    <dc:creator>dchas</dc:creator><description><![CDATA[Emergency officials who rushed to the scene of a toxic Norfolk Southern Corp. train derailment last month may not have had access to crucial information about which hazardous materials the train was carrying, the Department of Transportation said in a safety advisory Friday. 

Rescue workers and others responding to the 38-car derailment in East Palestine may not have had a mobile app used by first responders to quickly obtain information about the chemicals the train was carrying, the Department’s Pipeline and Hazardous Materials Safety Administration said.

“Emergency responders’ timely access to accurate train-consist information is crucial to understanding hazards present in a derailment and other incidents involving a train transporting hazmat,” according to the advisory, which asked railroads to review emergency responders’ access to the app, known as AskRail. 

The 149-car train that derailed in the Feb. 3 incident had 19 cars carrying flammable gases and liquids, corrosives and other environmentally hazardous substances, according to PHMSA. Among them was vinyl chloride — considered a carcinogen — as well as ethylhexyl acrylate and isobutylene, according to the Environmental Protection Agency. Eleven of those hazardous-materials cars derailed, the NTSB has said.

TJX, a Massachusetts-based retail company with over 300 locations across the California, was sued in October by 38 California Counties — along with the city of Los Angeles and the City of San Diego — for improperly disposing of goods with hazardous materials into normal facility trash bins, which end up in municipal landfills. 
Local facilities effected by the unlawful dumping are located in San Joaquin County, Stanislaus County, Tuolumne County and others.
These materials include batteries, aerosol products, electronic devices, cleaning agents, ignitable liquids and other flammable, reactive, toxic and corrosive materials as noted in court documents from the trial.
This violated numerous state health codes resulting high risk of environmental harm, according to the San Joaquin District Attorney's Office.

smoke emitting from a chemical tote containing corrosive material on the property of a business, according to a spokesperson for the City of Broussard. The initial fire had been extinguished by occupants onsite prior to arrival of the Broussard Fire Department.

Lafayette Fire Department Hazardous Material Team was requested to assist. While on scene, a second tote on the property containing the same material began emitting smoke and caught fire. All fire has been extinguished at this time. Haz-Mat crews are working to stabilize the corrosive materials involved in the incident, the spokesperson stated.

U.S. 90 was shut down in both directions — eastbound at Hwy. 182 and westbound at Ambassador Caffery Parkway — and remained shut down until hazards were no longer present. Estimated time of shut down was three hours.]]></description>

Lodi Fire says their crews responded to the 2300 block of Maggio Circle for a Hazmat situation.

At the scene, crews discovered that a "mild corrosive cleaner" had spilled out of a machine.

About 60-75 gallons of cleaner was spilled.

First responders were able to mitigate the leak and environmental health workers took over. Officials say no product got into Lodi's drainage system.

    <dc:creator>dchas</dc:creator><description><![CDATA[A company decided to stop making a chemical product. One of the input materials was toxic and corrosive phosphorus trichloride (PCl3). The lines to the storage tank and the process area were cleared of PCl3. A few years later, an operator found a leak outdoors. The investigation found that the leak was coming from the “empty” PCl3 line. No one was injured and the spill was contained.

During the clearing and flushing steps to shut down the PCl3 facility, someone missed clearing or flushing that section of line. It took a while, but the material corroded the “empty” pipe and caused a release.

An operator may have mistakenly opened the wrong valve, realized the mistake, closed it, but never told anyone. In that short time, some material may have passed through that section of pipe. It is difficult to predict what or when something may happen, but it could cause a serious problem.

We typically think of catastrophic incidents as occurring quickly, like an explosion. However, many serious incidents have occurred months or even years after a mistake was made.

The former Quality Chemical location still contains hazardous waste buried underground at  209-217 S. 21st St., located between Packaging Corporation of America and a small shopping center.

City of Newark Development Director Mark Mauter told Newark City Council on Monday the city worked with the U.S. Environmental Protection Agency and Ohio EPA to remove the hazardous materials in February, at no cost to the city.

A warehouse on the site contained 479 55-gallon steel drums and 111 plastic totes about the same size as the drums, and five pallets of smaller containers. There was a hazardous waste spill. Hwy 101 is completely closed. There is no estimated time of opening.

Recently retired Laytonville Fire Chief Sue Carberry told us northbound traffic is being directed to Branscomb Road to the coast then north up Hwy 1 to Leggett and southbound traffic is being redirected using the same route.

Details are skimpy at this point as the area is remote and cell phone/as well as radio access is difficult.

The CHP Traffic Incident Information Page reports that the truck was hauling corrosives and is now leaking.

The Governor’s Office Emergency Services Hazardous Materials Spill Report states that about five gallons of a Hydroxide sodium (peroxide phosphoric acid and soduium hydroxide) solution spilled. The report said that a “freight truck overturned at the listed location due to unknown reasons. The incident resulted in a chemical spill to the roadway. The CHP and the local fire department are on-scene. No fires or injuries were reported and no waterways were impacted. The release has been stopped and the owner is en route to assist with the cleanup.”

Olson is a chemistry professor at Texas A&M University-Corpus Christi, where he researches soft matter, nanoscience, and supramolecular chemistry. Glass reagent bottles are sometimes shipped in steel cans like this, surrounded by padding. “I am not sure how long the bottle sat there,” he says. “It was sitting towards the back behind a much larger bottle of another substance, easily out of view. What became clear once we began to clean it up was that the bottle inside the unopened can had somehow ruptured and was leaking,” he says.
The brown material is iron bromide (FeBr2), a relatively benign substance that formed as the acid leaked and reacted with the steel container. And the cabinet was properly vented, Olson says. Nonetheless, Olson says, they carefully moved the whole mess into a fume hood, took it apart, and disposed of everything in appropriate hazardous and chemical waste containers. In this case, the can corroded from the inside out, Olson says, but because other chemicals in the corrosives cabinet react with steel, whoever took delivery should have opened the can and placed just the bottle of reagent in with the others. “Opening the can immediately after delivery to the lab might have shown that the bottle cap had failed,” he adds.

The woman, who had been out walking in the area at the time, noticed the black drum and approached it.

It is understood she got an odour from the drum and called the emergency services believing it to be potentially dangerous.

Units of Clare County Fire and Rescue Service from Ennis station responded along with local Gardaí to what was reported as a ‘hazmat’ or hazardous materials incident.

On arrival, fire crews quickly cordoned off a large section of the green until the drum could be inspected. It was established that it contained Sodium Hypochlorite also known as bleach and considered a ‘dangerous and corrosive’ substance.

Firefighters who responded to the call around 11.30am said a drum of corrosive liquid chlorine was tipped over at a pool services company on Hillsborough Road spilling around 30 litres and reportedly leaving as many as five people with burns.

Paramedics were called just before midday and assessed those exposed, two of whom - a man and woman - were taken to John Hunter Hospital as a precaution, a spokesperson for the Ambulance said.

Meanwhile, firefighters from Charlestown and Cardiff Fire stations donned protective suits and breathing apparatus, and worked for more than an hour to clean up the spill, assess the building and declare the area safe.

    <dc:creator>dchas</dc:creator><description><![CDATA[On Oct. 11, 2008, a tank overfill of oleum (i.e., a solution of sulfur trioxide in sulfuric acid) spread a cloud of toxic and corrosive SO3/H2SO4 mist across three towns in Pennsylvania. Approximately 2,500 people were forced to evacuate or shelter-in-place. Fortunately, no one was seriously injured.

The facility was originally built with one power supply and three plug-in pumps, which prevented more than one pump from being used at a time. To prevent an overfill, the power supply was interlocked to stop the pump when a high-high level was reached in Tank 1501 or Tank 1502. However, in the 1980s, a temporary emergency power supply was added after several power outages in the main system. This emergency system was never added to the piping and instrumentation diagrams (P&IDs) or the operating procedures. Importantly, the high-high level switches (LSHHs) did not control the emergency power supply.

On the day of the incident, an operator began to pump oleum from Tank 610 to Tank 1502. To save time, he plugged another pump into the emergency power supply to transfer oleum from Tank 611 to Tank 1502. This practice had been passed from operator to operator for many years, but it was not documented nor managed within the process safety program. The LSHH was unable to stop the transfer from Tank 611, causing Tank 1502 to overfill and release oleum.]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[A hazmat situation occurred outside of the Green Oak Township Fire Department on Friday afternoon and subsequent clean-up efforts forced a road closure. 

Fire Chief Kevin Gentry says a person walked into the front door of Fire Station 81 and stated that a large drum had just fallen out of a semi-truck at Whitmore Lake Road and Silver Lake Road. He told WHMI Department personnel investigated and discovered a drum of hydrobromic acid that had cracked when it hit the ground and was releasing the acid into the air and onto the ground.

Gentry said hydrobromic acid is an extremely corrosive substance that is dangerous if it comes into skin contact but is also an inhalation hazard – meaning if inhaled it can damage the lungs - so it is a fairly hazardous substance that needed to be dealt with. 

The Fire Department and the Livingston County Hazmat Team secured the drum for removal by an environmental clean-up company. Gentry said investigation revealed that the drum had not been secured properly by the semi that was transporting it, which was located further down Whitmore Lake Road. He said apparently the barrel had not been secured in the back of the truck and bounced against the back door, which somehow came open and caused the drum to fall to the ground. Gentry said that matter is under investigation by the Green Oak Township Police Department.

Miroslaw Sek filed a complaint filed a complaint Nov. 17 in Harris County District Court against Contanda LLC, BWC Terminals LLC, Shrieve Chemical Company LLC and NRG Texas Power LLC doing business as Sprint Transport LLC negligence. 

According to his complaint, Sek was an owner operator of Sprint Transport and on June 8, 2020, was picking up a trailer load for Shrieve Chemical from Contanda and BWC. He claims that when he arrived at Contanda/BWC, he drove to a loading bay where the trailer was loaded by a Contanda/BWC loader. 

Sek further claims that he was told he did not need any "hazmat" documents despite the load having "a liquid solution of caustic soda" or Lye which is a corrosive hazardous material. He alleges that as he attempted to connect the hose from his trailer, the chemical discharged onto him and caused him to suffer chemical burns to his skin, face, eyes, nose, arm and other areas of his body. 

He claims the defendants' negligence for failing to properly label the chemical, secure the valves or have emergency showers at the off-loading site. ]]></description>

Fire officials said there is no chemical leak and no risk to the public right now.
The fire was reported around 9 p.m. Sunday at South Coast Terminals. The La Porte Fire Department responded after there were reports of smoke coming from the plant.
When firefighters arrived at the scene, they were able to quickly extinguish the fire but noticed thermal damage to the a container holding chemicals.
HazMat crews were called to the site to check for leaks and to conduct air monitoring. The fire marshal’s office said the chemical is not flammable but is corrosive.

Miami-Dade Fire Rescue crews responded to a call referencing a tank explosion at a warehouse along Northwest 33rd Street and 79th Avenue, just west of Miami International Airport, at around 11:20 a.m., Thursday.

Responding rescue personnel located two men who sustained burns from being sprayed by 40 gallons of alkali material that was on the ground.

The victims’ skin burned, and rescue crews treated them and rinsed their skin at the scene before transporting them to Jackson Memorial Hospital.

One victim was airlifted to the hospital and the other was transported by ground to JMH.

The two men were separated due to one of the victims having respiratory issues.

Hazmat crews have since responded to the scene to neutralize the toxic materials at the warehouse.

District attorneys from Solano, Contra Costa, Alameda, Marin, Monterey, Napa, San Francisco, San Mateo, Santa Clara and Sonoma counties were part of a lawsuit against Ulta Beauty Inc. that included a total of 34 district attorneys and city attorneys across the state.

According to the complaint filed by the district and city attorneys, Ulta Beauty stores across the state frequently handled flammable, reactive, toxic and corrosive materials like cosmetics, fragrances, nail polish and electronics and allegedly improperly disposed of them in standard trash containers and dumpsters rather than transporting them to a designated, legal hazardous waste facility.

The complaint also alleges that Ulta Beauty stores failed to properly document and store hazardous waste materials or train employees to handle and dispose of them.

“These are serious violations of hazardous materials,” said Scott Alonso with Contra Costa County District Attorneys Office.

As part of the settlement, Ulta will be required to pay $752,000 in fines and implement a compliance program to ensure they properly dispose of hazardous materials in the future.]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[LAFAYETTE, La. (KLFY) A Republic Services curbside hauler in Lafayette suffered burns on his arm when a dangerous and deadly corrosive material leaked onto him from a trash bin that he had been exposed too.

It happened Tuesday, Scott Fire Chief Chad Sonnier said.

He said firefighters were dispatched to a local refuse facility after the employee was reported to have obtained chemical burns from exposure to sulfuric acid which had been improperly placed in a residential trash bin.

The employee was initially transported to a local hospital, Sonnier said.

He was then transferred to the burn center where his condition is listed as serious, but stable.

“Due to the negligence of someone improperly discarding this dangerous material, in the normal trash, an employee received chemicals burns and is being treated at a local hospital,” Sonnier said.

According to Sonnier, the Lafayette Haz-Mat Team, carefully sifted through the remainder of the trash bin, checking its contents, making sure there were no further containers, with hazardous materials present.

Firefighters located a container with an unidentified liquid material in a trash bin, which the employee had been exposed to.

Haz-Mat technicians entered the container in protective suits to test the material and identify what the employee was exposed to quickly so that emergency room personnel could properly treat the burns.

The substance was "a dangerous and deadly corrosive material," which Scott FD says had been improperly disposed of in the trash bin. The Haz-Mat team sifted through the remainder of the bin, checking its contents to make sure there were no other containers with hazardous materials present.

Responders collected the material and the area was deemed safe. The employee received chemical burns and is being treated at an area hospital.

In more than half of the schools, she found highly hazardous chemicals. They included explosive picric acid, toxic mercury, and toxic and corrosive hydrofluoric acid. While mercury was mostly sealed in old thermometers, once Álvarez Chávez and her team fotund an open container about the size of a baby-food jar full of the toxic metal. After that incident, the team adopted a new rule during inspections: don’t touch anything.

The lawsuit claims that a concentrated and highly corrosive mixture of hypochlorite and sulfuric acid was released into a children's pool area at Hurricane Harbor Splashtown, located in the Houston suburb of Spring.

Amanda Regulus of Houston says in the lawsuit that her three daughters, all under the age of 13, immediately felt the presence of toxic chemicals in the air. Shortly thereafter, they began developing headaches, skin irritations, and sore throats. Despite the dangerous conditions, the family was not immediately evacuated by the park. Ms. Regulus says park employees asked her and other guests to join a promotional contest before exiting.

(WCJB) -A flipped semi-truck off of US 301 in Bradford County has kept the road closed off nearly all morning into the early afternoon as fire rescue crews from Gainesville and Bradford County worked to clear the scene of the accident.

“Upon arrival, we did find a tanker rolled over that was containing some hazardous materials,” said Bradford County Fire Division Chief, Ben Carter.

To be specific, the KC Industries truck held ferric sulfate, which is an acid harmful to people, the environment and aquatic life.

“Which is a type of acid, added Carter. “It was corrosive material based on the type of hazmat placard that was on the truck as well as the type of actual tank that it was being carried in and so we identified that pretty quick off.”

Crews from surrounding agencies responded dressed in hazmat suits, as officials say it was a highly dangerous corrosive material.

The incident caused major backups during rush hour.

"Basically all we did was we put some absorbent down to try to absorb it, however it was a little difficult to get off the ground. It seemed like it polymerized a little bit on the ground," said special ops coordinator Jonathan Al-Khal.

Crews spent several hours cleaning up the liquid from the Cedar Crest exit.

The disaster made it clear that a massive clean-up effort was needed, and Lebanese officials approached specialist German companies to begin the massive job.

Höppner Environmental Protection Consultancy, based in northern Germany, partnered with heavy lift transportation company Combi Lift to inspect 59 containers of concern that had been stored at the port for years, and then securely shipped them to Germany for safe disposal.

If these containers had caught fire, Beirut would have been ‘wiped out’, interim port chief Bassem al-Kaisi warned in November. In February, Combi Lift’s managing director Heiko Felderhoff said that these apparently forgotten hazardous chemicals were tantamount to ‘a second Beirut bomb’.

But before the hazardous receptacles could be despatched, the companies had to carefully examine them to make a full inventory of their contents.

In the 59 containers, significant amounts of highly concentrated hydrochloric acid, and other corrosive materials such as sulfuric acid, formic acid, sodium hydroxide and hydrofluoric acid were found, according to Johannes Melcher, a chemist with Höppner whose job was to identify the materials and ensure that they could be handled and shipped safely. Furthermore, he says, acetone and empty cans of methyl bromide were also uncovered.]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[PORTAGE, Wis. (WMTV) - Two people were injured in an accident involving a semi-truck filled with flammable materials overturned on Interstate 90 Friday afternoon.

According to a news release, troopers were called to the site of a possible four vehicle crash on I-90 near MP 116, near Poynette.

The troopers continued saying when they arrived, the found a semi truck had overturned while hauling hazardous, corrosive and flammable materials.

The semi had significant damage as a result of the crash and some of the hazardous material leaked out of the truck, troopers noted.

I-90 westbound was closed at State Highway 60 near Lodi and the Portage Fire HAZMAT team was called to the scene. The HAZMAT team said they rendered the scene mostly safe after an evaluation.

The chemical is a corrosive, acidic, organic propanetricarboxylic acid, said Williamsburg Fire and Rescue Chief Larry Todd.

“We only had three to five gallons on the ground. The rest had leaked out elsewhere,” Todd said.

The chemical leak was discovered Thursday between 6 – 6:30 p.m. at Pilot Travel Center in Williamsburg, which prompted the evacuation of a one-tenth of a mile area from the Exit 11 southbound ramp west along Highway 92W to just passed Walmart.

Personnel are still at the scene, according to the Grant County Sheriff’s Office.

The Grant County Emergency Operations Center says the initial call came in just before 3:30 a.m. of a car fire, and while emergency crews were responding to that scene, a crash involving two semis occurred.

Investigators say a semi-truck carrying vegetables was approaching the scene and was slowing down when it was hit from behind by a second semi-truck carrying a tanker of corrosive material.

Officials say that crash caused the corrosive material to leak. Authorities say the material is not lethal, but will cause irritation if inhaled. They are working to clean the spill, which could take a couple of hours.

    <dc:creator>dchas</dc:creator><description><![CDATA[Mangaluru, Oct 20: After corrosive hydraulic fluid which was being transported in a tanker from Karwar to Kochi leaked from the tanker and fell on the ten km stretch of the highway between Mulky and NITK tollgate, Suratkal in the early hours of Friday October 19, there was apprehension that it might cause problems. However, after the tollgate staff observed the leakage, fire brigade was called. The fire brigade poured water all along the road and averted any untoward incident. However, several vehicle users complained of eye burn.

Suratkal station house officer, K G Ramakrishna, senior officials of the department and officials of Mangalore Chemicals and Fertilizers visited the spot. As leakage from the tanker could not be plugged, it was parked on a ground on the adjacent road leading to Lord Sadashiva temple. Police and fire brigade personal guarded the spot. The police said that there are plans of transferring the fluid to another tanker.

Some people who have experience of using this chemical said it might cause danger to eyes if it is too strong. It is said that some gas fumes were coming out of the chemical which had spread on the highway and fire brigade personnel who sprayed water for about half an hour suffered some side affects.

According to Tim Dickerson, Pike County Emergency Management Agency director, a pump for a cooling tower failed, causing it to spill a lubricating oil on the floor inside a maintenance room.

Sarah Junk, public information officer for the emergency management agency, explained the chemical was classified as an inhalation hazard. A material safety data sheet provided to the Gazette also explained the chemical is toxic if ingested, combustible, and corrosive to the eyes and skin.

Pike County Commissioner Fred Foster, who was at the scene Wednesday morning, said he and Dickerson agreed to evacuate the building as a precaution. While the chemical was contained to the maintenance room, because the doors were open to the rest of the building, it put off a bit of a smell. As soon as officials discovered the smell, Foster said they sent everyone home for the day.

North Island Mussels Limited was handed a $219,375 fine in the Tauranga District Court on Friday.

The worker received $60,000 in reparation for the January 2017 incident.

The worker was decanting a cleaning product when a piece of tubing flicked him in the eye.

The corrosive product and the impact of the tubing left him with damage so significant that his eye had to be removed. The resulting scarring also meant the victim could not be fitted with a prosthetic eye.

The teacher, who was exposed to the solution, was evaluated and declined further treatment, the spokesman said. Several people in the building were evacuated.

According to authorities, a forklift operator inadvertently pierced a hole in a 500-gallon container of peracetic acid around 11:40 a.m. The hazmat incident occurred at the main building at 55 Hosier Street, right where live-haul trucks are backed into the plant, which sometimes briefly stops traffic.

Mountaire uses peracetic acid as a disinfectant. It’s related to peroxide, a compound of carbon, hydrogen and oxygen, but can be corrosive.

“Mountaire received it in diluted form, [which] basically turns into a very strong vinegar, and that’s what the smell was — of a strong vinegar — on the scene,” said Selbyville Volunteer Fire Company Chief Matt Sliwa. “We’re lucky that they get a diluted version,” he said, because at full strength, peracetic acid is lethal at an exposure of between 1 teaspoon to 1 ounce per 150-pound person, according to the EPA.

"A box of chemicals fell off a cart. We immediately evacuated the building and called the response teams," said Whitney Point Central School District Patricia Follette.

A specialist was visiting the school to organize chemicals in the chemistry lab.

A custodian accidentally spilled nitric acid, a highly-corrosive substance.

The specialist let the school know right away and seven response teams came to the scene.

"A hazardous materials incident is quite involved, so it takes multiple resources so we like to do  Office of Emergency Services which is assisted by both HAZMAT teams, New York State Police and obviously the local fire departments, Whitney Point, Lisle, Chenango and Broome County Emergency Squad was here for EMS coverage," said Broome County Fire Coordinator Jeffrey Buckler.

By the time the teams made it into the building, officials said the chemicals had stopped reacting.

"They were no longer off-gassing, basically mitigated the hazard side of the situation in a very short time frame," said Buckler.

It happened at Complete Packaging Solutions LLC. About 2,500 gallons of a corrosive substance used in paper manufacturing leaked onto the roadway. DEC and hazmat crews were called to the scene.

Officials say the cause of the spill was mechanical failure.

A portion of Hiawatha Boulevard between Sixth North Street and Seventh North Street was closed so crews could clean it up. Several vehicles drove through the spill before it was reported.

"Anything that's not natural is not good to be on the ground," said Syracuse Fire Department District Chief George Cowburn. "So it's not something you want to be walking through, but there's no danger to the public at this point. There's no toxic plume or anything like that to worry about."

Police said the principal reported a chemical explosion Wednesday afternoon on the grounds of the school, which is located in the city’s northwest along 7th Avenue, just south of Sherwood Drive.

Early investigation from police indicates the boy allegedly mixed ingredients together in a closed container, which created an improvised explosive device.

The explosion injured a girl who was nearby. Officers said she was taken to hospital with injuries consistent with a chemical burn.

The boy’s home was searched. Police said they don’t believe this act was directed at any particular individual.

The boy is facing a number of charges, which include using explosives (throwing a corrosive substance), assault with a weapon and mischief under $5,000.

A Fire and Rescue NSW spokesman said a HAZMAT team was tasked to the scene on the northbound off-ramp of the Pacific Highway.

The truck lost control and veered into a 1m ditch earlier this morning.

Fortunately upon investigation the corrosive liquid being carried by the truck were not found to be leaking.

Recovery of the vehicle has now commenced.

Bangalow firefighters are still on the scene asisting the RMS with the clean up operation.

The crash happened around 4:30 a.m. on I-65 southbound near mile marker 43. Traffic was diverted off I-65 at exit 50. Around 11:30 a.m., state police said the northbound lanes were also being closed at exit 41. All houses within half a mile of the crash scene were evacuated, according to Sgt. Stephen Wheeles with Indiana State Police.

A passenger inside Okpeh’s truck was injured in the crash and was taken to an area hospital. His injuries aren’t believed to be life-threatening.

Okpeh’s vehicle was hauling hazardous chemicals, including a non-flammable gas, a corrosive liquid (weed killer), a caustic acid, an oxidizing solid (dantobrom), and a corrosive liquid (glutaradehyde). Some of the containers inside Okpeh’s tank ruptured after the collision and then spilled onto the road. Hazardous materials crews were called to the scene for cleanup.

State police briefly reopened the left lane around 10:30 a.m. and said the right lane was expected to reopen Wednesday afternoon. However, just before 11 a.m., ISP said both southbound lanes were closed again due to “complications from the cleanup.” Crews removing containers from Okpeh’s truck noticed smoke and a “volatile reaction” taking place inside the trailer, leading to the closure of the northbound lanes and the evacuation.

    <dc:creator>dchas</dc:creator><description><![CDATA[SALT LAKE CITY, Utah, Dec. 19, 2017 (Gephardt Daily) — Officials have reopened State Route 201 at Redwood Road following a HazMat situation Tuesday morning.

SR-201 was closed eastbound and westbound after a container containing formic acid fell from a UPS truck just after 11 a.m., Lt. Todd Royce, spokesman for the Utah Highway Patrol, told Gephardt Daily.

Royce said a small delivery truck was headed eastbound on SR-201, and it appears the back latch was not fastened on the truck and the door opened. A package fell out of the back containing five liters of formic acid, which in its concentrated form is corrosive to the skin.

“It’s a very corrosive acid, it’s very, very acidic,” Royce said. “West Valley City HazMat came out to neutralize that acid on the roadway, they tested that to make sure it was all neutral and we were able to open up traffic and traffic is flowing as normal.”

Public concern was raised over early November press reports stating that Los Alamos National Laboratory officials weren’t sure of the extent of a decades-old toxic chemical spill in Mortandad Canyon.

The revelation was reportedly made at a hearing held between state lawmakers and LANL officials about the status of a toxic chemical cleanup operation in Mortandad Canyon. The spill is decades old and involves hexavalent chromium, an anti-corrosive agent that was flushed regularly into the canyon from the cooling towers of a LANL power plant from the early 1950s into the mid 1970s.

The chemical is known to cause cancer in humans.

LANL has been working to contain the spill, which is thousands of feet underground and threatens a regional aquifer, from reaching drinking water wells in Los Alamos County and the San Ildefonso Pueblo.]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[MEACHAM, Ore. - A truck hauling a corrosive chemical crashed and overturned on icy Interstate 84 in Eastern Oregon early Monday, causing the trailer to rupture and prompting an isolation area and lengthy closure of the freeway, Oregon State Police said.

OSP troopers responded to the crash reported around 4 a.m. on westbound I-84 at milepost 23 near Meacham in Umatilla County, said OSP Lt. Mark Duncan.

Investigators said a green 2006 Freightliner truck hauling tetramethylammonium hydroxide (an ammonia solution, which is a corrosive) was traveling westbound on I-84 when the driver, Dona Carlson, 42, of Prescott, Arkansas lost control of the rig on the icy highway.

The vehicle combination went into the median and the trailer rolled onto its side and ruptured, Duncan said.

Carlson and her male passenger, Morrey Carlson, 42, also of Prescott, were not injured. 

OSP and the Oregon Department of Transportation initially set up an isolation distance of 1/2 mile in all directions due to the hazardous materials concern. 

The Umatilla County Fire District No. 1 Hazardous Materials Unit, the Umatilla Tribal Fire Department and the National Response Corporation Environmental Services responded to the scene. The investigation is ongoing, Duncan said.

The action was filed in Santa Barbara County Superior Court in Santa Maria. The complaint alleges the hotel illegally disposed of hazardous waste, specifically corrosive swimming pool chemicals, by placing them in an ordinary trash dumpster.

The environmental violations were discovered when fumes were emitted from the dumpster in which the chemicals were placed.

Hotel management called the Santa Barbara County Fire Department, which in turn contacted the Santa Barbara County Certified Unified Program Agency (CUPA).

CUPA is a unit of Santa Barbara County Environmental Health Services that regulates hazardous-waste compliance.

The call came from the 4700 block of Baxter Road, where a pet hospice is located. Arney said the employees were injured when their skin came into contact with the corrosive chemical.     <dc:creator>dchas</dc:creator><description><![CDATA[RICHMOND — Chevron Corp. has reached a settlement agreement with state regulators stemming from the 2012 fire at its Richmond refinery that will require it to spend about $20 million to improve safety at the facility.

The agreement calls for San Ramon-based Chevron to implement “extraordinary measures” that meet or exceed California’s pending landmark regulation to reduce risk at refineries, the state Division of Occupational Safety and Health (Cal/OSHA) said in a news release Monday.

“This means safer operations at the refinery, which will help protect refinery workers and those who work and live nearby,” Cal/OSHA Chief Juliann Sum said.

An investigation by the U.S. Chemical Safety Board found that the Aug. 6, 2012, fire, which endangered 19 refinery workers and sent more than 15,000 residents to hospitals, was caused by a combination of regulatory shortcomings, a flawed Chevron safety culture and insufficient emergency efforts that likely exacerbated the problem. It concluded that Chevron failed to replace aging and corroded pipes, and failed to shut down leaking equipment before it caught fire

The agreement announced Monday calls for Chevron to replace all carbon steel piping that carries corrosive liquids with chrome-alloy piping, which has better corrosion resistance, at an estimated cost of $15 million. It also calls for Chevron to spend an estimated $5 million to develop and implement criteria and procedures to monitor equipment to alert operators when equipment should be replaced.

In addition, Chevron agreed to provide specialized training for Chevron Fire Department workers and refinery operators, beyond the training that is already provided; donate $200,000 to a job-readiness program to help prepare students for jobs in the petrochemical and related industries; and pay the citation penalties originally proposed by Cal/OSHA in January 2013 of $782,700, plus an additional $227,300.

A bladder-like container of ammonium bisulfite was punctured and began leaking from a truck trailer shortly before noon Tuesday, according to the state Department of Transportation.

Sodium bisulfite is a corrosive chemical that is used in agriculture and photographic processing.

State Department of Ecology officials were on the scene, and a private contractor was cleaning up the foul-smelling material, said Ecology spokeswoman Sandy Howard.

The plan was for crews to pressure wash the pavement and then vacuum up the material, Howard said.

Delays are expected on southbound Route 202 between the Flemington Circle and Hart Boulevard as the right and center lanes remained closed as of 9 a.m., the state Department of Transportation reported.

The fire happened around 5 p.m. Saturday. The tractor-trailer stopped between the Spring Garden and Rich Haven roads intersection on southbound Route 202.

Southbound Route 202 remained closed for much of the evening, with northbound Route 202 reopening just after 7:30 p.m.

This was the third incident involving a truck hauling hazardous materials in Central Jersey in the past two weeks. Bridgewater residents were asked to shelter in place when a truck hauling chemicals overturned, and the Somerset County Hazardous Materials Response Team were called out to an accident in Hillsborough involving a truck hauling fertilizer.

    <dc:creator>dchas</dc:creator><description><![CDATA[DARDANELLE — Russellville Fire Department Hazmat 1 responded to State Highway 27 and Liberty Road in Yell County at approximately 4:30 p.m. Thursday after a two-vehicle motor vehicle crash with possible chemical spill that caused the highway to be closed from Dardanelle to Liberty Road for a few hours.

Upon arrival, the fire department’s Hazmat team found an accident that involved a pickup truck and trailer hauling approximately 1,700 gallons of a hazardous chemical. Crews determined the chemical to be UN3264 Corrosive Liquid (inorganic chromium). 

A two-person entry team donned Level B suits entered the hot zone and confirmed an active leak. The leak was stopped and the remaining spill was contained. Approximately 800 gallons was spilled. A Hazmat cleanup company was responding as of press time. 

The Russellville Fire Department worked the mutual-aid incident in conjunction with Yell County Office of Emergency Management (OEM), members of Dardanelle Rural Fire Department, Arkansas State Police (ASP), Arkansas Highway Police and other Dardanelle and Yell County law enforcement agencies.

    <dc:creator>dchas</dc:creator><description><![CDATA[A UPS driver was sickened Wednesday by a corrosive material leaking from a 5-gallon barrel he was moving in a College of the Canyons parking lot in Santa Clarita.

The worker became ill around noon in the 26000 block Rockwell Canyon Road, according to Los Angeles County Fire Department Inspector Richard Licon.

“(The UPS driver) was moving around the barrel filled with a corrosive when he started having an irritation to his eyes and then suffered nausea,” Licon said. “We responded with 24 firefighters, paramedics and we were joined by L.A. County Health and Hazmat.”

The area was evacuated and the driver taken to a hospital for treatment, Licon said.

The hazardous materials team remained at the scene to help UPS take care of the barrel of corrosives, Licon said.

    <dc:creator>dchas</dc:creator><description><![CDATA[HARTFORD — A bill that would have required state road crews to use natural alternatives to the magnesium chloride, the effective ice melt that has resulted in complaints that it corrodes truck and car bodies, died Friday when the legislative Transportation Committee failed to act on the measure.
Rep. Antonio Guerrera, D-Rocky Hill, chairman of the committee, said he declined to bring it up for a committee vote because magnesium chloride has been effective in prepping state highways before snow storms.
“The bottom line is safety,” Guerrera said in a brief interview after the brief committee meeting. “This stuff works. We’ve seen it in the last two or three snow storms we’ve had in the state of Connecticut, with the temperatures dropping and so forth. When the temperature drops, it works.”
While motorists and the trucking industry have complained about the corrosive effects of the chemical, experts have said the effects are negligible, while the melting capabilities are obvious. “You may have to wash your car a little more often,” Guerrera said. “Once a snow storm hits, wash it twice that week, or whatever it may be. But that will get rid of that stuff from the bottom of your car.”
According to a staff assessment of the bill, the DOT spends about $780,000 a year for the chemical. The bill, which unanimously passed the Environment Committee before being referred to the transportation panel, would have required the DOT, in consultation with the Department of Energy and Environmental Protection, to phase out the use of the chemical by July 1, 2022.

    <dc:creator>dchas</dc:creator><description><![CDATA[TOWN OF ULSTER >> The Hampton Inn hotel on Ulster Avenue was evacuated, 12 workers were exposed to caustic fumes and a contractor who inhaled them was hospitalized Tuesday afternoon after accidently mixing a cleaning chemical with pool water containing chlorine, the local fire chief said.

Ulster Hose Chief Shawn Heppner said firefighters were called to the hotel at 1307 Ulster Ave. at 1:23 p.m. for an automatic fire alarm activation after a contractor working near the hotel’s pool accidently mixed a “corrosive cleaner” with pool water. The contractor initially thought the resulting fumes were caused by a fire, Heppner said.

Twelve contractors, including the one that was hospitalized, were evaluated by Mobile Life Support Services. Eleven of the workers refused treatment, Heppner said.

The contractor, who was not identified, suffered injuries that were not life-threatening injuries, the chief said.

    <dc:creator>dchas</dc:creator><description><![CDATA[A multiple vehicle crash on Highway 401 near Mallorytown, Ontario, has shut down all travel lanes. A hazardous material spill prompted law enforcement to issue a mass casualty event response due to the number of people exposed.

A chain reaction crash involving at least a dozen tractor-trailers and several ancillary collisions occurred near the township of Leeds and the Thousand Islands on Tuesday. At least one of the commercial vehicles involved in the primary crash was transporting a hazardous material that turns into hydrofluoric acid when exposed to heat, according to a Facebook post by the Gananoque Police Service. Hydrofluoric acid is a highly toxic, highly corrosive and poisonous solution harmful to skin, lungs and eyes.

The Gananoque Police Service is not the agency responding to the crash, but is sharing updates about the crash via its social media accounts.

The transport is believed to have been carrying between 7,000 and 10,000 liters (between 1,800 and 2,600 gallons) of the chemical at the time of the crash, according to a report from KingstonRegion.com. The report states that several people, including firefighters and others in the area, were covered in the hazardous liquid. A decontamination station was set up in the eastbound lanes of the 401 for those who were exposed, per the report.

The police service post notes that emergency responders are requesting anyone in the area to remain in their vehicles and await further instructions and evacuation by first responders.

The township of Leeds issued a news release at 3 p.m. Tuesday stating that emergency responders were on-site and coordinating the medical response.]]></description>

The boy was 15 years old in February 2015 when a friend of his sprinkled some sodium hydroxide into his hair as a joke.

The compound can be found in products used to clear drains.

While the boy did not immediately feel anything, he later felt a burning sensation and decided to ease the pain with ice and snowflakes.

That exacerbated the situation because sodium hydroxide becomes corrosive when mixed with water.

The boy was treated in hospital every day for about six weeks and had to undergo plastic surgery. The judge's ruling said the chemical product reached his brain.

The teenager who put the powder on his friend's head testified he did not know his gesture was dangerous. He said he saw their teacher touch the substance and tell the class it felt funny.

The victim's parents sued the school where the incident occurred and the Commission scolaire de Laval school board.

Quebec court Judge Richard Landry ruled the safety guidelines concerning the use of sodium hydroxide were not as clear cut as suggested by the teacher.
