment, which has caused dozens of accidents and injuries through the years. Poole “took a shortcut, and you can’t take shortcuts when it comes to safety,” Rubinowitz says. Poole, who had taught chemistry for several years, now works for the city’s department of education advising other teachers.

Other students have won lawsuits after being injured in rainbow experiment accidents, though award amounts have been much lower. The jury awarded Yanes almost $60 million to address pain and suffering both since the accident and into the future. Rubinowitz says Yanes, now 21, is in college.

Nick Paolucci, press secretary for the New York City Law Department, says that the safety of students is the department of education’s top priority, and that the experiment has been banned as a result of the accident. “While we respect the jury’s verdict, we are exploring our legal options to reduce the award to an amount that is consistent with awards that have been upheld by the courts in similar cases.”

Embattled educator Anna Poole struggled to explain why her testimony about the Jan. 2014 accident contradicted the recollection of Alonzo Yanes, the student disfigured in her Beacon High School classroom, as well as that of his classmates, experts, police officers and firefighters.

Trembling and stuttering, Poole insisted she’d properly conducted the “rainbow experiment” involving mineral salts, methanol and fire, just as she’d done for previous students. She said Yanes could not have been sitting a mere two and a half feet away from the demonstration, as he had testified.

“In your opinion did you perform the demonstration appropriately?” city attorney Mark Mixson asked.

“Yes,” Poole, 36, replied.

But attorney Ben Rubinowitz confronted her with sworn testimony she gave in the case in 2017. During a deposition, Poole had said she had no memory of the moments prior to the near-deadly chemical reaction, or any recollection of other times she performed the demonstration.

“We were trying to find out what in the world happened on Jan. 2, 2014,” Rubinowitz said.

“I’d like to know that, too,” Poole replied.

She was in the fourth year of a doctoral program in chemistry. It was around 8 on a Friday night and Wangtrakuldee was alone, conducting an organic synthesis — combining chemicals such as ketones, aldehydes, and isocyanide with a carboxylic acid — for an anticancer drug. As she poured the mixture into another container, Wangtrakuldee spilled a large amount onto her lab coat, which soaked through to her jeans and onto her legs, the methanol in the concoction causing it to absorb into her skin quickly.

With no one else around, she quickly stripped off her clothing, but not before she sustained what felt like a bad sunburn for a week.


“I was really lucky because it could have been so much worse,” Wangtrakuldee said in a recent interview.

Alonzo Yanes, now 21, took the stand Tuesday in his family’s civil case against New York City and his former teacher regarding the January 2014 experiment at Beacon High School, the New York Post reports .

The lawsuit says the experiment, which involved mineral salts being set on fire and used methanol, was performed despite warnings sent to the city’s education department from the U.S. Chemical Safety Board that it was dangerous. The suit says the students were not adequately protected.

A lawyer for the defense told jurors in opening statements that the fire was an accident.

Yanes testified that when a giant fireball flared, he put his arms out in an instinctive attempt to protect himself from the flames coming in his direction.

U.S. Middle District Judge Matthew W. Brann on Wednesday gave Karen Green of Locust Gap the opportunity to file an amended complaint but questioned whether she can successfully allege any plausible claims.

The averments in the dismissed complaint appear to be more legal conclusions than factual allegations, the judge wrote.

Green failed to allege facts that infer the defendants were aware of the risks of the experiment, he said, noting chemistry teacher Tammy Michaels had performed it for 23 years without incident.

The complaint contains no facts that suggest Michaels directed the Aug. 30 explosion at the girl, identified as M.G., who was seated in the first row, Brann wrote.

The judge’s opinion gives this account of what occurred that caused M.G. to suffer second-degree burns:

Michaels poured methanol into a container and attempted to ignite it. Unsatisfied with the result, she added methanol and attempted again to light it with a match.

The explosion happened in the garage portion of the dealership.

An employee says that a mechanic was trying to open a 55-gallon drum containing some methanol. The chemical interacted with an open flame and exploded. The fire marshall said that the flame was not from a cigarette.

Two people were sent to the hospital with severe burns. One of them had second to third-degree burns.

“To prevent what happened here today, you cannot have an open flame near flammable gases. And that’s just a very common safety concern that people should have when they’re around the shop,” said Nicholasville Fire Marshal David Johns.

A fire that started immediately after the explosion has been extinguished.

We are told the methanol was used for windshield wiper fluid.

The student was not wearing eye protection at the time, having failed to re-don safety glasses upon reentering the lab after a break. The requirement to wear safety glasses in this lab is task-dependent - IF work involving certain materials is going to be performed, THEN specified PPE is required. It is left to the individual to be knowledgeable about the work controls, identify the need, and don safety glasses when appropriate. Some other labs, including the only other laboratory this student works in, have mandatory PPE requirements.

All personnel entering these labs are required to don the specified PPE upon entry regardless of the work location or task. Recommendation: Implement donning some level of PPE into the process for entering any lab that routinely uses potentially hazardous chemicals or materials, or potentially hazardous forms of chemicals or materials.

It happened around 6:30 this morning in the driveway to the Southwest Energy facility in Jane Lew.

The tanker was carrying methanol, a flammable chemical, into the facility, when it tipped as it was turning in.

The Jane Lew Fire Department said the tank began to leak after the crash, and officials from the Department of Environmental Protection were containing the spill.

The immediate area was evacuated until another tanker could be brought to empty the first truck.

The West Virginia Air Quality Board granted the Department of Environmental Protection’s motion to dismiss an appeal that the group People Concerned About Chemical Safety filed, agreeing with the DEP that the issues raised were beyond the scope of the state agency’s job. The decision means the board won’t hold a full hearing with testimony about the citizen group concerns.

In arguing against the appeal being dismissed, Bill DePaulo, attorney for the People Concerned organization, urged the board members to make clear what ruling for the DEP’s motion would be saying to the public.

“Win or lose, I’d just like a clear ruling,” DePaulo told board members. “Do it in big, bold letters: ‘We do not consider the human health and safety in issuing this permit.’ ”

US Methanol hopes to start production in mid-2018 at the plant that would convert natural gas to methanol, a common industrial feedstock. The facility, located at the Institute property now operated by Dow Chemical, would use parts from a deconstructed plant in Brazil.

Shortly after midnight Wednesday, the Woodstock Fire/Rescue District responded to 4609 Franklinville Road, Chief Michael Hill said.

A pickup truck had left the road for unknown reasons and hit a parked trailer. The trailer was carrying a 55-gallon drum of, at the time, an unknown liquid that began to leak. Hazardous material technicians were called to the scene about 1 a.m. to determine what exactly was leaking.

“It was the unknown that was the risk,” Hill said. “Until we knew what we were dealing with, we had to take every precaution.”

Madison fire crews responded at 1:36 p.m. to the Animal Sciences building on Observatory Drive, the fire department said.

As the crew was led to a 10th-floor laboratory where the spill occurred, an EMT stayed outside with the patient to begin medical evaluation until paramedics arrived. The patient was experiencing eye irritation after a bottle containing chloroform/methanol was dropped on the floor, the report said. 

The lab manager assured firefighters that the scene was no longer hazardous, but the crew proceeded to the lab donning personal protection equipment as a precaution, according to the report. Among the shards of broken glass, the crew found a crystalline residue left behind by the chemicals, which had already evaporated.

Two UW chemical safety specialists quickly arrived to the lab to take over cleanup duties. They offered further assurance that there was no danger to occupants.

Firefighters retrieved the patient’s belongings and took the person to the hospital. The MFD Hazardous Incident Team was briefed on the incident, but was not activated.

She and her classmates at The Yellow School, a day school at Memorial Drive Presbyterian Church in the Memorial Villages area, had been watching their teacher conduct a seemingly innocuous science experiment Tuesday.

The teacher poured an accelerant - in this case methanol - onto chemical salts in bowls, said David Foster, police chief with the Village Police Department.

The salts would burn at different light frequencies, creating a rainbow.

But when the teacher didn't see an undetected flame already in one of the bowls, the teacher poured in more methanol from a plastic bottle, Foster said.

The results were frightening. The additional methanol onto the flame essentially created a rocket effect, shooting fire out of the bottle and into Kate and her classmates, seated about 10 feet away.

The so-called Rainbow Fire experiment is common nationwide as a way to show differences in atomic structures, said Kristen Kulinowski, a board member of the U.S. Chemical Safety Board.


The safety board has been asking teachers since 2013 to question whether the experiment is really necessary in their classrooms. And, in a setting like The Yellow School, where students are between 3 and kindergarten, Kulinowski questioned whether it was worth the risk.

Students were sitting outside watching the experiment around 12:30 p.m. , supervised by at least half a dozen teachers, Foster said.

"It appeared they took as many precautions as they could," Foster said. "Short of not doing the experiment."

The accident happened at Yellow School - Memorial Drive Presbyterian Church just after noon.

According to the Memorial City Fire Department, preschool students at the 240 block of Blalock Road were conducting some type of science experiment outside when a flash blast occurred.

Of the 12 students who were injured, 11 of them suffered burns, one student was trampled and six of them were taken to the hospital. All of the students are 3 years old, a fire official said.

"It was an experiment that went wrong. There was a brief moment of flame and it was put out fairly quickly," church business administrator Bob Giles told ABC13.

As parents and grandparents picked up children who were not injured, some of the kids told Eyewitness News that they were involved in some type of color-changing fire experiment.

"Fire was changing colors and the last one wasn't working, so we put it a little bit more, and then it exploded," said Kate Earnest, a 5-year-old who was part of the group that participated in the experiment. "That's how the other kids got burned, and they were crying."

The victims were taken to Children's Memorial Hermann Hospital in the Texas Medical Center for treatment. Five of the children have been released, and one remains hospitalized in good condition.]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[A massive fire broke out at the National Chemical Laboratory (NCL) in Pune’s Pashan area on late Monday evening.

According to officials, the blaze which originated in the methanol lab (in Pilot Plant III) at around 8 p.m. and inflicted extensive damage, seriously gutted the ground and the first floors. While there was a major loss of property, no casualties or injuries were reported.

“Since the premises were under renovation , no experiment was being carried out in the laboratory and no scientist or student was near the pilot plant at the time of the conflagration,” said Dr. Prakash Wadgaonkar, Chairman, Safety wing.

The cause of the blaze has yet to be ascertained. Dr. Wadgaonkar further informed that a committee has been constituted under the Director, CSIR-NCL to probe the cause of the fire.

10 fire tenders were rushed to the spot and a team of 50 jawans were engaged in fire-fighting with a quarter of an hour of the fire. The inferno was brought under control within an hour.

The pilot plant occupies around 10,000 sq.ft. and houses reactors and allied equipment.

WFD received the call for assistance around 9:30 p.m.

The fire department sent a full crew to the area of Seven Springs to help the Pricetown Fire Department. When crews arrived, they found a tanker with 6800 gallons of methanol in the ditch. The tanker was partially rolled over.

A WFD spokesman says the driver was trying to turn around to avoid flooded roadways when his trailer slid off into the ditch. Methanol was leaking from a 2″ pipe on top of the tanker.

The team was able to slow the leak significantly. Another tanker was brought in to transfer the product from the damaged tanker into it. The team was there throughout the night and returned to Wilmington this morning. It is estimated that approximately only 100 gallons of methanol escaped. No one got hurt.]]></description>

On Nov. 25, 2013, Tatiana Schwirblat was 16 when she participated in a class experiment involving methanol vapor at the school at 2001 N. Orchard St., according to a lawsuit filed Thursday in Cook County Circuit Court.

Schwirblat, now 18, alleges her science teacher failed to follow safety guidelines and knowingly added methanol to an open flame, causing a fire that burned her severely, according to the suit.

She alleges the Board of Education was negligent in allowing the teacher to perform an unsafe experiment.

Fire officials said at the time that methanol was being burned inside a container that shattered, and the chemical spilled. A total of three students were taken to the hospital.

A Chicago Public Schools spokeswoman at the time attributed the fire to a “mechanical malfunction,” adding that the instructor quickly put out the flames and followed additional safety protocols.

The lawsuit further claims that the teacher pulled off Schwirblat’s clothes in front of her classmates and pushed her into a shower.

The teacher named as a defendant in the suit still teaches science at Lincoln Park High School, according to the school’s website.

No injuries were reported in the fire at the Agrileum plant on Deadrick Avenue, near the intersection of Lamar and Airways in South Memphis.

The company's website says it produces biodiesel from vegetable oil, animal fats and processed used cooking oil. The person listed on Agrileum's website as company president, Brandon Sheley, said he'd retired at the end of 2015 and didn't have information on the fire. He declined to provide contact information for current company leaders.

The fire was reported shortly before 2 p.m., fire department spokesman Lt. Wayne Cooke said. Employees evacuated and the fire department checked nearby houses to make sure residents left, too. Students at nearby Magnolia Elementary School were ordered to remain indoors temporarily and parents were briefly told not to pick up their children.

The chemical that caught fire was methanol, which is used in the process of making biodiesel, Cooke said. The flammable liquid somehow escaped a 2,500-gallon tank and began burning on the floor.

Cooke said the material is "toxic if inhaled or if it gets on the skin it can potentially cause burns." Firefighters who were exposed to the methanol had to go through decontamination procedures, he said.

The methanol spill occurred at Colonial Terminals at 1002 South Front St. just after 10 p.m. Sunday, according to Wilmington Fire Department Battalion Chief David Hines.

The spill occurred from an overflow when a valve was mistakenly left open and caused a tank to overfill, Hines said. It was estimated that more than 20,000 gallons of the chemical spilled in the area.

“The safety of our students and staff is always a top priority in all of our schools,” FCPS Superintendent Karen Garza said in a Nov. 2 email to parents of all students.

Garza directed that schools conduct a thorough review of science curriculum and of the guidance currently being given to science teachers to “ensure all concepts are taught in the safest possible manner and setting”. In addition, she required that all high school science teachers undergo science safety updates that they must complete by the end of the semester.

In the email, Garza also said that families of all the students injured in the Woodson fire reported that their children are in “good spirits.”

These announcements come after a fire that ignited around 9:30 a.m. on Oct. 30 at Woodson in Fairfax, Va., and injured five students and one teacher.

Two of the students had serious injuries and were immediately transported to the MedStar Washington Hospital burn center in Washington, D.C. Their injuries were not life-threatening, according to Fire Chief Richard Bowers, who responded to the emergency with the Fairfax County Fire and Rescue Department. One of the students has since been released from the hospital.

The other three students were taken to a hospital near Woodson after suffering burns from exposure to fire products. They all returned to school when it resumed Nov. 4.

The injured teacher was treated on-site at the time of the incident.

Woodson principal Scott Poole and FCPS communication and community relations public information officer John Torre declined to comment on the specific nature of the incident, because the Fire and Rescue Department’s investigation is ongoing with no concrete timeline yet set for completion.

However, according to a report by NBC4, the fire could have been started by a demonstration called “the rainbow experiment”, which illustrates how different mineral salts produce flames of different colors when mixed with methanol, a highly flammable chemical.

The fire comes amid growing concern about use of fuels such as methanol in educational demonstrations. C&EN counts that at least 46 people have been injured in similar incidents across the U.S. since September 2013.
Of those injured at Woodson, two students were flown by helicopter to area hospitals. One was released on Nov. 2, and the second was still hospitalized as of C&EN press time. The other three students were treated at a local hospital and released the day of the incident. The teacher was treated at the school.
The American Chemical Society, National Fire Protection Association, National Science Teachers Association, and Chemical Safety & Hazard Investigation Board have each issued safety alerts or recommendations to prevent similar incidents.

But now, experts want more done to prevent an accident like the one seen at W.T. Woodson High School from happening again.
“Any time you have hands-on learning there is a potential for some accident happening,” former Virginia State Superintendent for Public Instruction Dr. Jo Lynne DeMary told WTOP. “Schools have the responsibility to maintain a safe environment for students.”

Fairfax County Fire Chief Richard Bowers said Friday that the demonstration the teacher was performing involved different color flames. It’s a common demonstration called the rainbow experiment.

“The rainbow experiment is designed to demonstrate to students that different metals will burn with different colors,” U.S. Chemical Safety Board member Kristen Kulinowski said. “This is the same phenomenon that gives fireworks its different colors.”

Unfortunately, it’s an experiment the agency has seen turn wrong too often.

“Typically, a small amount of metal powder is put into some kind of small bowl,” she told WTOP. Then methanol, or some other flammable liquid, is placed on top of it and then ignited.”

The demonstration runs as expected and then the flames start to die down.

“When the flame starts to die down, in order to keep the demonstration going, additional solvent is added to the bowl to reignite the flame,” Kulinowski continued.

“The problem is that causes what’s called a flashback in which the flame travels through air basically, through the vapor that’s being released by the larger bottle, into the larger bottle. As that liquid vaporizes from the fire, a large pressure is built up in from within the receptacle and it has to go somewhere. So, it goes out the narrow opening at the top of the bottle and effectively causes a flaming jet, which then results in the larger injuries.”

    <dc:creator>dchas</dc:creator><description><![CDATA[A chemistry class demonstration at a Near West Side high school on Halloween 2014 ended with a student injured by an exploding pumpkin, a newly filed lawsuit alleges.

The four-count suit was filed Tuesday in Cook County Circuit Court by Bryanna Romero, who was a student at UIC College Prep in October 2014.


Romero’s suit alleges that during her chemistry class on Oct. 31, 2014, her teacher tried to show the chemical reaction between boric acid and methanol when the two were placed inside a pumpkin.

During the demonstration, the teacher “created a chemical explosion” that injured Romero’s eyes and face, the suit stated.

The teacher who allegedly performed the demonstration — also named as a defendant — is still employed at the school, according to UIC Prep’s website.

A spokesperson for the Noble Network of Charter Schools could not be reached for comment Wednesday evening.

The suit alleges negligence, willful and wanton conduct and violations of the Eye Protection in Schools Act. It seeks an undisclosed amount in damages.]]></description>

Seven of the victims were burned to death while another died after consuming the chemical. Several people are admitted to various hospitals following the incident.

The lorry had been seized by government officials at Queens Factory, a spirits manufacturer in the area. The factory manufactures Blue Moon, a popular brand of alcohol.

"The lorry was carrying the methanol seized by government officers for disposal. A group of youths volunteered to help the officials to offload chemical and boarded the lorry," said Kayole OCPD Ali Nuno.

The youths are suspected to have started drinking the methanol immediately after they boarded the lorry and therefore could not escape when the fire broke out.

Police are suspecting a person angered by the crack down set the lorry ablaze.]]></description>

Leon County Emergency Medical Services responded to the call, along with the Tallahassee Fire Department and the Sheriff's Office. Two students were admitted to Tallahassee Memorial Hospital with burns suffered during the incident, and another student was released to parental care. Both hospitalized students are in stable condition, confirmed Chris Petley, spokesman for Leon County Schools.

A flame test being demonstrated by an experienced teacher during an AP chemistry class resulted in the accident.

"The teacher is devastated," Principal Allen Burch said. "But she handled everything correctly."

Burch said that students were wearing protective gear and that the teacher had successfully carried out flame tests in the past. His first concern is the students' recovery.

"As far as next steps – we'll sit down and look at how the flame test was conducted," he added. "We'll see how or if we missed anything."

A flame test is designed to analyze mineral salts. Flames produced from burning a substance in question emit certain colors, allowing observers to determine the presence of specific elements. Several elements in a type of a common flame test called the rainbow experiment release vivid colors and can be fascinating to watch, especially for students.

However, a string of disastrous accidents in high school chemistry labs in the U.S. indicates that the experiment – despite education or entertainment value – may not be worth conducting at all, according to national media reports.

The experiment is performed underneath a fume hood on a lab bench. A flammable solvent – in Friday's accident it was alcohol – is used to ignite the flame. But it also creates the conditions for a flash fire.

Flame tests are high-risk and are "totally uncalled for," said Dr. Kenneth Roy, chief science safety compliance officer at the National Science Teachers Association.

Roy, who has recently noticed a marked increase in reporting of flame test accidents in high schools, too often serves as an expert witness in cases involving victims of flame test accidents. He reported there were at least five flame test accidents this year that resulted in severe burns to students.

"Some kids for the rest of their lives will wear these scars," he said. "Parents think they're sending their kids to a safe place, and in these cases they weren't."

Roy's major point of contention is that flame test accidents are almost entirely preventable. Teachers must be aware of all safety aspects and have proper training.

"What were these school's standard operating procedures when handling hazardous materials? Were science teachers certified? Are they using safety equipment? Are they incorporating best practices?" he added.

In a follow-up interview, Petley said that the district has strong oversight on safety in science classrooms, and the accident at Lincoln was not a case of negligence.]]></description>

The incident happened in a classroom in the 2300 block of East Mall Road.

Spokesman Brian Hutchinson said the fire department deals with this type of situation on a regular basis ​when chemicals are mixed in labs. In this case, he said it's believed nitric acid was mixed with methanol.

One patient was transported to hospital in a routine manner, Hutchinson said, meaning without lights and sirens.

When fire crews arrived at the scene just before noon, they found four 10,000-gallon biodiesel tanks on fire outside the facility.

Crews immediately evacuated the building and area businesses.

"Our biggest concern was we were not going to be able to contain the fire," Fire Bureau Chief Dan Harshburger said.

He said the top of a methanol tank on the property blew off while crews battled the fire.

Harshburger said sand was hauled in to help create a berm to prevent runoff and crews used foam to put out the blaze.]]></description>

The EPA’s report is just one part of the agency’s long awaited fracking study, which will assess the impacts of hydraulic fracturing on drinking water supplies. The full report is due out this spring.

The EPA researchers say less than one percent of frack fluid in their analysis of 39,000 wells contained additives, while water made up 88 percent of the fluid, and sand, or quartz, made up ten percent. The agency identified 692 separate frack water ingredients. Maximum concentrations of these chemicals were usually below 2 percent of the total mass, while half of the chemicals were below 0.3 percent of mass. EPA science advisor Tom Burke told reporters on a press call that the chemical additives and volumes of water varied greatly from well to well. Water usage for each fracked well ranged from 35,000 gallons to 7.2 million gallons.

“While these maximum concentrations [of chemical additives] are low percentages of the overall fracturing fluid,” said Burke, “more than half the wells had water volumes greater than 1.5 million gallons. So a small percentage may mean hundreds or thousands of gallons of chemicals could be transported to, and present on, the well pad prior to mixing on the fracking fluid. Remember one percent of a million gallons is a large number — 10,000 gallons.”

The three top chemicals used in the frack fluid were hydrochloric acid, methanol, and hydro-treated light petroleum distillates. Hydrochloric acid is used to keep the well casings free of mineral build-ups, while methanol is used to increase viscosity. Petroleum distillates are refined products like diesel, kerosene, or fuel oil, and are used to make the fluid “slick,” or soapy, and thereby reduce friction.

Yantic Fire Chief Frank Blanchard said firefighters contained the mixed chemicals and packed them into a sealed drum. The drum was removed from the hospital for disposal by a contractor. 
Hospital spokesman Shawn Mawhiney said there were no injuries and no impact on patients. The first-floor laboratory is not in a patient care area, he said. Employees were evacuated out of their work space and into other parts of the building, he said. 
Employees were let back in about an hour later.

Discussion: A worker was refilling squeeze bottles of methanol for use in a laboratory. While pouring from a manufacturer’s large amber bottle expected to contain pure methanol, the employee immediately noticed that the contents were a bright fluorescent pink (figure 1), and did not match the characteristics of methanol. A review of the bottle providing this liquid showed some faint scratch marks over the original product labeling (figure 2), but nothing definitive was added to indicate what the new contents were.

Analysis: A review of the source bottle indicated that it originated in a nearby facility, and individuals relocated this bottle, along with several unopened bottles, in an attempt to reuse chemicals when the directorate initiated a lab cleanout initiative. Further review of past operations in the original building indicated that the pink color was a result of adding a laser dye to a solvent.

The attempted relabeling of the amber (source) bottle was inadequate to communicate the contents to the new owner(s). Nothing indicated the presence of a laser dye, some of which are toxic or even carcinogenic. Furthermore, the relabeling would probably be insufficient for use within the lab by knowledgeable individuals. ]]></description>

The Orange County Fire Authority received a call at 6:30 p.m. about the spillage in the pathology lab where the employee worked, Capt. Steve Concialdi said.

The employee spilled the liquid chemical, Formalin, on himself and immediately showered and sought treatment in the emergency room. The chemical is a formaldehyde solution that contains a small amount of methanol. It's used for preservation of specimens in a lab setting. Contact with it can cause skin burns and blistering.

The hospital called a private hazardous materials team to clean up the spill. The Orange County Health Care Agency and California Division of Occupational Safety and Health were notified by the hospital.

The employee was listed in moderate condition, Concialdi said. CalOSHA is investigating the incident, he said.]]></description>

(AP) — State and academic officials are assessing the damage to Winona Lake after thousands of gallons of water used to battle a blaze at a nearby chemical plant mixed with chemicals and flowed into the popular northern Indiana lake, the mayor said.

The Indiana Department of Environmental Management is gathering information from Warsaw Chemical Co. for a remediation order, and Grace College’s Center for Lakes and Streams is playing “an important role in the assessment,” Warsaw Mayor Joe Thallemer said in a statement Monday.

The fire in a chemical-storage building at the car-wash fluid production plant on the south side of Warsaw burned for three hours Friday, forcing the evacuation of 50 homes and sending at least seven people to the hospital with eye and lung complaints.

State natural resources officers estimate that several thousand gallons of water mixed with chemicals, mostly methanol, and washed into Winona Lake as firefighters doused the fire.]]></description>

(WANE) – The Indiana Department of Natural Resources is concerned about hazardous materials leaked into Winona Lake after an explosion at a chemical plant in Warsaw on Friday. Conservation officers are asking residents to stay off the ice and to not eat any fish from the lake until further notice. 

Multiple fire departments responded Warsaw Chemical to reports of a fire, followed by an explosion. Warsaw Chemical, located at 2300 E. Durbin St. is well known for producing car washing products.

The Indiana DNR reports an unknown amount of chemicals, mostly methanol, made its way into the storm drains and into Winona Lake, which turned the lake and snow blue and green from the dyes in the chemicals. Conservation officers said it’s possible thousands of gallons of chemical and water mixture went into the lake.

On Friday, two fire departments arrived at the plant at 11:15 a.m., according to our partners at WSBT. Residents within 1,000 feet north and east of the plant were asked to evacuate. The area remains under evacuation until further notice and until air quality tests have been performed.

Lt. Kip Shuter of the Warsaw Police Department said eight people were injured. Out of those eight people, five were with the Warsaw City Waste Water Treatment and two were firemen. Their injuries included burning eyes and lungs. One person was also treated for burns on his hands.

Calling such incidents a national problem, the Chemical Safety & Hazard Investigation Board (CSB) on Oct. 30 released a safety bulletin with recommendations for preventing them.

On Sept. 3, a demonstrator at a museum in Reno, Nev., was attempting to create a colored “fire tornado” when she poured methanol from a 4-L container onto a smoldering cotton ball. On Sept. 15, a teacher at a high school in Denver was demonstrating flammable properties when he poured methanol from a 4-L container onto an open flame. On Oct. 20, a Cub Scout group trying to produce a green flame poured methanol-containing antifreeze from a 355-mL bottle onto a fire in Raymond, Ill. And on Oct. 31, high school students in Chicago were also using methanol to create a green flame.
In at least the first three cases—the fourth is still being investigated—the fire propagated back into the fuel bottles and ignited the remaining liquid, CSB investigator Mark Wingard says. Pressure built up in the containers and the fiery fluid was expelled, burning people in the vicinity.]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[The parents of a Bronx teenager whose ear was melted in a chemistry lab fire at Beacon High School is suing the city for $27 million.

The suit was filed Thursday in Manhattan Supreme Court by Yvonne and Claudio Yanes on behalf of their 16-year-old son, Alonzo.

Alonzo and another student, Julia Saltonstall, 16, of Manhattan were injured last January when their chemistry teacher poured methanol into four hot Petri dishes containing nitrates that had been aflame only moments earlier, according to a city report.

The heat combined with the methanol and the nitrates triggered a fireball that flew across the table where Anna Poole’s students were gathered.

Yanes had second- and third-degree burns on his hands and face. A custodian who responded to the put out the fire said at the time that his ear looked melted.

Saltonstall had first-degree burns on her forearms and was released after getting emergency room treatment. Her father has filed a notice of claim with the city, which says he intends to sue for $10 million.

In court papers, the Yanes family lawyer, Jeffrey Bloom, said that accident exposed “gross negligence” and “abject dereliction and reckless disregard” for the safety of students and staff by the city Department of Education.

Bloom said that only two weeks before the Jan. 2 accident, the DOE had been warned by the U.S. Chemical Safety Board that the Rainbow Experiment, which Poole was conducting, “posed risk of severe injuries if performed.”

The first incident described in the CSB safety bulletin is the September 3, 2014, accident at the Terry Lee Wells Discovery Museum, known as “The Discovery,” in Reno, Nevada, where thirteen people, most of them children, were injured. Two CSB investigators were deployed to the site and interviewed personnel who were directly involved. 
 
Just 12 days after the fire in Nevada, a second similar accident occurred on September 15th at the SMART Academy in Denver, Colorado, severely burning a 16-year-old high school student. Most recently, on October 20, 2014, less than five weeks after the incident at SMART, three Cub Scouts and one adult were injured during a demonstration using methanol at a Cub Scout event in Raymond, Illinois.]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[DENVER – Three weeks ago, Daniel Powell conducted a science experiment that went so wrong, four students were injured, one seriously. He has now been fired and state records show he did not have valid teaching license.

"It's a horrible, unfortunate incident and our hearts bleed for the families and for those students," Nora Flood, president of the Colorado League of Charter Schools, said.

Powell was a teacher at SMART Academy which is part of a network of charter schools called Strive Prep. He was trying to show students a chemistry experiment with fire and methanol when a fire started causing serious burns to one student.

"It could've been anybody at any school," Flood said.

In a search of the data base which shows records of teacher licenses, there is no listing of a Daniel Powell with a current Colorado teacher's license. Charter schools are not required to have licensed teachers in the classroom.

Flood says that's by design.

"There are always challenges in finding good teachers and so we're actually able to go outside the realm of the traditional teacher licensure program to try to find the best fit for our schools," Flood said. "It's one of the things we hold dear and that is, that it is up to the school to be able to determine for themselves whether they want to have that licensure piece."

Flood says that allows, for example, a science-based school to seek a professional engineer to teach students about engineering.

"You have to be an expert in the field you are going to teach," Flood said.]]></description>
