    <dc:creator>dchas</dc:creator><description><![CDATA[MARTINEZ, CA — A chemical spill Thursday at a Martinez business landed one person in the hospital.

The Contra Costa County Fire Protection District responded to a hazardous materials incident shortly after 11:30 a.m. Thursday at a laboratory in the 100 block of Mococo Road, a spokesperson for the fire district confirmed.

"Appears one employee was exposed to a hazardous chemical," Con Fire Spokesman Steve Hill told Patch. "The one employee was transported to a hospital at 12:14 p.m."

The spill, which was initially reported as possible cyanide, was limited to a chemical hood in a lab, Hill said.

As for whether the chemical was confirmed to be cyanide, that was not yet known, Hill said.

    <dc:creator>dchas</dc:creator><description><![CDATA[On Monday, the United States of America and the State of Indiana filed a complaint in the Northern District of Indiana against Cleveland-Cliffs Burns Harbor LLC and Cleveland-Cliffs Steel LLC for a 2019 discharge of toxic chemicals into Lake Michigan, which resulted in the killing of marine life and the closure of several public beaches in Indiana.

The defendants own the Burns Harbor Facility, “one of the largest fully integrated steel mills in  North America, with the capacity to produce approximately 5 million tons of raw steel per year.” The facility has a federal NPDES permit which authorizes it to “discharge treated wastewater, stormwater, and non-contact cooling water, as well as treated sanitary sewage wastewater from the Town of Burns dHarbor’s wastewater treatment plant.” The plaintiffs alleged that Cleveland-Cliffs sends once-through, unrecycled water into the treatment plant despite not being allowed to do so. Because of this, the facility allegedly discharged toxins including cyanide and ammonia into Lake Michigan. Furthermore, in August 2019, an alleged malfunction led to millions of gallons of intoxicated wastewater to be dumped into Lake Michigan, which Cleveland-Cliffs failed to notify the federal authorities of for several days. This caused the closure of several public beaches, the killing of thousands of marine life, and a major cleanup mission to detoxify the lake.

The United States of America sued Cleveland-Cliffs seeking injunctive relief permanently enjoining the defendants from further violating the Clean Waters Act (CWA), Comprehensive Environmental Response, Compensation and Liability Act (CERCLA) and Emergency Planning and Community Right-to-Know Act (EPCRA), civil penalties, reimbursement, disbursement, and other relief.

For months, residents of Durban have been pleading to know which toxic chemicals they were exposed to when the UPL chemical warehouse was set alight during the July 2021 unrest.

Now, finally, they have some answers.

The UPL warehouse in Cornubia was storing up to 19,000 tons of pesticides. At about midnight on 13 July 2021, the building was torched as part of the ongoing violence in KwaZulu-Natal.

In the days that followed, a toxic soup of chemicals flowed into the Ohlanga River, turning the Umhlanga Lagoon a luminous turquoise and killing thousands of fish. The fire also blanketed northern Durban in acrid smoke for more than a week, leaving residents struggling to breathe and with few answers about what was in the air.

Last week, the provincial government finally released an atmospheric impact report, commissioned as part of the mammoth clean-up of the chemical disaster.

The report, produced by Airshed Planning Professionals in Midrand, identifies 62 chemicals that were likely present in the smoke, the “most significant” being hydrogen cyanide, hydrochloric acid and bromine, as well as pollutants more commonly associated with coal power plants: SO2, NO2 and PM.

Police said the driver of the truck escaped without injury and unhooked one of the trailers.

The CFS said a large amount of a chemical the truck had been carrying had spilled on the road and there was also a large amount of toxic smoke.

It said the chemical is believed to be cyanide-based, and that two CFS trucks were on the scene.

The Eyre Highway has been closed in both directions and is expected to remain closed for some time.

2 On Your Side has learned through a Freedom Of Information Law request that air, soil, and water tests conducted on July 21, a little more than 14 hours after the fire started, determined nearly three dozen "pollutants of concern" were detected at the site. 
According to the documents obtained by WGRZ, the testing was conducted by John Richter, Pretreatment Coordinator of the Environmental Control Division at the EPA/NYSDEC. 
In an email to state and federal officials, Richter says cyanide and dioxane "may have entered both the Town of Amherst Sewer Lateral System and the Town of Amherst Storm Water Collection system."
According to a spreadsheet of all chemicals detected at the site of the fire, 1.23 mg/L of cyanide were discharged into the sewer system, "slightly above town standards," Richter said in that email to state and federal officials.

“While e-commerce is a significant driver of our economy and provides convenience to millions of consumers, there [is] also [a] certain degree of risk to the population in general,” DENR Usec. Juan Miguel Cuna said over the weekend.

Rreferring to the online selling of regulated chemicals or chemical products, Cuna said: “With these products being highly accessible to anybody who has access to online trading sites, risk of human exposure or environmental contamination is highly possible, especially when sellers or suppliers do not have permits from government regulatory agencies.” 

Cuna made the remarks last Friday, April 16 during a virtual interagency meeting involving environmental, health and trade regulators, e-commerce executive, and chemical safety advocates. The topic of the meeting was the supposed unfitting use of online shopping platforms to sell regulated chemicals such as cyanide and mercury.

A presentation by the EcoWaste Coalition illustrated how third-party dealers are taking advantage of digital commerce to sell regulated chemicals in blatant disregard of existing regulatory controls.

Fire crews are currently on the scene trying to extinguish the flames at Cahaba Pallets in the 300 block of Airport Road in Centreville. As of 6 p.m., the fire was under control.

Centreville Mayor Mike Oakley said all the equipment inside the business was reportedly destroyed and 3 acres of the property is still burning.

The fire is now a chemical situation involving cyanide. Tuscaloosa Fire Chief Rodney Smith says there are no signs of the material at this time and likely burned up in the fire.

At least 12 fire departments have responded to the scene but have been cleared as of 8:45 p.m. The cause of the fire is unknown at this time.

Josh Lee is a Powell firefighter and was rushed to the hospital over the weekend.

"Some of us are going up there everyday," said Chief Garmon.

We caught up with the chief of Powell's volunteer fire department. He described Josh Lee, the firefighter sent to the hospital.

"Hardworking, dependable, a very busy young man," said Garmon.

Chief Garmon said Josh Lee has worked as a firefighter in DeKalb County for more than a decade, and just recently started working at Powell Fire. He was battling a fire Saturday morning when he went down. He was originally taken to DeKalb Regional Hospital, then flown to Erlanger in Chattanooga, Tennessee.

Investigators think Lee is suffering from cyanide poisoning.

"That's very, very, very uncommon," said Garmon.

The DeKalb County Sheriff's Office said fires can create cyanide gas. Right now, they don't expect foul play.]]></description>

“The reopening is in response to three consecutive days of water testing that indicated cyanide levels well within the US EPA's standards for drinking water,” National Park Service spokesman Bruce Rowe said in a release. “In addition, two of the sampling days indicated no detection of cyanide.”

The agency also reopened the Little Calumet River within the Indiana Dunes National Park, he said. Both were reopened after sign offs from state and federal environmental agencies.

“The National Park Service will continue monitoring the ongoing water testing to ensure the health and safety of park visitors,” Rowe said.

    <dc:creator>dchas</dc:creator><description><![CDATA[A Shrewsbury man has been sued for illegally demolishing a backyard shed containing dynamite, causing a hazardous fire and explosion in the fall of 2016, Attorney General Maura Healey announced Wednesday.

On Oct. 24, 2016, Edgar Muntz Jr. directed P&M Asphalt Services Inc. of Sutton to demolish an old wooden shed at 393 Oak St. and to remove the construction and demolition debris from the property. Mr. Muntz, who lives at 10 Grove Meadow Lane, owned the 393 Oak St. property as trustee of 393 Oak Street Realty Trust.

Inside the shed were about 550 glass jars and other containers holding hazardous materials, including dynamite, hydrofluoric acid, mercury, sodium cyanide, arsenic, chloroform, toluene, and chromium, according to a complaint filed by the attorney general’s office in Suffolk Superior Court.

During demolition the shed caught fire, releasing hazardous materials to the air and soil, according to the attorney general’s office. The fumes caused P&M workers and a neighbor to suffer chest, throat, and respiratory discomfort, the lawsuit says.

After a neighbor alerted authorities to the explosion, the Massachusetts Department of Environmental Protection, along with other state and local agencies, took remedial action to secure the site and clean up the hazardous materials.

P&M workers previously had knocked down a house on the property. They backfilled the cellar hole of the demolished house using soil mixed with debris from the shed. That debris included jugs and plastic containers, some labeled “Poison,” that held bright multicolored powders, some of which were spilling out of cracked and broken containers, the lawsuit says.

But until the plant’s owners quietly received approval for a $100 million expansion from the Jefferson Parish Council last year — an approval that was recently rescinded, sparking a legal battle — few residents knew the toxic chemical was being produced there.

Fewer still are likely aware that in the past several years, the plant, known as the Fortier Manufacturing Complex, has been found responsible for two accidents involving the chemical, and has also been found on multiple occasions to have violated federal laws aimed at protecting communities from pollution.

An examination of reports from state and federal agencies, as well as other documents, shows a history of fines and regulatory actions by the U.S. Environmental Protection Agency and the Louisiana Department of Environmental Quality against the plant stretching back decades, with several actions occurring since Cornerstone Chemical took ownership in 2011.

The HazMat team arrived at 3:35 p.m. and most students returned to the Mods at 4:45 p.m., per the same email. According to a 3:22 p.m. email from Vice President for Campus Operations James Gray to the University community, a hazardous chemical substance was found in the Foster Mods. A later email on Friday at 5:14 p.m. from Gray confirmed the chemical was sodium cyanide. Gray clarified that the sodium cyanide was purchased commercially and was in the form of a non-volatile powder in an email yesterday to the Brandeis community. Additionally, he said there was no evidence that the purchaser intended  to harm anyone with  the sodium cyanide and “the material was identified, isolated, and removed appropriately and expeditiously.” “Out of an abundance of caution, the Mods were evacuated. Over 175 students living in a Brandeis University campus dorm were evacuated after a hazardous chemical substance was found on campus.

The school said sodium cyanide was found on campus in the Foster Student Living Center, known to people on campus as "The Mods."

Waltham police and fire departments and a hazardous materials team responded to the scene and removed the chemical.

The school said there was no danger to the community and no one was hurt.

"Out of an abundance of caution, the 177 students who live in the Mods were evacuated; we anticipate students will be able to return very soon," the university said in a statement. "Waltham Police do not anticipate making any arrests related to this incident."]]></description>

Geotek is a fiberglass manufacturer, and according to their Vice President of Quality Safety, employees complained of a strange smell and itchy eyes just after the company's lunch hour.

Geotek then called the Stewartville Fire Department, who brought in the State Chemical Assessment team out of Rochester.

Five people were transported for evaluation as a precautionary measure.

Stewartville Fire Chief Vance Swisher tells KIMT that the facility was evacuated and tested using monitors, which tested positive for hydrogen cyanide.

However, given the chemicals at the facility, they believe that the monitors had a faulty alarm.

As of now, Swisher says the most likely cause is that the manufacturing line at Geotek had a failure of some kind, causing the chemical mixutre not to be consistent with what they normally run.

The Inter-Ministerial Crisis Management Team (CMT) constituted by the government, in a statement recently, said while it is working with stakeholders to find amicable rehabilitation solutions to the damaged TSF (Tailings Storage Facility) it is safe for residents to carry out normal activities, including farming and schooling in the area.

It may be recalled that there was a chemical spill when a section of the geo- membrane of the TSF ruptured at the MNG Gold facility on September 27 affecting the victims, who were taken to Phebe Hospital in Suakoko for immediate medical attention.

The rupture resulted from an uncontrolled discharge of ‘slurry’ containing a high concentration of cyanide into the Sein Creek and the surrounding environment at MNG Goldmine in Korkoya.

An Environmental Protection Agency (EPA) release revealed that much has been done to restore the livelihood of the people of Sayewheh Town.

The incident occurred at Chilworth on Campus Drive, shortly after 11 a.m.

The company provides safety-related consulting, training, and testing for a number of industries worldwide, their website says.

Employees were working in a "fume hood" and their readings of a test showed a presence of cyanide and chlorine gas, West Windsor Emergency Services Director Jim Yates said. 

The quantities they were working with were very small and company officials called the fire department to check on the readings, Yates said. Two firefighters donned chemical suits to take readings in the area and found no escape of the chemicals into the building.

Yates said the firefighters also found no harm to the public or environment, nor to building employees or occupants and the building was later reopened.]]></description>

The Bozeman Police Department confirm the chemical that was reported at the Sundance Apartments was potassium cyanide gas.

Potassium cyanide releases hydrogen cyanide gas, a toxic chemical asphyxiant that interferes with the body's ability to use oxygen, according to the U.S. Centers for Disease Control & Prevention. Exposure to potassium cyanide can be rapidly fatal. 

Police, Gallatin County Sheriff’s deputies, and the Bozeman Fire Department were called to the apartment complex at around 10 p.m.

Electro Plating Services  at 945 E. Ten Mile Road was given multiple chances to comply and rectify the situation, according to Michigan Department of Environmental Quality Director Heidi Grether, but it continued to "ignore the most basic requirements of public safety."

“Its location, within 500 feet of residential neighbors and overlooking I-696, poses an imminent threat that could cause untold damage to people and the environment," Grether said in a news release. "We have worked with the Madison Heights Fire Department to help EPS meet its obligations to the community and under law. Several emergency crews, including police, fire and the DEEP emergency response team, responded to Waterbury Hospital. DEEP spokesman Dennis Schain said they were cleaning up “a chemical contamination incident.”

It all started when, Friday morning, Waterbury Fire Department and EMS were called to American Electroplating at Thomaston Avenue for a person “in distress with a health issue.”

The patient went into cardiac arrest and was transported to Waterbury Hospital, Schain said.  At the hospital, a plastic bag was found in his pocket that had “sodium cyanide” written on it, and was a salt in powder form. Powder was found on the patient’s clothing.


The man died, according to DEEP Emergency Response Coordinator Ken LeClerc, but he could not say if it was due to the chemical exposure or something else.

How and why the man was exposed to the chemical in the first place is being investigated by Waterbury Police.

The fire department shut down the Emergency Room and isolated the heating and ventilation system so as not to spread the chemical. American Electroplating also hired an environmental contractor to do the decontamination of the ER and ambulance that transported the victim.

Normal operations have since resumed, and no other victims showed signs of exposure to the sodium cyanide.

DEEP will not test the powder “given the circumstances,” but law enforcement may further investigate.

    <dc:creator>dchas</dc:creator><description><![CDATA[MARYVILLE, Tenn. — Police, firefighters and other first responders sign up for inherently dangerous work, but that doesn't give citizens or businesses a blank check to harm them without financial consequences, a federal judge has ruled in the case of a Maryville train derailment.
Chief U.S. District Judge Tom Varlan has turned aside a bid by CSX Transportation Inc. and Union Tank Car Co. to dismiss a class-action lawsuit filed by emergency responders in a July 2015 freight train derailment in Maryville that sent poisonous smoke into the air and more than 100 people to the hospital.
A broken axle on a single rail car hauling 24,000 gallons of a toxic chemical derailed the 57-car train, causing a fire that burned for 19 hours, authorities said.
About 5,000 people in a 2-mile radius in Blount County were forced to evacuate their homes. At least 87 people had to be treated, with 36 admitted to the hospital, and 10 first-responders also required treatment for the effects of exposure to the noxious smoke. A fish kill was later reported, and area wells tested.
The rail car was carrying a chemical, acrylonitrile, used in the manufacture of plastics. The substance is considered carcinogenic, and exposure can burn the skin, inflame the lining of the lungs, throat and nose, and cause headaches, nausea and dizziness. Cyanide is a byproduct of burning acrylonitrile.
Union Tank Car Co. manufactured the rail car at issue. CSX is accused, among other things, of dragging the rail car nearly 10 miles after the axle broke, which, in turn, caused it to rupture and the derailment to occur. Both companies face class-action lawsuits in U.S. District Court from emergency responders in one action, and property owners in another.
The firms wanted the emergency responders' lawsuit tossed out in its entirety, arguing Tennessee has what's known as the Policemen and Firemen's Rule. Under the rule, police and emergency workers are barred from suing citizens and business owners for injuries those responders suffer on the job.

BSO Fire Rescue spokesman Mike Jachles said a port employee noticed a sodium cyanide tank was leaking.

The report prompted the evacuation of some 65 workers about 7 p.m. Sodium cyanide is poisonous and may be fatal if inhaled, swallowed or absorbed through the skin, according to the National Oceanic and Atmospheric Administration. 

Jachles said the crews determined the substance that was in the tank that had just returned from the Dominican Republic was not a hazardous chemical. Authorities suspect the leak may be liquid left over from when the tank was cleaned out after transporting sodium cyanide.

Fontaine kept fighting, though, until it was extinguished. But his chief told him to get his sore knee checked at the hospital.

While in the emergency room at Roger Williams Hospital, Fontaine lost consciousness. He fell into a coma, and was in critical condition.

"I was in the right place at the right time," Fontaine told NBC 10 News at his home in Chepachet on Friday.

He was suffering from the effects of overexposure to cyanide, which is a toxic chemical that can be deadly in large amounts. Fontaine's wife, Debbie, received a call from another firefighter telling her that her husband was not breathing.

"It's the call you never want to get," Debbie Fontaine said.

Fontaine is recovering now, fueled by a strict lung therapy regiment and large doses of prednisone.

"I've been through a lot of stuff, but nothing like this," he said.

Several other firefighters also suffered cyanide poisoning while battling the blaze.

The City of Providence opted not to purchase individual cyanide detectors for each firefighter to wear. Fontaine said firefighters are aware that they are being exposed to cyanide and most departments have crews that measure cyanide levels, however, the Providence Fire Department does not. The effects of overexposure are delayed, like in Fontaine's case, so it's hard to tell if you've breathed in too much until it could be too late.

Despite the threat of cyanide and other inherent dangers, Fontaine said that wasn't going to stop him, or his brothers and sisters on the department, from continuing to fight.

"I've got thirty one and a half years. That's what we do -- protect the community," Fontaine said. "They're going to have to force me off the job."

Fontaine received get well letters from Gov. Gina Raimondo, Rhode Island congressmen, as well as former Providence mayor Joe Paolino.]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[Several firefighters have suffered from cyanide poisoning following the blaze that ripped through a triple-decker on Eaton Street in Providence's Elmhurst neighborhood and spread to two others on Thursday, according to Providence Fire Union president Paul Doughty.

Capt. Joseph Fontaine, who was in critical condition after the fire, is now in stable condition,Doughty said during a Friday morning press conference at Rhode Island Hospital. The first thing Fontaine asked about was his men, according to Doughty.

A total of four firefighters, including Fontaine, have been tested and treated for cyanide poisoning and that number is expected to rise, Doughty said.

Symptoms of cyanide poisoning are similar to carbon monoxide poisoning, but one difference is that carbon monoxide poisoning doesn't pound your body, hydrogen cyanide does, according to Doughty. Hydrogen cyanide is a chemical agent that was used as a weapon by the Germans in World War II. But hydrogen cyanide poisoning usually occurs in patients with smoke inhalation from residential or industrial fires. Once they are treated with the antidote, they are fine, according to Doughty.

At last count, 21 firefighters were injured after the massive fire erupted engulfing the multifamily home. That's six percent of the department, which currently has 360 sworn members. They suffered burns, neck, back, knee and skeletal injuries during the fire, Doughty told NBC 10. T

The aggressive approach to the fire was necessary, according to Doughty who commended Capt. Kevin Jutras of Tower Ladder 2 for his management of the fire. One mistake while fighting that fire with the wind gusts the way they were could have resulted in the entire block burning down, according to Doughty.]]></description>

The fire broke out inside a recording studio at the College of Communication at 640 Commonwealth Ave. shortly before 9 a.m.

Nearly 100 firefighters converged on the building to fight the fire that broke out on the third floor.

"Someone said, 'It’s a real fire,’ so we all ran out," one student said.

The building was evacuated. Firefighters said high gas levels were detected in the building. 

One firefighter, a Boston University police officer and three students were taken to the hospital to be treated for smoke inhalation. None of the injuries is life-threatening.

“When we saw them, they had soot on their iPads and faces,” another student said. "Hopefully, they’re OK."

BU spokesman Colin Riley said all three students were released from the hospital after a few hours of observation. 

The studio’s sound insulation in the walls produced concerning levels of hydrogen cyanide. Firefighters had to vent the building, and crews rotated in and out to limit exposure.

“The fire itself was knocked down fairly quickly,” Boston Deputy Fire Chief Michael Doherty said. “There was a lack of windows in the building.”

MBTA service on the B branch of the Green Line was suspended from Packards Corner to Kenmore Square for a brief time.

Officials estimate the damage to be at $500,000. BU plans to reopen the College of Communication on Monday, after maintenance teams spend the weekend cleaning up the fire damage. ]]></description>

On 12 August, a warehouse in Tianjin erupted in an explosive blaze that killed dozens of people, and damaged and destroyed property. The warehouse had, among other things, been storing 800 tonnes of ammonium nitrate and 500 tonnes of potassium nitrate, as well as 700 tonnes of sodium cyanide and large quantities of calcium carbide. The death toll from the explosion now stands at 165, including 99 firemen, 11 police and 55 civilians, with eight people still missing.

The clean-up operation is now almost complete with nearly all hazardous chemicals from the blast having been cleared. Nine air monitoring sites and seven of the nine water monitoring sites report no findings of toxic chemicals. Cyanides have been identified at the remaining two water monitoring sites, but the concentrations are below safety benchmarks.]]></description>

The explosions in a warehouse storing dangerous chemicals devastated an industrial park in the northeastern port city of Tianjin late on Aug. 12. More than 700 people were injured and thousands were evacuated because of the risk posed by chemicals stored at the site.

The official Xinhua news agency said on Friday rescue crews were rushing to the site after four new fires broke out. It said one of the "combustion points" was in a logistics site for automobiles near last week's blast.

The other three were within the central blast area, it said without giving any explanation of the cause of the fires.

State authorities have confirmed that more than 700 tonnes of the deadly chemical sodium cyanide were stored at the Tianjin warehouse that blew up.

Several blasts occurred over about a minute, and the resulting fires burned throughout the weekend, leading to eight further explosions on Saturday. Over a hundred people were killed, and hundreds more are injured or missing. 

This, you'd think, would be bad enough – but as it turns out, chemical explosions have repercussions which reverberate beyond the initial massive explosions and raging fires.

On Monday night, it rained in Tianjin for the first time since the explosion. The rain which left behind a mysterious white foam and reports of stinging skin and lips from those who came into contact with it. These pictures show the foam the rain left behind, on a public square and on what appears to be a car window: 

It's unclear what exactly the foam contains, but authorities have announced that the rainfall is safe. The local Bureau of Environmental Protection measured levels of cyanide in the foam and found it normal, while several officials stood in the rain for ten minutes and reported no stinging or itching. Bao Jingling, chief engineer at the bureau, told China Daily that local hospitals had reported no cases of poisoning thus far.

Although authorities have also insisted that chemical releases into the environment were limited, local residents claimed falling rain caused skin burns and white foam formed on roads a few days later near the blast site. Some residents have also held demonstrations demanding compensation for property damage and more information on relatives unaccounted for in the disaster.
Despite the confusion and displacement of people living and working near the site of the disaster, authorities say the sprawling port resumed operations on Aug. 18. Chemical producers in the area say they are working normally.
Officials still haven’t indicated what they suspect as the cause of the explosions. However, they revealed that among the hundreds of tons of about 40 different chemicals stored at the Ruihai International Logistics hazardous chemicals warehouse at the center of the explosions were large quantities of ammonium nitrate. Commonly used as a fertilizer, ammonium nitrate is also explosive and the cause of numerous industrial accidents such as the 2013 explosion at West Fertilizer Co. in Texas that killed 15 people.
Officials’ main concern, however, has been disposing of hundreds of tons of sodium cyanide, which releases flammable and toxic hydrogen cyanide gas on contact with water or acids, that they said was stored at the warehouse. The chemical is used in the mining industry to extract gold and other precious metals.

The U.S. Environmental Protection Agency and the U.S. Department of Justice on Thursday announced the agreement involving the biotechnology company’s Soda Springs facilities.

Federal officials say the chemicals released are hazardous and can pose serious health risks. Monsanto in a statement noted there were no allegations that the releases exceeded state of federal standards, or that they contributed to any known health concerns in the Soda Springs area.

Federal officials said the releases occurred between 2006 and 2009, with the plant emitting hydrogen cyanide, sulfur dioxide, nitrogen oxides and mercury. Companies are required by law to report such releases immediately.

“Each of these chemicals are hazardous and can pose serious health risks to workers and the community if mishandled or released in an uncontrolled manner,” federal officials said in a statement.

Police were at the scene directing traffic.

A man admitted himself to hospital after he dropped a vial of potassium cyanide, a highly toxic and extremely dangerous substance.

A "small quantity of potassium cyanide" was dropped in the dental clinic, Bay of Plenty area Fire Service manager Murray Binning said.

Firefighters were cleaning up the spill and a unit was ready to decontaminate them and their gear.

Pittsburgh police and the Allegheny County medical examiner’s office continue to work to determine whether the death of Nicole Kotchey, 35, of Ross, is a homicide, suicide or accident. Some familiar with the case have said homicide is not a leading theory, nor has it been eliminated.

Ms. Kotchey’s co-workers found her unconscious near the desk at about 8:30 a.m. Nov. 12 in her Craft Avenue office. She died at 12:24 p.m. in Magee’s intensive care unit.

Medical examiner Karl Williams said workers in his office conducted a preliminary test there that looks for the presence of cyanide. When that test came back positive, they sent samples to another laboratory that could determine the level of cyanide, he said. Those results came back Friday morning.

“The cyanide level is high enough to explain her death,” said Dr. Willliams, who declined to release the cyanide level. “The difficult thing now is, ‘How did she get it?’ and that’s why we leave the manner as pending further investigation.”

Cyanide compounds sometimes are used in genetic research to block certain cellular processes, but it was not clear whether it was used in Ms. Kotchey’s studies. The U.S. Occupational Safety and Health Administration and the National Institute of Occupational Health and Safety have strict guidelines regulating its usage to prevent human exposure to the acidic salts, which are highly toxic.

Ms. Kotchey’s death came five days after former University of Pittsburgh researcher Robert Ferrante was convicted of first-degree murder in the death of his wife, neurologist Autumn Klein. Dr. Klein died of cyanide poisoning after she collapsed last year in the couple’s home.]]></description>

Yorktown Heights firefighters responded to the Thomas J. Watson Research Center at 1101 Kitchawan Lake around 11:40 a.m. on a reported hazardous-materials incident.

Police Lt. Kevin Soravilla initially said two workers were exposed to the highly toxic chemical and taken for observation. A spokesman at IBM later said the incident affected only one individual.

"A technician ... reported smelling an unidentified chemical while performing a routine semiconductor process step," said Chris Andrews in a company statement. "The technician was working in full protective gear, following all IBM safety procedures and all engineering safety controls were in place."

Andrews said the technician later returned to work after being released from the hospital.

Tim Ottinger with St Joes says Bryan FD set up special, inflatable tents outside the hospital where they could decontaminate the student before admitting him to the ER. Six emergency workers who treated him were also decontaminated and held for observation.

The entrance to the ER was blocked off and the hospital was put on diversion until 6:40 p.m. That means no other ambulances could be sent there during that time.

Texas A&M officials tell News 3 the University did not issue a code maroon because, in their words, there was no imminent danger. The University says first responders were able to secure the scene and keep students, faculty and staff at a safe distance.

Fire departments from three towns responded to 153 Labrador Pond Road following reports that chemicals being mixed for a spray foam insulation caught fire around 11 a.m.

Paris fire Chief Brad Frost said the chemicals apparently ignited and spread to a small amount of insulation in a basement. The blaze was quickly extinguished after crews arrived on scene.

The people were taken to Stephens Memorial Hospital in Norway.

A chemical-monitoring team from Paris was dispatched an hour later to the site over concerns that hydrogen cyanide, a component of some insulation types, was present. Frost said no traceable amounts of the compound were discovered.]]></description>

The potentially lethal substance belonged to her father and is used to clean jewellery.

Officials had found readings of hydrogen cyanide in about 100 homes in the neighborhood. But officials say they believe the odor is actually coming from another chemical.

Nearly 12 hours later, the Montgomery County Hazardous Materials Response Team are still working to determine what that odor is, NBC10 reported this morning.

About 150 people were initially evacuated to Skippack Elementary School, the station reported. The Red Cross said it helped shelter four families, for a total of 10 people who had no place to go for the night.

Officials said a chemist under contract with Barr Laboratories Inc. was pouring 50 milligrams of powdered potassium cyanide into a 100 milliliter liquid solution when the mixture boiled over about 4:50 p.m.

"The product splashed up on one of the scientists' face," said Chris Kear, Rockland County deputy fire coordinator. "He immediately notified the personnel around him and the scientist had water flushed to his face for about 15 minutes and was decontaminated within the unit."

The contract employee, whose identity was not released, was said to be conscious and alert during the emergency response. He was taken by the Haverstraw Ambulance Corps to Nyack Hospital for further decontamination and evaluation, officials said.

Two other scientists working inside the 1,500-square-foot room also were exposed to the chemical mixture. Those men, ages 29 and 31, were not identified, but were said to be employed directly by Barr Laboratories, which is operated by Teva Pharmaceutical Industries Ltd. All victims were expected to recover, Kear said.

Four volunteers from the Hillcrest Fire Department were decontaminated at the scene as a precautionary measure, Kear said. They were uninjured and did not require hospitalization.

The unknown substance was found after the man, who apparently has a history of mental disturbances, brought a container to the NYPD precinct in the Pelham Parkway section of the Bronx Friday and told officers that there was a dangerous substance inside, sources say.

The container tested positive for traces of cyanide, so officers brought the man in for questioning. During an interview, the man told officers he had ordered the contents online, sources say. He said he also bought another jar containing a ricin-like substance, which was still at his apartment.

The substance found Wednesday will be tested, police say.

According to the NYPD, two people were taken to the hospital from the apartment and were later released. The building has since been declared safe.

Authorities say someone dropped off a 30-gallon barrel containing zinc cyanide, an extremely poisonous chemical, at Black Bear Inn in Lyons on Nov. 7. The barrel was found next to flood debris.

Four days later, police say a similar discovery was made at an Environmental Protection Agency dump site. It was later determined the chemical inside the 20-gallon barrel was copper cyanide, which is also extremely toxic.

A homeowner at 43 Harriss Court dropped chemicals on Saturday while removing them for safe disposal, touching off a hazardous waste scare in the neighborhood.

Fire Chief John Nuttall said the chemicals mixed and formed a small vapor cloud. With the possibility that one of the containers had cyanide, Nuttall ordered the evacuation of five houses at 11:45 a.m. on Saturday.

One person in the home was taken to South Shore Hospital as a precaution. No one else was injured.

Nuttall said the resident was taking the chemicals to a safe collection area.

The exterminators were doing termite work under a home near Colusa Avenue and Thousand Oaks Boulevard on Friday afternoon when they came across the vessels buried in the soil, said Acting Deputy Fire Chief Avery Webb.

“There’s no telling how long they had been down there in the dirt,” said Webb. “They dug up the jars and they were intact. They weren’t leaking.”

One of the vessels, a glass jar, was marked 1927, and had a handwritten label that read “Sodium Cyanide Deadly Poison.” Two metal containers, both empty, also were found along with the jar, the homeowner told Berkeleyside.

The Berkeley Fire Department’s hazardous materials team responded, and conducted air monitoring and thermal imaging tests to ensure that the containers were stable. When it was determined that they weren’t leaking, they were turned over to the homeowner to dispose of. Webb said the homeowner was instructed to call a chemical disposal company for further instructions.]]></description>

A container holding the chemical spilled on the workers' legs, the Australian Nuclear Science and Technology Organisation (ANSTO) said in a statement.

A spokesman for the facility said the staff were not injured, but had been taken to Sutherland Hospital as a precaution.

'They're fine. They've been decontaminated. There's no injuries,' he told AAP.

Around 10 a.m. a man went to Fire Station No. 5 at 391 Oxford st. after finding containers labeled "cyanide" in his family's property. The man said he was cleaning out his recently deceased father's shed when he discovered the bags.

The man placed the containers in his car and took them to the fire station to have them inspected. Firefighters then roped off the area and called HAZMAT to investigate.

Cyanide is known to be highly toxic.

Officials tested the substance and disposed of it.

The accident was described as limited to just a container in that room – not a big spill covering any large area of the campus – and it happened on an afternoon when students are routinely released early from classes.

Bob Orrill, battallion chief for the Flagstaff Fire Department, said the chemicals were potassium ferricyanide powder and hydrochloric acid. When combined, the chemicals can react to create hydrogen cyanide gas.

Four people (three adults and one student) were taken to Flagstaff Medical Center midday Wednesday for precautionary evaluation, according to Flagstaff firefighters, a hospital spokeswoman and Coconino County’s emergency manager.

Students in an adjacent room to the storage room were evaluated at the school as a precautionary measure.

The accident happened as teachers were cleaning up a storage room in preparation for summer break, said Karin Eberhard, a spokeswoman for Flagstaff Unified School District.

“The teachers that were doing this cleanup immediately put the toxic-spill kit on it,” she said, preventing any toxic gas from being formed before calling the fire department.

One person was taken from the scene, in the 4800 block of South St. Louis Avenue, in fair to serious condition to Holy Cross Hospital, the fire department reported.

A Level I HazMat — or hazardous materials — response was called about 12:50 p.m. for the spill at the factory, located in an industrial area of the Archer Heights neighborhood. The scene was secured by 1:35 p.m.

The spilled chemical was sodium hypochloride, the fire department reported. The chemical is widely used for its bleaching, disinfecting and oxidizing properties.

According to unconfirmed fire dispatch reports, an explosion was caused when a couple of chemicals, one of them cyanide, were mixed.]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[A Marin County toxic materials crew was dispatched to Novato on Friday to remove 40 gallons of cyanide and other chemicals a real estate agent removed from a home for sale, a fire official said.

Novato fire Capt. Alex Bowlds said the broker was showing a Novato home that was in a probate sale because its owner had died. The deceased homeowner was apparently an inventor who used chemicals, and the broker found the stash of cyanide, formaldehyde and other toxic compounds in liquid and powdered form, Bowlds said.

The broker removed the chemicals and brought them to her husband's auto body shop in Bel Marin Keys, thinking the business could dispose of them. The shop realized it could not handle the chemicals and called police. Police looked at the chemicals and called the fire department at about 1:30 p.m.

The fire department summoned the county's hazardous materials team in to removed the potential lethal assembly of chemicals, a process that took five hours. Emergency officials notified hospitals that the chemicals were being moved in case there was an accident.

The chemicals were taken to a Novato fire station for storage overnight, and the hazardous materials specialists planned to destroy them Saturday. No leaks or injuries occurred during the incident.]]></description>

The Phoenix Fire Department responded to a fire near Central Avenue and Baseline Road just before 3 a.m. after someone reported seeing smoke coming out of what appeared to be a home that had been turned into some sort of meeting hall.

When firefighters got there, they saw a sign on the door that the empty building was being treated with cyanide because of a rat problem.

That's when the hazmat crew was called.

"It's very dangerous for our guys to go in there without their masks and without air because it's a hazardous situation," said Larry Nunez with the Phoenix Fire Department.

Firefighters put out a small fire, which damaged the roof.

    <dc:creator>dchas</dc:creator><description><![CDATA[A spokesperson for the Woodridge Police Department said the friend of a condo resident in the 2200 block of Wharf Drive asked police to check on the resident’s well being Thursday afternoon – and when police entered the man's home at about 3:30 p.m., he “confirmed/advised” officers of the presence of a dangerous chemical.

Less than one gram of the chemical component was found, the spokesperson said, and investigators have not positively confirmed that the chemical is potassium cyanide, as has been reported in area media.

Police said preliminary investigation does not indicate the man intended to use the chemical in a crime, but criminal charges may be forthcoming pending the outcome of the investigation.

Police have not released the name of the man or the employer from whom he allegedly stole the chemical.

Several homes and restaurants are in the area, but the California Department of Toxic Substance Control did not alert the community because of the stable nature of the chemicals, officials said Friday.

The chemicals contained potentially explosive materials, including acids and cyanide, that if mixed together, could have blown up and created toxic smoke over Yuba City.

"We could have actually had a situation where we had a plume coming from this building that may have gone over the residential neighborhood," said Adam Palmer, a scientist with the Department of Toxic Substances Control, a division of the California Environmental Protection Agency.

Fortunately, there was no explosion, as investigators from the U.S. Environmental Protection Agency found the chemicals during a random inspection and started taking enforcement action against Custom Chrome and Bumper at 335 Garden Highway in Yuba City.

The chemicals were found in the building’s basement during a routine fire inspection, Police Chief Paul Tursi said.
Preliminary reports were that some of the containers contained hydrochloric acid and cyanide solution, said Larry Hajna, spokesman for the New Jersey Department of Environmental Protection.
Employees from the Lippincott-Jacobs Consulting Engineers were evacuated from their ground-floor offices as a precaution, Tursi said.
The Burlington County Health Department was notified of the discovery and sent a hazardous materials team to investigate, county spokesman Ralph Shrom said. There was no evidence of a spill or danger, but Health Department officials were trying to determine why the chemicals were left there, Shrom said.

According to the EPA's incident report, contractors working to clean up the old Baycote building left two different chemicals -- cyanide and sodium hydrosulfite -- too close together, causing them to spontaneously combust.

Sodium hydrosulfite is a highly reactive chemical, the report said, and the combination is probably what caused the fire to ignite, although the official cause of the fire is still under investigation.

The workplace accident happened at the Thomas & Betts company at 100 Longtin St. around 11:30 p.m.

Alarms went off as soon as the hydrogen cyanide was released, and 100 people inside the building went outside without any injuries.

A Hazardous Materials crew was quick to ventilate the building, and make sure no pockets of the gas remained.

Hydrogen cyanide is normally a liquid, but boils at slightly above room temperature. It is commonly used in electroplating.

By 5 a.m. the hazmat team had given the all clear and people were allowed to return inside.]]></description>
