    <dc:creator>dchas</dc:creator><description><![CDATA[At least five people were killed, one was missing and another one was injured after an explosion rocked China's chemical factory on Monday morning, reported The Global Times. On Monday, the incident took place at the chemical hydrogen peroxide production area in China's Liaocheng, Shandong province, reported The Global Times. Investigation is underway to determine the reason behind the blast. Meanwhile, according to another local newspaper, the fire had been put out as of Monday afternoon. The person, who has been injured, was rushed to the local hospital for treatment while rescue work is still underway.
Fire explosions recurring in China 

This was not the first time that China has witnessed an explosion. Earlier in January, five people died after an explosion occurred at a chemical company in Panjin, Northeast China's Liaoning Province on missing on January 16. Rescue teams had arrived at the scene of the accident and a search operation has been launched for people who may be trapped, reported The Global Times.  As these incidents keep on recurring in China, The Ministry of Emergency Management has continued to guide the on-site rescue and treatment of those injured. In the January explosion, Wang Xiangxi, minister of Emergency Management, had made arrangements and called for all-out efforts to rescue the missing and prevent secondary accidents. Meanwhile, local authorities have asked the officials to adhere to the principle of safety first and learn lessons from the accident. At this incident, which took place in January, 20 ambulance vehicles and 240 medical personnel and other staff were at the site, as per media reports.

The chemical spill was reported around 10 a.m. The call came from Maple Springs Laundry at 161 30th St. Place NW, Long View Town Clerk Heather Minor said in a news release.

Responding agencies reported seeing a vapor cloud and an active chemical leak. Additional resources were dispatched to assist. A perimeter was set up around the spill to remove the public from harm. Further investigation revealed that the leak was hydrogen peroxide from a stationary tank, Minor said.

The tank was located beside Maple Springs Laundry. A tanker truck was filling the tank when the leak occurred, she said.

The leak was contained around noon. A hazardous materials team from Catawba County stopped the flow of hydrogen peroxide into the creek downhill from the business. Proper environmental authorities were contacted regarding the spill and the hydrogen peroxide released into the creek as a result, Minor said.]]></description>

The evacuations were in response to a report from the University of Delaware’s Environmental Health and Safety office. The Delaware State Police investigated the materials, removed them from the building and transferred them to a safe outdoor location where they were ‘neutralised’ on the evening of 8 February, the university announced.


‘This was an isolated incident, and there is no threat to campus health or safety,’ the University of Delaware said. ‘No injuries were reported.’ The school declined to release further details of what was exploded and why or what reaction was being run. Chatter on social media suggested that a chemistry graduate student accidentally made triacetone triperoxide, also known as TATP, which is synthesised from acetone, hydrogen peroxide and an acid. A graduate student at the University of Bristol inadvertently made 30–40g of TATP in 2017, requiring a bomb disposal team to dispose of the material.

‘Our safety people did a top-notch job and responded quickly. It was just so sudden and unexpected,’ tweeted Mike Axiotes, an undergraduate student in the University of Delaware’s chemistry department. ‘We were talking about vinyl chloride burn in subgroups as a safety/making reactions count talk and then boom (pun intended) we’re being evacuated 3 hours later.’

    <dc:creator>dchas</dc:creator><description><![CDATA[A safer, convenient, and scalable synthesis for 3,4-bis(4-nitro-1,2,5-oxadiazol-3-yl)-1,2,5-oxadiazole-2-oxide (BNFF or DNTF) is described. The obtained products were fully characterized and further confirmed by single-crystal X-ray diffraction. The reaction process of tandem nitration–cyclization was optimized, and the thermal stability of the optimal reaction system was studied. 3-Amino-4-(carboxymethyl)furazan is oxidized and then treated with dilute mixed acid (HNO3 and H2SO4) to obtain BNFF in a yield of 52% and a purity of 99% after liquid chromatography analysis. Compared with some previously reported multistep methods that relied on high-concentration hydrogen peroxide solutions and suffered from dangerous exothermic profiles, this new method used to synthesize BNFF promises to be safer and more efficient.

    <dc:creator>dchas</dc:creator><description><![CDATA[A truck tractor semi-tank trailer rollover crash that occurred around 2am this morning and resulted in a chemical spill has closed down State Highway 36, just three miles west of Rising Star.

According to a press release from the Texas Department of Public Safety, a truck tractor semi-tank trailer was transporting a cargo tank containing hydrogen peroxide when it lost control and overturned on the roadway. The vehicle skidded on its side, causing the tank to rupture and spill some of its chemical contents.

State Highway 36 has been closed for cleanup and motorists are being urged to avoid the area if possible. Anyone traveling near the crash site should expect delays. Traffic traveling East on State Highway 36 is being detoured at Cross Plains, and traffic traveling west is being detoured at Rising Star.

At least 51 people, including 10 firefighters, were killed and over 200 injured in the fire and massive explosion at the depot on the outskirts of the country’s port city Chattogram on June 4. The fire was doused on June 8.

A member of the committee said that they had made 20 specific suggestions in their report regarding fire control at inland container depots.
The divisional commissioner told reporters that they would send the report to the ministry and the ministry would take further decisions.

A deadly blaze and several massive explosions have left 48 people dead and nearly 450 injured in Sitakund, near the port city of Chittagong, Bangladesh. The fire began on the evening of 4 June, and took 86 hours to douse with the army’s assistance. The explosions killed ten fire service personnel, with a further three among those still missing. DNA tests have been carried out to identify most of the bodies.

Explosions caused by stored hydrogen peroxide threw multiple containers into the air and released deadly fumes
The incident took place at BM Container Depot, a private facility storing clothing and other items for export. The exact cause is being investigated, but hydrogen peroxide appears to have played a significant role. The chemical was being stored in containers – allegedly without authorisation – destined for use in the textile industry. As the fire spread, eyewitnesses said that multiple containers exploded and were launched into the air. Clouds of toxic fumes formed, causing burns, eye injuries and deaths.

‘The massive explosions have traumatised the population the scale of which was hardly ever seen in Bangladesh,’ says Muhammad Ali Shahin, an environmental activist with the non-profit Young Power in Social Action based in Chittagong. ‘Nearly 200 people got critically injured and deaths are still rising, especially among badly burned people.’

Civil society organisations have called the fire a ‘structural murder’ and blamed the incident on Bangladesh’s apalling industrial safety record, poor infrastructure and institutional preparedness at odds with international safety standards. Activists say that no lessons have been learned from numerous previous accidents in Bangladesh. They expressed anger that eight BM employees were arrested by the police for negligence rather than the facility’s owners.

This article attempts to answer this question by describing how invention education first took root in a chemistry class at Ridgewood High School in Ridgewood, NJ, and then expanded to other students with diverse interests to find a solution to two worldwide problems: satisfying the need for safe drinking water and reducing plastic waste. Specifically, it describes the students’ journey step-by-step during the COVID-19 pandemic to invent a self-sanitizing and sustainable Solar Aqua Tech water bottle. The concept underlying this invention was to quickly produce hydrogen peroxide (H2O2), a well-known sanitizing agent, on-demand within the confines of a bottle using only water, dissolved oxygen (O2), and solar power to eliminate pathogens in untreated water. Students had to convert this chemistry classroom concept into reality without a prescription and without knowing if their goal was even attainable. Since the journey did not take a linear path but veered in different directions as unexpected problems arose, the students sought guidance from academics, mentors, and local experts. Along the way, they acquired new knowledge with deepened understanding and overarching perspectives. Success was achieved only after many setbacks. In the end, the sense of accomplishment for finding a solution to two global problems instilled confidence in the students that, with empathy, perseverance, and resilience, they are equipped with essential skills to tackle 21st-century challenges. Invention, therefore, can be a valuable complement to the standard chemistry curriculum and is unparalleled in sparking a lifelong passion for learning.

"Please be careful with any chemical you have around your house," the Nephi Fire Department said on its Facebook page. "This was a large volume of hydrogen peroxide."

Neville Marshall, of Conister Road in Douglas, admitted driving a vehicle with an unsecure load on Ballafletcher Road on 12 August last year.
The court heard he was a self-employed driver for a haulage firm and had not loaded the chemicals on to the curtained trailer himself.
Magistrates fined the 65-year-old £400.
He was also ordered to pay £500 in costs and given three penalty points on his licence.
The court heard he had been working as a driver for Graylaw and was given a job delivering six 1,000-litre containers to the Isle of Man Creamery in Tromode.
But when he tried to deliver the load to the creamery it was rejected as staff at the site did not have the machinery to offload the containers.
Three crates, which contained potassium borate, hydrogen peroxide and sodium hydroxide (caustic soda), fell off the back of the vehicle as he exited the site onto Ballafletcher Road, with one spilling some of its contents onto the carriageway.
The defence said he had played no part in loading or securing the chemicals onto the trailer he had been towing as that was done by employees of the haulage firm.]]></description>

Student Anoop Kumar a Form 4 student then at Sarava High School was rushed to the hospital after the experiment caused an explosion at his school science laboratory.

The incident which was reported by The Fiji Times on this day in 1978 had raised a lot of concern about the science course offered at schools.

According to the principal education officer science, Devakar Rao on Tuesday March, 7 the experiment was deleted from the Fiji Junior exam course because it was too dangerous.

Chemistry teacher Satend Singh was also injured from the explosion.

This newspaper reported that Kumar had received cuts from flying glass but was reported recovering at Lautoka Hospital after having glass fragments removed from his chest.

The experiment had involved mixing potassium chlorate and manganese dioxide to produce oxygen.

Mr Rao said that three years ago he had sent a circular to science teachers telling them to delete it from the syllabus because it was dangerous.

He had left them with the choice of preparing oxygen from either hydrogen peroxide or potassium permanganate — both considered to be safe methods. He said he failed to understand why the old and dangerous method was still being followed at the Sarava School.]]></description>

According to KFD, "multiple visitors" to the Fish Hospitality Pantry at 122 W. Scott Ave., in Northwest Knoxville picked up Tuesday one-gallon jugs of a hydrogen peroxide mix.

"The liquid mixture contains 50% hydrogen peroxide and is not intended for household or community use. The highly concentrated oxidizer poses a significant risk of first- and second-degree burns upon contact with skin," KFD said in a Wednesday release.

It appears 54 jugs went out and probably have ended up in the homes and vehicles of members of the public.

The powerful explosion occurred at around 8:51 p.m. at a chemical factory in western Incheon while hydrogen peroxide was being loaded into the truck. 

Seven workers were injured and taken to the hospital for treatment, while one was found dead early on Wednesday. 

The explosion did not cause a fire, but damaged parts of the building and nearby factories. 

Fire authorities mobilized some 50 fire trucks and equipment along with more than 140 people to contain the explosion. They along with the police plan to conduct a joint inspection of the site on Wednesday to find out the exact cause.

But on Monday, she experienced a first: Rather than getting a new respirator mask as she started her shift, she was given one she had worn before — decontaminated in a cutting-edge process meant to help address the dangerous equipment shortages that have plagued hospitals in the battle against the coronavirus.

There was, Mattola-Kiatos acknowledged, “a little bit of uncertainty not knowing how the whole process was going to work. Was it going to smell differently, was it going to feel differently? It’s actually better for me than taking a brand new one out of the box because I’ve already molded it to my face. It’s like it was custom-made for me.”

As part of a bold initiative involving hospitals across the state, MGH on Monday began distributing thousands of freshly decontaminated N95 masks to health care workers after the equipment went through an elaborate cleaning process at a site now up and running in Somerville. The treatment takes place inside a giant decontamination machine owned and operated by the Ohio nonprofit Battelle. Hospitals plan to use the machine to alleviate critical shortages of respirator masks for workers battling the coronavirus pandemic. At its peak, the system will be able to treat up to 80,000 masks per day — a prospect hailed as a “game-changer” for the region.

The Syracuse Fire Department’s Hazardous Materials Response Team was called to assist after the drum was found in the chemical room at the manufacturing plant. The drum contained 50% hydrogen peroxide and was bulging unsafely, officials said.

The building was evacuated and no workers or citizens were exposed, officials said. The chemical room also contained plastic drums of methanol, so crews said they exercised extra precautions.

The hazmat team depressurized the bulging drum within an hour, halting the threat. No one was injured, officials stated.

    <dc:creator>dchas</dc:creator><description><![CDATA[In 2019, the most-viewed video on TikTok—a short-form mobile video platform—was a chemistry experiment: a massive demonstration of the hydrogen peroxide–potassium iodide reaction known as elephant toothpaste. To date, the 12 s video of four mad scientists in David Dobrik’s backyard pouring four buckets of a mysterious yellow fluid into a giant test tube of blood-red liquid has 183.7 million views on TikTok. The yellow fluid, of course, was potassium iodide, the catalyst that rapidly decomposes hydrogen peroxide. In the video, the potassium iodide was added to a mixture of hydrogen peroxide, water, soap, and red food coloring to create water, iodine, and oxygen gas, which gets trapped in the soap to create the characteristic foamy eruption.

With that TikTok, Dobrik and Nick Uhas, another scientific social media maven, set out to make a world record for the amount of foam produced from an elephant toothpaste experiment. However, after the video went viral, there was social media controversy about the legitimacy of the record-breaking eruption. So, for their next attempt, the duo committed to quadruple the (approximately) 67.5 m3 of foam produced in the viral TikTok clip.

In a video with over 22 million views on YouTube, the scientists put a whopping 625 L of 35% hydrogen peroxide (in addition to the soap, water, and food coloring) in a 1,893 L plastic drum. They then rigged a pulley system, which involved placing a trash can on top of a lifeguard chair, to efficiently dump the 91 L of potassium iodide catalyst into the drum. In the video, a dramatic exothermic reaction is shown, producing approximately 200 m3 of foam.

It might look cool, but the Newscripts gang felt compelled to consult some safety experts.

“I am familiar with this stupidity,” David Katz, a chemistry educator and demonstrator, tells Newscripts. He says that the 35% hydrogen peroxide can cause severe burns.

Furthermore, “the blobs of foam seen floating down the valley could contain unreacted hydrogen peroxide,” which could hurt innocent bystanders, says Sandra Koster, a retired senior lecturer from the University of Wisconsin–La Crosse.

Yikes! Want to try an elephant toothpaste experiment at home? Stick with the safer 3% hydrogen peroxide you can buy at the drugstore. While yeast won’t produce the same dramatic explosion, using that as a catalyst will lead to a slower buildup of foam and be less likely to permanently stain walls. And always, always, always use personal protective equipment.

Fire crews, police and paramedics rushed to the Vicarage Road hospital just after 11am last Thursday (February 13) after a chemical leak was reported.

The site was temporarily closed and clinics postponed while firefighters worked to contain the spillage which hospital bosses say happened in a "secure, temperature controlled cabinet".

West Midlands Ambulance Service said paramedics assessed 10 people "who were showing symptoms relating to the incident" - nine of whom were discharged at the scene, while the tenth, a woman, was taken to Russells Hall Hospital as a precaution and was discharged the same day.

Diane Wake, chief executive of the Dudley Group NHS Foundation Trust, said: "Our staff followed all the appropriate procedures and evacuated staff and patients in that area affected. There was no risk to the public."

She confirmed the spilled liquid was hydrogen peroxide which she said "is used for cleaning and sterilising scopes" and which can cause eye and throat irritation, coughing and breathing difficulties if inhaled.

Our labs recently reported on the discovery of oxygen evolution during the LiOH/H2O2-mediated cleavage of Evans oxazolidinone. Faced with this previously unexpected hazard, the authors, herein, report on the development and demonstration of a methodology for the safe scale-up of this O2-releasing reaction. It was found that O2 in the headspace could be controlled by a combination of N2 sweeping and modulation of the LiOH addition rate. A design of experiment study was used to probe the multivariate parameter space in order to minimize formation of the unstable peracid intermediate, thus avoiding accumulation and subsequent risk of rapid and uncontrolled O2 release. To support scale-up, the well-mixed vapor headspace assumption was tested using computational fluid dynamics, and it was found that N2 sparging is effective at reducing heterogeneity in the headspace. Condenser conditions were evaluated to balance recovery of solvent—because of the high N2 sweep—and avoid the risk of condenser freezing—because of the presence of water in the condensate. An additional fail safe was developed, using a pH buffer to rapidly halt the base-catalyzed reaction in the case of an unexpectedly high headspace O2 concentration. The final process was validated and executed at up to 215 kg scale.

The Country Fire Service (CFS) issued a warning to people at Angas Plains and surrounds following a chemical spill at a commercial property on Tuesday, September 3.

Eight CFS resources, along with SA Police, the Metropolitan Fire Service and the Environmental Protection Authority, attended the site on Davidson Road, where two 1000-litre containers of hydrogen peroxide were spilt in a shed.

A CFS spokesperson said someone on the property saw white fumes coming out of the shed and called emergency services at about 11am.]]></description>

Firefighters responded to Deer Island at 7:20 p.m. for reports of one gallon of hydrogen peroxide leaking, officials said. The leak was classified as Level 3, which is the most dangerous hazmat situation.

Firefighters in hazmat suits shut down the leaking valve, vented the building and monitored the air quality, according to fire officials.

Northbound traffic was blocked between Hasley Canyon Road and Parker Road reopened just after 6 p.m., according to the California Highway Patrol.

The spill was reported to firefighters around 2:30 p.m., said Supervisor Melanie Flores with Los Angeles County Fire Department.

Officials were advised that the truck was leaking hydrogen peroxide. Although they were told the spill was about 12 to 15 gallons, Flores said it was larger, with about a gallon leaking every five minutes.

It happened around 5:30 a.m. east of New Boston.

The Texas Highway Patrol said the driver of an 18-wheeler hit the brakes to keep from hitting a car that swerve in front of him.

When the driver tapped the brakes, all of cargo shifted forward and three containers inside the trailer broke open.

Officials said about 1,000 gallons of hydrogen peroxide spilled onto the ground.

"It's not a danger to the public. It's a small percentage, 5 percent peroxide. The worse it will do is irritate your skin. It's a light corrosive," explained Scottie Taylor, Bowie County Deputy Fire Marshal.

"Finally got a GSP helicopter in here and they flew over the scene and there's crews down there now. It appears that they have 6 train cars that have left the track," said Mayor Robert Morris.
 
The calls came in close to 8:30 p.m. on January 6th. The derailment caused Hydrogen Peroxide to be released into the air. Those who live in Bartow were told to evacuate immediately.
 
Mayor Morris said, "Stay out until we tell you to come back. This is going to be an on-going event and no telling how long it's going to last."
 
At 2:00 a.m. on January 7th, Jefferson County EMA Director Jim Anderson confirmed the evacuation radius of Bartow shrunk from seven miles to three.

A truck delivering the chemical had backed up to the building when a leak was discovered in a 330-gallon tote, Martinez said. The main building and a neighboring building were evacuated, affecting 10 to 15 people.]]></description>

The ninth and tenth floors of the hotel were evacuated while officials assessed the situation.

Students and staff at the school were advised to stay inside as a precaution. 

According to officials, the spill happened around 8:30 a.m. at a town sewer pump station near Critzer Elementary School. 50 to 100 gallons of hydrogen peroxide leaked from an onsite chemical storage tank while being filled by a third-party contractor. 

The chemical spill has been contained and clean-up is underway.

A Town of Pulaski spokesman said it doesn't appear that nearby streams or groundwater has been contaminated.

The leak took place at a 1,000-gallon tank diluted with hydrogen peroxide. The Newark Fire Department estimated that 200 gallons of the non-flammable chemical spilled during the incident.

The incident occurred at Aramark Uniform Services at 110 Glenn St. as a chemical, hydrogen peroxide, was being offloaded from a truck, Fire Chief Brian Moriarty said. 

As the unidentified male worker was unloading the chemical from a truck, the fill pipe broke and hydrogen peroxide splashed into the worker's face and spilled onto the floor, Moriarty said. 

The injured man, who appeared to be his 30s or 40s, was taken by ambulance to Lawrence General Hospital for treatment. His condition was unknown but Moriarty said he suffered burns to his face. 

Due to the chemical spill, a Tier One response, the lowest response, was activated. Members of a regional team of firefighters trained in hazardous materials clean up response and clean up went to Aramark, Moriarty said.

The investigation showed that there was no explosion or fire; the accident was a release of hydrogen peroxide and sulfuric acid being mixed to make commercial grade disinfectant.

Buildings surrounding a radius of 600 meters were evacuated as a standard precaution and the area will remain evacuated until air monitoring on the property is completed.One employee suffered minor injuries, he was taken to a local hospital and is expected to be OK.

The FFD hazardous materials crews are conducting air monitoring operations to ensure the environment is safe prior to releasing the building back to the owner.]]></description>

The three Coastal Processing workers were exposed to PAA, which Jefferson County EMS Director Maj. Chad Parker said is used as an antimicrobial. The chemical is a combination of peracetic acid, hydrogen peroxide, acetic acid and water.

Jefferson Hospital CEO Lou Semrad said all three were treated and released. Semrad confirmed the individuals were decontaminated.

“When they use it to clean chickens, it’s in a diluted form. They were exposed to a concentrated version,” Semrad said.

Jim Anderson, the county’s EMA/911 director, said the chemical leak would not require a report to the Occupational Safety and Health Administration .

According to Rajashekar Puranik, Environment Officer at the board, about 1,000 litres of hydrogen peroxide was sprayed on the water from 4 p.m. to 6.30 p.m. on Saturday. It acted as an oxidiser to increase the dissolved oxygen level in the stagnated water making it diluted.

He told presspersons at the dam site on Sunday that as a result of the chemical spray, pollutants in the stagnated water have diluted and its black colour has diminished. A preliminary report from the National Institute of Technology-Karnataka (NIT-K), Surathkal, has said that the dissolved oxygen level in the stagnated water was below one mg per litre against the normal 3.5 mg per litre.

Mr. Puranik said that the NITK, the board and the College of Fisheries would submit three reports on the contents in the polluted water to know the reasons for pollution. The reports are expected in three days.]]></description>

Hydrogen peroxide vapor was visible, and dangerous, because of its concentrated form.

“He’s hauling approximately 4,5oo  gallons of hydrogen peroxide at a 50 percent ratio, which is a pretty stout mixture of hydrogen peroxide. Hydrogen peroxide at this mixture can burn your skin, infect your lungs, where you can’t breathe and can put eyes out,” said Curt Clayton, director of Union County Emergency Management Agency.

Clayton helped coordinate the response and cleanup. As soon as authorities determined that neither the truck’s driver, nor anyone else was injured, firefighters began dousing the leaking chemical with water to dilute it.

In the meantime, authorities shut down all four lanes of I-22 between Glenfield and Myrtle as environmental experts came up with a game plan to clean up the spill, offload the chemical into another tanker, and open the roadway.

“This is a major thoroughfare, a lot of economics involved in this, we are going to try and get this highway open as soon as possible for the citizens, and traveling public,” said Ernie Shirley, with Mississippi Department of Environmental Quality.

Traffic was diverted onto Highway 178. The two-lane highway is not built to handle a large influx of vehicles.

More than 160 farms resorted to the chemical in 2015 to tackle parasites such as sea lice, according to the Scottish Environment Protection Agency (Sepa).

Enough hydrogen peroxide was used to fill eight Olympic-size swimming pools, including more than 1m litres at a site operated by Marine Harvest, the Norwegian-owned firm.

Hantsport deputy fire chief Paul Maynard said that at about 9:30 p.m., hydrogen peroxide and sodium hypochlorite were accidentally mixed together. About 1,500 litres, most of that hydrogen peroxide, was involved.

The reaction was causing a gas to form when Hantsport fire crews arrived on the scene. They called the Kings County hazardous materials team, which is composed of firefighters from Kentville, New Minas and Wolfville, to assess the situation.

“What (mixing of the chemicals) does is cause a reaction that produces oxygen and a lot of heat,” Maynard said at about 12:30 a.m. Thursday. “It's not overly dangerous, but with an oxygen-rich environment, we have to watch for ignition sources.”

Among them, one man had direct contact. He was injured in the face and ear. One woman felt sick after inhaling the gas leaked by the spill.

The two were sent to hospital around 13:15 pm after being disinfected in a mass decontamination pod. The other six felt unwell but recovered after they were given first aid.

A total of 112 people were evacuated from the building.

The Fire Services Department (FSD) had mobilized 13 fire engines and seven ambulances, according to a spokesman.

An investigation was launched in this regard, according to the department.

The police received a call at 11:33 am about a suspected chemical explosion on the second-floor lab of Beijing Genomics Institute-Hong Kong on Tai Fu Street, according to the spokesman.

He said it is suspected the incident was caused by a spill of expired hydrogen peroxide. Whether an explosion happened remains to be confirmed, the spokesman said.

Three medical workers were decontaminated after contracting injuries.

According to officials, a local pool-maintenance worker mistakenly added 160 liters of hydrogen peroxide to the waters on August 5, which partially neutralized the chlorine used for disinfection. With chlorine disarmed, the officials said that “organic compounds”—i.e. algae and other microbes—were able to grow and turn the water a murky green in the subsequent days.

The revelation appears to contradict officials’ previous assurances that despite the emerald hue, which first appeared Tuesday, the waters were safe.

“Of course it’s an embarrassment,” Gustavo Nascimento, director of venue management for the Rio Olympics, told the New York Times. “We are hosting the Olympic Games, and athletes are here, so water is going to be an issue. We should have been better in fixing it quickly. We learned painful lessons the hard way.”

Nascimento said it took a while to figure out what happened because hydrogen peroxide wasn’t supposed to be used at all, and it was not detected by initial tests. “The electronic monitoring system that measures the amount of chlorine in the water was betrayed by this chemistry,” he said. Officials had previously blamed the pools’ swamp-like shade on algae, people, and a shortage of an unnamed chemical.

The mixture created a noxious cloud forcing the resident out of the home.

Two residents at the home safely evacuated; they were checked by medical personnel but declined treatment. Crews monitored the air, ventilated the home and tested the substances that were mixed.

Chester County Chief HazMat Coordinator Jeff Emmons said the resident grabbed the wrong bottle causing the accident. He said the HazMat team is called for these kinds of accidents throughout the county every couple of months. Emmons recommended that county residents should clearly mark home chemical bottles and not mix chemicals unless called for in the instructions.

The fire was created by a combination of hydrogen peroxide and cleaning fluid, possibly ammonia, according to local officials.

“They were burning some feed sacks and evidentally some of the embers got into the building and ignited some of the combustible materials,” West Stanly Fire Department Chief Kenny Crump said.

WSFD was on the scene at the farm from 11:30 a.m. to around 4 p.m., along with units from Oakboro, Ridgecrest, Midland and New Salem volunteer fire departments — around 40 responders total.

The N.C. Hazardous Materials Regional Response Team of Charlotte was called in because of the amount of hydrogen peroxide involved, along with cleaning up the hazardous materials spilled and containing excess unspilled liquids. More than 350 gallons were stored in the containers; however, not all of the chemicals spilled, Crump said.

Crews will be working through the night to safely remove about 400 to 500 gallons of the highly-concentrated chemical.

“When something like this happens, it's quite a shock to the folks here,” said Peroxy Site Manager Joe Ainsworth.

It happened at about 12:45 p.m. at PeroxyChem in the 12000 block of block of Bay Area Blvd.
According to Pasadena police, about 1,000 gallons of an oil-based cleaning solution leaked, causing the explosion.

“Plant employees and contractors were conducting a routine function when the contractor’s equipment exploded," PeroxyChem officials released in a statement.

Two PeroxyChem employees and a contractor were taken to a nearby hospital for treatment. One victim suffered a broken arm and two needed treatment for chemical exposure, police said.
Another contractor died at the scene.

“It's a chemical they work with all the time. It's not significant toxic properties, but it's a precaution we send them to the emergency room anyway to have them treated and looked at,” said Ainsworth.

The plant which was 90 acres produces hydrogen peroxide. The site manager said it has an,"outstanding safety record."

There were no injuries in the incident on Interstate 20/59 northbound, which began when two trucks collided at around 6:18 p.m. and one of them began leaking chemicals, police said.

The truck, carrying hydrogen peroxide, sodium hydroxide and sulfuric acid, later exploded, sending a huge fireball into the sky, NBC affiliate WVTM reported.

An area within half a mile of the wreck was evacuated, according to WVTM, and northbound lanes of the highway were shut down.

    <dc:creator>dchas</dc:creator><description><![CDATA[CEDAR RAPIDS — Pieces of pumpkin went flying into the air inside Thomas Commons at Cornell College Tuesday morning, grabbing the attention of students and faculty walking by.

That was the exact reaction the Cornell College Chemistry Club was looking for.

“I think if you tell people there’s going to be an explosion, people are going to come,” Cornell College Chemistry Club president Bethany King said.

The club decided to celebrate National Chemistry Week by using a chemical reaction to explode pumpkins. King said the group used a combination of Hydrogen Peroxide, Calcium Carbide and water. She said that combination produces flammable acetylene gas and calcium hydroxide. After a minute, the group ignited the gas inside the pumpkin, causing a contained explosion.

King said securing faculty approval for the project wasn’t easy.

“I asked a lot of people on campus if I had permission to do it. I had to asks six people in our department, and then Cornell campus security.”

She said the group practiced with smaller explosions, eventually working their way up to the full explosion.]]></description>

City staff noticed a leak in one of the tanks Monday afternoon, which prompted the school to keep students off the football field and move an activity to another campus’s field.

Mansfield Utilities Director Joe Smolinski said at 2:30 p.m. a plant manager noticed a tank leaking while doing a regularly scheduled inspection. He added the chemical spilled right into a special containment area and no one was in danger.

“We have containment areas around each of the tanks that are out there at the treatment plant,” he said. “If one of those tanks were to leak or rupture it would capture anything that leaks out of it so none would leak into the environment.”

He said the staff followed procedure and notified the fire department and the school. Students were not allowed on the football field, which is adjacent to the tanks, he said.

Fire Chief Barry Bondurant said the normal evacuation radius of a similar leak is 300 feet, not even enough to surpass the water treatment plant’s boundaries.]]></description>

Around 4:15 p.m. on Wednesday, fire, police and haztmat crews were called to NC State Pesticide Storage Warehouse on Reedy Creek Road.

According to NC State Director of Environmental Health and Safety Ken Kretchman, about 5 gallons of  hydrogen peroxide, acidic acid and water leaked onto the ground of the facility.

According to the Florence County Emergency Management/911 Central Dispatch Facebook page, there was a leak of approximately 20 gallons of 50 percent hydrogen peroxide.

There were no injuries and the situation is under control, the Facebook page states.

The Olanta Fire Department, Florence County EMS, the Florence County Sheriff’s Office, the Olanta Police Department and the South Carolina Department of Transportation assisted Olanta Fire with the hazmat spill.]]></description>

“Hydrogen peroxide is a chemical commonly used in bleaching, dyeing, and in paints, among other uses. However, its oxidation capacity is high and it explodes when it comes into contact with metal,” Vijay Dambal, Additional Superintendent of Police, told The Hindu . He added that the chemical was manufactured in Pune in Maharashtra.

Yerriswamy, a rag picker, died after the plastic container he reportedly found in a bush and tried to open the lid using an axe-blade exploded. The high intensity explosion blew off his legs and hands. Leaves in a couple of trees in the vicinity have withered because of the heat following the explosion.

According to Maremma, the victim’s wife, Yerriswamy, had returned home with the blue plastic container and wanted to empty the content to sell the container as scrap, when the explosion occurred.

Mr. Dambal said that three teams had been formed under the supervision of Suresh, Deputy Superintendent of Police, Ballari Rural, and Prasad Gokhle, Circle Inspector of Police, to find who had kept the container and for what reason. “The team will be primarily enquiring with the jeans washing units, paints factories in the city to ascertain whether their chemical stocks were intact or some containers were missing. The team would also verify from where they procured the chemical,” he said.]]></description>

Lake Dillon Fire was called to a laundry facility operated by Keystone on Tennis Club Road at 9:30 a.m., where they neutralized a large bin full of smoking and burning towels using water and soda ash.

Steve Lipsher, public information officer for Lake Dillon Fire-Rescue, said the towels were used to clean up a spill of 35 percent concentration hydrogen peroxide — more than three times the strength of hair bleach — that was used to clean a pool near the facility.

“The workers there mopped it all up using a bunch of towels, which were tossed in the laundry bin,” Lipsher said.

As the wet towels sat in a laundry bin, they began to oxidize due to the acidic hydrogen peroxide, and the cotton in the towels began to smolder.

“And, we had a case of spontaneous combustion going on,” Lipsher added. “It’s kind of like a high school chemistry experiment in some respects.”

The fire was quickly extinguished, and never escalated to a structure fire.

Firefighters evacuated the building, and ventilated it, clearing the area of smoke and toxic vapors. Two workers were treated at St. Anthony Summit Medical Center after they opened the laundry bin to investigate, inhaling some of the vapors and smoke, reporting shortness of breath and skin irritation.

Shortly before 9 a.m., an unknown chemical got into a barrel of hydrogen peroxide and began heating rapidly, according to Kalispell Fire Chief Dave Dedman.
Employees at Valley Linen on Fifth Avenue West were outside by the time fire and police crews arrived. 
“The workers there did the right thing,” Dedman said. “They cleared out and called us.”
The mixture of chemicals had reached 186 degrees by the time the fire department’s hazardous-materials crew arrived. Less than a gallon of liquid spilled.
With people clear of any gaseous hazard, Dedman said firefighters waited for the reaction to finish and made sure the area was safe. They were at the scene for an hour and a half. 
Valley Linen was responsible for disposing of the barrel, he said.]]></description>

Hydrogen peroxide is used in the port’s stormwater treatment process, re-introducing oxygen into the water before it is released into Budd Inlet.

The hydrogen peroxide leak originated in a pipe leading to a large tank at an $11.5 million stormwater treatment plant that opened in December, said port spokeswoman Kathleen White.

Evacuations were ordered for a quarter-mile radius and employees cleared from several nearby businesses while the leak was located and contained, she said.

The evacuations remained in place for about three hours while the Washington State Department of Ecology assessed potential hazards.

"There were no injuries and the site is stabilized," White said.

    <dc:creator>dchas</dc:creator><description><![CDATA[OLYMPIA — A chemical leak at the Port of Olympia forced evacuations of some nearby businesses for about three hours Wednesday.

“At 11:37 this morning, a hydrogen peroxide leak within a storm water treatment containment system at the Port of Olympia today forced the cordoning off of over 20 city blocks,” Olympia Deputy Fire Chief Greg Wright said.

“Several businesses were asked to evacuate and another six-to-eight businesses were asked to shelter in place. The Fire Department called for the evacuations within a quarter-mile of the incident as a precaution due to the potential health risks associated with a large Hydrogen Peroxide release. No injuries were reported by citizens or first responders.

“The origin of the leak was a 3,300-gallon chemical storage tank within the storm water treatment system.  The Fire Department worked with the Port of Olympia and the Department of Ecology during the initial stages of the event.  After determining the extent and potential effects of the spill, all evacuated areas were reopened” at about 3 p.m.

“The Department of Ecology will determine the severity of the spill and any adverse impacts to the marine area,” Wright said. “There are no drinking water concerns as the results of this spill.  The white vapor seen issuing from the spill and storm water system was water vapor.]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[A large drum of hydrogen peroxide was spilled at the Ritz-Carlton hotel in Northwest D.C. on Tuesday, bringing D.C. firefighters to the hotel to clean up the potentially hazardous substance.

D.C. fire department spokesman Tim Wilson and a receptionist at the hotel both said that no one had reported any injuries and that the hotel was not being evacuated.

Wilson said that the spill appeared to be from a 55-gallon drum of hydrogen peroxide.

Though hydrogen peroxide is frequently used, in a watery solution, for cleaning and other common purposes, it can cause burns to skin and can trigger explosions in high concentrations.

Wilson said he was not sure why the chemical was at the hotel, but suggested it might be for cleaning. Hotel officials could not be reached.]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[Piranha solutions are used to remove organic residues from substrates. Typically a 3:1 mixture of concentrated sulfuric acid to 30% hydrogen peroxide, it is highly corrosive and a powerful oxidizer. Simply mixing the solution is dangerous.
And mixing piranha begs the question: Add the acid to the peroxide, or the other way around? Everyone hopefully learned in chemistry labs to “never cover an acid”–that is, when diluting, always add acid to water, not the other way around. For piranha, however, best practice is to add the peroxide to the acid.

No one was injured, though BSF personnel and Pakistani Rangers went into a tizzy after the powerful explosion which saw the debris scattered “in an area of 2000 square yards”.
A magisterial probe has been ordered and the Pakistani side has been informed about the incident, officials said, adding a forensic team from Chandigarh has been dispatched.
