Victims were instantly overwhelmed by gas, which left high concentrations of hydrogen sulfide in air

India’s National Disaster Response Force, the State Pollution Control Board and the police are all investigating the incident
As people were heading towards a milk vendor in Ludhiana, India, early in the morning of 30 April, highly toxic gas suddenly overwhelmed and killed 11 people, including two children, in the street and nearby houses. Four others were injured but have since recovered.

Preliminary investigations indicate that there was a high concentration of hydrogen sulfide in the air which smelled pungent and of rotten eggs. ‘There was no respiratory distress and the gas is suspected to contain neurotoxins,’ says Swati, a Sub Divisional Magistrate for Ludhiana, who is heading the magisterial inquiry.

The investigators from the National Disaster Response Force, the State Pollution Control Board and the Ludhiana police are investigating the exact cause and source, which remains unknown. The Punjab state police has also formed a five member special investigation team. The police have warned the pollution control officials that if they don’t help the police in finding the culprits, they too will be prosecuted.

Whether industrial chemicals were illegally dumped in the sewers is yet to be determined

‘We are checking sewage connectivity as well, but the concentration of hydrogen sulfide has to be very high to be able to kill instantly,’ says Swati, who uses a singular name. The circumstantial evidence indicates that those exposed to the gas, or mixture of gases, died almost immediately, she adds.

SAN JUAN, Puerto Rico -- The Environmental Protection Agency on Monday ordered the owners of an oil refinery in the U.S. Virgin Islands to remove thousands of pounds of chemicals from the site, warning that its pipes and valves are severely corroded and in disrepair.

The EPA estimates that more than 40,000 pounds (18,000 kilograms) of anhydrous ammonia and more than 37,000 pounds (16,000 kilograms) of liquefied petroleum gas are stored at the refinery in St. Croix. It also ordered the removal of hydrogen sulfide.

The order comes after the EPA inspected the refinery in September and found serious deficiencies that could lead to a fire or chemical release, including piping and valves on LPG units that are severely corroded and in disrepair. Last month, the EPA announced the oil refinery could not resume operations without obtaining a clean air permit.

The refinery was forced to pause operations in May 2021 following an EPA emergency order, with the agency noting that the facility is located near marginalized communities where people have been sickened and schools forced to close.

Air monitoring data from Monday and Tuesday from CCH's Hazardous Materials Program (HazMat) show concentrations of the chemical that may cause symptoms such as headaches, nausea and irritated eyes in people exposed to it for long periods.

CCH and its partners have not detected levels of hydrogen sulfide in Crockett's air that would be considered immediately dangerous to public health.

The threshold at which hydrogen sulfide in the air may cause symptoms is .03 parts per million (ppm). The Bay Area Air Quality Management District (BAAQMD) measured a 1-hour average reading of .036 ppm near the plant on Friday, Oct. 7, prompting the health advisory from CCH.

Sept 28 (Reuters) - BP Plc laid off most contractors at the approximately 160,000 barrel-per-day Toledo, Ohio, refinery it owns with Cenovus Energy Inc (CVE.TO), according to sources familiar with the matter on Wednesday, indicating that the plant will experience a prolonged shutdown following last week's explosion and fire.

The explosion killed two United Steelworkers members, identified as brothers Max and Ben Morrissey.

The more than 100-year-old refinery has been offline since the middle of last week following the explosion and could be shut for several months.

At least one contracting company is on site assessing the damage. The U.S. Chemical Safety Board is also investigating the incident, which it said additionally caused the release of sulfur dioxide and hydrogen sulfide and significant property damage.

Cenovus has referred comment to BP, which has declined to comment further.

The fire raged for 4 hours and two badly burned workers died. The incident also released sulfur dioxide and hydrogen sulfide and caused significant property damage.
The investigation of the incident will be the first that the CSB has initiated in 14 months.
The refinery employs 840 people and processes up to 160,000 barrels of crude oil daily. The 100-year-old facility had recently completed a major overhaul of its isocracker unit, the company notes. The refinery was shut down as of C&EN’s deadline.

At its height, six fire engines from Warrington, Lymm, Chester, Northwich and Tarporley fire stations attended the incident on Percival Lane, Runcorn, at about 7.45pm on Friday.

The blaze occurred a week after the same road saw a huge fire break out at a pallet yard, with plumes of smoke being seen for miles around.

On arrival at the scene on Friday, crews found a fire smouldering in the roof space of the unit where maintenance work had been carried out.

Additional fire engines were deployed to the scene owing to the fire producing hydrogen sulphide gas.

Firefighters wearing protective kit attempted to tackle the fire using the site’s own fire suppression system.

The bridge began reopening shortly after 5 p.m. A chemical was found inside a vehicle that was parked near the bridge, the Placer County Sheriff’s Office said.

Deputies found a warning letter on the car but are not disclosing the details if it. Hazmat crews later found two chemicals mixed together to make hydrogen sulfide in the car.

Additionally, Placer County firefighters broke the window to find a person dead inside.

“In smaller doses, that gas can be a throat irritant, coughing, nausea, vomiting, headaches, but at high concentrations and since the vehicle was closed, it can cause death,” said Battalion Chief Matthew Rea of the Placer County Fire Department.

City of Nederland emergency response crews are still monitoring a transport trailer that leaked its contents resulting in a HAZMAT cleanup operation and an awful odor that affected a wide area in Southeast Texas, with residents from adjacent cities complaining that they too could smell the spill.

On Dec. 12, the city of Nederland issued a press release warning citizens that Beauxart Garden Road was closed due to hazardous material leaking from a transport trailer located at Nederland Tank Wash. On Dec. 15, Nederland Tank Wash stated that the leak had been detected Dec. 11 at about 5 p.m. by staff who heard a hissing noise coming from the trailer.

The product leaking from the trailer has been identified as Lubrizol 1389, a mineral oil blend that was producing low levels of hydrogen sulfide gas, according to the city. City Manager Chris Duque told The Examiner last week that the tank wash facility had hired air monitoring firm Center for Toxicology & Environmental Health (CTEH), and Pulido Transport, the owner of the trailer, had since taken leadership of the cleanup operation.

CTEH, which is also reportedly still in the city performing tests, has stated in subsequent news releases following the leak that the community is safe from the potentially lethal gas, in spite of the noxious pervading odor nearby residents say still remains. According to CTEH, monitoring performed on Dec. 12-13 did not detect sulfur-based compounds in the community and although residents could smell the rotten egg stench associated with those chemical compounds, “These low concentrations do not represent a public health risk to the community.”

BEAUMONT, Texas — With three chemical leaks in less than three weeks, people are asking how to stay informed and what can they do to protect themselves.
First, it was the Saturday, Dec. 11 hydrogen sulfide from a tanker at a Nederland business.
Then on Wednesday, Dec. 15, a benzene leak from a Port Arthur plant triggered a brief shelter in place. And Tuesday night, a pipeline leak in the area of LaBelle Road lead to a shelter-in-place order that lasted for nearly 24 hours.
Some people are concerned about how they can protect themselves when they were never alerted of the leak.
"I have children, and I have to consider the well-being of me, myself, and my kids," said Beaumont resident Joanna Freeman.
Freeman said she smelled gas in the air, but she never received an alert about a pipeline leak.

"There would have to be a formal warning. So we would know, because, you know, seeing it on Facebook, it's not acceptable," said Port Neches resident Vickie Smith.
"The residents of this area that have experienced chemical explosions, the TPC explosion, and now we've got gas leaks. I just feel like there's no one that has our backs," Smith said.
Fire Chief of ESD No. 4 Jeremy Hansen said the primary system of alerting residents in case of emergency is the Southeast Texas Alerting Network or STAN.

"It primarily exists here for the petrochemical hazards that exist and the chemical hazards that exist in this region,” Hansen said.

But registering for STAN isn't a one-and-done deal.

"Even if you've been registered in the past, it's important that you update your registration annually and make sure that you're subscribed to receive the top alerts on your cell phone and/or home phone," Hansen said.

Early investigation found that one victim was found dead in a vehicle, Eugene-Springfield Battalion Chief Mike Caven said.

"It sounds like we had somebody commit suicide with a hazardous gas in the vehicle," Caven said Monday night. "They're locking down the area, (asking people) shelter in place until they can make sure everything's ventilated."

The gas released was hydrogen sulfide, and the chemicals have been isolated to the one vehicle, said Battalion Chief Anthony Bucher, who responded initially.

"They've just been mitigating the hazard, which will probably take a couple more hours, that's why the area is cordoned off," Bucher said.

Emergency response crews spent most of Monday monitoring and suppressing the chemical leak at Beauxart Garden Road near Highway 69. The incident was first reported on Sunday but began Saturday afternoon.
The leak has closed the road and surrounding neighborhoods are being closely monitored.
Officials released multiple statements saying the chemical released by the leaking trailer may linger in the air for several days. This has left Beauxart Garden residents concerned, as they have already been experiencing mild side effects.
The leaking product, known as Lubrizol 1389, is producing a low level of hydrogen sulfide gas, according to a City of Nederland press release. Lubrizol 1389 is a mineral oil blend used as a lubricant additive.

The smell, described by locals as similar to rotten eggs, was first reported on Oct. 3. Now, officials say that the stench was caused by waste from a beauty and wellness product warehouse that caught fire and spilled chemicals into a canal.

The task of determining the cause of the “stench of death” was left to South Coast Air Quality Management District. The agency said on Friday that the warehouse fire on Sept. 30 and subsequent chemical spill caused were at the heart of the matter. The chemicals killed plants and other organisms in and around the canal, producing hydrogen sulfide. This colorless gas is flammable and harmful to human health.

Since the stench began, Carson residents have reported various symptoms. Many have complained of headaches, fatigue and respiratory problems. Some have endured more serious symptoms, such as constant nose bleeds that require hospitalization.

The South Coast Air Quality Management District said in a statement it issued notices of violation to four companies and Los Angeles County, which is responsible for maintaining Dominguez Channel.

The notices allege the emissions of hydrogen sulfide caused a public nuisance.

The fire began Sept. 30 at a warehouse property in Carson where two companies stored large amounts of wellness and beauty products, and chemicals including ethanol subsequently flowed into the channel, the district said.

Cain is staunchly opposed to state mask rules and called the chemical plant mask order "sweet libt--d hell."

A Texas legislator called a face mask and shelter-in-place directive "sweet libt--d hell" after a chemical leak occurred in his legislative district.

Briscoe Cain represents District 128 in the Texas House of Representatives. A chemical plant in La Porte, a city in his district, reported an "odor incident" around 8:30 p.m. on Monday.

Officials originally believed the strong sulfur-like and rotting smell was the result of hydrogen sulfide, a highly toxic and flammable gas used in mining and gas refining, and quickly instituted a shelter-in-place order and mask order. Local authorities also asked residents to turn off air conditioning systems to prevent the chemical from entering homes and businesses.

WINDSOR, ONT -- Municipal officials are “shrinking” the evacuation order down to just two properties and allowing all 31 residents to return home.

“We can’t carry this on forever,” Chatham-Kent Fire Chief Chris Case told CTV News Saturday.

“We cannot tell people it's safe. We can tell people that there was a risk, that risk is still there but the actual gas is no longer present that’s all we can tell them.”

On June 2, 2021, a hydrogen sulphide gas leak was detected inside 15 Erie St. North in Wheatley.

Chief Case describes it as 'toxic and highly explosive gas coming out of the ground in sufficient quantities that it could have exploded.'

First responders shut off all utilities to a block of buildings at Erie and Talbot, evacuated 31 residents and closed 12 businesses.

Chief Case says firefighters have been on scene ever since testing the entire perimeter every 15 minutes for gas.

A federal judge has awarded $1 million in damages to a former Intel contract worker who was exposed to toxic gas on the company’s Chandler campus in 2016.
Ahmad Alsadi, 29, claimed he sustained damage to his respiratory system after inhaling toxic fumes of hydrogen sulfide while one of Intel’s buildings was being evacuated for a gas leak.
Alsadi later sued Intel, Chandler’s largest employer, for negligence and accused the technology company of failing to protect workers from harmful chemicals.
After a six-day bench trial earlier this month, U.S. District Court Judge David Campbell awarded $921,188 to Alsadi and another $250,000 to the plaintiff’s wife.
Campbell concluded there was evidence to suggest Intel had been advised to install gas monitors and exhaust systems before the 2016 incident, yet the company failed to do so.
“Because Intel did neither in the face of multiple instances of (hydrogen sulfide) off-gassing and its knowledge that H2S is extremely hazardous for workers, the court finds that Intel breached its duty with respect to operation of the (wastewater) system,” Campbell wrote in his ruling.
Linda Qian, an Intel spokesperson, said the company was “disappointed” by the judge’s ruling and said Intel acted properly during the events described in Alsadi’s lawsuit.

Santa Barbara police and firefighters were dispatched to the cul de sac at the ocean end of Santa Cruz Boulevard shortly after 7 a.m.

The woman was found deceased in the vehicle, an apparent suicide victim, according to Anthony Wagner, a police spokesman.

Hazmat crews were brought in because the woman used a "chemical concoction" in taking their own life, Wagner said.

Police had previously said the death was by carbon monoxide poisoning, but Wagner said that was incorrect.

The chemical mixture reportedly was hydrogen sulfide, which was created by combining readily available household products, but that could not be confirmed.

LYTTON — A potentially deadly cloud of hydrogen sulfide gas that developed near a well Monday afternoon sent two people to a hospital.

The incident happened at about 1:15 p.m. in Lytton.

Both men, one of whom had briefly stopped breathing, were taken to Loring Hospital in Sac City. A report on their conditions was not available Monday evening.

A crew from Sargent Drilling, of Carroll, was injecting 110 gallons of hydrochloric acid into the well when a chemical reaction occurred, according to Fort Dodge Fire Chief Steve Hergenreter. He said a large cloud of what was later identified as hydrogen sulfide was created.

A worker in the well building was overcome by the gas, Hergenreter said. One of his co-workers went into the building and pulled him out.

Hergenreter said the worker who was overcome was not breathing when he was pulled from the building. He said Calhoun County 911 dispatchers instructed the second man in performing CPR. As a result, the patient was breathing when emergency personnel arrived.

The Region V Hazardous Materials Response Team operated by the Fort Dodge Fire Department was called to the scene.

Hazardous materials team members in full protective gear identified the gas as hydrogen sulfide. Hergenreter said at that time the amount of gas in the well building was 10 times the level at which it is considered lethal.

The hazardous materials team flushed the acid in the well, eventually dissipating the hydrogen sulfide.

The US Chemical Safety and Hazard Investigation Board (CSB) issued two significant reports in late July. One praises voluntary actions of Airgas after a deadly incident at a nitrous oxide facility. The second is a factual update to the ongoing investigation of an incident at an Aghorn Operating facility in Texas in which a worker and his wife died from exposure to toxic hydrogen sulfide gas.

The CSB is an independent, nonregulatory federal agency that investigates chemical accidents.

In a Safety Spotlight report, the CSB lauds Airgas, now a subsidiary of Air Liquide, for rapidly developing and implementing a robust process safety management program (PSM) in response to an explosion in 2016 that killed one worker.

CSB stresses that the effort was voluntary, as nitrous oxide handlers are not required by US federal law to develop a PSM program. The incident occurred during the loading of a trailer truck at an Airgas facility in Cantonment, Florida. In the transfer, a pump heated the nitrous oxide above its safe operating limits, causing decomposition and the explosion.

According to the CSB, after the incident, the company began to review its safety programs for its nitrous oxide production facilities, trucking fleet, and cylinder-filling operations. The review resulted in several reforms, such as better audit practices and inherently safer designs.

According to the Gladewater Fire Department, it happened in the 2600 block of South Rodeo Street just west of Gladewater. And several residents were evacuated when first responders saw signs at the oil lease indicating the presence of hydrogen sulfide gas.

According to Gladewater Fire Chief Cory Crowell, firefighters weren’t sure what they were up against when they arrived at the scene.

“They found that there had been an explosion on a well site, and there was a gas release. And so they erred on the side of caution and started evacuating a few of the residents around there,” Crowell said.

The words may not have been explicit, but oilpatch contractor Matthew Linnitt says he read between the lines: lie on official documents about an incident that could have killed him, or someone would be fired.

The tacit threat, he alleges, was handed down by his supervisor at Canadian Natural Resources Limited (CNRL) after a close call with hydrogen sulfide on a northwestern Alberta well site on May 2, 2016.

Hydrogen sulfide, also known as H2S or sour gas, is a toxic substance that can be fatal in high concentrations, and is sometimes leaked from the wellheads, pump jacks, pipes, tanks and flare stacks of oilfields.

At the time, Linnitt considered himself lucky to have escaped with his life — he was working alone on a remote site in Karr Creek, Alta., when a geyser of fluids and poisonous sour gas erupted from its depths. A valve hadn’t been properly shut down.

Staff at the Nova Chemicals site in Corunna, south of Sarnia, were told to evacuate at about 4:45 p.m., the plant source, who asked not to be identified for fear of reprisal, told the Star and Global News. An alert issued by the nearby First Nations community of Aamjiwnaang stated the chemical spilled was hydrogen sulphide.

It’s not clear when the leak began. The same source said Nova staff were told Wednesday morning that a leak “happened last (Tuesday) night and (was) still not contained.”

The company, which employs about 500 people at the plant, called an all-clear just after 6 p.m. A spokesperson for the Ontario Ministry of the Environment, which must legally be told about any chemical spills, didn’t say if Nova notified the government of the incident but said it’s now aware of an “alarm” at the plant.

KINGSPORT, TN (WJHL) – An Eastman Chemical company spokesperson told News Channel 11 that a potentially harmful chemical released during a fire and explosions this week was a small amount and never left the plant site.

News Channel 11 also obtained an incident report from the National Response Center provided to us by TDEC or the Tennessee Department of Environment and Conservation. The document lists what caused the explosions an equipment failure.

Eastman said the incident happened during what it called an “equipment swap” in the coal gas area.

The Tennessee Department of Environment and Conservation said for the first time that, “Emissions resulting from the explosion at Eastman on Oct. 4 were in excess of the limits set by TDEC and that such excess emissions were atypical from a normally operating process at Eastman.”

The NRC incident report lists an equipment malfunction as the reason that “Hydrogen Sulfide” was released into the air.

During a press conference earlier this week Eastman officials told said “Hydrogen” and “Carbon Monoxide” were two agents emitted when the explosions happened.

In an emailed statement an Eastman spokesperson said the amount of Hydrogen Sulfide released was “significantly less” than is required to be reported to the government and that the company believes, “The small amount of Hydrogen Sulfide was consumed at the time of the explosions and therefore did not leave the plant site.”

Hydrogen sulfide — a potent gas that lingers around sewers, wastewater treatment plants, oil and gas water tanks and wells — is found where organic material decomposes, said Wayne Vanderhoof, a workplace safety expert and president of RJR Safety Inc., based in Claysville, Washington County.

Mine Safety Agency Issues Serious Accident Alert On Safe Handling Of Chemicals

MSHA highlighted a November 3, 2017, accident in which an explosion occurred when incompatible chemicals were mixed in a tank. A miner had unloaded a truckload of sodium hydrosulfide (NaHS) solution into a chemical storage tank that contained calcium polysulfide (CaPS) solution. NaHS and CaPS are hazardous chemicals that give off hydrogen sulfide (H2S), a flammable and toxic gas that produces a “rotten-egg” odor. MSHA said the operator had installed a charcoal filter on the tank’s vent line to control the odors. “When these chemicals came into contact in the tank, an explosive concentration of H2S gas was liberated,” the agency noted. According to the agency, “The concentration of H2S exceeded the filter’s capacity, resulting in a chemical reaction generating enough heat to ignite the filter. H2S vapors in the tank ignited, causing a violent explosion damaging the tank, surrounding area, and building. This accident occurred because the operator changed its processes, which resulted in the mixing of incompatible chemicals.”

To avoid similar accidents, MSHA issued specific “best practices” for operators to implement. These include:

Prior to implementing a process change, such as the introduction of a new chemical, thoroughly evaluate hazards, including chemical compatibility and reactivity.
Establish procedures for the safe handling and storage of chemicals, ensuring chemical compatibility with the process and equipment.
Equip chemical storage tanks with properly designed pressure relief devices that are safely vented.
Ensure that chemical filters and scrubbers are properly designed and rated for the intended application.
Conduct continuous H2S monitoring in areas where it may be liberated.
Do not use pressurized air to off-load NaHS solution.
Train miners on chemical hazards and emergency response procedures.
Use proper personal protective equipment during chemical handling.
For additional safe practices and precautions when handling NaHS, refer to the U.S. Chemical Safety and Hazard Investigation Board’s Safety Bulletin No. 2003-03-B (November 2004).
In 2004, the U.S. Chemical Safety and Hazard Investigation Board, an independent federal agency investigating chemical accidents, issued a report on “preventing harm” when using sodium hydrosulfide. It noted, “Despite its pungent rotten egg odor, H2S can deaden the nerves that detect odors, thereby preventing those exposed from being able to smell life-threatening airborne concentrations. This condition is referred to as ‘olfactory fatigue’ and must be considered when designing NaHS safety systems.”

Paying heed to the demand of Koparkhairane residents, the civic body will clean the longest Khairane nullah after MPCB tests confirmed unhealthy presence of hydrogen sulphide in the nullah water.
Secondly, NMMC has forwarded MoEF authorized lab test reports of partially treated industrial waste released by some industrial units in the TTC industrial belt to MPCB for the first time.
These units have been releasing hazardous effluents into the city nullahs and NMMC hopes to mount pressure on MPCB to ensure it takes action under Environment Protection Act, against these errant chemical units.
The residents of new sector 11, Koparkhairane, who were protesting for months about polluted nullahs has reason to cheer. City engineer, Mohan Dagaonkar has cleared the proposal of cleaning the longest Khairane nullah. Paulsboro Refinery released 130 pounds of sulfur dioxide and nine pounds of hydrogen sulfide Friday night, according to Philadelphia police.

PBF Energy, which runs the refinery, apologized and said Saturday that a brief loss of power at the refinery shortly before 8 p.m. Friday resulted in "flaring and odors." The refinery is located just across the Delaware River.

The odor was not dangerous, officials said, and no injuries were reported.

The release was not at a level high enough to require reporting by the company, said a police spokesperson.

SOUTH SIOUX CITY — South Sioux City residents who were forced to leave their homes by a horrible odor from a nearby energy plant said they're concerned that one resident was recently sent back to his home before levels of hydrogen sulfide gas were deemed safe.

Chris Cornell was among the residents displaced from his home on Oct. 28 due to the odor from Big Ox Energy. City administrator Lance Hedquist told Cornell he could return to his home last week, before initial tests results had been processed.

"I was shocked," Cornell told the City Council on Monday.

Hedquist said he thought the home was safe based on discussion with other people working on the home, but Cornell said alarms in his home showed gas levels were more than double the recommended range. Gas levels should be lower than 7.17 parts per billion before residents can safely return to their homes, according to the state Department of Health and Human Services.

Other residents also expressed frustration to the City Council on Monday, saying they feared their hotel stays could be cut short like Cornell's situation, even though the odor problem hasn't been resolved. About 14 residents are still staying at hotels.

The pungent, colorless gas may have come from the car battery in the Porsche Cayenne SUV, though officials will need to do more tests to know for sure, said Carrie Proudfit, a spokeswoman for the Orange-Osceola Medical Examiner's Office.

Latifa Lincoln, 46, and Maksmilla Lincoln, 3, were found dead on June 2, after the SUV bumped into a guardrail on Florida's Turnpike and came to a full stop north of mile marker 224 in Osceola County. They were driving to Miami, according to a voicemail Lincoln left before her death.

Their deaths remained unexplained for months as officials investigated and waited for toxicology results.

A Florida Highway Patrol trooper and two Osceola County deputies walked up to the SUV and saw that the two didn't seem to be breathing, according to their reports. The engine was still running. Lincoln was in the driver's seat, and her daughter was buckled into a child's seat in the back.

When oil and gas companies extract fuel from the earth via fracking, they routinely add biocides such as glutaraldehyde to the high-pressure water they use to fracture rock formations deep underground. These compounds are a preemptive strike against microbes that produce hydrogen sulfide, which can corrode pipelines.
New research, however, calls into question the across-the-board addition of toxic biocides to water used in fracking. Jason Gaspar and Pedro Alvarez at Rice University and colleagues, including scientists at energy firm Statoil, performed numerous tests at fracking sites in the Bakken Shale Formation in the north-central U.S. and in Canada. There, they found that H2S might be formed in fracking wells by geochemical reactions, rather than microbes (Environ. Sci. Technol. Lett.2016, DOI:10.1021/acs.estlett.6b00075).
At the Bakken sites, the team found, temperatures at the depths that the biocide-laden water is injected run about 120 °C, too hot for sulfogenic microbes to survive. In addition, DNA tests of injected water samples showed no evidence of microbes, and sulfur isotope analysis indicated that H2S recovered from the samples was produced abiotically.

FERNDALE
The Phillips 66 Ferndale Refinery was hit with a $324,000 fine by the state for several workplace violations revolving around fire and hazardous chemical safety.

The refinery received the fine for violations that took place in 2014 and were not corrected, according to a news release from the Washington State Department of Labor & Industries.

The 2014 citations are under appeal, but state law requires employers to correct hazards even if the violations are under appeal, according to L&I.

The original fines in 2014 totaled $20,500, said Elaine Fischer, a spokeswoman at the state agency. The new fine is a result of state inspectors visiting the refinery in June 2015 and finding that no changes had been made.

The refinery was cited for three violations, each carrying a $108,000 fine. Two involve the refinery’s firefighting and fire suppression systems. According to the state agency, Phillips 66 did not inspect or follow recognized and generally accepted engineering practices for the firefighting water tank or the buried firefighting water distribution piping. The company also failed to address the potential loss of firefighting water, according to the report.

The third violation involved not consulting industry material before writing a policy about chemical piping. Following the company’s policies potentially could expose workers to hydrogen sulfide, a poisonous gas, and other flammable vapors, according to L&I.

Around 160 residents were forced to evacuate Tuesday morning when a well blew out near the intersection of County Road 107-I and County Road 102, just east of Seminole.

It happened around 6:30 a.m. on a Tabula Rasa site in the area. Residents have not been allowed to return as of Tuesday evening.

Wild Well control from Odessa has been called in to try and fix the leak.

The evacuation was necessary because the leak involved hydrogen sulfide, which can be fatal.

Gaines County Sheriff Ronny Pipkin said, "We did evacuate everybody within about a two-mile area. No one is hurt. No one was injured on the well site. There was a pull unit on the well at the time."

The emergency management team in Seminole has opened up the Gaines County Civic Building as a temporary shelter for anyone who was evacuated and doesn't have family in the area.

St John ambulance spokeswoman Victoria Hawkins said the 58-year-old man was in a critical condition and was undergoing decontamination by Fire Service staff at the Grand Hotel Chancellor in Hobson St but died at the scene at 12.10pm on Tuesday.

Ambulance staff had assessed a further 10 patients at the scene who did not need to be taken to hospital.
....
It was too early to confirm whether it was a drug-making operation, Fire Service assistant area commander Dave Woon said. He said authorities were unsure what chemicals were used but that different chemicals had been combined together. The mixed chemicals created a gas, which formed a "rotten egg smell that you would typically find in Rotorua".

Mr Woon said it was a rare situation and he had never seen anything quite like it and said police were now leading the investigation

Several stores were evacuated as a precaution, according to the Kern County Fire Department.

Multi-gas detectors initially found 1 part per million of hydrogen sulfide inside the store. The fire department said 100 parts per million of H2S is dangerous to a person's health.

The department says the trace chemical came from outside, but the exact source is still unknown. Anyone who knows its source is asked to call the Air Pollution Control District at (661) 392-5500.

The fire department said it has received many reports of the smell in north Bakersfield and Oildale.

And in a seemingly unrelated incident, residents in northwest Bakersfield reported a chemical smell that environmental health officials said Thursday afternoon may be at least partially attributable to a 20 gallon spill of naphtha at Amber Chemical Inc., located in north Bakersfield, just west of the 99.

Naphtha is found in oil fields and gives off a pungent odor. Health officials, however, say such a small leak doesn't pose any risk to public safety.

KINGSPORT, TN (WJHL) – An Eastman Chemical Company spokesperson confirmed there was an equipment failure in the coal gas operations Thursday around 11 a.m.

Eastman’s Corporate of Communications Director Betty Payne said the equipment failure led to a quick depressurization of a gasifier and a minor fire occurred in the immediate area.

According to a National Response Center report, hydrogen sulfide — a flammable gas that is listed as extremely hazardous — and carbon monoxide were released in the incident.

The response center reportedly received a call around 12:15 p.m. from Eastman saying that a line ruptured on a processing unit and released a mixture of carbon monoxide and hydrogen sulfide into the air. Eastman told News Channel 11 that no injuries were reported and there is no threat to human health or the environment.

News Channel 11 spoke with a man who says he saw the entire incident. Lee Cowan says he was fishing today when he heard steam blowing off. He told us that he worked at Eastman for 32 years and knew something bad was going to happen. He says he heard an explosion and looked up and saw black smoke and fire.

SAN DIEGO — A man was found dead in a sedan filled with deadly gas at a Sorrento Valley industrial park Thursday in what authorities said was an apparent chemical suicide.

The car was covered in notes warning people to keep away, a fire official said. Hazardous materials crews spent more than three hours neutralizing the gas and cleaning up the potential danger to others.

Authorities were called out to the parking lot at Miramar Road and Production Avenue about 4 a.m. by a Starbucks employee headed to work, San Diego Fire-Rescue Department Capt. Joe Amador said.

The employee was parking his car at the back of the lot when he noticed a man slumped over the steering wheel of a car and a strange chemical odor. Amador said the outside of the car was plastered with printed signs saying, “Keep away,” “Danger, do not open doors,” and other warnings.

Police and firefighters cordoned off access to the parking lot, closed off Production Avenue and called a hazardous materials team.

Amador said there was a bucket of hydrogen sulfide on the car seat next to the man. A few breaths of the chemical will kill a person, he said.

Something stinks in Manassas.
City water and sewer officials say they’re working hard to deal with a foul odor resulting from wet weather and a chemical found in waste water generated from the nearby Micron microprocessor plant.
Residents of Sandy Court, who live in a subdivision near Godwin and Battlefield drives, have complained to the city about a bad smell described as a combination of sewage and spoiled eggs.
Tony Dawood, deputy director of the city’s water and sewer department, said the smell is coming from a nearby side storage pond that fills with stormwater during significant rainfall. The pond also serves as a temporary holding point for runoff from the nearby Micron plant.
That effluent contains the chemical hydrogen sulfide, created in the Micron manufacturing process, that discharges the odor when the chemical transforms from a liquid to a gas.
Dawood said the odor is not harmful because gas levels in the area are well below limits that would cause concern. Still, he said the city understands the smell is a nuisance and is working to make it better.

Environmental Treatment Solutions (ETS) director Jock Germany could not be reached for comment about Thursday’s 25-metre chemical spill, which forced the evacuation of St Joseph’s Catholic School and hospitalised eight workers and a police officer when they were overcome by fumes.

The Environment Protection Authority (EPA) and WorkCover are investigating the incident.

EPA south director Gary Whytcross said the investigation would take several months.

“The EPA will inspect the ETS premises, require a report from the company on the incident, interview ETS staff and where required issue a statutory notice to the company to provide specific information,” he said.

“Initial investigations suggest that the gas emission is thought to have been hydrogen sulphide.”

Hydrogen sulphide, commonly known as rotten egg gas, is highly toxic and explosive.

A WorkCover spokesman said the fumes were created by a chemical reaction during the neutralisation of hazardous substances at the centre.

The business is barred from using the process while investigations are underway, but can continue other operations.

It is the latest in a long running battle over the plant’s handling of coal ash and scrubber waste, despite past fines and $1.5 million spent by LG&E to fix problems involving dust and odors for a coal-fired plant that is scheduled to close next year. The neighbors also sued the company late last year.

Air pollution control district officials were out there on Friday, and when residents continued to complain about persistent rotten egg smells Saturday, one official notified the health department and called on a hazmat response to measure hydrogen sulfide levels.

The Justice Department says Bowman admitted to not properly protecting employees from exposure to hydrogen sulfide, a poisonous gas. Truck driver Joey Sutter died in December 2008 after exposure.

Bowman also admitted that he told employees to falsify transportation documents to hide that waste water was coming from his company, which was in operation from 2008 to 2010.

(STMW) — A hydrogen sulfide leak at a southwest suburban oil refinery in March was the subject of a complaint filed in court by state and Will County officials hoping to prevent a recurrence.

The complaint was filed in Will County Court on Thursday against ExxonMobil Oil Corp., where the alleged air pollution violations occurred March 1 at a refinery near Interstate 55 and Arsenal Road in unincorporated Channahon, according to AG’s office spokesman Scott Mulford.

An equipment malfunction at the Channahon Township facility caused an “uncontrolled release of hydrogen sulfide that lasted more than 9 1/2 hours,” the complaint, filed by Attorney General Lisa Madigan and Will County State’s Attorney James Glasgow, charges.

It seeks to force ExxonMobil to take immediate corrective actions to prevent a similar occurrence, and asks for civil penalties of more than $50,000.

They put their faith in their boss and in OSHA, but mostly in their boss, that workdays will be merely dangerous, not fatal.

But sometimes they still come home dead, the tank workers. Joey Sutter did. He was 36, married, with a son, when he died Dec. 18, 2008.

Within seven months there were two others, all working tanker trucks, all employed by Matthew Bowman, former president of Port Arthur Chemical and Environmental Services, and also Chemical and Environmental Services of Houston.

On Thursday, Matthew Bowman stood at the lectern in a blue and dark wood courtroom in the Beaumont federal courthouse with his hands free, unlike the defendant who preceded him, and pleaded guilty to causing Sutter's death.

Moments before, the mood in the staid court was collegial. Defense counsel joked about ex-wives; the government man mentioned an acquaintance who's become a preacher.

A worker died Friday after breathing hydrogen sulfide fumes while cleaning large-diameter pipes at a wastewater treatment plant Thursday at the Port of Tampa.

Officials say seven employees of Lanzo Lining, Inc. were cleaning the 96-inch pipes at the Howard F. Curren Advanced Wastewater treatment plant when hydrogen sulfide fumes appeared inside a sewer pipe.

Two workers inside the pipe were overtaken by fumes.

Two other workers went into the pipe to help their co-workers, when they too were overcome by fumes. The remaining workers attempted to render aid to the other four, but were also affected by the fumes.

Tampa Fire Rescue responded to the scene and transported a total of seven workers to Tampa General Hospital for medical treatment.
