At about 8:55 p.m. ET, a Norfolk Southern train of around 150 cars derailed near the Ohio town of East Palestine, 20 of those cars carrying hazardous materials.

The derailment caused a huge fire and, fearing a massive explosion that would release noxious gases and shrapnel into the surrounding area, emergency responders intentionally breached five cars to let out the chemicals inside.

The chemicals were diverted into a trench and burnt off. Officials warned, however, that it would send toxic gases phosgene—used during World War I—and hydrogen chloride into the atmosphere.

What initially was thought to be smoke from a structure fire Friday night west of Mount Airy was determined to be a “chemical cloud” deemed potentially life-threatening, according to Surry County Emergency Services.

After it was traced to an acid leak, precautions were taken including sending a 911 message to all residents within one mile of the scene advising them to shelter in place with their HVAC systems turned off, which was in effect for several hours.

A report concerning the fire was received by Surry County Communications around 7:45 p.m. Friday in reference to an old abandoned building at 4432 W. Pine St. (N.C. 89), Mount Airy, which was said to be “smoking heavily.”

The location involved is just east of Interstate 77 near its interchange with Interstate 74 in the vicinity of the James River Equipment location and Gene Hill Road.

That report led to the Pine Ridge, Skull Camp, Franklin Community and White Plains volunteer fire departments being initially dispatched to the scene. Firefighters arrived to discover what appeared to be “smoke” blanketing the area and a large portion of N.C. 89 nearby.

Fire personnel quickly learned that the smoke was a chemical cloud instead and notified the Surry County Hazardous Materials Team.

Upon arriving, responders reported an irritating and choking sensation.

The cloud appeared to be “following” the topography in the area due to the moisture present, according to Surry County Emergency Services, prompting the 911 alert to residents.

Meanwhile, the N.C. Department of Transportation was contacted and rerouted traffic around the scene.

Also responding to the incident were the Surry County Emergency Medical Service, Surry County Sheriff’s Office, Surry County Fire Marshal’s Office and Surry County Emergency Management.

Firefighters and hazardous materials technicians were able to survey the building in question and located a leaking industrial tote with a label of muriatic acid, which online sources identify as another name for hydrochloric acid.

It is a corrosive strong acid that can be used for tasks such as removing salt deposits from rock or stucco materials and rust and stains from concrete and brick.

The leaking chemical appeared to be reacting to moisture in the soil along with organic materials, producing a chemical gas that continued to blanket the area. The location was sealed off and hazardous materials technicians were sent in to mitigate the leak and attempt to neutralize the reaction using protective equipment.

That led to the leak being stopped and the initial reaction was slowed, which produced less gas.

Due to size of the leak and the amount of gas still being generated, Ultimate Towing and Recovery was contacted for a site clean-up.

The incident happened at Tsubaki Automotive Division on Lonczak Street.

A Tusbaki employee was treated at a hospital and is expected to be okay. The company said because of their emergency protocols, they were able to keep the spill contained and shut the valve off immediately.

A spokesperson tells Spectrum News 1 the hydrochloric acid was contained in a holding tank. It wasn’t released into the waste water treatment plant and there is no danger to the public.

The spill is expected to be cleaned up within 24 hours and the incident is still under investigation.

A shelter-in-place advisory has been lifted after fire crews responded to a chemical spill in West Chester on Monday.

Crews were called out around 11:18 a.m. to 4544 Muhlhauser Road for a burst chemical pipe at a business, according to the fire department.

Hazmat crews identified the leak as hydrochloric acid and contained the spill for the most part to the building.

Some of the spill leaked into a creek and is now being handled by the EPA.

A shelter-in-place advisory was issued for residents within a half mile of the spill out of an abundance of caution. The shelter in place has since been lifted.

Officials gave an update at 1 p.m. saying crews are continuing to clean up the scene and Muhlhauser Road will be closed for at least another hour.

The truck driver, Omar Pleasont, 35, of Portage, was pronounced dead at the scene. The accident happened shortly after 8:30 p.m.

Pleasont was hauling a tanker filled with hydrogen chloride. The Indiana State Police said in a press release that Pleasont was headed east on the Toll Road in his 2020 Freightliner when for reasons unknown, he drove off the road near the 129.5 mile marker and landed in the road’s center median. Both the truck and trailer rolled over in the center of the road’s median. The force of the accident caused the tractor to separate from the trailer. The tractor eventually came to rest on its roof in the center of the right eastbound lane.

The tanker finally came to rest in the median, badly damaged and leaking the toxic chemical.

After combining with water vapor the hydrogen chloride creates hydrochloric acid, a dangerous inhalation irritant.

CHATTANOOGA, Tenn. — Alex Haws wasn't in school Friday because of the explosion at Wacker, but he was working hard on an extra-curricular project after he witnessed the catastrophe.
"I heard a real loud bang boom. I don't know it was a big old thud, and the school shook," he said.

Soon after he saw steam puff up into the sky. Friday afternoon, Wacker clarified that the chemical released is called trichlorosilane. Wacker officials says when trichlorosilane mixes with moisture in the air, it makes another compound called HCl. That's what they were worried about people breathing in.
Professor Bradley Harris his chemical engineering class is very familiar with HCl.
"As an inhalant, something that's in the air, it's mostly just going to be an irritant," Harris said. "It's going to make you feel uncomfortable. It's going to bother your mouth, your esophagus, your lungs-- make you cough."
That's why Haws created this petition online that now has more than 500 signatures.
"Gas masks should be provided for each student teacher and faculty," he said.
Haws didn't experience symptoms after the explosion Thursday, but wants to know he'll be safe in the future. Even if the school can't afford masks, he says he wants a safety discussion to happen.

Records show the Wacker Polysilicon plant in Charleston was fined $3,500 in April 2016 by the Tennessee Occupational Safety and Health Administration over control of hazardous energy.

In August 2016, a $6,450 fine was issued over process safety management of highly hazardous chemicals and respiratory protection.

Authorities said Wacker's hydrogen chloride release Thursday briefly closed a busy interstate section and spurred shelter-in-place orders.

Bradley County schools were closed Friday.

The Bradley County Sheriff's Office said eight people went to the hospital for related symptoms. Wacker said one employee was in the hospital for observation.

On Wednesday, a spokesman for the Boston Fire Department said a store owner was apparently letting a friend experiment with making fish oil supplements. Large bags of yeast, omega 3 acids and 55-gallon barrels of hydrogen chloride were found in the basement.

The basement area was messy and it took crews a while to make sense of the materials inside, officials said.

Alexander Weixelbaumer: “An ex-colleague didn't secure the valve on his HCl gas bottle correctly. Over night the whole lab was gently flooded with the extremely corrosive gas. The ventilation wasn't strong enough and all electronic equipment was damaged beyond repair.”

Simon Wheeler: “Once inhaled way too much acetyl nitrate while analysing fatty acids. Like inhaling all the malt vinegar in the world in one go!”

Radioactive gaffe

Jacqui Lee Mendez-Johnson: “Spilled radioactive buffer while in the cold room spilling all over myself and shoes; panicked and ran down the hallway requiring the entire wing of the building being blocked off for days for decontamination.”

"The threat is over," Anderson County's deputy emergency manager proclaimed.

It's a claim Littleton had been anxious to make since he learned Monday morning that a truck carrying a leaking tank of hydrogen chloride was discovered on northbound Interstate 85, just a few miles inside South Carolina.

The truck spent the next 55 hours at a weigh station on Mile Marker 9, while Littleton and representatives of more than 20 other agencies plotted a safe solution.

Cleaning and inspection work, led by the Environmental Protection Agency, will continue much of the day Thursday. But for Littleton and most of the crews involved in public safety, the job essentially ended when the contents of the leaking tank were successfully transferred to a safe one late Wednesday morning.

"There is still plenty of work to do --- they'll have to eliminate any vapor in the tank, and they'll have to decontaminate every piece of equipment out here," Littleton said.

ANDERSON COUNTY, S.C. —A hazardous chemical that caused concern on Interstate 85 in Anderson County on Monday, was still leaking early Tuesday, according to hazmat crews at the scene.

About 5:10 a.m., WYFF News 4 crews covering the leak at the weigh station on I-85 northbound at mile marker 9 in Anderson County were told to move back about 200 yards, due to a shift in wind direction.

Crews first responded to the weigh station at 8:30 a.m. Monday, according to Taylor Jones with Anderson County Emergency Management.

Jones said a tractor-trailer at the weigh station is leaking hydrogen chloride, and the plume radius is about 300-feet.

He said the liquid is vaporizing and going into the air. Jones said it would only become dangerous if winds pick up.

Capt. Matthew Littleton with the Anderson County Sheriff's Office expected crews to remain at the scene until sometime Tuesday morning, and called it a "long drawn-out operation."

CLEVELAND — Hazmat was called to a rental car facility near Cleveland Hopkins International Airport Friday evening after a suspicious substance was found in a rental car.
According to Cleveland police, an employee observed white powder in the backseat and truck of a returned rental car around 8:30 p.m. The interior trim and spare tire were missing and a hole had been burnt through the trunk floor.
An employee identified the man who returned the vehicle who was still walking nearby. The suspect, Christopher Frank, was found on a shuttle bus.
Frank told police he was with his girlfriend on Wednesday when they entered an abandoned building and removed 5 glass bottles, 3 of which contained liquid. One was labeled as hydrogen chloride. The bottles, along with some other items, were placed in the trunk of the rented vehicle.
Frank told police he later noticed a strong odor coming from the vehicle that made breathing difficult. He found two of the bottles containing liquid had spilled in the trunk.
The couple drove to a car wash and sprayed water in the trunk. A friend told Frank to apply baking soda. They then discarded the bottles, interior parts and a spare tire into a trash can.
According to police, the car is a total loss.

Utah Highway Patrol spokesman Todd Johnson said the truck’s driver sustained only minor injuries and was treated at the scene. However, the 7:30 a.m. crash temporarily closed both directions of travel near mile post 179 — and one of the truck’s fuel tanks ruptured.

Additionally, an estimated 50 pounds of hydrogen chloride spilled, Johnson said. Hazmat personnel were dispatched to the scene near Echo Reservoir. By 6 p.m., the chemicals had been cleaned up, UHP reported.

However, traffic through the area was diverted to alternative routes until early Wednesday afternoon, when all but one lane was reopened. At 6 p.m., workers were still fixing the guardrail, which required the lane to remain closed, Johnson said.

The Harrison Pharmacy School building was evacuated at about 8:45 a.m. because of a corroded canister of hydrogen chloride gas, according to the Auburn Fire Department.

A canister of hydrogen chloride, a potentially deadly gas, was found to be leaking.

“The regulator [on the canister] is stuck,” said Lee Evans, dean of the School of Pharmacy. “They had planned to remove this canister this evening. When the University was notified that we had a canister that could potentially leak, they pulled in the militia.”

All access points to the building were blocked off with yellow caution tape. An AU alert went out at 9:29 a.m. via a phone call and 9:40 a.m. via text message, warning students of a reported hazardous material release and cautioning them to avoid the area.

“This is really being extremely safe,” Evans said. “Essentially, from a safety perspective, [the University] said we need to get it out of here now.”

Safety precautions included members of the Auburn and Opelika HazMat team and Auburn Fire Department.

“They’re in level-A suits in there,” said an Auburn firefighter.

When sealed in one of these suits, the wearer is completely isolated from the outside atmosphere. The suits are composed of up to a dozen protective layers, as well as a sealed hood and respirator mask.

On a scale of one to 10, however, Evans said the event was only about a 0.5.

Lee Lamar, Auburn fire chief, said as far as they know, the canister was found to be leaking this morning. He didn’t know if they were planning to move it or not.

“This wouldn’t be standard protocol for moving it as long as it was in a good state,” Lamar said. He said at 10 a.m. they were looking at one to two more hours of cleanup.

William Ravis, professor in the pharmacy school, said the canister had been “sitting up there for years.” Arrangements were apparently made last night to remove the canister.

Officials are currently setting up a decontamination team.

The fire department will make a formal statement on the situation later in the day.

“There are very few agencies in the United States that can assess, contain and clean up a hazardous material spill of this magnitude,” said city Emergency Manager Steve Endacott. “Given this challenging scenario, this is the way it was designed to go down.”

Due to previous emergency planning, city officials were confident from the beginning the 10,000 gallons of corrosive chemical would not seep into the environment. The city installed a specialized trough beneath the hydrochloric acid tank capable of holding 100 percent of the chemical housed in the building.

“It was fully contained the first day it occurred,” said Mayor Ken Tedford Jr. “The way the building was designed worked perfectly.”

ANCHORAGE, Alaska (KTVA-CBS 11 News) Anchorage Police say they evacuated the Fred Meyer store off of Northern Lights Boulevard and New Seward, after a chemically activated bomb went off during a late night back to school shopping session on Saturday. APD says no one was hurt, however the people who were out late Saturday night back to school shopping, still got quite a scare.
Police say just before 11 pm, what looked like a plastic water container exploded from gas pressure in isle four in the Fred Meyer store. APD says the store was evacuated for a period of time, but they are not sure exactly how long that was.

Anchorage Fire Department officials say after everyone cleared out of harms way, they tested some samples from the bottle with their hazardous materials equipment. It tested 97% muriatic acid, a hydrochloric acid commonly used for industrial cleaning.

Fire officials say they were able to neutralize the substance with baking soda, provided by Fred Meyer. APD says this goes beyond a prank and is a very serious offense.

"Lots of times, young people especially, because that's who I am seeing on the internet constructing these kind of devices, young people think that either their a young person, or this is just a funny thing so it's not a serious offense. This caused a great deal of disruption, but it's also placing other peoples lives and well being in jeopardy, and we will prosecute criminally when we determine who is responsible for this act," said

The truck overturned on the Ohio Turnpike shortly after 11 p.m. Tuesday, closing a portion of the toll road and prompting the evacuation, according to the Streetsboro Fire Department.
The department’s public information officer, Kevin Grimm, said residents returned to their homes Wednesday morning after sleeping at the homes of friends and relatives.
“The Red Cross was contacted, but their assistance (in providing lodging) wasn’t needed,” Grimm said.
Jon Barber, director of Portage County Department of Homeland Security and Emergency Management, said the area was deemed safe at about 3 a.m. Wednesday.
The driver, a 46-year-old man from Ohio City, was able to get out of the wreckage and was found outside of the truck. He was quickly assessed by paramedics and moved a safe distance from the scene in an ambulance.

Hazardous materials crews and firefighters called to the scene just before 3 p.m. found a gallon container of the acid lying near the intersection of the two roads, and then used a red 5-gallon container to isolate it before more of the acid could spill.
"The HAZMAT crews assessed the hazard, and using the equipment they have, took samples and determined the substance to be muriatic acid," said Assistant Chief Brett Bowman of the Prince William County Fire and Rescue Department.
The container could have fallen off the back of a truck transporting hazardous materials, but there is no way for investigators to know for sure, Bowman said.
It took crews less than an hour to identify the substance, which is commonly used in household cleaners and production of polyurethane and PVC plastics.

“When Matt and I walked up to the car we smelled the toxic fumes that is associated with the meth process and spotted what appeared to be items commonly used to manufacture methamphetamine in the vehicle,” said Deputy Gullett. “We then called for Sheriff’s Investigator Charlie Wilder to assist us at the scene.
“We have heard about the ‘shake and bake’ method of manufacturing methamphetamine but this was the largest mobile lab that we have found in this county,” said Wilder. “During our search of the vehicle we found some finished product, assorted drug paraphernalia, digital scales and all the items necessary to make meth including Coleman fuel and Muriatic acid. The chemical process was definitely underway to manufacture meth.”
