I-95 collapse in Philadelphia: Highway hazmat accidents on the rise

PHILADELPHIA (CBS) -- From the look of the images of black smoke and the gaping hole in the interstate, you might think Sunday's I-95 collapse in Philadelphia was a freak accident. However, a CBS News investigation found hazmat road accidents in the U.S. have more than doubled in the past decade.

Over the last 10 years, the number of big rig accidents involving hazardous materials has increased by 155%. According to data from the Pipeline and Hazardous Materials Safety Administration (PHMSA), in the last decade, there have been 52 fatalities and 160 injuries due to hazmat incidents involving tractor-trailers in transit.

With Philly's dense population, parts of I-95 are among the busiest stretches of highway in the country.

RELATED: Highways pose greatest danger for potential Pa. hazmat situation

"We don't recognize that driving down these roads, we're driving next to one the most dangerous things that can be produced," Timothy Boyce, the head of emergency management in Delaware County, said last month. "If you're cutting off a truck or tailgating, or you think you want to challenge that truck, think of the consequences you may have for your family and others."

According to PHMSA data, hazardous materials such as nitric acid, sodium chlorate, batteries, and compressed gas have led to 17 Philadelphia highway fires – about one every three years – since 1974. Just five of those have happened since 1997, and Sunday's incident was the city's first highway fire caused by gasoline since 1984.

Drivers are found to be at fault in nearly one in five hazmat crashes. A variety of safety technology is available, such as cameras, lane assistance, rollover prevention, and automatic braking. It's not known how many smaller trucking companies can afford measures like these, and a CBS News investigation found there's no federal mandate for companies to put them in place despite NTSB recommendations.

About 2:08 a.m., city police responded to the intersection, where a Dalton Delivery Service tanker truck carrying about 6,000 gallons combined of gasoline and diesel fuel overturned, according to a news release from police Capt. Nicholas P. Lechman.

“We are in the process of determining what residences are safe to return to, and clean-up will continue throughout the day,” Lechman said. “The events leading up to the accident are still under investigation.”

West Union Boulevard remains closed between Pennsylvania and 16th avenues, as well as side streets in the area, he said. Drivers should expect delays and avoid the area if possible.

Colleen Connolly, northeast regional spokesperson for the state Department of Environmental Protection, said the agency’s emergency response team responded to the spill.

“We’re just assessing the environmental impacts at this point,” she said. Earlier in the morning, she reported some of the material reached Monocacy Creek. However, just after 10 a.m., she said sand berms below 14th Street had protected the creek.

Firefighters were sent to the Toot 'n Totum at 3201 S. Coulter Street at about 6:22 p.m.

Officials said a person drove away from the pump after filling up and forgot to remove the nozzle.

The vehicle ripped the hose from the pump causing the fuel to continue flowing.

A Level II Hazmat was called at about 6:45 p.m.

An employee was able to engage the emergency shutoff switch.

Fire crews damned up the driveways and applied absorbent to prevent runoff.

About 40 gallons of gasoline were spilled, officials said.

The Office of Emergency Management and Environmental Protection Agency have been contacted for further guidance.

The Chemical Safety Board says refineries need to strengthen safeguards surrounding the use of hydroflouric acid, and the EPA should take steps to improve oversight. The recommendations are part of a final report on the investigation into the June 2019 explosion and fire at Philadelphia Energy Solutions in southwest Philadelphia.
One of the most dangerous aspects of the incident was the release of more than 5,000 pounds of hydroflouric acid (HF) into the air. Nobody was injured by the chemical release. But it’s one of the most deadly industrial chemicals in use, and swallowing just a small amount of it or getting small splashes on the skin can be fatal, according to the Centers for Disease Control and Prevention.
Integral to the creation of high-octane gasoline, it is used as a catalyst and is combined with highly flammable hydrocarbons at 46 alkylation units in the United States, including three other refineries in the Philadelphia region.

The snapshot of potential impacts — reported to the federal government by local agencies, corporations and private citizens — already paints a grim picture:

Thousands of gallons of boat fuel from sunken vessels fouling Florida’s waters; chemicals released from a storm-damaged warehouse; more than 2,000 gallons of disinfectant leaking from a county-operated water utility pipeline.

The list goes on: A storage tank with hundreds of gallons of gasoline dumping into a historic river; a boat leaking fuel just outside a St. Petersburg home; a man who reported trouble breathing after finding a “green sludge” in his flooded Lee County condo.

These reports were logged by the National Response Center between landfall on Sept. 28 and Tuesday. The center, staffed by the U.S. Coast Guard, is an emergency call center that keeps track of potential environmental emergencies related to oil and chemical spills, then notifies the proper response agencies.

The spill has not created risk to groundwater, area irrigation ditches or the nearby Clarks Fork of the Yellowstone River, nor has it presented harm to livestock, officials said.

Barriers were set back 500 feet around the area to protect against exposure to vapors from the crash, and no one was injured, officials said. Evacuation warnings have been issued for the area.

The rail cars were carrying sorghum, animal fat, coal, gasoline and other products, according to officials. Photos from the scene show damaged tanker cars and containers from Walmart.

Several roads, including East Bridger Road and South River Road, have been closed in the area of the cleanup. Drivers are asked to avoid the area.

HONOLULU (AP) — The Honolulu Board of Water Supply said Thursday it has detected a small amount of a chemical naturally occurring in coal, crude oil and gasoline in a monitoring well near a Navy fuel storage facility that spilled jet fuel last year.

The utility said in a news release it found “very low levels” of the chemical, polycyclic aromatic hydrocarbons. It said it shared its data with the state Department of Health and the U.S. Environmental Protection Agency and both agencies agree these low levels are not expected to cause any health effects. Still, they agreed the situation needs attention and continued monitoring, the utility said.

The Board of Water Supply said the discovery heightens its concern that fuel spilled from the Red Hill Bulk Fuel Storage Facility is migrating through the aquifer located under the tank farm.

PHILADELPHIA (CBS) — Police are investigating a hazmat situation after more than 100 plastic milk jugs filled with gasoline were found inside an abandoned home in West Philadelphia. First responders were dispatched to an abandoned home on the 100 block of North 59th Street for reports of a strong smell of gas around 8:30 p.m. Monday.

Police say upon arrival, first responders found about 154 one-gallon jugs of gasoline. You can imagine how strong the smell was coming from that house which is what tipped off the police to this potentially dangerous hazmat situation.

First responders went right inside because the house appeared to be partially burned out from a fire and completely vacant.

They say the plastic milk jugs filled with gasoline were found on the living room floor and on the steps.

“Very, very bizarre, 154 gallons and plastic milk containers. Clearly hazardous material incident, clearly unsafe in this mainly residential neighborhood,” Chief Insp. Scott Small said.

The Corps Commander, Federal Road Safety Corps (FRSC), Anambra State Command, CC Adeoye Irelewuyi, disclosed this in a statement issued to newsmen in Awka.

The statement reads: “At about 1700hrs of today 03-07-2022, Anambra State Fire Service, Awka, Nigeria received a distress call of fire outbreak in two (2) petrol tankers of 33,000 litres opposite At Anthony’s Catholic Church, Amansea.

“Immediately, we deployed our fire truck and Firefighters to the fire scene and fought the fire to standstill with our chemical foam compound and there was no loss of life.

“The avoidable fire outbreak emanated during the trans-loading of the petrol from the faulty tanker to another tanker without the presence of Fire Service to be on standby. The mistake of the pump operator caused the fire outbreak.

“The road is now free for motorists to move on.

According to DOTD as of 4:07am, the southbound lanes are closed, while the northbound lanes are opening. For more information on closures and updates on traffic visit 511la.org

It’s estimated to take 8-10 hours to offload the fuel to another tanker and clear the scene. This could extend into the early commute hours of Monday. The roadway near the scene will be closed off to traffic for safety reasons.

“Our victim did succumb to his injuries this morning. This is still under investigation,” said Whiteville Emergency Management Director, Hal Lowder, Jr. “We’re praying for the families there.”

According to Lowder, the explosion happened just before 4:40 Monday afternoon. Whiteville Fire, along with other area fire departments were called to control the flames.

“The scene was contained pretty rapidly,” Lowder remembered. “The guys really hit it aggressively, hit it hard. And it was confined to one area.”

The source of leaking gasoline in Watseka March 1 has been contained and will not cause any issues for the public, according to local officials.

Emergency personnel were on scene at both the pond south of city hall on Brianna Drive and on Veterans Parkway at the Shell station about the issue.

Watseka Fire Chief Ken Baier said he was called Feb. 28 about the matter. “There was a slight smell coming from one of the storm sewers at RP Lumber. The workers themselves had contacted the city. Code (inspector Eric Brandt) had gone over there and he called me about it. Nobody else in the area had smelled anything and there was nothing else to be concerned about. So I said OK we’ll come by in the morning and check it out. In the meantime I got in contact with the sewer department. They have a few things that we could use if need be to check the sewer and also I wanted to get a hold of the sewer maps to see where everything drains to.

“We started out (March 1) at RP Lumber and immediately we could smell a petroleum based product of some kind in the storm sewer. From there we started working out way south following the storm sewers. We were picking up levels of gas within the storm sewers themselves. We ended up in the north parking lot of the Shell gas station. They have two storm drains there. Once we got to the west storm drain on the north side we were able to actually physically see product going in to the storm sewers. At that point we had duty to act. I made contacts that needed to take place.”

Sheriff Ed Gonzalez said three of the four injured workers were taken to hospitals by Life Flight.
Three suffered flash burn injuries, and the fourth employee suffered a broken leg, authorities said.

In 1921 the US automobile industry was struggling with an awkward technical problem. An internal combustion engine works best when the fuel–air mixture explodes at the optimum point in its power cycle. But premature ignition – indicated by a distinctive noise known as ‘knocking’ – reduces efficiency and damages engines. In December of that year, Thomas Midgley announced his discovery of an effective remedy for it. Midgley showed that adding traces of lead to commercial gasoline prevented knocking.

Lead’s toxicity had long been known – the Greek physician Dioscorides (c. 40–90 CE) mentioned its harmful effects in his widely-read De Materia Medica, and numerous subsequent authors observed tremors, convulsions, and madness afflicting workers in lead-based industries. Nevertheless, Midgley and his backers insisted that leaded gasoline posed no risk to public health. Decades passed before the full consequences of this error became apparent. Today, persistent lead residues remain its enduring legacy.

What was the substance spilled last month in Rocky Creek that killed fish and brought out the Augusta Fire/EMA Hazmat Team? According to Kevin Chambers, director of communications for Georgia Environmental Protection Division, it included both gasoline and diesel fuel.

In an email, Chambers said the emergency response contractor found the soil that had been impacted by the spill was properly remediated and no further clean up is required. The investigation into the source of the spill is ongoing, and EPD had no further information at this time.

On Nov. 17, residents near the creek, which extends from Barton Chapel to Regency Mall, were told to be careful if they went outside and to return indoors if they smelled the gasoline odor of the spill, although no one knew at the time what the substance was that had spilled.

Around 8:30 a.m. Wednesday, the Augusta Fire Department’s hazardous materials team responded to a portion of the creek near North Leg Road and Commerce Drive after receiving reports of an odor in the area.

Responders say they found an unknown substance in the creek.

The Savannah Riverkeeper is describing the substance as a chemical that has interacted with the waterway and has killed fish along the creek.

The smell is reportedly very intense in the area and smells similar to gasoline.

Residents who live in the area are asked to stay indoors and the public is urged not to interact with the creek which runs from Barton Chapel Road to Regency Mall. It’s unclear at this time whether the substance is a danger to the public.

The team is trying to contain the substance while working with other agencies to investigate its source.

A gas leak originating at a nearby Ford plant is likely the source of a spill into the sewer system of Flat Rock, which has led to the evacuation of a number of homes in that Downriver community.

Gov. Gretchen Whitmer declared a state of emergency as workers dealt with reports of chemical odors from the city's sewers over the last five days, which her office said had "the potential to be explosive." Bob Holycross, Ford's vice president of sustainability, environment and safety engineering announced Friday that the company's plant was the source of the leak.

The affected area in Flat Rock is "a 4-square-mile perimeter including the areas south of Vreeland Road, east of Cahill Road, north of Woodruff Road and west of Interstate 75," according to The Detroit News:

The Michigan Department of Environment, Great Lakes and Energy said in an email late Friday that an estimated 1,000 to 3,000 gallons of unleaded gasoline are estimated to have spilled into the sewer system.

As the company works to execute a "corrective plan," Holycross said, the plant — which builds Mustangs — will be shut down over the Labor Day weekend as previously planned. Ford said it also is in the process of isolating wastewater on its property from the city's system, assisting with cleanup efforts and "identifying people whose lives have been disrupted by this."

EGLE said in a Friday evening statement that the city of Flat Rock would use firefighting foam to "suppress" the vapors. The foam is "designed to bond with hydrocarbons" such as gasoline "to help break down the organic compounds." It will be applied inside sanitary sewers that were affected by the issue. The foam does not contain a group of chemicals known as PFAS.

The leak came from a line carrying gas used to fuel vehicles in the plant. While Holycross said the initial leak was thought to be small, it was eventually discovered to be much larger.

Federal, state and local personnel are responding to an apparent spill of an unknown chemical in the Flat Rock sanitary sewer system.

Wayne County Chief Executive Officer Warren C. Evans on Wednesday declared a state of emergency in Wayne County over the spill, seeking emergency assistance from the state and federal government.

Evans noted that the odors were first detected in the sanitary sewer system on Monday, and spread to homes in and around the Hickory Ridge subdivision, in a southeastern portion of the city south of Gibraltar Road. On Tuesday, the odor was detected by city personnel at a sanitary sewer lift station.

The U.S. Environmental Protection Agency, the Michigan Department of Environment, Great Lakes and Energy, and Downriver, Western Wayne and Washtenaw hazardous materials response teams have all converged in the city. The exact substance spilled, where it came from and how much spilled have still not been determined.

"My sense is that it's gasoline," said Flat Rock Mayor Mark Hammond, who is retired from 25 years as a firefighter at Detroit Metropolitan Wayne County Airport.

According to the Hancock Fire Department, the gasoline tanker, carrying about 8,500 gallons of gasoline, tipped over Santori’s Corner on US-41 at about 8:00 a.m. June 24.

“Currently, it is unknown how much spilled,” the fire department said in a release.

The fire department said here have been no injuries caused by the incident and spill, and that drinking water has not been affected.

Hancock Fire Department says some gasoline has gone into the storm sewer system, and ended up into the Portage Canal. Because of this, the Western Upper Peninsula Health Department, Michigan EGLE and the EPA have closed the Portage Canal and beaches. Read more about that here.

Many areas in the vicinity have been evacuated. Evacuations are in place in west Hancock from Santori’s Corner on South Lincoln Drive (US-41) to Michigan Street, Michigan Street up to Elm Street and down to the Jutila Center, and Minnesota and Ohio Streets between Ethel Avenue and Michigan Street (see map below).

Since early Sunday morning, the Kingsville Fire Department has been at the scene of a convenience store and gas bar at Essex County roads 20 and 23, along with the Ontario Provincial Police and the Windsor Fire and Rescue Service HAZMAT team — but they've hit some snags in their cleanup effort, and it's unclear when the intersection will reopen.

"There's a larger issue than we they originally thought," said Andrew Plancke, Kingsville's director of municipal services, one day after homes and businesses within 500 metres of the scene were evacuated.

Town officials said they thought the leak would be a quick cleanup but it's turned into a bit of a mystery. Fuel is leeching into the ground and they are not sure where it is coming from.

With Hurricane Laura threatening the Gulf Coast region, the oil refineries and petrochemical plants that stretch across the Houston area and Southeast Texas took their customary precautions of shutting down operations. In doing so, they released about 4 million pounds of pollutants into the air.

In reports to the Texas Commission on Environmental Quality, the Beaumont Gas to Gasoline Plant announced the release 3.5 million pounds of carbon dioxide, and the Motiva Chemicals plant in nearby Port Arthur reported that it would release more than 130,000 pounds of polluting chemicals, including benzene, a known cancer-causing substance.

Instead, Motiva’s emissions totaled nearly 90 tons, including a half-ton of benzene. The plant reported releasing more hazardous pollutants during the hurricane than it did in all of 2018.

Environmental groups are growing increasingly alarmed at the health risks to nearby residents as a result of shutdown and startup pollution releases. The pollution releases due to Hurricane Laura were anticipated to be about half the amount released during Hurricane Harvey in 2017.

Equipment breakdowns, process malfunctions or operator error during the startup and shutdown of equipment can cause unauthorized pollution releases, according to Environment Texas, an environmental watchdog group.

Firefighters arrived on scene to find a HAZMAT level 2 incident at Hamilton Road and Meadowlily Road.

Fire officials say the fuel truck operator took quick action to ensure more fuel wasn’t spilled on the road.

The truck driver told CTV News that he was refueling when a piece of equipment failed. A broken fueling elbow was the cause of the spill.

The truck driver quickly shut off the pump and called 911 and the fire department.

That fast action meant no fuel entered the catch basin. The driver told CTV News he has been driving for 25 years and has never had this happen before.

ZANESVILLE – A pump at a Marathon gas station was struck by a semi-truck last night. When the pump was struck, piping was sheared off from below the pump’s emergency shutoff valve. This caused the pump to activate and emit gas.

“Gasoline got out into 40, into the storm sewers, now its traveling downstream towards some ponds. Zemba Brothers, their division that does HAZMAT got the gasoline stopped before it got to the two ponds so its just going to be an all day cleanup,” EMA Director Jeff Jadwin said.

Jadwin says that 500 to 1,000 gallons of gas spewed out of the machine. He confirms that it is safe to walk around the gas station. The biggest concern is the effect the spill has on the environment.

“What’s here has been cleaned up. It’s what’s in the storm sewers and what’s in the streams downstream that we got to worry about right now; the environmental impact. The EPA is here, O-DOT’s been here, a lot of agencies are involved this morning,” Jadwin said.

Jadwin hopes that the job can be fully taken care of by the end of the day. The nearby South Pleasant Grove Road will be closed until work is complete.

The Wichita Falls Fire Department responded to a Hazardous Materials call and is investigating an underground fuel leak they believe came from fuel storage tanks at a convenience store at Kemp Boulevard and Call Field Road.

Assistant Fire Chief Donald Hughes said the amount of fuel leaking is enough for people to smell but not enough to be volatile or in danger of igniting.

A tank carrying 1,500 gallons of cooking oil exploded and caused a fire at an oil terminal in Chelsea on Tuesday morning, Chelsea firefighters said.

The 25,000-gallon biodiesel tank overheated at the Global Chelsea Terminals at 11 Broadway around 5:30 a.m., Chelsea Deputy Fire Chief Robert Houghton said.

“We’re thinking it was a possible overpressure of the product inside. It heated to a high temperature that caused the explosion,” Houghton said. “It was in the rear of the yard protected by a concrete wall.”

Firefighters put out the fire in about 10 minutes, Houghton said. A hazmat officer responded to the scene.

"Early this morning a small tank released biofuel and resulted in a minor fire at our Chelsea Terminal ... All product was confined within the tank’s concrete containment dike. There was no impact to adjacent infrastructure and no injuries,” Global Partners, a gasoline company that owns the terminal, said in a statement.

Chelsea firefighters, Massachusetts State Police, US Coast Guard officials, US Environmental Protection Agency officials, and the Massachusetts Department of Environmental Protection are investigating the explosion, Houghton said.

OLEAN — The Olean Fire Department extinguished a vehicle fire Friday at the Kwik Fill on Main Street, but not before the gas station’s fire suppression system blanketed the area in white powder.

The engine of a red Kia Sedona minivan ignited into flames at approximately 4:40 p.m., causing a Kwik Fill employee to activate the fire suppression system, said Olean Fire Capt. Patrick Zink.

However, the minivan was not parked under the gas station canopy and the dry chemical agent did not extinguish the flames. Firefighters then arrived on scene and used a handline to extinguish them.

“Obviously gasoline is quite flammable so anytime we get a call for a fire near a gas pump it can be more dangerous, but it’s really no different from any other fire,” said Zink, adding gas stations have emergency switches to shut down its fuel pumps as well.

No one was injured by the flames, but a few individuals on scene were exposed to the extinguishing agent and checked out by EMS personnel. Zink said they all refused transport to Olean General Hospital, which is located directly across the street from the Kwik Fill.

Zink said the minivan operator had pulled into the station to get some gas, but noticed an issue with the vehicle and spoke with a Kwik Fill employee. Zink said the operator left the scene prior to the fire and was not on scene when firefighters arrived.
