Selangor Fire and Rescue Department director Norazam Khamis said in a statement that firefighters from the Pandan Fire and Rescue Station and Hazardous Materials Unit (Hazmat) teams from the Shah Alam and Tun Razak Fire Stations were deployed to the scene following a distress call at 9.55 am.

He said the KKM officer entered the restaurant to help the two female restaurant workers who had fainted.

The victims were brought to the Ampang Hospital for further treatment, he said, adding that the restaurant is closed pending an investigation.

Meanwhile, Ampang Jaya police chief ACP Mohamad Farouk Eshak said the fainting spell was caused by the air ventilation fan not being activated while the restaurant workers were frying chicken, resulting in smoke and fumes from a gas leak not being expelled through the exhaust fan of the kitchen.

“The content of carbon monoxide that exceeded the content of oxygen caused the three victims to inhale smoke and gas until they passed out,“ he said in a statement today.

He said all the victims who received treatment at the Ampang Hospital were in stable condition and the case was classified as ‘Refer to Other Agencies’, namely to the Hazardous Materials Special Team (Hazmat). - Bernama

A Review of Safety Issues and Risk Assessment of Industrial Ammonia Refrigeration System

Several industries like cold storage and ice plants heavily rely on ammonia (NH3) due to its low cost, excellent thermophysical traits, and zero effects on the ozone layer. Despite its widespread use as a refrigerant, the hazardous nature of ammonia remains a significant health concern. Since the industrial revolution, ammonia leakage has led to numerous major accidents, including fire and vapor cloud explosions. This paper reviews the safety issues and the current practices of risk assessment with regard to the industrial ammonia used in refrigeration systems. In addition, previous works related to the risk analysis of ammonia-based industries are reviewed. The majority of ammonia refrigeration system-related works applied the traditional static tools for risk analysis such as fault tree, event tree, and bow-tie that are not suitable to handle common causes of failures or conditional dependencies, which are absolutely significant aspects for a reliable safety assessment. Thus, it is vital to maintain and improve the safety levels of diverse industries using ammonia-based refrigeration systems. Various characteristics of ammonia-based refrigeration systems and their safety issues related to industry, present development, and future trends are comprehensively underscored. This review may act as a guide to develop a better understanding of dealing effectively with the hazards of ammonia-based refrigeration systems and risk assessment, thus enhancing the overall safety of humans and the environment.

TUARAN (May 10): Members of the public are advised to steer clear of the Sungai Rugading area here, after a chemical spill was detected at the river on Monday (May 9) evening.

Sabah Fire and Rescue Department (JBPM) assistant director (Operations) Misran Bisara said a hazmat detector had discovered the pollution, which was in the form of scheduled waste.

He said the Tuaran Fire and Rescue Station received an emergency call from MERS 999 at 6.34pm, before a team was dispatched to the scene, situated 10 kilometres away.

“Although the pH level of the river water is 6.8 or neutral, another occurrence of chemical spill can change the reading (to polluted). As such, we don’t declare it as safe today,” he said when contacted by Bernama.

It was learnt that the incident occurred at about 10.15pm after two women from the village had trouble breathing and fainted due to the strong odour.

Following that, many other residents noticed the pungent chemical odour and immediately contacted the Fire and Rescue Department for assistance.

Johor Fire and Rescue Department’s Hazardous Materials Unit (Hazmat) chief Saiful Bahri Safar confirmed receiving a distress call at 10.22pm and said a team was immediately despatched to the location.

He said the first team arrived at Kampung Sri Aman within 15 minutes.

“Two female victims in their 20s and 40s were rushed to the Sultanah Aminah Hospital and Sultan Ismail Hospital by the department’s Emergency Medical Response Service team after they had difficulty breathing.

Saiful Bahri said the Hazmat team then detected the suspected source of fumes at the nearby river.

“The personnel isolated and used damping technique in the river after they found spillage of a solid liquid upstream,” he said.

A total of 24 firemen and five fire engines from the Kempas, Larkin and Johor Jaya fire stations have been dispatched to the village.

The establishment of the Globally Harmonized System of Classification and Labeling of Chemicals (GHS) is one of the global initiatives to promote effective chemical hazard communication worldwide. Ultimately, the GHS aims to enhance safe use of chemicals, as well as to facilitate chemical trading across national borders. Countries that are now adopting GHS into their respective national legislations include the EU, US, Japan, Korea, Taiwan, and Malaysia. Nonetheless, challenges exist that might impede GHS implementation worldwide. These challenges have been identified and are discussed in this paper, namely, (i) adoption of different versions of the GHS document, (ii) adoption of building blocks, (iii) list of classified chemicals, (iv) different chemical mixture classification throughout the supply chain, and (v) the definition of GHS implementation. The challenges discussed in this paper are not to discourage countries from implementing GHS but, rather, to serve as input for the relevant entities to further strengthen GHS implementation worldwide.

An industrial gas supply firm and its chief executive officer failed to take necessary measures to ensure workplace safety, leading to a fatal blast that killed a chemist in 2015.

New mother Lim Siaw Chian, 30, was killed and at least seven others were injured after an explosion at Leeden National Oxygen's Specialty Gas Centre Quality Control Laboratory on Oct 12 that year.

Ms Lim had returned to work from maternity leave a week before the tragedy at the laboratory in Tanjong Kling Road near Pioneer Road.

The former Malaysian had received her Singapore citizenship just a month before she died. She left behind a daughter who was six months old at the time.

Leeden National Oxygen and its CEO Steven Tham Weng Cheong, now 69, each pleaded guilty yesterday to an offence under the Workplace Safety and Health Act.

Among other things, they had failed to take measures to ensure that unsafe modified regulatory valve assemblies were not used when testing combustible gases. They had also failed to ensure that there was a system in place for the accurate tracking of gas cylinders.

State Housing, Local Government, Environment and Green Technology Committee chairman Datuk Tey Kok Kiew said the gas release began at 10pm and would take about 10 hours.

He said Hazmat had sprayed water in five locations in the farm to help release the trapped gas into the air, with gas readings indicating a drop from 500 ppm (part per million) to about 30 ppm within 45 minutes.

The department was also conducting gas readings every two hours in areas within a 3km radius of the factory, Tey said at a press conference at the operations centre here late last night, adding that if the readings in the areas indicated that the air was polluted, the release of the gas would be stopped.

Nevertheless, the school was quick in seeking treatment at the Ayer Molek Clinic for the 14 pupils aged between eight and eleven.

It also called the Malacca Fire and Rescue Department, which immediately dispatched six personnel and a fire engine to the school laboratory.

"Upon arriving at the location at 12.28 pm, the team found that the mercury had spilled from the thermometer and used special equipment to clean up the area," said a department spokesman.

The pupils were later sent home after receiving outpatient treatment. - Bernama

This is what National Poison Centre statistics have found, said the centre's pharmacist (drug and poison information unit) Asdariah Misnan.

Sixty six percent of cases recorded during the 10-year period comprised accidental poisoning involving household chemical products (33%) and medicines (33%).

Among the accidental cases, 46% involved children aged one to nine.

The household chemicals include laundry detergent and bleach, as well as dishwashing liquid, she told Bernama when met at the centre at Universiti Sains Malaysia recently.

Fifty percent of all cases involve children, with the centre receiving calls about such poisoning cases on a daily basis.

An earlier report on the incident said there was an explosion, but this turned out to be inaccurate.

A spokesman from the Fire and Rescue Department said all the students were in a stable condition, with only one sent to Penang Hospital to receive treatment.

State Environment Committee chairman Phee Boon Poh added that the Form Four students were conducting an experiment which involved iodine.

“Iodine was heated during the experiment to test its chemical reaction.

“As there was insufficient ventilation in the laboratory, around 20 students who were in class experienced breathing difficulties and started coughing,” he said, adding that no explosion occurred during the experiment.

JOHOR BARU, March 21 — It was a spill of sulphuric acid that caused a lockdown at the Johor Department of Environment’s (DoE) building here, the authority confirmed today.

Johor DoE director Datuk Mohammad Ezanni Mat Salleh said the spilled chemical was concentrated sulphuric acid.

“Initial investigations revealed that the spilled chemical was only sulphuric acid and contained no other substances.

“The acid was in its pure form and considered highly hazardous if spilled.” said Mohammad Ezanni when contacted by Malay Mail tonight.

Concentrated sulphuric acid is extremely corrosive and can cause serious burns when not handled properly. This hazardous chemical is capable of corroding skin, paper, metals, and even stone in some cases.

If sulphuric acid makes direct contact with the eyes, it can cause permanent blindness. If ingested, this chemical may cause internal burns, irreversible organ damage, and possibly death.

Mohammad Ezanni said the 30-year-old DoE technician involved in the incident is reported to be in stable condition after being treated at the Sultanah Aminah Hospital (HSA) here.

“He is recuperating after he lost consciousness in the laboratory, believed to be from the fumes,” he said, adding that the incident was a minor incident.

Sungai Masai is the third illegal dumping site discovered since toxic fumes emitted by waste dumped into Sungai Kim Kim hospitalised more than 3,000 people and forced the closure of 111 schools on March 7.

Police and the Fire and Rescue Department’s hazardous material unit (Hazmat) were at the latest site, and personnel from a private environment firm were seen removing the barrels from about 2pm.

A 300m stretch at Kilometre 17 of the Pasir Gudang-Johor Baru Highway towards the state capital were closed, resulting in a traffic congestion until 7pm as authorities loaded the barrels into a lorry.

Hence, he hopes that people will not worry as the impact was not as severe as exposure to the original cyanide gas.

“The pure cyanide which is used as a chemical weapon is dangerous.

“The hydrogen cyanide (cyanide from the pollution of Sungai Kim Kim) is harmful but it is the result of the reaction of waste materials. The reading rate is also not very harmful. So, I hope this can be rectified,” he told reporters here, today.

State Health, Environment and Agriculture Committee chairman Dr Sahruddin Jamal said that, of the total, 21 victims had been taken to the Sultan Ismail Hospital, six to the Pasir Gudang Health Clinic and three more were treated at the Penawar Hospital.
He said that all were reported to be in stable conditions.
“Two victims at the Sultan Ismail Hospital are provided with breathing assistance (intubate) to prevent a worse situation as the breathing conduits can be blocked after breathing in the chemical,’’ he said.

He also said samples of the pollution at the river had already been taken by Hazardous Materials and Waste Management Unit (Hazmat), Fire and Rescue Department and the Department of Environment Thursday morning to be analysed.

Nazrin Hassan, the CEO of a Malaysian venture capital fund, died after one of his cellphones reportedly caught fire while it was charging in his bedroom. Conflicting reports have attributed his cause of death to blunt force trauma and smoke inhalation, but authorities say that they need more time to investigate exactly what happened.

Hassan was the Group CEO of the Cradle Fund, a VC that claims to have helped fund over 700 Malaysian tech startups. In a press release dated June 15, the Cradle Fund confirmed Hassan’s death saying “the post mortem report concluded the cause of death as being complication of blast injuries attributable to an exploding hand phone that was being charged next to him.”

That account echoes a social media message from Hassan’s brother-in-law that was circulated in local reports about the incident. According to The Malaysian Insight, the brother-in-law claims that one of Hassan’s phones overheated and exploded causing an object to hit him in the back of the head. “He had two phones, one Blackberry and a Huawei. We don’t know which one exploded. Who would have thought such an innocuous routine procedure is the reason three young kids will grow up without their father by their side,” the brother-in-law reportedly wrote. Previously, however, a police spokeperson told Malaysia’s Star newspaper that the cause of death was “smoke inhalation and the body being burnt.”

City Fire and Rescue Department director Khirudin Drahman said the quick action of the hospital's engineering team to move the tanks and shut off the electricity and the oxygen pipelines avoided a larger disaster.

He added that a short circuit is believed to have sparked off the fire at the storeroom.

The incident which happened around 3am today, was said to have happened because the lorry was trying to avoid hitting a car.
According to preliminary reports, the 37-year-old victim is known as Surazru Mohamed Sakeri.

The Fire and Rescue Department did not rule out the possibility that the man could have died after inhaling poisonous gas emitted from the spilled acid, apart from suffering severe injuries.

The body was sent to the Tuanku Ja'afar Hospital (HTJ) for a post-mortem.

The hazardous materials unit (Hazmat) is currently cleaning the acid spillage.

The 4.30am incident destroyed at least 60 per cent of the warehouse, which was used to store paint and tyres, among others.

As of 2pm, some 50 firemen from Selangor and Kuala Lumpur stations dispatched to the scene were still working to put out the blaze.

Selangor Fire and Rescue Department director Azmi Osman said the fire was noticed by a security guard who alerted the authorities at 4.41am.

He said 56 firemen with eight fire engines and water tankers, including two from the Kuala Lumpur FRD, rushed to battle the blaze, which was already raging when the first team arrived at 4.56am.

"When firemen arrived, they saw very thick smoke which signified that the fire had been spreading for quite some time.

"Within an hour, they managed to control the fire from spreading to another warehouse, which stores industrial chemicals.

"It is important to make sure that the blaze does not spread there as the chemicals there are hazardous. InshaAllah, from the current status of this operation, we are confident that the blaze is under control and contained," he said.
