—
The early-morning fire at an e-bike store in lower Manhattan that left four dead was caused by lithium-ion batteries, the city’s fire commissioner said Tuesday.

“It is very clear that this was caused by lithium-ion batteries and e-bikes. There is a very large number of both batteries and e-bikes. This location is known to the fire department, we have written violations at this location before, and we have conducted enforcement in this location before,” Commissioner Laura Kavanagh said.

Kavanagh called lithium-ion batteries “incredibly deadly” because so much fire is created when they ignite. “It can often be too late, as soon as the fire has begun,” she said.

The city’s Chief Fire Marshal Dan Flynn said fire officials inspected the store in August and issued summonses for violations related to electrical wiring, charging of batteries, and the number of batteries. The owners of the store were found guilty of the violations and were fined. A reinspection of the property would have been scheduled following the guilty verdict, but the reinspection had not yet occurred, he said.

The same property was also found guilty of violations in 2021, Flynn said.

A group of New York Democrats in April announced support for federal legislation aimed at regulating lithium-ion battery safety standards after a spate of fires caused by the batteries malfunctioning or overheating.

An explosion at a rocket and explosives plant in Turkey caused a building to collapse Saturday, killing all five workers inside, an official said.

The explosion occurred at around 8:45 a.m. at the compound of the state-owned Mechanical and Chemical Industry Corporation, on the outskirts of the capital, Ankara, Gov. Vasip Sahin told reporters.

Sahin said the explosion was likely to have been caused by a chemical reaction during the production of dynamite. Prosecutors have launched a formal investigation, he said.

LA PORTE, Texas – The U.S. Chemical Safety and Hazard Investigation Board (CSB) has released its final report on a deadly 2021 acid leak in La Porte.
The findings in the report show that 164,000 pounds of extremely hot acetic acid were released at the LyondellBasell La Porte complex in July 2021.
According to the report, workers had mistakenly removed a valve cover while trying to repair a pipe leak.
Two workers died and two others were seriously injured in the incident. In addition, the facility received an estimated $40 million in property damages.
“As this tragic incident shows, even a simple task can turn deadly if it is not performed properly. There have been similar incidents around the country involving plug valves being inadvertently taken apart when removing connected equipment. It is time to improve the design of these valves and take other protective actions, such as signage and training, before more workers are killed or injured,” said CSB Chairperson Steve Owens.
CSB investigators said better valve design and more worker training is needed to prevent future accidents.

What was once an emerging threat has become a full-blown crisis in New York City with 220 lithium-ion battery fires recorded last year, equaling to about one every 36 hours.

So far this year, 76 fires have killed seven people including two this past weekend in Washington Heights.

With 60 people injured, the city is outpacing all of last year. The vast majority of fires are in the homes of the 65,000 delivery bikers who live across the five boroughs.

One of the solutions to the city's dilemma is over 800 miles away at the UL Solutions laboratory for battery testing in Northbrook, Illinois, outside of Chicago.

Dr. Robert Slone is the chief scientist and the man behind making lithium-ion batteries safe.

For batteries to receive a UL logo, it has to endure what is called "torture testing." But experts said the problem in New York is uncertified batteries, often cobbled together in the black market.

Victims were instantly overwhelmed by gas, which left high concentrations of hydrogen sulfide in air

India’s National Disaster Response Force, the State Pollution Control Board and the police are all investigating the incident
As people were heading towards a milk vendor in Ludhiana, India, early in the morning of 30 April, highly toxic gas suddenly overwhelmed and killed 11 people, including two children, in the street and nearby houses. Four others were injured but have since recovered.

Preliminary investigations indicate that there was a high concentration of hydrogen sulfide in the air which smelled pungent and of rotten eggs. ‘There was no respiratory distress and the gas is suspected to contain neurotoxins,’ says Swati, a Sub Divisional Magistrate for Ludhiana, who is heading the magisterial inquiry.

The investigators from the National Disaster Response Force, the State Pollution Control Board and the Ludhiana police are investigating the exact cause and source, which remains unknown. The Punjab state police has also formed a five member special investigation team. The police have warned the pollution control officials that if they don’t help the police in finding the culprits, they too will be prosecuted.

Whether industrial chemicals were illegally dumped in the sewers is yet to be determined

‘We are checking sewage connectivity as well, but the concentration of hydrogen sulfide has to be very high to be able to kill instantly,’ says Swati, who uses a singular name. The circumstantial evidence indicates that those exposed to the gas, or mixture of gases, died almost immediately, she adds.

Tucson hospitals and county health officials weren’t immediately notified about a nitric acid spill on Interstate 10 in February, among communication missteps in a mostly strong and effective emergency response, says a new report.

The after-action report by the Pima County Emergency Management Office, released to the public on Friday, assessed strengths and weaknesses in agencies’ response to the hazardous material spill.

On Feb. 14, truck driver Ricky Immel, 54, was driving near Kolb Road on I-10 when he slowly veered off the road and onto the median, where the uneven ground caused the truck to roll onto its left side. Containers carrying about 18,200 kilograms of nitric acid ruptured in the crash, causing the Arizona Department of Public Safety to shut down both directions of the interstate and tell residents and other people in the area to shelter in place.

Immel died in the crash. He had a blood alcohol level nearly eight times above the state’s legal limit, the county medical examiner found.

With the help of the Tucson Fire Department and DPS, the county Emergency Management Office conducted a survey and held two debriefing sessions to discuss what went well and what could have gone better. The first session was for first responders, and the second session included community stakeholders and support agencies, the report said.

An explosion at a Seqens chemical plant in Newburyport, Massachusetts, early on Thursday, May 4, killed one worker. The cause of the explosion has not been determined.
The incident is the third serious accident at the plant, which manufactures pharmaceutical chemicals under contract, since 2020 when an explosion damaged a building. A chemical fire occurred at the site in 2021.
The body of Jack O’Keefe, an employee at the company for more than 25 years according to a local news report, was removed from the site late Thursday afternoon. Four other workers on site when the explosion occurred were hospitalized for precautionary reasons, but were uninjured, according to Barry Salt, Newburyport deputy fire chief.
“They had just wrapped up a typical manufacturing procedure that they’d done numerous times daily,” Salt says. “Obviously there was some sort of accident to cause the explosion, but what that was or how that happened is still in the beginning stages of investigation.”
The plant manufactures both active ingredients and intermediates of pharmaceutical chemicals. Chemicals on site at the time of the accident, according to Salt, included acetone, isopropyl alcohol, and methanol.
Salt says air quality has been monitored by the Massachusetts Department of Fire Services beginning 1.5 h after the explosion. Local residents have been informed that there is no risk in the area.
The area of the plant damaged in the explosion has been in operation for more than 20 years. The facility was built in the 1980s. Novacap, a French firm now called Seqens, became the site’s owner when it bought PCI Synthesis in 2018.

Corteva Agriscience subsidiary has pleaded guilty to criminal negligence, 8 years after an accidental chemical release killed 4 employees and injured several others at a now defunct insecticide plant in La Porte, Texas.
On April 24, a US district judge ordered the subsidiary, EIDP Inc., formerly known as E. I. du Pont de Nemours and Company Inc., to pay a $12 million penalty and make a $4 million community service payment to the National Fish and Wildlife Foundation. EIDP must also serve 2 years of probation, during which time the company must provide the US Probation Office full access to all its operating locations.
The accident, which occurred on Nov. 15, 2014, at what was then a DuPont facility, released nearly 11 metric tons of highly toxic methyl mercaptan after plant workers tried to clear a section of blocked piping. A US Chemical Safety and Hazard Investigation Board probe following the incident determined that flawed engineering designs and a lack of adequate safeguards were to blame.
The US Department of Justice also found that Kenneth Sandel, a former manager who was responsible for making sure employees understood and followed safety procedures, failed to provide sufficient instructions on how to safely clear the blockage. Sandel pleaded guilty alongside the company and was sentenced to 1 year of probation.
“The Company deeply regrets the tragic events that occurred on November 15, 2014, at a former EIDP facility in La Porte, Texas,” says a Corteva spokesperson in an emailed response. “We are committed to the highest safety standards, and safety is a core value.”

At least five people were killed, one was missing and another one was injured after an explosion rocked China's chemical factory on Monday morning, reported The Global Times. On Monday, the incident took place at the chemical hydrogen peroxide production area in China's Liaocheng, Shandong province, reported The Global Times. Investigation is underway to determine the reason behind the blast. Meanwhile, according to another local newspaper, the fire had been put out as of Monday afternoon. The person, who has been injured, was rushed to the local hospital for treatment while rescue work is still underway.
Fire explosions recurring in China

This was not the first time that China has witnessed an explosion. Earlier in January, five people died after an explosion occurred at a chemical company in Panjin, Northeast China's Liaoning Province on missing on January 16. Rescue teams had arrived at the scene of the accident and a search operation has been launched for people who may be trapped, reported The Global Times. As these incidents keep on recurring in China, The Ministry of Emergency Management has continued to guide the on-site rescue and treatment of those injured. In the January explosion, Wang Xiangxi, minister of Emergency Management, had made arrangements and called for all-out efforts to rescue the missing and prevent secondary accidents. Meanwhile, local authorities have asked the officials to adhere to the principle of safety first and learn lessons from the accident. At this incident, which took place in January, 20 ambulance vehicles and 240 medical personnel and other staff were at the site, as per media reports.

Roberto Longoria, 42, passed away on Aug. 29, 2021, after he was exposed to hydrogen sulfide gas during a workplace incident at Danny’s Truck Wash, located near 99th Avenue and McDowell Road.
Another employee, Jose Perez, reportedly got inside the tanker trailer first to fix a drainage issue. After Longoria noticed Perez was unconscious, he grabbed a respirator, told the driver to call 911, and entered the trailer.
Longoria soon fell unconscious too and both men had to be removed from the trailer by firefighters. Neither of them survived.
The Carnegie Hero Fund Commission has recently selected Longoria as one of the 15 civilians who will receive the organization's hero medal. Each medal recipient or their surviving family will be awarded a financial grant.
The medals are awarded to U.S. and Canadian citizens who enter "extreme danger while saving or attempting to save the lives of others," according to the commission.

Dhaval Antapurkar, Director at the Directorate of Steam Boilers, a Maharashtra government body, said in a statement, "The three boilers out of the five at the Jindal Poly Films in Igatpuri were of waste heat recovery or running on thermic fluid. It does not use any inflammable material to prepare steam."

Sulphur, LA (KPLC) - State police are investigating after a man died in a hazardous chemical accident at Holeshot Tank Wash.

State police say Alvin Lively, 48, of Lake Charles, was inside a tank while working at Holeshot Tank Wash off Hwy 108 in Carlyss.

Police say Lively was washing the tank when he was found unresponsive Tuesday afternoon. Police say troopers received a call to investigate the incident around 2 p.m.

Trooper First Class Derek Senegal could not confirm whether Lively was wearing protective equipment.

“He was found inside of the tank unresponsive. Have to make sure that whatever type of, if he had some type of whether he had some type of PPE, personal protective equipment on, that he had the right stuff that’s on. I can’t say that he did, but that’s all part of the investigation,” said Senegal.

Senegal says the truck, owned by Republic Services, was dropped off for cleaning on Nov. 18. It contained pyrolysis gas, or “pygas,” which displaces oxygen.

Senegal says Lively was dead at the scene after apparently becoming overcome by fumes.

The U.S. Chemical Safety and Hazard Investigation Board (CSB) released an update into its ongoing investigation of a fire and explosion at the Yenkin-Majestic OPC Polymers resin plant in Columbus, OH on April 8, 2021.

The update provides a detailed incident description, background information on the facility and production, as well as a history of the equipment. The large explosion and fire led to one fatality and injured eight others.

“This was a very serious incident which led to a fatality, several injuries, and serious damage to the facility and the surrounding community,” said CSB interim executive Steve Owens. “This update provides important information about the events leading up to this incident while our investigation is ongoing.”

The CSB update provides details of the events on the night of the incident. Chemical reactions occurred when a process in a unit referred to as kettle 3 was nearly complete. The CSB determined that during the addition of a solvent to kettle 3, an agitator (stirring mechanism) had not been running for more than an hour. When a worker realized that the agitator had shut down, he turned it on and the product inside the kettle began to quickly vaporize. This increased pressure inside the kettle, which continued to rise until a mixture of resin liquid and flammable solvent vapor released from kettle 3’s manway into the enclosed room where the kettle was located.

SAN ANTONIO (AP) — A drug lab and explosive devices have been ruled out as the possible causes of a deadly explosion that killed four people in San Antonio last week, authorities said Tuesday. They said they are still investigating other possible causes, including potential problems with a propane tank.

The explosion just before midnight Friday destroyed K-Bar Services, which San Antonio Fire Department officials described on Tuesday as an underground residence and construction business located on 75 acres (30 hectares) of property. About 100 vehicles, including construction machinery, cars and recreational vehicles, are also located on the property, the officials said at a news conference.

Wet weather, muddy conditions, structural issues and a large radius of evidence produced by the magnitude of the blast have slowed the investigation, they said.

"It was absolutely unsettling as far as the devastation that we witnessed," said San Antonio Fire Department Chief Charles Hood. "As far as first responders, we don't see things like this type of explosion inside of a building like this."

Witnesses who said they had been on the property prior to the explosion told officials that they could smell gas, and a propane tank on the property had been worked on recently, the fire officials said. They added that foul play has not been ruled out and the investigation is ongoing.

Washington D.C., December 7, 2022 - Today, the U.S. Chemical Safety Board (CSB) released a new safety video about the fatal explosion that occurred on April 3, 2017, at the Loy-Lange Box Company in St Louis, Missouri. The incident occurred when a severely corroded pressure vessel catastrophically failed, causing an explosion that launched the pressure vessel into a neighboring building. One worker and three members of the public were fatally injured.

The safety video includes an animation of the sequence of events leading to the incident, and interviews with both the CSB’s Interim Executive Steve Owens and Lead Investigator Drew Sahli.

During its investigation, the CSB found that over the course of many years, an area of the failed pressure vessel had thinned due to a known corrosion mechanism that was poorly controlled at Loy-Lange. And the CSB found that Loy-Lange repeatedly ignored clear warnings that corrosion was causing major problems within its operations. In fact, the CSB found that prior to its failure, Loy-Lange ran the pressure vessel normally despite knowing that it was leaking.

In the video, Interim Executive Owens summarizes the CSB findings: “The incident at Loy-Lange resulted from a tragic series of circumstances -- ineffective corrosion management, inadequate pressure vessel repair, a lack of inspections, and the absence of sound safety management systems. The result of those factors was that a severely corroded pressure vessel, which presented a serious safety hazard, was allowed to operate until it ultimately failed, resulting in the death of four people.”

The potential for corrosion was a well-known problem at Loy-Lange. During its time operating the pressure vessel that failed, Loy-Lange experienced at least three leaks due to corrosion. In response to a 2012 leak, a repair company removed most of the area of the pressure vessel where the failure occurred. The company patched it with new steel but left some of the original steel in place. The original steel, however, was also thinned from corrosion, and between 2012 and 2017, it continued to degrade. The CSB found that it was the unacceptably thin remaining material from that repair that failed, initiating the incident.

A trucker from Illinois was killed Saturday night in a single-vehicle crash on Interstate 80 in the southern Panhandle not far from Sidney. A resulting chemical spill closed the freeway into Sunday.

Cheyenne County Attorney/Coroner Paul Schaub says 42-year old Mohammad Pirzad of Buffalo Grove was pronounced dead at the scene near Potter.

Cheyenne County Sheriff’s deputies responding to reports of the accident found the cab and trailer of a semi blocking both westbound lanes with Pirzad dead inside the cab.

Witnesses told deputies the truck was traveling about 70 mph when it drifted onto the grass shoulder, then continued on the shoulder for a distance before Pirzad turned sharply back onto the pavement.

That caused the tractor and trailer to tip onto the passenger side. The witnesses said they never saw the truck’s brake lights come on.

Another witness told deputies he went up to the windshield of the cab and saw Pirzad on the passenger door. He and another person then broke the windshield to get a better look, then concluded Pirzad was dead.

County Attorney Schaub says the truck was hauling a load of chemicals that spilled in the crash. Those chemicals included acetone, isopropanol alcohol, ethanol, and ammonia. Hazard mitigation and cleanup last well into Saturday.

The fire involved natural gas, according to a tweet from the NTSB, which investigates pipeline mishaps.

A fire department statement said crews responded to a northwest Atlanta home around 8:30 a.m. on Dec. 3, the Atlanta Journal-Constitution reported. Atlanta Fire Rescue Department officials said a gas leak was found in the front yard after crews extinguished the heavy blaze.

Other news reports said Atlanta Gas Light, the largest natural gas distributor in the Southeast, attributed the cause of the fire to the leak. Fire officials said the origins were still under investigation.

In December 2016, Alton Zeigler, Jacob Jackson and Kevin Vann — maintenance employees of DAK Americas LLC — attempted to remove a faulty pump on a chemical production line in Calhoun County, South Carolina, and owned by Eastman Chemical Co., which contracted work with DAK, which then contracted with another maintenance company, Mundy Maintenance Service and Operations LLC.

While attempting to remove a chemical pump without draining it first, the three workers loosened the bolts securing the pump, an explosion erupted in which molten liquid killed Mr. Zeigler and severely burned Mr. Jackson and Mr. Vann.

In April 2017, the two workers and a surviving spouse filed separate personal injury actions against Eastman and Mundy in federal district court, alleging that Eastman’s employees “were negligent in their management of the retained line, and that Mundy’s employees were negligent in their attempt to unclog the drainpipe prior to the explosion,” according to case No. 19-1643, consolidated by the 4th U.S. District Court of Appeals.

Two other people were injured in the blaze that tore through the building in the city of Anyang starting at about 4:30 p.m. Monday. It took firefighters about 3 1/2 hours to bring the flames under control, the Wenfang district government said.

One or more welders operating in violation of safety rules appear to have let loose sparks that ignited cotton fabric in the building, according to officials cited by the official Xinhua News Agency. The investigation was ongoing, with company managers and local government officials being questioned, Xinhua said.

Indian generics firm Maiden Pharmaceuticals has been implicated in the deaths of 66 children in The Gambia, West Africa.

On 5 October, the World Health Organization (WHO) issued a special alert focusing on four substandard cough syrups: Promethazine oral solution, Kofexmalin Baby Cough Syrup, Makoff Baby Cough Syrup and Magrip N Cold Syrup, all manufactured by Maiden, which is based in the north Indian state of Haryana. These products are ‘potentially linked with acute kidney injuries and 66 deaths among children’ in The Gambia, the report states. ‘WHO recommends all countries detect and remove these products from circulation to prevent further harm to patients,’ director general Tedros Ghebreyesus stated.

Toxic ethylene glycol and dietheylene glycol were discovered in samples taken from the implicated cough syrups, which are often used to treat children
According to the WHO, at the end of July 2022, the Gambian government received information about an increase in acute kidney injuries among young children from Edward Francis Small Teaching Hospital, the country’s main hospital. By 30 September, 78 cases had been reported including 66 deaths. About 72% of these cases were under two years old, reported from six out of the country’s seven health regions.

The link with cough syrups was discovered after samples of medicines used by the children were sent to labs in Senegal and Ghana – since The Gambia has no such facilities. Testing identified toxic contaminants diethylene glycol and ethylene glycol. The government began recalling the products from 2 October and has banned further sales. A door-to-door search aims to recover those already distributed and the police are investigating. More than 16,000 products have been located and confiscated for destruction. The Gambia’s President Adama Barrow has stated that his country will go to the bottom of this tragedy and is negotiating with the World Bank to create testing facilities.

Columbia River Fire & Rescue officials said the explosion was caused by oxygen tanks stored in the apartment where the fire started at about 12:30 a.m. Oct. 5.

Linda Newman, 74, died in the fire. Her son, Adam King of Kelso, Washington, told KATU his mother was the life of the party, who liked to sing and dance, and was a “kick in the pants.”

St. Helens Police officers responded to the fire and rescued several residents from the building by pulling them through windows.

Firefighters found Newman unconscious in the hallway of the building where the fire started. They got her out of the building and despite efforts to revive her, she died at the scene, authorities said.

Sept 28 (Reuters) - BP Plc laid off most contractors at the approximately 160,000 barrel-per-day Toledo, Ohio, refinery it owns with Cenovus Energy Inc (CVE.TO), according to sources familiar with the matter on Wednesday, indicating that the plant will experience a prolonged shutdown following last week's explosion and fire.

The explosion killed two United Steelworkers members, identified as brothers Max and Ben Morrissey.

The more than 100-year-old refinery has been offline since the middle of last week following the explosion and could be shut for several months.

At least one contracting company is on site assessing the damage. The U.S. Chemical Safety Board is also investigating the incident, which it said additionally caused the release of sulfur dioxide and hydrogen sulfide and significant property damage.

Cenovus has referred comment to BP, which has declined to comment further.

The Department of Justice says PCA violated the Clean Air Act’s General Duty Clause and Risk Management Program Regulations.

Three workers were killed and seven injured in the explosion, which launched a 100,000-gallon storage tank into the air and over a six-story building. The tank landed 400 feet away on mill equipment.

The United States and the Louisiana Department of Environmental Quality outlined nine violations of the Clean Air Act in a complaint filed with the settlement.

“Sadly, it took an explosion and the loss of lives to highlight PCA’s failure to adhere to some of these guidelines,” said Brandon Brown, U.S. attorney with the Western District of Louisiana.

“PCA violated the Clean Air Act and accompanying regulations at its DeRidder mill, resulting in an explosion that caused the senseless deaths of three workers, while placing other workers and the surrounding community in danger,” said Todd Kim, assistant attorney general with the Justice Department’s Environmental and Natural Resources Division.

The fire broke out on the third floor of the house on 130th Street in College Point at around 7:35 a.m., authorities said.

New York City firefighters found Stephanie Villa Torres, 8, unconscious and unresponsive. She pronounced dead at New York Presbyterian Hospital-Queens. Her father and brother were treated for burns and smoke inhalation, officials said.

Fire marshals determined that the fire was accidental and was caused by a "lithium-ion battery from an e-mobility device."

Authorities have blamed at least 130 fires from lithium batteries so far this year. In the last two years, fires started by batteries for e-bikes and e-scooters have killed at least nine people and hurt dozens and dozens more.

We're outraged by the news of a deadly blast that occurred at the BM Inland Container Depot in Sitakunda, Chattogram, killing at least 49 people (as of Sunday 7pm) and injuring hundreds. The sheer ferocity of the fire and subsequent explosions that swept through the facility since Saturday night can only be rivalled by the sheer meaninglessness of the tragedy that followed. The fire, which couldn't be fully doused even after about a day, is the latest in an ever-growing list of tragedies that put Bangladesh's appalling industrial safety record once again under the spotlight.

The death toll will no doubt rise as many of the injured are said to be in a critical condition. Horrifying scenes of panic and devastation have been reported both on the spot and at the hospitals. Some of the explosions – there were many – were so large that their sound could be heard several kilometres away. One eyewitness described seeing "fireballs falling like rain". The challenge for the firefighters and military rescuers, who joined them later, has thus been quite daunting as they struggled to bring the fire under control and pull charred bodies from the debris. At the hospitals, the air is thick with grief and anxiety.

What caused the fire? We don't know yet. But there were reportedly containers with chemicals in the depot, including a huge quantity of hydrogen peroxide – an oxidising chemical – which can intensify fire and is a major fire hazard when combined with other chemicals and substances. Apparently, the fire service wasn't informed of the unauthorised storage of chemicals (leading to at least nine firefighters being killed). This—in a depot containing millions of dollars of garments—will likely be a central focus of the investigation. Whatever caused the fire and subsequent explosions, we hope that investigators will find out soon and that those responsible will be held accountable.

A federal judge sentenced Tampa Electric Co. to a $500,000 fine and three years of probation for a 2017 explosion that killed five people at its Apollo Beach power plant.

The sentence from Judge Charlene Honeywell of the U.S. District Court for the Middle District of Florida, announced in a news release Friday from the Department of Justice, follows a plea agreement filed in the Tampa-based court in May in which TECO pleaded guilty to violating Occupational Safety and Health Administration standards.

The fine is the maximum allowed under law for a violation such as this, according to the release. TECO also must follow a safety compliance plan.

“Our office is proud to have partnered with DOJ’s Environmental Crimes Section, the FBI, and the Department of Labor, Office of Inspector General to shine a light on this willful violation of OSHA safety standards in order to deter such conduct and ensure that workers are protected in the future,” U.S. Attorney Roger B. Handberg for the Middle District of Florida said in the release.

In June 2017, TECO sent workers to clear a blockage of hardened slag — a molten coal by-product that drips from a furnace into a cooling tank underneath it.

TECO did not shut down the furnace when the blockage occurred and instead called in a contractor to clear it with high-pressure water while the system still was running. The slag gushed out from the tank and onto the workers.

The US Chemical Safety and Hazard Investigation Board recently released a final report detailing the cause of an industrial incident that launched a large pressure vessel 150 m, killing four people—one employee of the company and three of a neighboring facility. The April 2017 explosion occurred at the Loy-Lange Box Company, a 71-employee manufacturer of corrugated cardboard in St. Louis, Missouri.
CSB interim executive Steve Owens noted at a July 29 briefing that the report “took far too long to complete” and was the only CSB accident report issued in the last 10 months. That view was seconded by board member Sylvia E. Johnson, who acknowledged CSB difficulties. The CSB has 17 unfinished reports, “probably the biggest report backlog in CSB’s history,” she said. Johnson urged “grace and patience” from public stakeholders as the board attempts to correct problems.
On a Friday 5 years ago, Loy-Lange operators noticed a leak in the bottom of the pressure vessel as the plant shut down for a regular-scheduled weekend break. The company did not repair the vessel, which was a steel cylinder 5.3 m long and 76 cm in diameter. When the plant restarted on Monday morning, the leak expanded, releasing pressurized water and steam and launching the vessel into another nearby plant, killing three people there as well as one worker at Loy-Lange.
According to the report and a presentation by CSB investigator Drew Sahli at the board’s briefing, extensive dissolved oxygen had led to excessive corrosion in the vessel’s bottom head. The vessel had never been inspected in the 20 years it was operating, either by the company or the the city of St. Louis, which regulated pressure vessels within its jurisdiction. In 2012, a similar leak had been inadequately repaired by the now defunct Kickham Boiler and Engineering company.
The CSB investigation yielded several recommendations. One is that Loy-Lange establish a process safety management system. The agency also recommends that St. Louis and the National Board of Boiler and Pressure Vessel Inspectors fill gaps in their regulatory oversight programs.
Owens noted that one of the reasons the report was delayed was the high turnover of CSB staff, particularly investigators. Owens and Johnson, like past CSB leaders, promised to address the problem.

On April 3, 2017, a large industrial boiler exploded at the Loy-Lange Box Company on Russell Boulevard. A substantial piece of the boiler then traveled around 500 feet and crashed through the roof of the Faultless Healthcare Linen building.

Kenneth Trentham, 59, was killed in the initial explosion at the box company. When the equipment crashed into the Faultless Healthcare Linen building it struck two employees, 43-year-old Tonya Gonzalez-Suarez and 46-year-old Christopher Watkins, killing them. A fourth person, Clifford Lee, 53, was trapped under the hot boiler after it crashed into Faultless Healthcare Linen. He later died as a result of his injuries.

The CSB discovered an area of the failed pressure vessel had thinned due to a known corrosion mechanism that was poorly controlled and Loy-Lange repeatedly ignored clear warnings that corrosion was causing major problems within its operations.

MADISON CO., Miss. (WLBT) - At least six people were injured after suffering severe burns following a Friday morning oil explosion in Madison County.

The incident happened on Virlilia Road near Jubilee Road at the Salt Water Disposal Site.

Madison County Fire Coordinator Minor Norman said the victims suffered burns on at least 50 percent of their bodies, with one person being burned from head to toe. Two people were airlifted to the University of Mississippi Medical Center for treatment.

“There was a huge fireball and they were caught up in it,” he said. “Some were critical.”

The explosion occurred before 8 a.m. Madison County Sheriff’s Department received a call into dispatch around 7:50.

“It was one of the victims of the explosion making the call,” Norman said. “The fire was put under control at 9:02.”

Norman said the cause of the explosion was still under investigation.

The Mississippi Department of Environmental Quality was going out to the site to access the situation.

“We’re not sure what caused it. There were four storage tank facilities, to hold products from oil wells. They [the workers] were in the process of changing out two tanks with new fiberglass tanks. They were getting ready to go into service when the older tank exploded,” Norman said.

“You had some type of petroleum product in it. Because it was a huge fireball, it severely burned six workers.”

The workers were airlifted to seek medical treatment, Madison County spokesman Heath Hall said.

Minor said the explosion occurred in the Kearney Park Fire Department’s response area, but the Gluckstadt Fire Department was the first on the scene.

(WIS) - The South Carolina Highway Patrol is investigating a deadly crash on I-77 southbound in Fairfield County that caused significant traffic delays, and a chemical spell which led a neighboring town to shelter in place.

The crash happened just before 4 A.M. near mile marker 31, which is about three miles from the town of Ridgeway. It involved a Peterbilt tractor, and two other cars.

One person has died as a result of wreck, according to the SC Highway Patrol.

In the aftermath of the crash, a shelter in place was issued for residents in Ridgeway as first responders worked to determine what chemical had spilled onto the highway from the truck.

As the Fairfield County Fire Service put out the fire that resulted from the crash, that chemical got kicked up into the smoke.

“Smoke is always a concern, whether if it’s chemical or whether it’s free-burning,” he said. “But you just want to make sure that nothing is toxic. And the biggest thing is nobody knew at the time what was burning other than seeing the fire burning.”

The chemical was later identified by the Fairfield County Fire Service as ammonium sulfate.

At least 51 people, including 10 firefighters, were killed and over 200 injured in the fire and massive explosion at the depot on the outskirts of the country’s port city Chattogram on June 4. The fire was doused on June 8.

The report submitted to Chattogram divisional commissioner Md Ashraf Uddin by committee chief Mizanur Rahman, also the additional divisional commissioner (development), found that the fire originated from the unauthorised chemical hydrogen peroxide stored in some containers at the depot.
Al Razi Chemical Complex Limited, a sister concern of the depot owner Smart Group of Industries, owned the hydrogen peroxide-laden containers.
A member of the committee said that they had made 20 specific suggestions in their report regarding fire control at inland container depots.
The divisional commissioner told reporters that they would send the report to the ministry and the ministry would take further decisions.

FLINT, Mich. (WJRT) - The investigation into what caused a deadly explosion on Hogarth Avenue in Flint likely will remain a mystery through the summer months.

Investigators from several agencies who are trying to pinpoint a cause are not planning to meet again until Sept. 7, according to Michigan State Police and the Flint Fire Department.

They will discuss the results of testing conducted on evidence collected from the scene in February.

The explosion on the evening of Nov. 22 killed 3-year-old NuVeah Lucas, who lived in the house at the center of the blast, and 55-year-old Lisa Rochowiak, who lived in the house next door.

Three homes were completely destroyed in the explosion and dozens more in the neighborhood on Flint's west side sustained damage. Nearly 30 families were displaced after the blast and at least five homes will require extensive repairs before they can be occupied again.

gaseous explosion on the ground floor of the Enyobeni tavern which resulted in chemical asphyxiation, was probably the cause of the death of 21 young people in the early hours of Sunday morning, a government pathology expert has said.

Dr Solomon Zondi did not rule out the possibility that the deadly substance could have been swallowed in drinks provided to patrons in the tavern. But he suggested that a gaseous substance was released in the enclosed downstairs area – “this is where the pandemonium started” – and that people who were closer to the release epicentre were most affected.

Zondi said videos he had seen gave no indication of a teargas canister explosion, but people could be seen complaining of respiratory symptoms. During the post-mortem examinations on the deceased, Zondo said he was cautious not to inhale any odour emanating from any of the bodies. At one stage his heart started racing as he examined a body.

“I was becoming dizzy and had to stop for a while,” he told the Mail & Guardian. “The children died of poisoning – the question now is whether it was inhaled or ingested.”

Zondi spoke at the Woodbrook, East London, mortuary and pathology facility where the bodies of the deceased were taken after Sunday’s horror incident.

At least 13 people died and an estimated 250 people fell ill following the industrial accident at Jordan port.

The tank was dropped by a crane loading it onto a shop in Aqaba earlier today.

State TV broadcast the moment dockworkers were sent fleeing for their lives as the thick yellow cloud rolled towards them.

The Public Security Directorate, which initially described it as a gas leak, said authorities sealed off the area after evacuating the injured and sent specialists in.

State-run Jordan TV said 13 people were killed, while Al-Mamlaka TV, another official outlet, said 199 were still being treated in hospitals.

R. Lakshmi, along with her family, was living on the premises of the SBVK function hall at Bahadurpally and taking care of it. The family hails from Nagireddypet in Kamareddy district and had moved to Hyderabad six months ago for better job prospects.

Police said Lakshmi was trying to open the 25-litre can to clean and use it as a container to get diesel for the generator. It is suspected the paint can was old. A bomb disposal team arrived at the scene at around 11 p.m. and examined the contents. Samples were also sent to a forensic laboratory.
