Cylinders kept exploding one after the other due to the fire, causing fear and panic in the area. A worker at the shop, Asif, suffered burn injuries and he was taken to the burns ward of the Civil Hospital Karachi.

Warehouse fire

A fire broke out in a chemical warehouse in Liaquatabad. Nearby houses and buildings were evacuated due to the fire. Three fire brigade vehicles brought the fire under control after the hectic efforts of hours.

According to police, the warehouse owner had been warned a year ago to evacuate the warehouse as it was located in the residential area. The warehouse owner was detained on the instructions of the assistant commissioner of the Liaquatabad area.

Firefighters said that no one was injured in the incident as the factory was still closed. A few security guards who were on duty first alerted the police and the fire department. Officials suspect that a short circuit caused the fire.
At least 15 fire tenders were rushed to the spot from the IMT Manesar, Sector 29, Udyog Vihar, Bhim Nagar, Sector 37, Maruti Vihar and Pataudi fire stations, owing to the severity of the fire. Gulshan Kalra, deputy director, fire department (Gurugram), said that the rear end of the factory collapsed due to the fire but officers managed to save the front section. “It took at least six hours to bring the flames completely under control,” he said.
Fire officials said that they received the information about the fire at 4.57am. Only three tenders were sent to the spot initially, but other fire stations were soon alerted to send their tenders. Foam-based fire tenders had to be used as there were several types of chemicals and gas cylinders kept inside the factory, officials added. Several explosions also took place due to metallic containers containing chemicals and gas cylinders which were stored in the factory.

Emergency services were called to the large fire on College Road in the Darwin suburb of Berrimah at about 10:30am on Sunday, according to NT Chief Fire Officer Mark Spain. 

"On arrival, the building was already engulfed in fire," he said. 

"Fire crews did manage to get inside to contain that fire, but it got away from them at that point in time."

Mr Spain described the building as a "health storage facility" and that the "initial estimate" of the damage was $30 million.

    <dc:creator>dchas</dc:creator><description><![CDATA[The US Chemical Safety and Hazard Investigation Board recently released a final report detailing the cause of an industrial incident that launched a large pressure vessel 150 m, killing four people—one employee of the company and three of a neighboring facility. The April 2017 explosion occurred at the Loy-Lange Box Company, a 71-employee manufacturer of corrugated cardboard in St. Louis, Missouri.
CSB interim executive Steve Owens noted at a July 29 briefing that the report “took far too long to complete” and was the only CSB accident report issued in the last 10 months. That view was seconded by board member Sylvia E. Johnson, who acknowledged CSB difficulties. The CSB has 17 unfinished reports, “probably the biggest report backlog in CSB’s history,” she said. Johnson urged “grace and patience” from public stakeholders as the board attempts to correct problems.
On a Friday 5 years ago, Loy-Lange operators noticed a leak in the bottom of the pressure vessel as the plant shut down for a regular-scheduled weekend break. The company did not repair the vessel, which was a steel cylinder 5.3 m long and 76 cm in diameter. When the plant restarted on Monday morning, the leak expanded, releasing pressurized water and steam and launching the vessel into another nearby plant, killing three people there as well as one worker at Loy-Lange.
According to the report and a presentation by CSB investigator Drew Sahli at the board’s briefing, extensive dissolved oxygen had led to excessive corrosion in the vessel’s bottom head. The vessel had never been inspected in the 20 years it was operating, either by the company or the the city of St. Louis, which regulated pressure vessels within its jurisdiction. In 2012, a similar leak had been inadequately repaired by the now defunct Kickham Boiler and Engineering company.
The CSB investigation yielded several recommendations. One is that Loy-Lange establish a process safety management system. The agency also recommends that St. Louis and the National Board of Boiler and Pressure Vessel Inspectors fill gaps in their regulatory oversight programs.
Owens noted that one of the reasons the report was delayed was the high turnover of CSB staff, particularly investigators. Owens and Johnson, like past CSB leaders, promised to address the problem.

Two St John ambulances, two rapid response vehicles, and a manager, rushed to the campus in Hillcrest at 8.16am on Monday, a statement said.

An email to staff from Waikato University’s senior deputy vice-chancellor professor Alister Jones said it was a “minor explosion” from a gas cylinder outside a science laboratory near the EF link building.

“The site of the explosion has been secured and it is safe to be on campus.”

Fire and Emergency sent three fire trucks from Hamilton and Chartwell, a spokesperson said. The last fire truck left at 8.53am.

More information would not be provided by Fire and Emergency due to industrial action.

WorkSafe was notified of the incident and was making enquiries.

"As our Hazmat team was performing a flaring operation to empty the leaking container, a strap gave away causing the propane cylinder to slip in the grass," Boise Fire said. "This created another leak."

Thankfully though, in addition to there being no injuries, there was no reported property damage.]]></description>

Police said the chemical factory is in Brahmpuri area, and as fire started, around 700 cooking gas cylinders were shifted to safety from the godown.

The chemical factory was being operated illegally, chief fire officer Devendra Meena said.

The fire was controlled in time otherwise a major incident could have happened, he said.

The fire has been controlled now, and the matter is being investigated, the police said.

There was a blast in an LPG cylinder in Silica Scientific, which produces raw materials for laboratories. We doused the fire in an hour and the cooling operation is in progress. Over 13 shops were gutted in the fire and we are yet to establish the damage. Once the cooling operation is completed, we will investigate further.”

They informed he died of flame burns today morning adding that medically survival of such patients is impossible. An official source at GSSH said that Shakir caught fire first and later on a blast occurred which damaged the first floor of the hospital badly. Window panes, doors, and gates were shattered, the source said and added that debris fell on two parked cars which were totally damaged. The hospital faced a great loss, the source informed and added the lab technician was a nice chap.

MS, Dr Rao Amjad informed this news agency that the deceased was only staffer in the lab at night duty and had returned to duty after saying Isha prayer. He stated that normally he offers ‘Taravi’ too but last night he did not. The MS noted that infrastructure was vandalized with windows, doors, gates, his office 8mm table mirror besides shattering of windows and gates of pharmacies and bank in front of the hospital. Replying a question, DrAmjad said that according to Bomb Disposal Squad, Civil defence and police officials conducting inquiry into the incident, apparently gases of Air Conditioners-two split units and a standing AC installed in the lab caused the blast. To another question, he replied that an inquiry committee constituted by DC Amir Karim met today where he briefed it about the explosion. He quoted Shakir as telling hospital colleagues during his statement that when he sat on his table after returning from prayers, he caught fire suddenly from AC side.

Deputy Commissioner Amir Karim Khan reached the hospital and supervised the rescue work. He said the nature of the incident and the extent of damage was being worked out. He said three technicians and a guard were injured in the incident.

Commissioner Dr Irshad Ahmed also visited the hospital and said the explosion had taken place in the laboratory on the first floor. Forensic teams joined the investigation launched by Health Secretary South Punjab Tanvir Iqbal Tabassum. He directed Additional Secretary Admin Qamar Zaman to probe the matter.

"As the firemen were dousing the flames, an LPG cylinder exploded resulting in injuries to six firemen and three civilians," the official said, adding the condition of the injured is now stable.

The official confirmed that there have been no casualties in the incident, however, there was certainly a massive loss of property. The cause of the fire is yet to be ascertained.

A hissing sound interrupted the pleasantries.

It was March 26, 1962, at the Firestone Research Center at Wilbeth Road and South Main Street in Akron. More than 140 people worked at the Firestone complex, including chemists, physicists and technicians involved with research and development of new products.

A whooshing noise began about 9:25 a.m. in a plastics laboratory on the second floor of the three-story brick building. It sounded like a steam line, but it turned out to be a leaking cylinder of propylene gas.

Christian E. Best, 46, a chemist, was walking down a hallway when he heard the commotion and saw two technicians emerge from a room. Knowing the lab contained flammable materials, he asked Everett Tackett, 23, and Gerald Morneweck, 25, if there were any open flames.

The fire at the metro site near Bamboo Bazaar in Shivajinagar is reported to have started around 4 pm.

It erupted in a storehouse where chemical canisters, wood, thermocol sheets, cables, mechanical spare parts, cylinders and other materials were stacked up, according to a senior officer in the fire department.

Roofed with corrugated steel sheets, the storehouse was located at the edge of the metro site. A narrow alley separates its boundary with a row of tightly packed houses in the densely populated neighbourhood.

It was the explosion of three cylinders one after one that set alarm bells ringing in the locality, said Ameerjan, a local resident. Residents of the surrounding residential buildings immediately evacuated, he added.

Coral Springs-Parkland Fire crews responded to the scene at 1900 Augusta Terrace, Monday afternoon.

Officials said there was a problem with a propane tank in the backyard of the home.

Crews attached a line to the damaged cylinder and began burning off the fuel. Firefighters put foam on the ground nearby to keep the tar from melting.

Crews from Coffee County, Manchester and Tullahoma assisted Winchester Fire around 7:30 a.m. Saturday after a call for a leaking chlorine gas cylinder. Crews placed a special device on the 1-ton cylinder to stop the leak.

No injuries were reported in the incident and no gas was released from the building as there are air scrubbers installed to clean the air before it is released from the building.

Cal Fire/Riverside County Fire Department responded to the scene of the blaze at 12:33 p.m. near Dinah Shore Drive and Pacific Avenue and found a container on a train had caught fire.

At 3 p.m., the agency reported the container was carrying hazardous materials, including 25 pounds of lithium batteries, consumer drills, cell phones, 28 standard car batteries, 11 airbag deploy gas cylinders, and other toxic substances.

The incident outside an alleged illegal cracker godown claimed two lives and four injured. The cops' team swung into action raided several godowns stocking hazardous material in New Tharagupet.

As a result of the successful raids, the police seized 16 cooking gas cylinders weighing 40 kg each stored illegally in godowns.

According to the report, the raided godowns are located near Chamarajpet where the cracker blast took place. The city police have arrested proprietor Ganesh Babu.      

While commenting on the police raid, a senior officer said, “We will continue raiding godowns in and around Chamarajpet, Goods Shed Road, KR Market, and surrounding areas," as quoted by the Times of India.

A woman and her children were killed and five others were injured in an explosion that shook a residential building near Mosamyat Chowk on University Road on Tuesday morning.
Initial investigations suggest the explosion likely occurred due to a gas leakage.
The blast, which ripped through a pizza outlet located in the building – Crown Gardens, left the eatery and two neighbouring realtors' offices completely destroyed while three other shops were partially damaged.
Panic ensued and chaos reigned when the explosion occurred. The scale of the blast was such that a van of the K-Electric overturned and added to the casualties.

The Mount Airy Volunteer Fire Company says units were dispatched just before 11:00 AM for a vehicle collision involving an overturned stake body truck carrying a mix of compressed gases, liquid nitrogen and dry ice. Arriving crews found gas cylinders strewn across Route 27, along with a leak from a liquid tank and dry ice scattered all over. At that time, Route 27 and I-70 was closed to traffic.

The Haz Mat teams from both Carroll and Frederick Counties were dispatched to the scene. After personnel were equipped and a recon was done, the teams determined that several gas cylinders and the liquid tank were compromised in various ways. The materials involved were Oxygen, Acetylene, Nitrogen, Liquid Nitrogen, and dry ice. The air was monitored to make sure there were no explosive mixtures and ignition devices were removed.]]></description>

Maj. Bernie Faldas, chief of Norala municipal police station, said Monday the incident happened past 9 a.m. at the residence of the Rapista family in Sub-Purok Bliss, Purok Reloquemas in Barangay Poblacion.

He said a balloon vendor identified as Neilfren Cabuhay, 40, and several family members had just finished inflating balloons using what looked like an LPG tank when it suddenly exploded.

Citing accounts from witnesses, Faldas said one of the victims reportedly mixed unknown chemicals and filled the tank with them before the incident.

“The investigation is still ongoing and we’re mainly determining the chemicals that caused the explosion,” he said in a report.

    <dc:creator>dchas</dc:creator><description><![CDATA[A fire and several explosions at a chemical factory in Durban, South Africa injured 13 people on December 8. Over 100 workers were evacuated from the factory and two of the injured are reported to have suffered life-threatening injuries.

The incident happened at a factory belonging to Zeta Laboratories in New Germany, outside of Pinetown in Durban and occurred just days after another explosion in the city at an oil refinery operated by Engen. 

Firefighters tackled the blaze for several hours before it was extinguished. A lack of fire hydrants in the area had made it difficult for firefighters to tackle the flames until fire trucks arrived at the scene. Paramedics attended the scene at around 11:00 local time and all 13 inured people were taken to local hospitals. 

Local officials said that they believe the fire started in the processing area of the factory and quickly spread to 20 gas cylinders which caused an explosion. A number of chemicals were stored on site, including 10 tonnes of sanitisers, acetone, hydrogen peroxide, caustic soda, and petroleum jelly tanks which all helped the fire to spread and caused further explosions.

Multiple emergency services were dispatched to the area in effort to extinguish the destructive blaze in which at least 13 people were injured, Zeta Labs manager Nikesh Singh told TimesLIVE.  

Dennis Govender, the eThekwini fire department's divisional commander for the western region, said an investigation was under way.

He said emergency service found the factory “well alight with numerous explosions”. He said 9kg gas cylinders were exploding.

“We made attempts to extinguish the fire, but we couldn’t, purely because there was no water in the facility and surrounding areas. We had limited water in the tanks that we carried,” Govender said.

The agency said Airgas, an Air Liquide Company, rapidly implemented a “robust” process safety management program in response to its recommendation following CSB’s investigation into an August 2016 fatal nitrous oxide explosion at the Airgas facility in Cantonment FL.

Following the incident, the company quickly began a comprehensive initiative to review the safety programs for its nitrous oxide production facilities, trucking fleet and cylinder-filling operations. The scope of this safety initiative includes 17 different areas for process safety. Some of the company’s most significant improvements include initiatives typically associated with the most effective safety management components, including the hierarchy of controls, improved audit practices and inherently safer design.

“Taking proactive, effective action, similar to what Airgas did following this incident, is the hallmark of driving chemical safety change,” said Katherine Lemos, CSB chairman. “I applaud the company for beginning implementation of these important process safety initiatives before the CSB completed its final report. This is especially noteworthy given that OSHA’s Process Safety Management standard does not apply to its facilities.”]]></description>

Leeden National Oxygen and its CEO Steven Tham Weng Cheong, now 69, each pleaded guilty yesterday to an offence under the Workplace Safety and Health Act.

Among other things, they had failed to take measures to ensure that unsafe modified regulatory valve assemblies were not used when testing combustible gases. They had also failed to ensure that there was a system in place for the accurate tracking of gas cylinders.

The CSB is an independent, nonregulatory federal agency that investigates chemical accidents.

In a Safety Spotlight report, the CSB lauds Airgas, now a subsidiary of Air Liquide, for rapidly developing and implementing a robust process safety management program (PSM) in response to an explosion in 2016 that killed one worker.

CSB stresses that the effort was voluntary, as nitrous oxide handlers are not required by US federal law to develop a PSM program. The incident occurred during the loading of a trailer truck at an Airgas facility in Cantonment, Florida. In the transfer, a pump heated the nitrous oxide above its safe operating limits, causing decomposition and the explosion.

According to the CSB, after the incident, the company began to review its safety programs for its nitrous oxide production facilities, trucking fleet, and cylinder-filling operations. The review resulted in several reforms, such as better audit practices and inherently safer designs.

According to officials, the fire started at around 9.30am on Monday, when Ami Patel, a PhD student, opened the laboratory after two months of lockdown. All the laboratories and classrooms at the campus were closed since the third week of March.
Patel could sense the heat after opening the doors of the laboratory, but she ignored it assuming it to be trapped heat inside the locked up rooms. She turned on the electric supply and switched on fans and lights along with water purifier.
“It was then that she noticed an electric spark and sudden fire after which she called for help,” said SPU vice-chancellor professor Shirish Kulkarni.
Hearing Patel’s cries, non-teaching staff members immediately informed the fire brigade. But till then all major equipment, chemicals, computers, files, furniture among other items inside the laboratory caught fire while a gas cylinder also blasted.]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[A year after an accident resulted in the death of a student, an advanced technologies lab involved in hypersonic and shockwave research at the Indian Institute of Science (IISc) has come back to life after extensive safety upgrades. 

Faculty and staff said inspections were being carried out to ensure safety standards were in place at the Laboratory for Hypersonic and Shock Wave Research (LHSR) and that experiments had restarted at the 50-year-old facility in December 2019.

“We have had three papers published through experiments after the lab reopened,” Professor K P J Reddy said.

On December 5, 2018, the explosion of a gas cylinder killed one engineer and seriously injured three others. The deceased, Manoj Kumar, 32, and the three injured engineers were employees of a promising new startup called Superwave Technology, helmed by two professors of the IISc’s Department of Aerospace.
However, a faculty member with knowledge of the matter said the lab was still undergoing safety checks. “It is still working up towards operability,” he said. 

Cause of accident

The IISc has been officially reluctant to discuss the cause of the accident. The institute’s director, Dr Anurag Kumar, referred the matter to the registrar, V Rajarajan, who did not respond to queries for information.

An eyewitness to the accident told DH that a worn-out valve on a liquid hydrogen cylinder had been responsible for the explosion. The facility employs four sophisticated shock wave tubes using liquid hydrogen, oxygen, nitrogen and helium to generate shock waves.

    <dc:creator>dchas</dc:creator><description><![CDATA[An 18-year-old worker died while he was refilling a fire extinguisher in a private fire service company in Badlapur on Saturday afternoon, after the nozzle of the extinguisher blew off and pierced his chest.

The victim, Ganesh Mhaske, had started working at Woodpecker India Fire Services four days ago and did not have the training to handle such chemicals, said the police. The police have arrested the owner of the company for not providing Mhaske with safety gear.

LM Sariputre, a senior officer of Badlapur (West) police station, said, “Around 12pm, Mhaske was filling dry chemical powder (DCP) in the extinguisher cylinder when due to pressure, the nozzle exploded and hit his chest. He was taken to Dhanwantari Hospital, where the doctors declared him dead. We have arrested the owner of the company, Gaurav Suresh Bihade, 39, who was present when the incident took place.”

Sariputre said there are certain safety measures that should have been followed while dealing with the chemicals. “The deceased was not wearing any safety jacket or safety gear. He [owner] also asked an untrained man to handle such chemicals. It could have resulted in a major fire mishap.”

    <dc:creator>dchas</dc:creator><description><![CDATA[A suspected cylinder blast killed a 32-year old technician and injured three others during an experiment in a laboratory of the Indian Institute of Science in Bengaluru on Wednesday, police said.

Four technicians of a private start-up company were involved in some experiment when an explosion occurred suddenly at around 2.20 pm, they said.

 

Technician Manoj from Mysuru died on the spot while three others suffered injuries.

The injured have been admitted to a private hospital where their condition was said to be stable, police said.

A release from the IISc said an FIR had been registered by Sadhasghivanagar police and investigations were on.

“A case of suspected blast of a cylinder was received from the laboratory of Hypersonics and Shockwave research of the Aerospace Department in the afternoon…,” it said confirming the casualties.

Though the nature of explosion and its cause were yet to be established, it was suspected that the incident occurred due to a blast in a cylinder containing some gas, police said.

Assistant Commissioner of Police Niranjan Raj Urs told PTI that the start-up company Superwave Technologies Private Limited was having a tie-up with the IISc for carrying out experiments pertaining to its research.

“Forensic experts would ascertain the actual cause of the accident but it seems like a cylinder blast,” he said.

The blast near a slum a week ago left seven children dead and at least a dozen injured.

Police arrested the injured vendor Abu Sayeed and started a case against him on culpable homicide charges.

According to Alto Fire Chief Cory Kok, the chemical spill was reported at 7:40 a.m. Thursday. He said an employee was filling a chlorine cylinder, and it began leaking chlorine. The cause of the leak is unknown.

Kok said the cylinder stopped leaking, but there are concerns about chlorine gas in the building. Around 11:30 a.m. Thursday, he said they were starting the process of ventilating the building.

A Hazardous Materials team from the Fond Du Lac Fire Department assisted as well as units from Waupun Fire Department.

Emergency services were called to Yarram Herd Services in Gippsland, Victoria, at about 3am where 100 cryogenic cylinders containing cattle semen exploded in the blaze.

It took 10 fire crews more than two hours to contain the fire at the building, which is a cattle breeding service.

Yarram Herd Services’ committee vice-chairman Aaron Thomas said the loss would be a “huge blow” for farmers, with the cylinders worth between $500 and $1000 each.

“We’re coming into the artificial insemination (AI) season so there would have been substantial amounts of semen inside the tanks that we’ve lost which was owned by our local farmers, and it can range in value from $5 per straw to $95 per straw,” he told the ABC.]]></description>

Clarendon County Fire Department responded to find a small leak coming from the farmers pump house. Clarendon County Sheriff’s Department was requested to establish a safe perimeter around the leak. Sumter Fire Department was requested to respond with their hazmat equipment. Hazardous Material Technicians from Clarendon and Sumter Fire Department entered the pump house to find a 100 lb. chlorine cylinder with a small leak. The technicians noted that most of the gas had already released from the container and a small amount of gas was remaining. The cylinder was removed from the pump house for safety measures. There were no hazards to the public and no injuries from the release. The scene was placed under control around 11:10 AM.

Unbeknownst to the operator, someone had disposed of a closed helium balloon tank such as the one pictured below. Upon compaction, the cylinder over-pressurized and burst, spraying sharp metal pieces across the area. Luckily, the worker was not injured.



Helium balloon kits like these are available from many retailers and can be an attractive option when hosting an event or summer cookout. However, they are compressed gas cylinders under significant pressure and users are responsible for understanding the hazards they present, and for following manufacturer’s instructions for safe handling and disposal.

Besides the environmental impact (link is external) of the accidental or intentional release of helium balloons, disposal of these tanks can be problematic. THEY SHOULD NEVER BE DISPOSED OF IN THE TRASH. 

If helium balloons are a necessity for your event, consider ordering them pre-inflated rather than purchasing a tank. If a tank must be purchased, try to find a retailer that will allow you to return the tank.

If neither of those options are feasible, follow the manufacturers’ instructions on proper disposal. This will most likely involve emptying the tank, removing the stem or rupturing a disk on the tank to show that it is no longer pressurized and then recycling with a scrap steel recycler. Not all recyclers will take the empty cylinders, however, even if they’ve been relieved of pressure through a rupture disc or removal of the stem, and disposal may then incur a fee.]]></description>

A hazmat team responded and cleaned up the leak at 2:11 p.m. at OXARC Inc., 2513 Third Ave., according to a news release from the City of Lewiston. No one was harmed.

An employee who reported the leak saw a “150 pound cylinder of chlorine with a green cloud of smoke on the floor around the base of the cylinder,” according to the news release.

The leak was cleaned by 6 p.m., allowing evacuees to return home.

Currently at least half of a dozen of probe committees, formed by different government organisations, are working on to find out the actual cause of what sparked fire.

They offered different views, however, their initial comments are basically centring two core reasons- either a gas cylinder blast or a transformer blast. However only one point where almost all of them have similar view is that chemical stored at the area helped spread fire.

The NYPD hazmat team responded and raked through the truck’s contents. The FDNY also inspected the vehicle, called a “hopper.”

“We did not find any corrosives or any materials in the back of the hopper or any type of devices,” FDNY Chief Nicholas Corrado said.

The company and Miguel Rivera, who was the general manager at U-Haul’s location at Hunting Park Avenue and Front Street, did not admit in their pleas to filling the propane tank attached to the La Parrillada Chapina food truck, which ruptured on July 1, 2014, killing Olga Galdamez, 42, and her daughter Jaylin Galdamez, 17, and injuring 11 others.
Instead, they pleaded guilty to two counts related to using at least two untrained workers to fill propane cylinders at the Hunting Park Avenue facility in June and July 2014.
Assistant U.S. Attorney Elizabeth Abrams told U.S. District Judge Robert Kelly that plea agreements called for U-Haul to pay a $1 million fine, the maximum amount for the two counts, and to be on probation for two years, and for Rivera to pay a $2,000 fine.

The factory, Herbal House HTI Experts, is owned by Dr Rajesh Thaper.
Srishti Nath, station fire officer, said the fire started around 4 pm. After the fire brigade was informed, fire engines immediately reached the spot.

He said chemicals were reportedly stored in the house-cum-factory due to which the fire could not be controlled easily. Since there were no fire-fighting arrangements at the unit, the employees failed to control the fire at the initial stage.
He said seven firemen had a close shave when there was a blast in the burning house. He had asked the factory owner if there was any cylinder or inflammable chemical lying inside the unit. Had the firemen stood close to the point where the blast took place, they would have suffered serious injuries. “The exact cause of fire is still unknown,” he added.

In October of the year just ended, no fewer than 11 persons in Abuja suffered various degrees of burns from a gas explosion. A month earlier in Lafia, Nasarawa State, dozens were left dead with hundreds injured when a gas tanker exploded and started a blaze.

Three years ago there was a gas explosion when a chlorine cylinder exploded at the Plateau State Water Board treatment plant at Lamingo, Dogon Karfe, Jos South Local Government Area, leaving eight people dead, among them a pregnant woman and children. Dozens of others sustained varying degrees of injuries. Dr. Philemon Golwa, the Chief Medical Director of Plateau State Specialist Hospital, said that a leakage at the plant was responsible for the deaths as residents around the area inhaled the poisonous gas which enveloped the entire neighbourhood.

Around 7 p.m., crews were dispatched to a second-floor unit at the Kihei Villages.
By the time they got there, residents had extinguished the fire, but upon investigation, fire officials found a leaking five-gallon propane tank which is believed to have started the fire. MFD added that no Christmas trees were burned in the small fire.
“The cylinder which was still actively leaking was moved to an area away from residents and structures where the remaining propane was transferred in to a functioning tank by Hazmat team personnel,” MFD said in a news release.

    <dc:creator>dchas</dc:creator><description><![CDATA[A few days ago I received a message in one of my WhatsApp groups from a researcher friend at Indian Institute of Science (IISc), Bangalore, insisting us to be extremely cautious while performing experiments in our labs. Rightly so, the explosion, suspected to be caused by a leak in hydrogen cylinder in the Laboratory for Hypersonic and Shock Wave Research in IISc created shivers in the whole research community; it resulted in the instant death of one young engineer and injured three others, while also damaging the equipment in the laboratory. The accident is not unique of its kind, there have been many other accidents that have occurred in the laboratories of academic institutions. For example, in June, 2009, extremely inflammable hydrogen gas, having the potential of forming explosive mixtures with air, leaked during an experiment and resulted in an explosion injuring four research scholars at the University of Missouri biochemistry research lab in Columbia.

The extent and the severity of the damage caused by such accidents calls for an imperative need to take pro-active safety measures to enhance safety at the academic workplace. The need is further stressed upon by the fact that academic institutions are relatively less regulated by the government in terms of following safety protocols. The pro-active measures must begin from within and must start with creating extensive safety awareness among the individuals involved, by rigorously discussing the various accidents that have occurred in academia or in other related fields (to take home important safety lessons), and finally religious implementation of the same.

According to the reports, the cylinder explosion incident occurred on Wednesday around 2.20 pm killing researcher Manoj Kumar, 32 from Mysuru while his colleagues – Athulya Uday Kumar, Naresh Kumar and Kartik Shenoy sustained severe injuries, TOI reports.

The FIR names Prof Jagadeesh and KPJ Reddy as first accused in the case. They both own the Super Wave Technology Ltd (SWTPL) on the IISc premises. The IISc has been named third accused in the case.

Prof Jagadeesh is a senior scientist at the Laboratory for Hypersonic and Shock Wave Research (LHSR) of IISc’s aerospace engineering department.

Acting on complaint filed by MR Chandrashekhar, chief security officer for IISc, the Bengaluru Police have registered a case of ‘causing death by negligence’ under IPC section (304A) and ‘causing grievous hurt by act endangering life of personal safety of others’.

“Preliminary probe suggested no negligence. We are waiting for the forensic report and charges will be dropped against the suspects if it’s found that the blast was an accident,” a police officer said.]]></description>

The IISc security chief claims it has a special officer to ensure safety and an audit on the same is done often.

A former technical officer at the IISc, who requested anonymity, said that several suggestions put forward to step up safety measures at the institution long ago went unheard.

“I have over the years recommended that the IISc acquire a fire tender, but the recommendations have fallen on deaf ears. How many more young lives do we have to lose before we wake up,” the officer asked.

Admitting the hydrogen cylinder blast was the first such incident at the 110-year-old institution, the former officer recalled the fire mishap of 2007, which gutted the Molecular Cell Biology laboratory, due to an electrical overload on the air conditioners. Several students inside the building had a miraculous escape then.

The former officer shared details of a 2010 workshop — Fire Safety Engineering and Structures in Fire — conducted by IISc’s Department of Civil Engineering, where top scientists, professors from elite institutions and seniors members from various disaster management groups across the country were in attendance.

    <dc:creator>dchas</dc:creator><description><![CDATA[A day after a 32-year-old was killed and three others injured in a suspected cylinder blast inside a laboratory of Hypersonic and Shock Research Center in the Indian Institute of Science (IISc) campus, forensic experts and along with fire and emergency personnel continued to gather evidence from the mishap site.
The police, in their preliminary report, have declared that it was a cylinder blast. Besides, they have taken some samples with them for further procedures. A senior police officer added that the police team questioned IISc staff to find out how exactly the blast happened and what are the standard safety measures followed during the tests.
“We are yet to confirm whether it was an act of negligence. For now, we are treating it as accidental blast,” the officer said.
The police, on Wednesday, had said that the victims, all interning with Superwave Technology Private Limited, were conducting an experiment involving supersonic shock waves. A highly combustible mixture of hydrogen and oxygen was used for the process. The cylinder it was in exploded and Manoj Kumar, who was working next to it died on the spot. The impact was such that his body was thrown 20 feet away.]]></description>

In a statement yesterday, the Ministry of Manpower (MOM) said that Chemical Industries (Far East) had pleaded guilty to a charge under the Workplace Safety and Health Act for failing to take reasonable measures to ensure the safety and health of the people involved.

An MOM spokesman told The Straits Times last night that the company pleaded guilty in court on Sept 14 and was fined the same day.

The incident involved a chlorine leak in a cylinder stored in Chemical Industries' Jalan Samulun chlorine storage facility in Tuas.

The leak triggered an alarm from one of the facility's chlorine detectors and a worker activated the chlorine gas emergency scrubber and exhaust system. Two workers donned protective equipment and attempted to stop the leak but were unsuccessful. Chemical Industries then called the Singapore Civil Defence Force (SCDF) for help at around 9.20am.

Kolkata, Sep 16 (IBNS): As many as 30 fire tenders and over 100 firemen are currently engaged in a firefighting operation in Kolkata's Bagree Market as a massive blaze continued to rage the commercial hub in central part of the city even after 19 hours.

An official of state fire and emergency services department said that third, fourth and fifth floors of the six-storey building are still on fire.

"We have heard sounds of explosions from these three floors which indicates that several gas cylinders were stored there and besides air conditioning machines, those gas or chemical containing cylinders are also going off due to extreme heat," the official told IBNS.

"We have brought a small hydraulic ladder and we are trying our best to use that at the congested area," he added.]]></description>

Back in February, the Baytown Fire Department responded to a 911 call at the HP Gas facility on West Bay Road, and subsequently discovered hazardous materials being stored in an unsafe condition. The Chambers County Fire Marshal’s Office was then notified, and upon inspection, the fire marshal found unstable portable compressed gas cylinders and various chemical containers being stored improperly.
