A fire inside a tank of liquid asphalt at a facility in Madison, North Carolina, on Tuesday prompted officials to evacuate residents from the nearby area as firefighters determine how to extinguish the flames.

The fire broke out inside a 17,000-gallon tank that was filled with about 11,500 gallons of liquid asphalt at SealMaster on West Decatur Street, the Madison Fire Department told reporters.

"We have an evacuation zone right now of 1,000 feet in all directions and everybody within a half a mile is sheltering in place," Madison Fire Chief Jim Ritchey said.

Ritchey said that firefighters tried using Purple K, a dry chemical substance, to put out the fire but it did not work. He said that firefighters don’t want to add water or air into the tank at this time.

SANFORD, Fla. – Chemicals dumped near fuel pumps caused a mini explosion at a BJ’s Wholesale Club in Sanford on Tuesday, according to the Seminole County Fire Department.
Fire officials told News 6 a third-party company was working on something at BJ’s. According to the fire department, someone pressed a wrong button and trigged the fire suppression system.

The system expelled approximately 560 gallons of dry chemical — a type of powder used to subdue flames — near the fuel pumps, fire officials said.
Crews helped get powder off six customers who had been getting fuel from the gas station, the fire department stated.
According to the fire department, the powder can cause eye irritation or respiratory issues.
Crews said four patients were taken to the hospital, and another two were evaluated after some individuals cited difficulty breathing. No life-threatening injuries were reported, the department said.
Sylvia Maldonado was one of the patients taken to the hospital after the explosion. She told News 6 that people “were going crazy” because they didn’t know what was going on.

MADISON, Wis. (WMTV) - A lithium-ion battery started “popping, sparking, and smoking” while it was charging, causing a fire at Madison home early Saturday morning, one of its residents told fire investigators.

The man recounted waking up to sounds of the battery catching fire and racing to make sure everyone else in the home on the city’s west side made it to safety. Several of them tried using dry chemical fire extinguishers to put out the flames to no avail, the MFD report continued.

Fleeing the home, they called 911. When firefighters arrived at the house, in the 5200 block of Harbor Court, around 3:40 a.m., they reported seeing smoke coming from the second story window. They got to the room and used a water can to put out the fire and the positive-pressure fans to ventilate the house.

The room where the battery was suffered smoke and fire damage, the MFD’s statement indicated, and no injuries were reported.

A MFD spokesperson stated the battery was for an electric bicycle. litihium-ion batteries are commonly used in many rechargeable devices – including, but not limited to, cell phones, laptops, power tools, and even electric vehicles.

A fire near Cinderella Castle sprouted from a tree around 6:45 p.m. Tuesday at Walt Disney World's Magic Kingdom, according to Reedy Creek fire officials.

Fire officials say the fire could have been caused by debris from the classic fireworks show set off at the castle.

The fire department said Cinderella Castle was evacuated, along with surrounding areas.

In a "very unusual occurrence," Walt Disney World staff escorted the first responder trucks down Main Street, said Reedy Creek Fire Association president Jon Shirey.

No one was hurt in the incident except for one person who accidentally inhaled the chemicals from a chemical fire extinguisher used by the fire department, Shirey said. That person was treated on the scene.

A representative from Walt Disney World described the fire as small. Firefighters quickly extinguished the flames, according to Disney.

In the spring of 1867, a crowd of local residents gathered on Central Square in Keene to view a demonstration of a new invention. This new machine was a chemical fire extinguisher. Although portable fire extinguishers had been in use for several decades, this new development may have been a soda-acid extinguisher, which was patented the previous year. A committee was appointed by the selectmen to view the demonstration and make a recommendation concerning whether Keene should make use of the extinguishers.

A 12-foot-square shed was built in the middle of the street just south of the Keene common at Central Square. The grand finale of the demonstration was to be the use of the new machines to extinguish a fire in the shed, which had been filled with combustibles. As a preliminary event, however, a pile of oil barrels was set on fire at the northeast corner of the square.

As the demonstrators were spraying the burning barrels, a shout went up that someone had set fire to the shed on the other side of the common. The extinguishing crew abandoned the barrels, which were burning brightly, and ran to the now blazing shed. Just as they arrived at the shed, someone jokingly turned down the valves on the chemical tanks. The fire was so hot that the demonstrators could not get close enough to reach the flames with the diminished spray from the extinguishers. The crew finally gave up and watched helplessly as both the barrels and the shed burned in the street.

The crowd was highly amused by the disastrous demonstration. The committee appointed for the event did not recommend that the town rely on the extinguishers for use by its firefighters. They did state that such machines should prove invaluable in shops, hotels and factories where watchmen or other employees were available to make use of them immediately before a fire became too involved. Although the fire extinguisher has earned an important place in homes and businesses around the world, it is a good bet that none were sold following the hot demonstration on Central Square in April of 1867.

FDA’s 30 September warning letter to Marcus Research Laboratory found “significant deviations from current good manufacturing practice (CGMP) violations for active pharmaceutical ingredients (APIs)” in its manufacturing of povidone iodine (PVP-I) used for presurgical preparations. The warning letter followed inspections conducted in May.

The firm failed to adequately investigate the effect of temperature excursions on the quality of an API following a fire and use of an extinguisher.

During production, personnel observed PVP-I in a reactor cabinet that was “smoldering” and responded to an “imminent fire” by using a fire extinguisher. The API was subjected to “inconsistent temperature exposure and additional processing and hold times.” Yet the firm only tested the affected drums to determine the amount of iodine that was still present in the material and released them for further processing.

A similar situation occurred in August 2020 when production personnel observed a “smoking” reactor cabinet in the facility. Production stopped and the materials were “held in unknown conditions,” according to the letter. “Your firm again, solely tested the affected drums of material for percent available iodine and released them for further processing” after this second incident, noted FDA.

“In both instances, your firm created deviation reports but released drugs based solely on composite results of percent available iodine,” wrote the agency. “Your drugs were exposed to excess heat, fire, smoke, and debris. Additional contaminants or other particulates from the use of the fire extinguisher may have also compromised the material.”

sprinkler kept an oven fire under control at the UW-Madison Wisconsin Energy Institute on Tuesday until fire crews arrived to put it out, authorities reported.

Fire crews were sent to the building in the 1500 block of University Avenue at 1:20 p.m. and arrived at 1:24 p.m. to find no smoke or fire showing from the five-story building, though people were evacuating, Madison Fire Department spokesperson Cynthia Schuster said in a statement.

A building manager told firefighters there was a fire in an autoclave room, prompting Engine 4 to upgrade the call to a full structure fire response, and additional fire crews were sent, Schuster said.

As the other crews responded, Engine 4 went to the autoclave room, where a fire sprinkler above an oven kept a fire at bay. The oven was open, and a small amount of fire continued to burn inside. Firefighters extinguished that fire with a water can extinguisher, Medic 4 shut down the sprinkler system, and Ladder 1 assisted with ventilation, Schuster said.

The investigation determined that some plastic items that normally are cleaned in an autoclave, but are not oven-safe, were accidentally placed in the oven. Someone working in the lab noticed smoke coming from the room, and when they opened the oven to check on it, incoming oxygen fueled the fire and caused it to flare up. Attempts to put out the fire with a dry chemical extinguisher failed, and the fire alarm was pulled to prompt a building evacuation, Schuster said.

Thursday, firefighters showed off what they now use instead to put out certain types of fires.

They demonstrated an extinguishing agent known as F-500 at Chippewa Valley Technical College.

Though the material puts out many types of fires effectively, unlike traditional foam, it doesn’t have harmful man-made chemicals known as PFAS.

Eau Claire Fire and Rescue Deputy Chief Allyn Bertrang said all of his department’s in-service trucks carry F-500.

He also said firefighters don’t use it very often. Most fires only require water to be extinguished. Some blazes, however, including ones with flammables like gasoline and diesel fuel, as well as those with metals, need a more powerful compound.

Environmental authorities in Sri Lanka have ordered the still-burning container ship X-Press Pearl to leave the nation's waters as a precautionary measure.

The fire aboard the X-Press Pearl broke out on Thursday as she waited at anchor about 10 nm off the coast of the port of Colombo. The Sri Lankan Navy responded, sending two patrol vessels and one fast attack craft. Two tugs from the Sri Lanka Ports Authority (SLPA) joined the efforts. In a novel addition, the crew of a Sri Lankan Air Force helicopter bombarded the fire with sacks of dry chemical powder extinguishing agent.

As of Saturday, the fire aboard the X-Press Pearl was not yet entirely contained, Sri Lanka's Marine Environment Protection Authority (MEPA) confirmed. The two tugboats from the Sri Lanka Ports Authority are still engaged in fire suppression and cooling efforts, but - as is often the case with container fires - fully extinguishing the seat of the blaze is proving challenging.

As it continues to smolder, MEPA has expressed concern about the potential risk to the marine environment from the vessel's fuel and from some of the cargo, including 25 tonnes of nitric acid. The agency sent aboard a team to investigate the potential cause of the fire last week, and it said in a statement Friday that it believes a chemical reaction was the root cause of the blaze.

At 6:30 a.m. Saturday, Dec. 17, 1921, while fighting a small blaze at a home on Greenwood Avenue, the Kankakee Fire Department lost one-twelfth of its manpower. Veteran firefighter Arthur A. Stiers, one of the dozen members of the Kankakee Department, was killed instantly when a chemical fire extinguisher tank exploded.

The fire, at the home of Walter Fisher, 128 N. Greenwood Ave., “was not one of great consequence,” reported the Kankakee Daily Republican. “The firemen found a barrel of paper and kindling wood on fire near the furnace in the basement. It was evident that the chemical tanks would be sufficient to extinguish it.”

Stiers’ death occurred only moments after the department’s two fire trucks arrived on the scene. Details of what happened were revealed at a coroner’s inquest, reported in the Republican on December 19: “Stiers, Assistant Fire Chief Noah Betourne, and Fireman Pat Bochant were about to operate a hand chemical [tank] which had been secured from Fire Truck No. 2….Betourne was in the basement…Stiers and Bochant were on the outside of the house, ready to turn on the valve of the hand chemical [tank] when Betourne gave the word. When Betourne shouted ‘All right,’ Stiers leaned over the chemical tank to turn it on. Immediately, there was a terrific explosion, the tank shooting skyward” and hit Stiers.

The tank the firefighters were using, which contained six gallons of liquid under pressure, was made of thick copper. In service for about 20 years, it had last been used at a fire on Nov. 5, then recharged.

NIST Tool Will Help Military Vehicles Fight Fires Using Less Harmful Chemicals

Researchers at the National Institute of Standards and Technology (NIST) have developed a new, interactive spreadsheet that will help the U.S. military extinguish aircraft and vehicle fires using the latest environmentally friendly chemicals.

While it may sound old-school, the spreadsheet is actually an upgrade of a NIST computer program that has helped protect valuable military assets and lives for several decades, despite being based on the vintage operating system MS-DOS.

Fire protection is a major issue for military vehicles, which carry flammable fluids and can incur significant damage and casualties in accidents or combat. These vehicles carry hand-held fire extinguishers among other fire-suppression systems. A challenge is filling the bottles in such a way that the heat of a fire does not boost the pressure to the point of explosion. Calculations are needed to safely combine a fire suppressant agent with the nitrogen that expels and disperses the chemical. Another challenge is what agent to use. Efforts are underway to identify fire suppression agents with low or no global warming potential (GWP).

In the 1990s, NIST researchers developed a computer program for bottle-filling calculations called PROFISSY (PROperties of FIre Suppressant SYstems), which was distributed informally and has been widely used. But the old source code made it difficult to add new fluids. NIST has now replaced PROFISSY with a spreadsheet that has additional functionality and fluids and also links to NIST’s REFPROP database, which contains properties of many additional chemicals and mixtures. REFPROP has been widely adopted as a standard in the refrigerants community and also contains fire suppressing agents, including environmentally preferable alternatives to halons that deplete Earth’s protective ozone layer and hydrofluorocarbons that contribute to global warming.

According to a city press release, more than 20 firefighters from all five Farmington Hills fire stations responded. Because it’s dangerous to use water on an oil fire, they turned to dry chemical fire extinguishers, which worked well but quickly ran out.

Command staff contacted the Detroit Metro Airport Fire Department for an Aircraft Rescue Fire Truck, which carries several thousand gallons of water and firefighting foam. Most importantly, it also carries 500 pounds of Purple-K, a dry chemical that is highly effective in fighting oil fires.

Shortly after Truck #7 of the Metro Airport firefighting fleet arrived, a team of firefighters from both departments applied Purple-K into the oil vat and extinguished the fire by 6 p.m., with no injuries to employees or firefighters.

A foaming incident occurred after a fire extinguishing system was activated in American Airlines hangar, at Chicago O’Hare International Airport (ORD) yesterday.

At the time of the incident, two American Airlines Boeing 787s, registration unknown, were parked at the hangar. It is unclear if any additional aircraft were in the hangar at the same time.

Source says that the automated fire suppression system was activated by accident. In the video below, it is possible to see the huge quantity of fire retardant foam falling on the aircraft. There is no report of any injuries.

A fire suppression system is mandatory in maintenance hangars; it can fill the hanger in minutes and prevents a fire from spreading to any aircraft, also minimising any damage to the building itself.

This incident could be very expensive for American Airlines. Fire retardant foam, like the one in the images, is highly corrosive to an aircraft; this is due to the chemical components that help extinguish the flames.

This relatively new generation of batteries are used in so many devices today that it is hard to imagine not having their convenience.

From portable power tools, to flashlights, mobile phones, computers, toys, and so much more.

In December last year, a fire in a tool storage shed deep in an Ontario mine was contained after some difficulty.

Findings this week determined the fire occurred while charging a number of lithium-ion batteries used by the miners portable tools. While it couldn’t be accurately determined if the fire began in the battery, the charger, the extension cord, the presence of several of the batteries together made the fire much harder to contain once it started.

When conventional dry chemical (ABS) fire extinguisers were tried by two miners who discovered the fire, it only made the fire worse.

The investigation recommended Class D fire extinguishers be used in such cases.

In June last year, a Vancouver-bound flight was forced to return to Calgary shortly after takeoff when a warning light indicated a fire in the cargo hold.

Two firemen suffered injuries as a cemented shed on which they were standing collapsed.
According to sources, the fire broke out at Chopra Industries Private Limited around 10.30 am. A call was received at the fire control room at 10.53 am.

The fire officials said the fire broke out on the first floor where a huge quantity of leather goods, mainly seat covers, and machinery were lying. They suspect there was chemical stocked on the first floor which caused the fire to become intense.
The officials said there were no fire extinguishers at the factory. Nor had the factory owner obtained the NOC from the Fire Department.

“It took over six hours to douse the blaze. About 40 firemen from the Pune fire brigade, seven fire tenders and water tankers were pressed into service for fire-fighting. No one sustained injuries in the fire,” Prashant Ranpise, chief fire officer of the Pune fire brigade, told TOI.
Ranpise said prima facie, short-circuit could have led to the fire. “We had deployed two fire tenders at the spot,” he added.
Ganesh Pinguwale, sub-inspector of the Lonikand police, said the company, Rise n’ Shine Biotech Private Ltd, deals in biotechnology, floriculture and horticulture. “The company has three laboratories where technicians work in three shifts. Around 8.30am on Friday, the technicians in the laboratory number 2 alerted the supervisor about a fire in one of the units,” the officer said.
He said before the supervisor could reach the laboratory number 2, all the 50 persons working there had come out. “Within seconds, fire broke out in the laboratory,” Pinguwale said, adding, “The supervisor and others tried to douse the fire by using fire extinguishers, but it had spread in the entire laboratory by then.”

A fire-suppression system for gasoline pumps was set off accidentally, covering vehicles and the ground with white powder that made it look like it had snowed in June.

The snow-like substance is a white dry chemical agent used in the fire suppression system for the gasoline pumps.

The white stuff covered the grounds and cars at Stewart's Shop at 120 Oswego St. in Baldwinsville earlier today. Customers snapped photos and shared them on social media.

Stewart's Shop employees said the power went out on the gas pumps around 8 a.m., and when a worker went flip the switch to try to turn them back on the worker pushed the wrong lever.

According to the CP Adeoye “what happened was not an explosion, one of the students carelessly released the safety pin of the fire extinguisher in one of the classes thereby releasing the carbon dioxide causing panic and some students who inhaled the substance got unconscious and were attended to.

“We deployed our anti-bomb squad to the scene after my personal visit and their findings corroborated what I just said.

The school management appealed to students, parents and all relevant stakeholders not to exercise any panic saying all the 27 students who sustained several degrees of injury have been treated and some of them with lesser injury discharged.

Fire officials say they responded to a fire alarm Monday afternoon at a Cumberland Farms gas station in Rehoboth. Responding crews determined there was no fire, and the system had instead malfunctioned and sprayed the area with a chemical powder used in fire extinguishers.

The Rehoboth Fire Chief says four people were taken to local hospitals for breathing problems and skin irritation. The injuries are being reported as non-life-threatening.

A lithium battery caught fire at Southwest Research Institute in downtown Boulder on Thursday afternoon prompting the closure of roads around the building as fire crews responded.

Boulder Fire Rescue Battalion Chief John Nuñez said staff on the fifth floor of the building, located at 1050 Walnut Ave., called 911 shortly before 3 p.m. to report the fire, which staff were trying to put out with a fire extinguisher.

Nuñez said that firefighters responded and extinguished the battery, but it took quite a while to ventilate the building. However, there doesn't appear to be any smoke damage.

No injuries were reported. Boulder police temporarily shut down portions of 11th Street and Walnut Avenue while fire crews were on scene.

Nuñez said it is unknown why the battery, which was being charged, caught fire.

The Southwest Research Institute employs about 80 people and is a nonprofit applied engineering and physical science research and development organization that is headquartered out of San Antonio, according to its website.

The incident was reported around 4:30 p.m. at the end of an access ramp on 18th Street.

A man and woman had parked their newly purchased electric bikes on the beach and were sitting next to them when the battery on one of the bikes malfunctioned, said Mike Halphide, lifeguard battalion chief for the Newport Beach Fire Department.

“Lifeguards on scene reported that the battery was popping and sending projectiles dozens of yards from the fire,” Halphide said. “It’s something I’ve never seen before.”

The couple were not injured and firefighters used a dry chemical to extinguish the blaze. One bike was destroyed and the other was damaged, he said.

The school is located at 24801 Monroe Avenue in Murrieta.

The fire, that was reported around 11 a.m., prompted four engine companies, one truck company, a battalion chief, and a fire marshal to respond to the high school.

Murrieta police officials and school resource officers also responded to the reported emergency to assist with student evacuations as well as traffic and crowd control.

While firefighters and other emergency first responders were rushing to the school, a teacher – who has not yet been identified – managed to extinguish the fire before it could grow out of control.

“A quick-acting school teacher used a 10 pound dry-chemical fire extinguisher to extinguish a burning plastic water bottle,” a fire official explained in a social media press release after the fire.

“Crews stated they believed a hydraulic hose broke,” wrote firefighter Brian Ackerman in a report. Employees said sparks from the factory likely ignited oil on the forklift.

When Bellevue firefighters arrived on the scene, the fire was already out, having been handled by Magretech employees with three chemical fire extinguishers.

The ground around the vehicle was covered in oil, which was cleaned up using Oil-Dri.

The Magretech crew seemed to handle the fire quite capably themselves, as Ackerman notes in his report.

“Employees had removed a propane tank prior to arrival (and) crews were beginning to work on dismantling the crane device,” he wrote.

Since the official recall of the Samsung Galaxy Note 7, phone manufacturers have been assuring customers that their devices are safe to use. Some, like LG, are even testing their batteries in extreme conditions to make sure that they don’t overheat or catch on fire. Thanks to a research team at Stanford University, we may have less to worry about when batteries explode…

The Stanford team has built their own lithium-ion battery which includes a capsule of triphenyl phosphate (TPP) residing within the battery’s electrolyte fluid. If the battery’s internal temperature reaches 150 °C (302 °F), the capsule will melt and release the TPP chemical.

TPP is a type of chemical fire retardant that — as tested by the team — is capable of extinguishing a battery fire in 0.4 seconds.

This is a much-needed safeguard as lithium-ion batteries tend to be temperamental, with a simple manufacturing or charging defect capable of igniting the battery.

Although Samsung has yet to officially release a statement on their Note 7 explosion findings, it is believed that the battery had a defect that led to the handsets catching on fire.

Hopefully, technology like this will help prevent accidents in the future especially as we become more dependent on lithium-ion batteries to power both our mobile handsets and items like electric vehicles. You can read more about this new “fire extinguisher” battery technology in a peer-review journal published by Science Advances.

(NEWSCHANNEL 3) -- Following the discovery that some galaxy note 7 cell phones explode, several airlines are taking precautions to make sure defects like this won't cause them any problems.
Virgin and Alaskan Airlines now require fire suppression bags on all of their flights.
The bags are made up of several heat resistant materials that can withstand temperatures up to 3,200 degrees.
Alaskan Airlines first started requiring the bags on their flights last year after a credit card reader overheated and started a fire.
But after the cell phone explosions, airlines as large as Delta are even considering having the fire suppression bags on all of their flights.
Flight attendants are trained to first douse the flaming device with a fire extinguisher, then put it in to the containment bag and get it far away from passengers so the pilot has time to safely land the flight.
"Once a battery starts to implode on itself, it continues until all of the chemical has run out, and so it does take a lot of liquid in order to make sure that we try to keep the heat, reduce the heat, they burn very hot, they burn very fast, and it can be for quite a long period of time,” said VP of in-flight services for Alaskan Airlines, Andy Schneider.
The Federal Aviation Administration does have concerns about the fire suppression bags, warning in a 2009 safety alert that transferring a burning appliance into a burn bag may be extremely hazardous.

Springfield firefighters responded to a 67 Market St. following a call from a building worker at 12:38 p.m. and encountered fumes from the spill. A regional hazmat team is en route to the scene and the building remains evacuated as of this afternoon.

Deputy Chief Glenn Guyer said the fire suppressions system used carbon tetrafluoride – an extinguishing agent commonly used upwards of sixty years ago, but which fell out of favor due to its carcinogenic and corrosive properties.

"It's a type of extinguishing agent which is set up on a fusable link, so when the room gets to a certain temperature or a fire gets to the fusable link, it activates. The system is under pressure and it sprays it all out there.," Guyer said. "It's very good at putting out fires but it's very corrosive – it's not good for people."

Thousands of butane cannisters were the first clues to what sparked a fire that damaged a west Santa Rosa warehouse, fire officials said Wednesday.

Police and fire investigators suspect people were extracting oil from marijuana in a laboratory behind false walls built in the Condo Court warehouse off Coffey Lane, officials said.

Assistant Fire Marshal Paul Lowenthal said he suspects the Tuesday afternoon fire started with a hash oil setup involving butane gas and electric equipment like bucket heaters and hot plates.

Firefighters had to break through walls built behind doors to get inside the warehouse after the 911 call reported smoke coming from the roll-up garage door at 3:15 p.m. Tuesday, Lowenthal said. They forced their way in past heavy smoke and carved a hole in the roof to relieve the heat, Battalion Chief Matt Dahl said.

No one was inside, despite spent fire extinguishers indicating people had tried to put out the flames, Lowenthal said. It wasn’t clear whether they had abandoned hope of extinguishing the fire or if it appeared to be out but then continued smoldering until it reignited. The doors were locked.

“We’re not quite sure of the reason for the discharge,” Ames said. The dry chemical system is designed to discharge a yellow fire-suppressant powder from nozzles around all the gas pumps. Anyone who happens to be pumping gas or who is in the vicinity can either be covered with the powder or experience eye irritation, Ames said.
The gas station, the convenience store and the coffee shop inside were closed for the day, Ames said. The town health department was called due to concerns about food service at the coffee shop, he said.

BANGKOK (AP) — Eight people died at the headquarters of one of Thailand's largest banks when workers accidentally triggered the release of fire extinguishing chemicals during an upgrade of the building's safety system, officials said Monday.
Seven other people were injured in the accident, which occurred Sunday night in the basement of the Siam Commercial Bank's headquarters in Bangkok.
The bank said in a statement that an initial investigation found that the accident was caused by "negligence on the part of our contractors hired to improve fire protection in the building."
Workers were upgrading a fire extinguishing system in a document archive room in the bank's basement when the accident occurred, causing the release of Pyrogen aerosol, officials said. They did not elaborate.
Police were investigating the accident and the bank said it would release more information after a meeting of its board members Monday.
Pyrogen is a chemical fire retardant normally intended to deprive fire of oxygen and is typically used in places where extinguishing a fire with water would damage documents or equipment.
Pyrogen's website says the dense aerosol is "most effective in unoccupied areas such as data rooms, machinery and engine spaces" and other storage areas. It says people should avoid exposure to Pyrogen and any "accidental exposure to the aerosol should be limited to five minutes."
The bank identified those who died as a security guard and seven contractors.
Bangkok's Erawan medical emergency center said five people died at the site of the accident and three died later at a hospital. It said the apparent cause of death was suffocation from insufficient oxygen, but that the official cause would not be known until autopsies are performed.

It appears a fiery mop is to blame for a fire that broke out at a Detroit Lakes house around 8 a.m. Wednesday morning, according to Detroit Lakes Fire Chief Scott Flynn.

Detroit Lakes woman Rachel Gray and two of her children, including a 2- month-old baby, were home at the time but escaped without injury to a neighbor’s house.

“She smelled smoke, went downstairs, and there were flames taller than her,” said neighbor Rebecca Talbert, whose husband is also a firefighter. “So she grabbed the baby and her 3-year-old and rushed over here to let me know.”

Talbert says once they had the children and dogs safely in her house, they grabbed a fire extinguisher and went back over to the house, but smoke was thick, she said, so Talbert just grabbed the kids’ coats by the door and left.

Emergency and fire crews were quickly on the scene, and according to Flynn, the culprit appeared to be a mop in the basement.

Mystery fire has razed down 3 Science laboratories belonging to the Faculty of Basic Medical Science in Okuku, Ogoja campus of the Cross River University of Science Technology CRUTECH.

National Mirror learnt that the incident occurred at the early hours of last Tuesday. The fire also affected Histology, Biochemistry and Physiology Departments of the school. The inferno is said to have disrupted academic activities and at the moment, carcases of the burnt equipment were seen littered all over the place of the once adoring laboratory. However, about 20 expended cylinders of fire extinguishers used by the university community to tackle the inferno were seen around littering around the building. Students of the institution were also seen milling around the area counting the huge loss their school had suffered due to the fire incidence while lamenting how the incident is going to hamper their academic pursuit.

According a fourth year student, Christopher Adie, the pathetic situation was capable of retarding the progress of the academic calendar adding that the incident was a bad omen for them. However, the Dean of the school, Dr Esebonu Ugochukwu put the value of the loss at several millions of Naira stressing that the labs gutted by fire were demonstration facilities for 1st year students describing the inferno as the worst in the history of the institution. As at the time of filing this report, the cause of the fire is yet to be ascertained but there were speculations that the fire may have broken out due to electricity fault from public power supply.

Dr Ugochukwu averred it was unfortunate that all efforts made by the university community to quench the fire was to no avail as the fire service station complained of lack of equipment. Assessing the disaster, the DG SEMA Mr John Inaku described the disaster as very unfortunate and assured of govt intervention. However, the Vice Chancellor of the institution Prof. Anthony Owan who was contacted on phone via a text message to react on the issue, said that he is yet to lay fingers on the necessary details and as such he cannot comment on anything for now. His words; “I will be in Okuku on Monday to find out the true situation. I cannot place my fingers on anything at the moment. You will be contacted as soon as the facts becomes clear” The institution Vice Chancellor maintained.

There were no injuries as a result of the incident.

University of Massachusetts spokesman Larry Rivais said the fire began in an oven in Room 114, a laboratory on the first floor, at 3:15 p.m. and was put out by firefighters about 15 minutes later.

“The fire was contained to the oven, but there was a lot of smoke,” Rivais said.

Assistant Fire Chief Lindsay Stromgren said an experiment caught on fire inside the laboratory-grade oven. After the people conducting the experiment were unsuccessful in using a fire extinguisher to put out the blaze, they pulled the alarm, prompting the response in which firefighters were assisted by UMass Police and UMass Environmental Health and Safety officials.

Firefighters were able to get the fire under control, after it briefly burned outside the oven, by using a fire extinguisher.

“The oven itself is destroyed, and there was pretty significant smoke and soot damage,” Stromgren said.

Stromgren said soil samples were being cooked in the oven, but it was uncertain why the fire occurred because soil does not burn.

Smoking materials cause of fire at St. Charles complex, officials say
The extinguisher, believed to be from the 1950s, was dropped off at the station at an unknown date and time and then was placed in the garbage, according to Battalion Chief Bryan McMahan. A firefighter decided to release pressure from the canister and did so in the station. The extinguisher, however, held carbon tetrachloride — a known carcinogen, McMahan said.

Exposure to the material poses health risks to humans. According to the Environmental Protection Agency website, the human symptoms of "acute (short-term) inhalation and oral exposures to carbon tetrachloride include headache, weakness, lethargy, nausea and vomiting. Acute exposures to higher levels and chronic (long-term) inhalation or oral exposure to carbon tetrachloride produces liver and kidney damage in humans.

"Studies in animals have shown that ingestion of carbon tetrachloride increases the risk of liver cancer," the EPA website states.

"The station was evacuated, and a hazardous materials crew has been in there for the last couple of days. We are hoping to get back in there tomorrow," McMahan said Sunday.

The firefighter who released the chemical and two other individuals were taken to Advocate Sherman Hospital for treatment, he said. All were released and returned to full duty, he said.

A Chicago factory that makes chemicals for use in fire extinguishers was destroyed in a massive blaze Thursday night.

The extra-alarm fire broke out in the Archer Heights section of the city just after 9 p.m. at the industrial building on W. 38th Street, according to MyFoxChicago.com.

Within 30 minutes, the building, which housed several businesses in addition to the chemical factory, was completely engulfed in flames.

More than 150 firefighters responded to the scene and it took them nearly three hours to extinguish the fire. The crews had difficulty getting enough water to the building because of a lack of hydrants and had to perform an "inline operation," in which six trucks were spaced out over a mile and connected by hoses to pipe water, the Chicago Tribune reported.

High flames from the blaze could be spotted from miles away and thick black smoke choked a nearby highway.

The fire caused the roof to collapse and the building was damaged beyond repair. No one was injured and authorities are investigating the cause of the blaze.

“We got a call that somebody reported smoke and flames coming from a closet in one of the rooms,” Edwards said. “The person who saw it first grabbed a fire extinguisher, but he decided to pull the fire alarm and call 911.”

By the time firefighters arrived, the building’s sprinkler system had already put out the fire, he said.

The Green Bay Police Department says the officers responded to a possible hostage situation involving a knife Monday. A person involved sprayed a fire extinguisher inside a hallway in the apartment building before police arrived.

The four officers exposed to the chemicals were released to return to work.

Police say four people were arrested and two were referred to the Brown County district attorney for charges.

They say the fire department asked tenants to leave the building because of the chemicals. The American Red Cross is assisting them until they can return.

But instead of a fire, crews arriving in the 2900 block of Walnut Street discovered copious amounts of chemical fire extinguisher dust billowing out of the school, said Camp Hill Fire Company Chief Mark Simpson, who chalked the incident up to vandalism. Nonetheless, the dust, which coated the school hallways and spilled out into the parking lot, certainly resembled smoke from an actual fire, Simpson said.

"It was a large number of dry chemical fire extinguishers that were discharged inside. I definitely don't think it was accidental, it was vandalism, but it was more of a nuisance," the chief said.

Students in a Flagstaff High School chemistry class are OK after an experiment produced an unexpected powder that forced a classroom evacuation, fire officials said.

Firefighters said none of the students or faculty showed any adverse symptoms.

Team members mounted a hazardous material response and wore special protective gear. They entered the classroom, monitored the surroundings and obtained samples of the unknown product.

After analyzing it, team members determined the product was an inert extinguishing agent.

Investigators said that the chemical hood system functioned properly, delivering an extinguishing agent.

Firefighters concluded that there were no hazardous materials released from the incident.

Five fire engines and a ladder truck were sent to the scene about 2:24 p.m.

Incident commander Dan Mathes and Investigator Chuck Berlinger, who led firefighters into the building, said the fire apparently was caused by the accidental ignition of pyrophoric metallic compound left under an exhaust hood. No one was injured.

Kathy Kosco, spokesman for the state DEP, said it appears, upon initial review, that a break in the hose caused a spark that resulted in a fire destroying multiple pumper trucks at the site.

Twelve Brooke County and Western Pennsylvania fire departments were called at about 5 p.m. to extinguish the blaze, which occurred on property leased by Chesapeake Energy from Russell Hervey.

Franklin Community Fire Chief Larry Palmer said staff with FTS International, a subcontractor for Chesapeake, attempted to put out the fire with a fire extinguisher. When that failed, they quickly evacuated the area and called for help and no one was hurt, he said.

All four labs on the south end of the 922 Grant St. building will remain closed off until further notice, said Steve Hannan, director of public relations for the university. There were no reported injuries.

A shelving unit in a fourth-floor laboratory came off of the wall around 5:33 p.m., spilling several containers filled with chemicals that burst into flames when mixed, said Ashland Fire Department Chief Ron Workman.

A staff member was able to douse the flames with a fire extinguisher, but not before the smoke set off one sprinkler, Hannan said. Potentially chemical-laced water spilled over into all four labs on the south end, he said.

Faculty and staff are still assessing the damage — the shelving unit and cabinet below were burned and there may be some water damage — and will clear out the chemicals before reopening the labs to students, Hannan said.

The state Hazardous Materials Team responded to Sohier Road on Wednesday morning for a fire at a chemical storage unit. No chemicals caught fire as a result of the incident, which was caused by improper storage of rags used to clean up solution of potassium permanganate, according to Beverly Fire investigators.
...
Crews working in close coordination with CPI employees were able to gain access to the storage unit and use a hose line to extinguish burning materials that were stored inside the container. In addition to a solution of potassium permanganate, rags and buckets used for clean up were stored in the container.

“The fire involved the rags and a bucket. At no time were the containers of the stored chemical involved in the fire,” said Peter O’Connor, Beverly Fire Department deputy chief.
...
The container is owned by Shaw Environmental but kept on the property for ongoing operations at that site. It had two fire suppression systems installed including a dry chemical system that activated keeping the fire from intensifying before firefighters arrived and a sprinkler system that did not activate because the fire did not get hot enough to activate a sprinkler head.

Before any students in the Lower Dauphin School District do any science experiments, they are required to take a classroom safety test.

In some cases, they sign a written contract outlining safety procedures.

“In all of our classes we start with science safety instruction. It’s a requirement,” district spokesman Jim Hazen said. “Despite all of that, accidents happen. Things happen.”

An accident in the Carlisle Area School District this week underscored that fact.
On Wednesday, seven students in the Wilson Middle School were hospitalized after a fireball erupted during a chemistry experiment in an eighth-grade classroom. All seven students have been released from the hospital.

The science teacher, James Redington, poured rubbing alcohol into a beaker, then lit it with a flame, Carlisle Fire & Rescue Services Chief John Heberlig said. The teacher grabbed a fire extinguisher and doused the flames, which were contained to a lab table at the back of the classroom.

An investigation continues. Yet the incident brings up questions about what regulations are in place for school districts to follow to ensure students and teachers are safe.

A malfunctioning piece of equipment in a laboratory on the UW-Madison campus caused a fire Monday that forced the evacuation of the building but no injuries, authorities reported.

The fire was reported at 10:52 a.m. in the Service Memorial Institute, 470 N. Charter St., according to UW-Madison Police Sgt. Aaron Chapin.

The fire caused about $1,500 in damage to the equipment, and was confined to the one room in the building.

The building's alarms went off and occupants evacuated safely.

"A researcher was heating up some oil when the equipment malfunctioned and the oil ignited," Chapin told Madison.com.

Madison Fire Department spokeswoman Lori Wirth said the researcher was performing an evaporation experiment when the temperature control on the equipment failed, bringing the liquid to flash point and igniting.

"The researcher first used a CO2 extinguisher and dry chemical extinguisher to put out the fire but neither worked, so he put water on the liquid, creating a minor flash that dispersed the liquid so the fire went out," Wirth said.

A Deltona resident was transported to an area hospital after officials said the person dropped a glass fire extinguisher, causing its chemical contents to spill.

The resident of the home on the 1200 block of West Wellington Drive dropped the old fashioned fire extinguisher and a hazardous chemical spilled, requiring a Volusia County hazmat team to respond, Volusia County sheriff's spokesman Brandon Haught said.

Deltona Deputy Fire Chief Robert Rogers said the resident is in stable condition.

The chemical, carbon tetrachloride, is manufactured and can cause harmful effects if exposure occurs in high amounts, according to the Agency for Toxic Substances and Disease Registry. It used to be found in refrigeration fluids and propellants for aerosol cans, in cleaning and degreasing products, in fire extinguishers and in spot removers.

The chemical is only used in some industrial applications now because of its harmful effects.
