    <dc:creator>dchas</dc:creator><description><![CDATA[As investigators have released the majority of the Foundation Food Group plant back to the company, advocates in the community say workers are concerned about returning to the workplace that saw six deaths and 12 total people hospitalized.

The U.S. Chemical Safety and Hazard Investigation Board told The Times Tuesday, Feb. 16 it had released the majority of the facility which experienced a liquid nitrogen leak Jan. 28.


“There is one area which the (chemical safety board) has asked the company to preserve,” according to the chemical safety board. “This area is known as the exclusion zone and the company is able to run the operation without impacting this part of the facility. It is now up to the company to determine their start-up plans.”

The chemical safety board said the exclusion zone is a single room that houses a two-stage nitrogen freezer.

“If you were to be working at an office or a warehouse or anywhere where your coworkers died in front of you, it’s completely understandable that even you or me would have reservations about returning to work,” said Elliot Lepe, outreach paralegal with Southern Poverty Law Center.

    <dc:creator>dchas</dc:creator><description><![CDATA[After federal investigators pointed to the charging of lithium-ion batteries as a possible cause of the fire that killed 34 people aboard the Conception dive boat, the U.S. Coast Guard is now directing its inspectors to look more closely at how the batteries are used, charged and stored aboard small vessels.

According to a policy letter issued on Oct. 30, Coast Guard inspectors should look for potentially hazardous conditions such as batteries not being stored in cool, dry places away from combustible material, or batteries being charged using multiple extension cords, also known as daisy chains. Inspectors also should verify that crew members know how to extinguish small lithium-ion battery fires.

The letter follows a notice that the Coast Guard sent to boat owners in the wake of the Conception fire on Labor Day 2019 that warned of the dangers of lithium-ion batteries. And it comes weeks after the National Transportation Safety Board determined that the fire aboard the Conception began in the back of a middle deck salon where lithium-ion batteries were being charged.

    <dc:creator>dchas</dc:creator><description><![CDATA[BEAUMONT, Texas — The family of a man killed in a fiery 18-wheeler crash near downtown Beaumont earlier in October has filed a wrongful death lawsuit against the company he worked for, claiming the truck wasn't taken care of. 

Jose Flores, 57, was driving on I-10 on October 10 when his truck reportedly overturned and caught fire. 

According to the Houston law firm representing Flores' family, he was 'killed in a horrific fire that engulfed the cabin.' 

The firm said the crash happened before 5 a.m. The truck was owned by Total Chemical Service, Inc. 

The firm said while the investigation isn't finished, "hazardous chemicals being hauled by the 18-wheeler began leaking. The cabin then burst into flames trapping Mr. Flores in the cabin fire. Tragically, despite an off-duty Beaumont police officer’s attempts to free Mr. Flores, he was unable to be rescued from the fire and lost his life at the scene of the incident."

    <dc:creator>dchas</dc:creator><description><![CDATA[A US court has dismissed charges against Arkema relating to the peroxide fire and harmful emissions that occurred when Hurricane Harvey hit its Texas site in 2017.

The facility in Crosby, Texas, was flooded during the hurricane which caused a power cut and led to the ignition of organic peroxides, which need to be refrigerated. This led to the evacuation of around 200 people and 21 first responders were admitted to hospital after inhaling the fumes.

The US Chemical Safety Board (CSB) published its final report on the Arkema incident in 2018, noting that the risk of severe flooding was not documented by Arkema during its hazard analysis despite the site being located on a flood plain.

Criminal charges of reckless emission of air pollutants were brought against Arkema, the CEO of Arkema North America Rich Rowe, and former plant manager Leslie Comardelle by Harris Country District Attorney Kim Ogg in 2018. Arkema was facing a US$1m fine and Rowe and Comardelle were facing up to five years in prison if convicted. The prosecution argued that Arkema failed to prepare the plant for the hurricane by not moving the organic peroxides before the storm arrived.

    <dc:creator>dchas</dc:creator><description><![CDATA[PALM BAY, Fla. – The Department of Environmental Protection investigation into the September explosion at FAR Chemical in Palm Bay is still underway but investigators have found possible violation of Florida law, according to a letter sent to the company by the Florida DEP.

On Sept. 8, a series of explosions rattled FAR Chemical that could be seen for miles, sent dark smoking bellowing into the sky and shut down U.S. 1. No one was injured in the explosion.

The DEP responded to assess potential environmental impacts following the release of hazardous materials. According to its website, FAR Chemical has been “handling and processing complex and hazardous chemistries” at its Palm Bay facility for the past 35 years. The chemicals manufactured at the site are used in pharmaceuticals, coatings, adhesives, sealants, composites, flavors and fragrances.

A Texas judge has dismissed the criminal charges case against French chemical giant Arkema, the chief executive of the company’s North America division, and one of its plant managers, all related to emissions of hazardous chemicals during hurricane Harvey in August 2017. The storm caused flooding that knocked out power to the plant, which led to the organic peroxides stored there igniting once refrigerators, generators and backup cooling methods failed. The resulting fires forced 200 residents to evacuate and sent 21 first responders to hospital.

The US Chemical Safety Board’s concluded in its May 2018 final report that Arkema didn’t consider it a ‘credible risk’ that flooding could overwhelm its electrical and safety systems. The agency urged the chemical industry to better prepare for extreme weather events.

Recently, engineers at the University of Sheffield in the United Kingdom analyzed 16 such videos that were captured from different locations around the explosion and had an unobstructed view of the event and its aftermath. From this visual evidence, the researchers were then able to estimate the strength of the explosion, they reported in a new study.

Chairman of the Marine Environment Protection Authority Dharshani Lahadapura said that the laboratory report of the oil samples obtained from the location of the fire-stricken oil tanker submitted by the Government Analyst has been forwarded to the Attorney General Dappula de Livera Saturday (19).

She also said divers were still checking to see if there were any holes in the ship, which is now located about 90 nautical miles off the eastern coast of Sri Lanka.

MT New Diamond oil tanker carrying 270,000 metric tons of crude and another 1700 metric tons of diesel caught fire on 3rd September in the seas about 38 nautical miles off Sangamankanda Point in the eastern coast of Sri Lanka.

A combined effort of Sri Lanka Navy, Sri Lanka Air Force and Indian defense forces battling for six days put out the fire completely on 9 September. However, the damage to the engine room has caused the diesel from the ship to leak leaving kilometers long oil slicks.]]></description>

The association warns that what happened in Beruit should be a wake-up call for all chemical manufacturers, importers and storage facilities in South Africa.

"Ammonium nitrate should be treated, handled and stored with the utmost respect, as oxidisers can rapidly react with incompatible materials, including hydrocarbons and organic materials, to produce runaway fires and explosions, the organisation states.

RPMASA technical director Liz Anderson explains that ammonium nitrate is classed as an oxidiser and explosive, while potassium nitrate and sodium nitrate are also classed as oxidisers and components of fertilisers, which have been the cause of various chemical disasters over the years.

Arkema Inc, Chief Executive Richard Rowe and the former manager of the Crosby, Texas, plant, Leslie Comardelle, face criminal charges for recklessly releasing toxic emissions as organic peroxides burned during flooding from Hurricane Harvey.

State judge Belinda Hill found the prosecution improperly withheld evidence that might have benefited the defense. The judge issued a continuance for the defense and said she might issue further sanctions against the prosecution for the misconduct.

Pre-trial motions will now start on Wednesday and opening statements on Thursday.

Arkema and another executive, Michael Keough, Arkema's former vice president of logistics, also face assault charges for failing to disclose the danger to emergency workers posed by pollutants released by the fires.

Many of those exposed to the fumes were emergency responders, the U.S. Chemical Safety Board found in a report on the incident issued in May 2018.

"It is critical for the refinery to maintain an up-to-date and accurate Risk Management Plan," said EPA Pacific Southwest Regional Administrator Mike Stoker. "These actions ensure that facilities handling dangerous materials are minimizing potential impacts to the environment and the surrounding community.

An inspection found that Torrance Refining violated the Clean Air Act's Chemical Accident Prevention Program, due to inaccuracies in its Risk Management Plan, including; failure to properly conduct a hazard assessment; failure to properly document repairs on equipment; and failure to follow emergency operating procedures.

Torrance Refinery agreed to install a new automated water system to mitigate potential releases at its largest modified hydrofluoric acid tank, reported NBC Los Angeles.

    <dc:creator>dchas</dc:creator><description><![CDATA[A March 2019 explosion at a chemical plant in China’s Jiangsu province has refocused public attention on environmental issues across the country. The latest in a series of incidents, the blast is now forcing China to carry out a difficult balancing act between managing economic uncertainty and winning its self-declared “war on pollution.” The focus is quickly turning to environmental enforcement, in order to address questions regarding whether enough had been done to prevent this kind of tragedy from reoccurring.

Vigorous Enforcement Is the New Normal

Environmental regulation in China has become much more robust in recent years, with a corresponding increase in enforcement activity. Beginning in late 2015, a national inspection plan covering all 31 of China’s provinces began, lasting nearly three years. Leading up to the 19th National People’s Congress in late 2017, as many as 40 percent of the country’s factories were forced to shut down at some point, and more than 17,000 elected officials were investigated. In addition to the challenges created by the inspections themselves, firms have struggled with interruptions in their supply chains, and the inspections and closures have resulted in canceled and unfulfilled orders.

In addition to stepping up enforcement, China has also sought to address safety issues by relocating potentially hazardous industrial facilities (particularly chemical plants) to more remote industrial parks, far from population centers. For example, the State Council issued a guiding opinion calling for all companies manufacturing hazardous chemicals to be relocated to specialized industrial parks by 2025. Even prior to the explosion, Jiangsu province—a hub for chemical manufacturing—had issued a similar plan to relocate chemical plants away from population centers. Implementation of relocation efforts has been uneven, however, particularly with respect to compensation payable to the facilities.]]></description>

The school, at 128 Township Dr. in Hendersonville, was evacuated on May 9, 2018 when a science experiment caused a hazmat issue. Seventeen students and a teacher were injured. The teacher and eight students were taken to nearby hospitals.

The Hendersonville Fire Department’s investigation found that the incident was caused by mixing boric acid — a white, odorless substance — and ethyl alcohol. Investigators deemed the incident “unintentional,” according to a previous report.

The first complaint
Hendersonville parents filed a complaint with their son in the Circuit Court for Sumner County on March 5, alleging “personal injuries…caused by negligence” of the teacher employed by the board of education.

The family is seeking $100,000 and medical expenses, according to the complaint, signed by Attorney Joseph Y. Longmire Jr. of Hendersonville.

Stacey Tilcock’s husband, Paul, was found dead in a house where he was laying the floor for work.

At the time of his death, the 30-year-old appeared to be wearing a mask that was completely ineffective at protecting him against the chemicals he was working with.

His employer and the supplier have been fined £750,000 between them for the careless management and the supply of dangerous chemicals.

Brian Campbell, 31, was convicted with second-degree recklessly endangering safety, felony possession of improvised explosives and misdemeanor bail jumping.

Campbell forced an evacuation at an apartment building at Timber Lake Village due to a strong chemical odor coming from one of the rooms on Feb. 20, 2018. HazMat crews discovered evidence of experimentation and explosive materials inside.

Campbell will be sentenced on Friday, Feb. 22. The prosecution plans to ask for three years or less in prison for Campbell.

After a chemical explosion at a local lab, the Occupational Safety and Health Administration is looking deeper into the matter.

The small explosion and fire happened July 31 during a chemical process being done in a laboratory at Dietary Pros Inc., a company on Wausau's west side that makes dietary supplements, according to the Wausau Fire Department.

A worker suffered third-degree burns in addition to a broken hip from the explosion. 

OSHA has six months to complete the inspection, issue citations and propose penalties due to violations of safety and health regulations.]]></description>

Reactive organic peroxides decomposed and caught fire after floodwaters knocked out power and disabled refrigeration at Arkema’s Crosby, Texas, plant. The chemicals must be kept cool to remain stable.
In addition to the company, the indictment names Richard Rowe, CEO of Arkema’s North America operations, as well as plant manager Leslie Comardelle. If convicted, Arkema could face $1 million in fines and the executives could face five years in prison.
“Companies don’t make decisions, people do,” Harris County District Attorney Kim Ogg, whose office sought the indictment, said in a statement. “Responsibility for pursuing profit over the health of innocent people rests with the leadership of Arkema.”

A man was killed in a explosion, one week ago in Building 109 at the Village Glen apartment complex. The FBI did a controlled explosion last Wednesday, and Mayor Rebecca Glewen on Sunday confirmed there was another explosion last Thursday.

“There’s too many unknowns,” Glewen said, including what specific chemicals led to the explosion, and if there’s a connection to a chemical filled apartment discovered on Madison’s west side three weeks ago.

“We have on information connecting those at this time,” she said. “That’s part of the investigation that is ongoing.”

Residents of building 109 will not be allowed back in prior to the burn on Wednesday. “It was deemed just not safe for anyone to go in,” Glewen said.

The burn is set for 10:00  a.m. Wednesday. Precautions will be taken to save other parts of the complex, including a barrier to be placed in between Building 109 and neighboring Building #113.

As a result, classes that had been meeting there will also need to be reassigned to other parts of the building.

The district may be able to expedite the rebuilding of the science lab by seeking an emergency exemption from normal contractor bidding requirements under a New Jersey statute, 18A:18A-7, that allows for such exemptions in matters where public health and safety are involved, said DiBenedetto.

Still, he said, "I don't see that area (the science lab) being used for another three or four months."

Earlier Sunday, an insurance adjuster was on the scene assessing the cost of the damage from Friday's fire. No firm figures have been given yet, but school officials are apparently encouraged by what they heard.

"We have great people in the insurance area, and the information we received went beyond any hopes we first had as far as being covered by insurance," DiBenedetto said.

Friday's fire, which was said to have resulted from the discarded remains of a science experiment earlier that day, was first noticed at about 5 p.m. by a female custodian. DiBenedetto could not immediately confirm her identity but said it was his understanding that she immediately alerted students and others who were still in the building, and helped initiate an evacuation.

Within an hour, the fire was effectively out, but not before it had already done significant damage. DiBenedetto, however, credited the custodian's quick action with averting what might have been a much worse outcome.

"The custodian saw flames shooting out through the bottom of the door, and alerted the students and had the wherewithal to literally yell to any other adults who were in the building to get the students out," he said. "That custodian is a hero."

Crews were called to the high school at 134 Barren Road just after 10 p.m. to reports of people getting sick in the men's locker room.

Thirteen football players fell ill following their game against Radnor. Several were taken to the hospital.

A fire official tells Action News a ventilation system in the locker room malfunctioned.

"Upon arrival, crews discovered what we suspect to be an ozone machine in the men's locker room that was creating an irritant to a number of the football players," said Mark Kirchgasser.

Some of the players were taken to Riddle Memorial Hospital for treatment.]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[Expressing grave concerns over a series of industrial accidents reported in Karachi within a span of six days, the Pakistan Institute of Labour Education and Research (Piler) on Monday demanded of the government make speedy efforts to ensure workplaces across Sindh have occupational health and safety measures in place. 

In a statement, Piler Executive Director Karamat Ali said a number of factories and buildings caught fire in the city, some severe enough to have claimed lives of workers. 

Only this past Sunday two fires were reported in the city. The first one erupted at a 14-storey building and was so severe that three floors, including two popular eateries located on the building’s ground floor, were gutted. The second was reported to have engulfed a cardboard godown located near Shafiq More. No casualties were, however, reported in the two incidents.

However, in an incident reported on Saturday, three workers died while a fourth is still stated to be in a critical condition, after they fell into the chemical tank of a fish processing factory named Inter Market Factory, in Ibrahim Hyderi. 

On September 20, one labourer died while another suffered injuries when a chemical storage of a cosmetic factory exploded after a fire erupted in the factory, located on Sharea Faisal. The blast was triggered by insufficient safety measures in the factory’s basement where the storage unit was located.

    <dc:creator>dchas</dc:creator><description><![CDATA[Asbestos, trichloroethylene, pigment violet 29—these are just three of thousands of chemicals the U.S. Environmental Protection Agency is assessing for risks to human health and ecosystems under the revamped Toxic Substances Control Act (TSCA). Congress overhauled the chemical safety law last summer, with wide bipartisan and industry support. Many viewed the legislation as a much-needed update to old, feeble regulations. Now, though, the Trump administration may be undermining the reformed law.
After Congress amended the old chemical safety act, it tasked the EPA with writing what are called the “framework” rules for how the agency will implement the reformed law. Outside experts and environmental groups express deep concern that the EPA’s new framework rules for TSCA, which took effect in July, could seriously subvert the law’s purpose in favor of industry. “These are major rules that will set the conditions for how TSCA is implemented—potentially for the next few decades,” says Noah Sachs, director of the University of Richmond Law School’s Center for Environmental Studies.
The TSCA framework rules establish formal guidelines for how the EPA will assess tens of thousands of existing chemicals. For the most part, they specify how the agency will prioritize and evaluate chemicals for risks. The Obama administration had already proposed a version of the rules. The current administration took over and finalized them—but not without significantly rewriting them first. “The law is much, much less stringent” with the latest rules, says Rena Steinzor, a professor of law at the University of Maryland.
One of the most controversial parts of the framework is how the EPA changed a key term known as the “conditions of use.” It defines which applications of a chemical the EPA will examine in risk evaluations. For a given chemical, usages could range widely—from a consumer product like a kitchen countertop cleaner to various business and industrial applications. “Uses are critical, because they define exposure” to people and the environment, Steinzor says.
The Obama administration interpreted “conditions of use” broadly, experts say, but Pres. Trump’s EPA has significantly narrowed the term. For instance, the definition now excludes “legacy” applications—a past use of a chemical that has been discontinued. One example of this is a class of chemicals called polybrominated diphenyl ethers used as flame retardants, which were added to furniture cushions until recently. Experts say the EPA still needs to consider exposure to these legacy uses. “When you’re assessing a chemical, it’s important to look at all the uses to understand the actual risk in the real world,” says Richard Denison, a lead senior scientist at the Environmental Defense Fund. That’s because previous or ongoing exposure to a legacy use of a chemical could complicate a person’s exposure to the chemical’s present-day uses. “You have to recognize that the way someone responds to a new risk is partly based on what else they’ve already been exposed to,” Denison says. Sachs agrees: “Congress’s concern is about aggregate exposure, and that makes sense, because that’s what matters to human health,” he notes. “If this rule stands, it is a weakening of TSCA and not at all what Congress intended.”
Karyn Schmidt, senior director of Chemical Regulation, Regulatory and Technical Affairs at the industry group American Chemistry Council (ACC), disagrees. “It’s clear that the legacy uses are cases where EPA does not think it needs to prioritize its resources,” she says.

    <dc:creator>dchas</dc:creator><description><![CDATA[HOUSTON, July 13 (Reuters) - Exxon Mobil Corp has been fined about $165,000 by U.S. regulators for safety lapses including inadequate training and equipment maintenance over an explosion that injured four workers at an aging Baton Rouge, Louisiana, refinery last year.

The U.S. Occupational Safety and Health Administration (OSHA) issued nine citations, several of which echo previous cautions by federal agencies at two other Exxon plants. The citations, issued in May, were seen by Reuters this month.

A separate investigation by the U.S. Chemical Safety Board (CSB) is ongoing and its report on the incident is due by year-end.

Exxon said it is contesting the OSHA citations and fines.

The facility was faulted five years ago by the U.S. Environmental Protection Agency (EPA) for failing to address corrosion on pipes and valves and for inadequate shutdown and emergency procedures provided to workers.

The Nov. 22, 2016 explosion on a sulfuric-acid alkylation unit that makes octane-boosting components of gasoline in the sprawling Baton Rouge refinery and chemical plant injured four workers, two of them severely. Two of the affected workers declined to comment; others could not be reached.

A worker on the alkylation unit removed the cover of a malfunctioning valve on an isobutane line and used a wrench to turn the value stem, Exxon reported to the Louisiana Department of Environmental Quality in a letter. Volatile isobutane is converted in the alkylation unit to a component of gasoline.

As the operator turned the valve stem, portions of the valve fell out, releasing isobutane, according to the Exxon letter, which was ignited by a welding machine 70 feet away.

One worker was knocked off a scaffold next to the alkylation unit and left dangling over the fire, according to two sources. Another worker was burned over most of her body.

The fire in the city of Lengshuijiang broke out about 11.20pm (AEDT) on Saturday and firefighters were able to bring it under control - with no casualties reported.

The chemical plant's warehouse contained chemicals and plastic bottles, but had reportedly suspended its production beforehand due to an investigation into PVC additives.

Local residents were evacuated and local environmental authorities are monitoring the air quality for any potential harmful pollution.

Industrial facilities across China have been rocked by explosions in recent months.

Four days after the streets ran purple outside a massive fire that destroyed an industrial container refurbishing facility in Jessup, borough firefighters are waiting to see if 10 sets of fire gear have been rendered useless by chemicals in concentrated purple dye.

Mr. Fetcho, Jessup’s first assistant fire chief, sent the stained gear to a cleaner, Lion Total Care, which recommended the department take the gear out of service pending the results of chemical tests..

“If it comes back and everything’s normal, the color is not going to bother us,” Chief Fetcho said.

    <dc:creator>dchas</dc:creator><description><![CDATA[Six undergraduate students were exposed when a plastic container of waste chemicals ruptured in a Chemistry Annex teaching lab at around 4 p.m. Tuesday, according to an email Jonathan Sweedler, director of the School of Chemical Sciences, sent to SCS faculty, staff, postdoctoral and graduate students.
As a safety precaution, five of the exposed students used a safety shower and were taken to a medical facility. One of the students reported having a chemical burn on their nose, campus spokeswoman Robin Kaler said in an email.
Kaler also said a sixth student reported a minor irritation on their face to at TA a few hours later.
The investigation of the exact cause of the chemical reaction is ongoing, but initial findings show that a waste container exploded due to a buildup of excessive pressure.
“The Division of Research Safety believes the reaction was caused by nitric acid mixing with a reducing agent to produce a nitrogen oxide gas,” Kaler said in the email. Two containers of ammonium hydroxide broke due to the rupture, releasing ammonia gas into the air, she added.
Students were able to retrieve their belongings from the lab later that night, and the lab was opened for classes on Wednesday.
Kaler also stated in the email that “To prevent something like this from happening again, the laboratory instructors have removed nitric acid from this experiment pending a more detailed review of the laboratory’s objectives and whether the use of nitric acid is necessary for those objectives.”]]></description>

A flash fire that occurred at an Elite Pipeline Services Inc. Odessa location will end up costing the company $25,200, according to documents acquired from OSHA.

According to the documents, a flash fire began at 11 a.m. Feb. 29, injuring three of the four employees at the site and sending them to the hospital.

“While attempting to demolish old petroleum pipe, employees used a cutting torch to sever bolts on a valve,” according to the documents. “The system had not been purged in over a year and vapors were present, resulting in an explosion and flash fire.”

The employees were not properly equipped or trained for the hazards of their work and were not notified of the possible hazards at the McAfee Pump Station before they began the work, according to the OSHA complaint, which lists six specific violations.]]></description>

U.S. refineries must notify the National Response Center if they release hazardous substances in excess of reportable quantities, according to the Comprehensive Environmental Response, Compensation and Liability Act, commonly known as Superfund.

The factory at Gladstone in the mid-north exploded in 2006, killing three workers and injuring two.

The Industrial Court found the explosion was triggered by a cook-off effect inside a poorly-maintained chemical-mixing machine.

The company Quin Investments and its chief operating officer Nick Kuzub were convicted of breaching workplace safety laws and fined $95,000 each.

Lawyer Grant Germain has argued on appeal that the magistrate's findings were implausible and Mr Kuzub should have his conviction quashed.

The employee at Abacus Chemical Ltd was mixing two chemicals together on 7 May 2009 when they exploded, causing him to be drenched in a hot, toxic chemical solution.

The company was prosecuted in a joint case brought by the Health and Safety Executive (HSE) and Environment Agency following the incident at its plant on Oil Sites Road in Ellesmere Port.

Chester Magistrates' Court, sitting in Knutsford, heard that the company had mixed 22 kilograms of sodium cyanide pellets with hydrogen peroxide to make them less toxic. By taking this action, Abacus avoided having to pay a licensed hazardous waste company to dispose of the pellets at a cost of less than £300.

Workers suffered minor respiratory ailments when a plastic drum of waste nitric acid apparently ruptured under pressure around 7 a.m., the Fire Department said. The rupture sprayed about a gallon of the chemical around the laboratory, said Ed Coletta, spokesman for the state Department of Environmental Protection.

The spill was contained to the room where it happened, Coletta said.

Chemistry professor Jacob de Boer told television programme Nieuwsuur the amount of lead – up to a thousand times the permited level - is 'extremely worrying'.

Officials have so far maintained there is no danger to human health from soot released in the blaze.

Nevertheless, on Tuesday Breda mayor Peter van der Velden said the plant may have broken its licencing conditions and could be liable.

Van der Velden was speaking on behalf of the public prosecution department and refused to answer any further questions, news agency ANP said.

A department spokesman told ANP officials had carried out searches of the Chemie-Pack offices, and that revealed possible infringements of the licence.

But he too refused to go into further details.

A spokesman for the company also refused to elaborate on the allegations

Dioxin

Meanwhile, it also emerged on Tuesday that cancer-causing dioxins had been found in the soot which had fallen in a 10-mile radius of the blaze. But the concentrations 'were not unusual' in the winter, Dordrecht mayor Arno Brok was quoted as saying by Nos tv.

A high concentration of dioxin found in grass growing 3 km from the fire was being looked into, he said.

Nevertheless, previous advice not to eat winter vegetables and to keep children and livestock indoors remains in force, Brok said.

The current affairs programme EenVandaag, which has seen inspection reports dating back to 2002, says that despite this it continued to be granted permits.

As early as October 2002 the Public Prosecutor's Office threatened Chemie-Pack with sanctions for contravening fire safety regulations. The company's automatic sprinkler system was not operating properly and the roof was not sufficiently fire-resistant. Between 2007 and 2009 inspectors reported that chemicals were not stored correctly.

Fierce chemical fires broke out at Chemie-Pack's plant in Moerdijk, south of Rotterdam, on Thursday last week. It took firefighters over 30 hours to get the blaze under control. 

Professor of Environmental Science Lucas Reijnders, who studied the documents for the TV programme, says the company should have been shut down.

Early on the morning of Jan. 6, 2005, a 42-car Norfolk Southern train crashed into two locomotives and two rail cars. The freight train was carrying more than 250 tons of toxic chlorine gas which was released in the crash.

Nine people died as a result of the toxic fumes, more than 250 people were sent to hospitals and about 5,400 people were evacuated.

Officials said the accident was caused by a displaced manual switch on the train tracks, which should have been put in the proper place by Norfolk Southern employees.

The lawsuit, filed in Hennepin County, demands 3M pay for the damage, which the state says includes the loss of fish and damage to groundwater, surface water and sediment. The lawsuit doesn't specify the cost of the damage.

"3M made a mess, they contaminated the waters," Attorney General Lori Swanson said. "We want them to step up and make it right."

3M spokesman Bill Nelson said Thursday that 3M has stepped up, pledging to pay the state up to $8 million to cover some direct costs of cleanup and $5 million to fund environmental research into the chemicals. 3M also is working on remediation at its disposal sites.

The lawsuit focuses on 3M's disposal of perfluorochemicals, or PFCs, and their compounds. The Maplewood-based 3M began producing PFCs in the 1940s, and legally disposed of them in landfills until the early 1970s — when the company built a corporate incinerator to handle the waste.

The report from Fire Marshal Stephen Coan's office into the July explosion in Norfolk instead pointed to another possible cause — a phenomenon known as "odorant fade."

That can occur when the chemical that gives propane its distinctive "rotten eggs" smell fades over time. Odorant fade can occur in new tanks or tanks that have run dry and are only partially filled.

The 1,000-gallon tank at the condominium construction site had only 200 gallons of propane in it at the time of the explosion, which killed an electrician and injured three others workers and two firefighters. Workers said they did not smell gas before the blast.

The blaze at the warehouse on Friday night killed six workers, five on the spot, and left five others critically injured. The deceased workers are Almas, Tofazzal, Aminul, Yusuf, Russel and Anwar. Of them, Russel, Yusuf, Tofazzal and Aminur were of the same family.

The injured were having treatment at the Burn and Plastic Surgery unit of Dhaka Medical College Hospital (DMCH). Six of them were brought in early yesterday, but Anwar, 26, succumbed to his injuries.

According to sources, the fire broke out when workers, basically labourers with little skill, were puncturing cans to let out the chemicals and gases. The cans had expired their best-before-use dates.

They said the liquid chemical from the punctured cans was kept in large barrels that also caught fire, resulting in a blow up.

In March 0f 2005, the Colorado Department of Public Health and Environment instituted the Colorado Board of Health Regulations Pertaining to the Cleanup of Methamphetamine Laboratories. It states after a industrial hygienist has cleared a property, the owner can request it be removed from any government lists.
Cuddie attempted to get his home removed in 2005. But said he ran into problems with the health department because his home was contaminated and cleaned up prior to the new regulations.

And there's no major route in or out of the city where hazardous materials can't be found: I-16, I-516, I-95, Ga. 21, Bay Street, Martin Luther King Jr. Boulevard, DeRenne Avenue, U.S. 80, U.S. 17.

But he hasn't completed a flow study that will give a more detailed picture of which materials are being transported on which roads. Savannah State University students will help him complete that study by actually tallying the identifying triangular hazmat placards they observe on trucks going by designated study spots.

Council requested the study in October after discussions of proposed LNG trucking revealed a lack of detailed data about the types and quantities of all hazardous materials flowing through the city.

There were 14 willful and one serious citation filed for allegedly failing to implement measures to prevent chemical fires and explosions at its plant in Eau Claire, where an OSHA investigation took place after a June 29 explosion and fire in a solvent sludge feed tank.

The tank's roof blew off, and a nearby tank also exploded. "Employees had been working in the area of the solvent sludge feed tank immediately prior to the explosion. Fortunately, there were no reported injuries," OSHA stated in its news release.

"Even after WRR Environmental Services experienced a devastating fire that destroyed the facility in 2007, the company still failed to implement an adequate program to ensure safe operating conditions," said OSHA Assistant Secretary Dr. David Michaels.

The update from the National Transportation Safety Board also noted that "no physical evidence suggests that a pre-existing leak occurred in the ruptured pipe pieces."
In the days immediately following the blast, some experts had speculated that the 54-year-old steel gas transmission pipe may have been weakened by corrosion or by work in 2008 on a sewer line in the same location.
Saying the agency is
continuing to analyze the chemical and mechanical properties of the pipe, as well as unspecified "environmental factors at the accident site," the report added, "the investigation is still in an early phase and there is much factual information to be developed."
Lingering uncertainties over what caused that section of pipe to rupture has aroused widespread concern that other sections of PG&E pipe may have similar vulnerabilities, especially since PG&E officials insist they had inspected that portion of pipe and found nothing wrong. In the first report on the accident, which was issued Oct. 13, the safety board noted only that the steel pipe fractured lengthwise and at welds that held it together.

The deceased was identified as Mohammad Sujan Ali, 30, a tenant of the four-storey building.

Doctors declared him dead around 6:30 in the evening.

Doctors also said 42 percent of his body got burnt in the deadly fire.

The fire broke out while refilling cigarette lighters at a building storing liquid petroleum gas and lighter refill canisters on Nazimuddin Road.

Sujan’s relative Ripon, 28—also got injured in the fire and he is still undergoing treatment at the burn unit of the same hospital.

Ripon had burns over 28 percent of his bodies, hospital sources said.

Earlier on October 6, at least 10 workers were burnt to death at a chemical factory in the capital's Dakkhin (South) Jatrabari. Four others, including three women, were severely injured.

On August 22, two persons died in a fire at a chemical warehouse in Lalbagh.

On June 3, 122 people, mostly women and children, died and scores were hurt in a deadly blaze, which originated from a chemical store in the densely populated Nimtoli in Old Dhaka.]]></description>

The names and ages of the teens were not released because they are juveniles, according to Lt. Shawn Freitas. The charges against them will proceed in Juvenile Court, he said.
Police said the mail box exploded into pieces, with some landing more than 20 feet from the post. Pieces of the mailbox had a strange brownish liquid on them and the bottle had a chemical smell.
The mailbox was blown up Sunday morning by what police describe as a "works" bomb, a chemical reaction bomb made from common chemicals and powerful enough to deliver second or third degree chemical burns, serious injuries or death.

"It won't be appropriate to conclude that the fire incident had resulted out of negligence on the part of the concerned authorities," minister for environment Sanjay Deotale told the state assembly in a written reply on Saturday.

While they don’t have a turnkey strategy to deal with a situation of this rarity, explosives experts said the developing blueprint seems solid.

“The plan is tight,” said Neal Langerman, a San Diego-based chemistry expert.

The three-bedroom home on Via Scott is so cluttered with dangerous chemicals — all of which terrorists have used to make explosives — that incinerating the home is the only way to safely remove everything, authorities said.

County leaders said they don’t know how much the operation will cost.

The prescribed fire is expected early next week, but county air officials said it won’t start until the winds are blowing about 3 mph toward lesser-populated areas to the east and there’s no inversion layer that could trap smoke. That’s usually the case in the late morning.

“It’s all science-based and theoretical right now, and I think the challenge is to make sure the proper public safety precautions are being taken,” said Don Plain, head of emergency response for the state Department of Toxic Substances Control. “It’s an unknown situation, to a certain extent. This is a variable that has people, I think, a little nervous, but I don’t really think (officials) have another option.”

The house was rented by George Djura Jakubec, 54, who is being held on $5 million bail on 28 counts, mostly for manufacturing or possessing explosives. He has pleaded not guilty.

Bethlehem police said the collection of chemicals they found in Jason D. Sanchez's storage area in the basement of his Cherry Arms apartment building posed "a grave risk of death" to other residents in the Delaware Avenue complex.

"He had apparently been setting up some type of laboratory in there," Deputy Police Chief Timothy Beebe said. While investigators have not determined what Sanchez, 24, intended to build, Beebe said "the potential for danger was high" and that the setup did not appear to be a makeshift laboratory for manufacturing crystal meth.

Sanchez is a graduate student at Rensselaer Polytechic Institute and a teachers assistant at Albany's William S. Hackett Middle School.

Confiscated from the basement were acetone, Xylene, sulfuric acid, a propane torch, butane fuel and laboratory-grade nitric acid, Beebe said at a news conference Wednesday.

Twenty-four-year-old Jason Sanchez is facing a number of charges after police say they found dangerous chemicals in the basement.

While there are some answers, police still have a lot of questions in the investigation.

David Cohen of Delmar says, "It's idiotic."

Many who live in the Town of Bethlehem are shocked that Sanchez allegedly had dangerous chemicals in the basement of his apartment building. 

Cohen says, "there should be some way of checking where they came from."

Police say what they found in the basement called for multiple agencies to respond. Police say they seized acetone, xylene, laboratory grade nitric acid, sulfuric acid, a propane torch and butane, along with a homemade commercial grade vacuum chamber.

Officials have been holding meetings behind closed doors as they try to organize the huge operation planned for next week. On Thursday, crews will begin cutting back trees and brush around the house on Via Scott.
The burn was planned after bomb technicians and Hazmat crews discovered homemade explosives and hazardous chemicals strewn throughout the house rented by 54-year-old George Jakubec, who according to a search warrant released Wednesday confessed to robbing three banks and trying to rob one of them twice.
Pictures taken inside the house showed clutter on every surface. Mixed in with that clutter, were grenade casings, a jar of explosives, blasting caps, and chemicals. A huge wall will be built around the entire house before firefighters light the house in fire.

The Insurance Council is alarmed that the number of claims for damage to property from P lab activity is increasing.

Property damage from P labs is caused by the chemical contamination that results from the cook-up process and, in worst cases houses may need to be demolished.

There are also a number of instances of fire damage due to P lab explosions.

Insurance Council spokesman, Terry Jordan, says property losses are becoming more significant and insurers are becoming increasingly concerned about the extent of these losses.
