    <dc:creator>dchas</dc:creator><description><![CDATA[WRENTHAM — The state Hazardous Materials Response Team was called to assist local firefighters after a chemical reaction filled a downtown art studio with carbon monoxide Wednesday morning.

Firefighters responded to the third-floor studio at 54 South St. just before 10 a.m. after the the studio tenant went across the street to the fire station and complained about not feeling well, Fire Chief Antonio Marino said.

The woman was checked out by EMTs but declined medical treatment, according to the chief.

Carbon monoxide is a colorless, odorless gas that can be fatal.

When firefighters went to the studio, they detected high meter readings for carbon monoxide inside and called for the hazmat team because of the presence of chemicals, Marino said.

It was designated a Tier 1 hazmat response, the lowest state classification, he said.

The high carbon monoxide readings were confined to the studio. The second floor of the building is vacant and a restaurant on the first floor had no readings and was able to stay open.

About five or six hazmat team members dressed in protective suits went inside the studio to investigate.

The chemicals were an epoxy and an acid that the woman regularly used previously without incident to make etchings and other art works, Marino said.

“For some reason the amount of gas that was emitted was other than normal,” he said.

The chief said the woman used a different epoxy and heat to help the curing process.

State and local fire officials and local health and building officials are continuing to investigate the storage of chemicals in the studio, according to Marino.

    <dc:creator>dchas</dc:creator><description><![CDATA[THE US Chemical Safety Board (CSB) has released a new safety video about the 2020 fatal incident at the Evergreen Packaging paper mill in Canton, North Carolina, US.

On 21 September 2020, a fire erupted in a confined space when a heat gun fell into a bucket of flammable epoxy vinyl ester resin, killing two contract workers. Evergreen Packaging is a packaging and containers company, making paper and paperboard products. The incident occurred during a planned shutdown of its Canton paper mill, while maintenance and repair activities were ongoing throughout the facility.

CSB’s new safety video includes an animation of the events leading up to the incident. While completing repairs in an upflow tower, Universal Blastco contractors began using a heat gun in the confined space. A fire erupted when the gun fell into a bucket of flammable resin and though the Blastco contractors were able to evacuate, two Rimcor contractors in the connected downflow tower were unable to escape.

The video explains that in its investigation, CSB identified four safety issues that led to the incident, including hot work and confined space safety. CSB Chair Katherine Lemos said that these factors are common safety issues in many major incidents investigated by CSB. The video included commentary from both Lemos and Lead Investigator Drew Sahli.

Two people went to a nearby hospital with respiratory issues that may have been related, though an air quality expert noted that eye, skin and respiratory symptoms could take 24 hours to develop after exposure.

The incident was reported around 8:40 a.m. at Huntsman Chemical at 4541 W. Electronics Place, according to Los Angeles Fire Department spokeswoman Margaret Stewart.

Officials initially said the incident occurred after a small valve broke, but by Thursday evening, the precise cause remained unclear. A spokesperson from the South Coast Air Quality Management District said the agency believed it may have been a chemical reaction rather than a leak.

According to a hazardous material spill report from the California Office of Emergency Services, a rupture disk was released during the packaging of an epoxy product known as Reninfusion 8610, “spilling a small amount of the product onto the concrete floor and allowing smoke to release into the atmosphere.”

The accident occurred at 5:23 in the morning. The driver needed assistance to exit the vehicle. Firefighters freed him with the help of hydraulic tools and handed him over to the Emergency Medical Service (NMPD).

The homeowner had evacuated from the building himself.

The site is cordoned off. As the Salacgrīva administration points out, the substance is not toxic but emits a strong smell, so if the smell bothers residents, they can be evacuated to the Salacgrīva school dorms.

Meanwhile, Latvian State Roads warned drivers that traffic may be disrupted near the scene of the accident. At the site of the event, traffic is regulated by the State Police, while the VUGD is performing removal of the substance.

It led to a "shelter-in-place" order for about three hours in Roberta on Thursday. 
Crawford County Fire Chief Randy Pate says they're continuing to monitor the plant.
A Georgia EPD spokesperson says the chemical reaction is now contained on a concrete slab.

They'll dispose of the chemicals once the reaction actually stops. 
"We're just standing by monitoring. That's all," Pate said.
Pate says they got the call from the Olin Proxy Plant around 9:15 Thursday morning.
People downtown, about a mile from the plant, say they could smell a strong odor, like something electrical burning. 
Right before noon Thursday, nearly three hours after the call was originally made, the county issued a "shelter-in-place " order out of precaution. Pate says it took them that amount of time after first responders were dispatched to make a plan.

 Around 7:33 a.m., state troopers were requested to investigate a crash in the area on I-35. Preliminary investigation revealed a 2015 Peterbilt truck was traveling north on the interstate when for unknown reasons the trailer caught fire, according to a press release from Tackett, a Texas Department of Public Safety spokesman.

Tackett said the driver, 61-year-old Guy Mulcahey of Springtown, disconnected the truck from the trailer and moved forward. The trailer was loaded with an epoxy resin which is considered a hazardous material.]]></description>

Peterborough firefighters were called to the plant before 9 a.m. Thursday after it was reported that some employees were exposed to an epoxy.

Siemens communications spokesperson Laura Heidbuechel said the incident was minor.

"This morning the Siemens Peterborough facility experienced a minor incident in its factory in which a pipe burst and a small amount of chemical compound leaked," Heidbuechel stated.

"Five employees received medical attention. The Peterborough Fire Department has been through the entire building and has determined that there is no further risk to employees."

The Peterborough Siemens plant designs and makes measurement instruments for process industries in the oil and gas, environmental, chemical, food and beverage, pharmaceutical and mining sectors.

A growing body of research is raising questions about the safety of alternatives to bisphenol A (BPA) – an endocrine-disrupting chemical that regulators are cracking down on – that are being used to make plastics and epoxy resins.

There is a real danger of what researchers are calling ‘regrettable substitutions’, where BPA is replaced by a structurally similar chemicals that elicit comparable health concerns.

‘When BPA-free products started to be advertised we knew enough about BPA to be concerned about regularly exposing people to this chemical, so that seemed like a victory until we started looking at the replacements,’ says Laura Vandenberg, an environmental health researcher at the University of Massachusetts-Amherst in the US. ‘We have seen enough scientific evidence that we are starting to say “Ok, these are regrettable”, and if we need the amount of evidence of harm that we have for BPA, then we are going to have to wait another two decades or more, and that seems unreasonable.’]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[LYNN — Firefighters and a state hazardous materials team cleaned up a potential chemical spill inside an 18-wheeler parked off of Boston Street Monday afternoon.

A UPS driver was making a delivery just after noon when he noticed a chemical smell inside his truck’s trailer, according to Lynn Fire Chief Stephen Archer. The driver immediately closed the truck, parked it deep in a lot, away from the street and businesses, and called the fire department.

“He did everything right,” Archer said. “He did what he was supposed to do.”

At no time was the public in danger from the chemical, Archer said.

There were eight 55-gallon drums of chemical inside the truck’s trailer, according to Archer. While the driver and firefighters on scene didn’t see any leaked chemicals, the state’s Department of Fire Services Hazardous Material Response team would determine what, if any, cleanup was required.

State investigators believe the chemical involved is triethylamine, a chemical compound used in creating resins, epoxies and other materials. Exposure to the chemical’s vapor can lead to “eye irritation, corneal swelling, and halo vision,” according to guidance from the Environmental Protection Agency.

It happened at Royal Adhesives and Sealants on Washington Street Wednesday night. According to a release issued by the president of the company, the problem was caused when a mixer producing epoxy adhesive overheated. The smoke filled most of the plant.
Initially first responders were concerned about chemicals leaking into the air, and people from nearby homes were evacuated and roads were closed. However, a representative from the emergency response section of IDEM verified there was no chemical issue and the smoke was not a threat to the public.
Two employees were taken to the hospital to be evaluated and were released. Operations within the production room have been discontinued until the investigation is finished, according to Ted Clark, Royal Adhesives president and CEO.

    <dc:creator>dchas</dc:creator><description><![CDATA[NORTH ANDOVER – A chemical reaction Wednesday afternoon at Atlee electronics hardware manufacturing company in North Andover caused four employees to evacuate and prompted a response from a regional hazmat team.

At 12:39 p.m., the North Andover fire received a call for a chemical reaction causing smoke inside 10 Bayfield Drive, the building that houses Atlee, part of the business park off of Route 114.

Fire chief William McCarthy said the reaction was caused by a common epoxy resin hardening process gone wrong.

"They mixed larger quantities than normal and it started to overheat and react, and they developed a smoke condition in the building," he said.

The epoxy resin is normally used as a liquid that is poured over a circuit board or other similar electronic hardware and then hardens.

When the fire department arrived, McCarthy said the four people who were inside the building at the time of the incident had already exited, with no injuries.

Discussion: On Friday, April 8, 2016, LINAC personnel were in the process of disposing of legacy chemicals that were owned by an employee who retired late last year. During the disposal process, a Collider-Accelerator Department (C-AD) technician accidentally dropped a 1 quart glass bottle of Epi-Cure 3223 Epoxy Curing Agent (CMS 56250). Epi-Cure 3223 Epoxy Curing Agent is 70% to 100% Diethylenetriamine. The technician immediately notified their supervisor and cleaned up the <<spill>> using GREEN STUFF Absorbent, which was available in a nearby <<spill>> kit. The technician placed the waste into a metal garbage can that was lined with a plastic bag. The absorbed <<spill>> material was considered to be benign and was not taken out of the area as waste immediately.

On Wednesday, April 13, 2016 at 03:45 am, BNL Fire-Rescue responded to an activation of the Highly Sensitive Smoke Detection alarm system for the area where the garbage can was stored. The garbage can was found smoldering with no signs of flames. The can was moved outside and water was applied to extinguish the contents.

The BNL Fire Marshal analyzed the material in the can and found that the GREEN STUFF and the <<spill>>ed chemicals had formed a semi-rigid mass that was charred in the middle, and there were small charred scraps of paper and wood. Additionally, the plastic bag used to collect the cleanup debris was partially melted.

Two schools and a busy state route were closed after workers reported potentially noxious gases seeping into the air, authorities said.

The incident was reported at 1:30 p.m. Tuesday at Contractors Chemical, located at 3130 Depot Road. The company is out of business but still stores chemicals at the site, including epoxies and catalysts that solidify concrete, said Don Nicholson, a spokesman with the Hayward Fire Department. The all-clear was given about 4:20 p.m., police said.

On Tuesday, workers began mixing large amounts of chemicals in an attempt to turn them into solids, then dispose of them, according to Nicholson. But a reaction spread the vapors through the neighborhood and prompted a shelter-in-place alert.

Students and staff at Ochoa Middle School and Mount Eden High School as well as residents living within a 1-mile radius of the business were asked to stay indoors and close all doors and windows. Nearby residents and businesses were also asked to turn off heating, ventilation and air conditioning units.

The Frederick News-Post (http://bit.ly/1gyur9e ) reported Sunday on a report it obtained through the Freedom of Information Act on last year’s fire at the new U.S. Army Medical Research Institute of Infectious Diseases.

Investigators couldn’t determine the cause of the blaze. But they concluded it started in a room where a welder had been working without a required permit or a co-worker watching for fire.

The report recommends a fire watch for all welding operations. It also urges reassessment of an epoxy wall coating cited as the fire’s primary fuel source.

The fire pushed the building’s completion date back from 2014 to May 2015.]]></description>

Green Bay Metro Fire Department officials characterized it as a “hazardous materials-type incident” and said epoxy paint was involved. Firefighters used at least four hoses and an aerial truck to soak an area north of the Carboline Co. building.

Lewis said it was not immediately clear what was causing the problem, but that police had been told that the material could be “highly flammable.”

The non-injury industrial accident in the 5300 block of Custer Street occurred about 1:15 p.m., according to the San Diego Fire-Rescue Department.

Police and firefighters investigated, then called in a hazardous-materials team to determine the source of the caustic vapors permeating the commercial district just north of Friars Road, SDFRD spokesman Maurice Luque told reporters.

Authorities evacuated about 50 people from buildings in the immediate area.

Crews determined that epoxy in a metal barrel had heated up, apparently due to a reaction with some other chemical, to the point where it no longer could be contained, according to Luque. The lid of the drum then blew off and the some of the hot contents spewed out.

"As soon as we stepped out of the building she was trying to fall asleep -- literally fall asleep in the back of the truck -- which is not OK," Edlin Elizondo said.
Her daughter was one of three people treated early Monday morning outside the Richland Apartment Homes in the 7700 block of Woodchase Drive.

San Antonio firefighters evacuated 13 people from eight units, then called hazardous material teams to investigate a downstairs apartment.

An SAFD spokesperson told KENS 5 the source of the dangerous fumes was a can of epoxy resurfacing material left open.

Danville Fire Department’s Hazardous Materials Unit was called in to ChemCast, 1009 Lynch Road, around mid-morning today regarding an epoxy material. Smoke was reported as a result, but no flames, according to radio traffic.

Danville Director of Public Safety Larry Thomason said because of the situation homes to the north of the business were being evacuated as a precaution. The Vermilion County Sheriff’s Department was handling the evacuations.

    <dc:creator>dchas</dc:creator><description><![CDATA[MONTGOMERY TOWNSHIP — A chemical spill Wednesday afternoon that forced employees of ITW Polymers Coatings to evacuate their building at 130 Commerce Drive in Montgomery Township — in an industrial park off Stump Road — was quickly contained by emergency responders, and no injuries were reported.

Shortly before 2 p.m. on Wednesday, a chemical reaction occurred in ITW’s research and development lab that created a potentially dangerous vapor cloud, Fire Department of Montgomery Township Fire Chief Bill Wiegman said at the scene. “There was some off-gassing which we were concerned about,” said Wiegman, who did not name the chemical but stated that it was utilized in the processing of an epoxy made by the company.

The building was evacuated; more than two-dozen employees milled about in a nearby parking lot as members of Montgomery County Hazmat Station 81 went into the building to do air and temperature checks. Ambulances from Volunteer Medical Service Corps stood by; joining FDMT crews was Wissahickon Fire Company with Ladder 7 to handle decontamination “in case anybody gets any product on them,” Wiegman explained.

It happened at ITW Polymers Coatings in Montgomery Township on Wednesday afternoon. Montgomery Township fire Chief Bill Wiegman says the chemical that spilled was used in the processing of epoxy made by the company.

About 30 workers at the plant were evacuated when alarms went off around 1:45 p.m. and fire officials called in a hazardous materials team. A preschool in the area was also ordered to shelter in place while crews investigated.

Authorities eventually determined the gases given off weren't harmful and the spill was contained to the building.]]></description>

Employees were allowed back inside about 3:40 p.m. after the air conditioner was vented and tested twice, said Kent Donohue, special assistant to the city manager.

About 12:30 p.m., employees began noticing a strange smell throughout the building, especially on the second floor, Donohue said. Not long after that, the building was evacuated and Port Orange Fire Rescue, South Daytona Fire Rescue and a Volusia County hazmat team began investigating.

Donohue said roof workers using epoxy happened to be near the air conditioning intake unit and the vapors made their way into the building.

One City Hall employee was treated at the scene and sent home for the day, Donohue said.

He also said the city's public works staff, city manager's office and the construction contractor will be meeting Monday to discuss having workers perform the construction at different times to avoid similar issues.]]></description>

The failure to report the spill could leave the city with fines totaling more than $1 million.
“I would hate to imagine the fines that could result from this mess,” Doug Dowdin, a city storm water enforcement official says in a Feb. 2 email to fellow employees. “I am sure that I don’t have to stress the potential liability.”
In late January 2011, an angry city employee told several subordinates to pour out cans of acetones (solvents), varnish, epoxy, creosote, enamel paint and latex paint. The chemicals were dumped on an asphalt parking lot that abuts up to a grassy area. Paint and chemicals swirled together creating areas thick with paint and a lower section that included open soil coated with acetones, varnishes and creosotes.]]></description>

The Fire Department said it is a "low burning" fire and no one is at risk.
A hazmat crew is using dry ice to extinguish the fire.

Firefighters went up to the roof and came down with a bucket of epoxy that was being used for some sort of roofing work.

Just after 1 p.m., 911 began receiving calls from several people who reported hearing explosions coming from the Polystar Chemical plant at 206 Brookhollow Industrial Blvd., an operator said. No one was in the building when the fire began, fire officials said.
Dalton Fire Chief Bruce Satterfield said around 6 p.m. that officials had not yet begun to investigate the cause of the fire.
“Things are just now cooling down,” he said. “I don’t know how much they’ll even (investigate) today.”

Polystar is a 5-year-old company headquartered in Dalton that produces “a full line of materials for epoxy systems,” according to the company’s website, www.polystarusa.com. “Our core chemistries are amine and epichlorodhydrin based, and so our key products are hardeners, diluents, adducted resins and related surfactants,” the website states.
