The sum includes a nearly $5 million civil penalty and another $5 million to fund local environmental projects.

“With this agreement, the Department of Environmental Protection is taking steps to hold Shell accountable and protect Pennsylvanians’ constitutional right to clean air and water while encouraging innovation and economic development in the Commonwealth,” said acting Secretary Rich Negrín.

The “ethane cracker,” as the plant is called, is a 384-acre-wide industrial complex that heats ethane — a byproduct of fracking in the region — and “cracks” it under high pressure into ethylene to produce polyethylene pellets, a building block for plastic.

It officially switched on last November, with an interim permit that allows it to emit a yearly total of 516 tons of “volatile organic compounds” — chemicals such as benzene, toluene and naphthalene, which are linked to a range of adverse health effects, from respiratory irritation to nerve damage.

But state records show that before operations even began, Shell had exceeded its 12-month VOC emissions ceiling due in part to “flaring” events — when excess gasses are burned off instead of released into the atmosphere.

“It’s not normal to look up in the sky and see flames,” said local resident Hilary Flint, 31. “The sky has been a completely different color since they’ve become operational.”

The enforcement action comes three weeks after NBC News and the Global Reporting Centre first started asking Shell and DEP questions about the plant, and not long after environmental advocacy groups sued Shell over its excess emissions. The agency’s consent order details a range of violations. In addition to repeatedly breaching emissions limits for VOCs, nitrogen oxide, carbon monoxide and “hazardous air pollutants” — otherwise known as air toxics — DEP cited a slew of malfunctions at the plant dating back to June 2022.

    <dc:creator>dchas</dc:creator><description><![CDATA[AURORA, Colo. (KDVR) — A carbon monoxide leak was reported and multiple people were sick at the Gaylord Rockies Resort and Convention Center on Tuesday afternoon, fire officials said.

Emergency responders were triaging and treating patients who were suffering illness from the leak, Aurora Fire Rescue said. “Several people who were feeling sick” reported the issue just after 5 p.m. A total of seven people were assessed, with one transported to a hospital.

Fire officials later said the seven people were employees and “cooking equipment” caused the issue.

“The equipment has been shut down and is no longer causing a carbon monoxide problem,” Aurora Fire Rescue said Tuesday afternoon.

None of the patients were suffering from life-threatening illnesses.

By about 6 p.m., Aurora Fire Rescue firefighters were taking final air quality readings in the building. No further updates were expected on the incident, the department said.

An individual from the lab called Ottawa Fires Services (OFS) around 7:53 p.m. on Friday, Dec. 16, reporting the five people in the lab could smell propane and were experiencing symptoms of "watering eyes." The building was evacuated and the OFS Hazadous Materials (HazMat) team dispatched.

The HazMat team discovered elevated levels of carbon monoxide in the building, peaking at 30 part per million. An acceptable level of carbon monoxide is considered to be less than 25 parts per million as an average over an eight hour period, according to the Ontario regulation for occupational exposure limits.

A fire crew inspecting the roof found a faulty heating system to be the cause. The system was shut down and tagged for inspection.

The Chicago Fire Department responded to a level one hazmat situation around noon at the New Philadelphia Baptist Church, located at 5452 S. State St. 

People were gathered for the afternoon service when they noticed an unusual smell. 

Fire officials said 12 people were taken to local area hospitals in good condition.

The pastor is just thankful firefighters got there in time. 

"A lot of people's eyes were watering and some were getting dizzy, so we had to call the fire department," said Rev. Dennis Dow. "We couldn't tell what it was."

    <dc:creator>dchas</dc:creator><description><![CDATA[WRENTHAM — The state Hazardous Materials Response Team was called to assist local firefighters after a chemical reaction filled a downtown art studio with carbon monoxide Wednesday morning.

Firefighters responded to the third-floor studio at 54 South St. just before 10 a.m. after the the studio tenant went across the street to the fire station and complained about not feeling well, Fire Chief Antonio Marino said.

The woman was checked out by EMTs but declined medical treatment, according to the chief.

Carbon monoxide is a colorless, odorless gas that can be fatal.

When firefighters went to the studio, they detected high meter readings for carbon monoxide inside and called for the hazmat team because of the presence of chemicals, Marino said.

It was designated a Tier 1 hazmat response, the lowest state classification, he said.

The high carbon monoxide readings were confined to the studio. The second floor of the building is vacant and a restaurant on the first floor had no readings and was able to stay open.

About five or six hazmat team members dressed in protective suits went inside the studio to investigate.

The chemicals were an epoxy and an acid that the woman regularly used previously without incident to make etchings and other art works, Marino said.

“For some reason the amount of gas that was emitted was other than normal,” he said.

The chief said the woman used a different epoxy and heat to help the curing process.

State and local fire officials and local health and building officials are continuing to investigate the storage of chemicals in the studio, according to Marino.

Selangor Fire and Rescue Department director Norazam Khamis said in a statement that firefighters from the Pandan Fire and Rescue Station and Hazardous Materials Unit (Hazmat) teams from the Shah Alam and Tun Razak Fire Stations were deployed to the scene following a distress call at 9.55 am.

He said the KKM officer entered the restaurant to help the two female restaurant workers who had fainted.

The victims were brought to the Ampang Hospital for further treatment, he said, adding that the restaurant is closed pending an investigation.

Meanwhile, Ampang Jaya police chief ACP Mohamad Farouk Eshak said the fainting spell was caused by the air ventilation fan not being activated while the restaurant workers were frying chicken, resulting in smoke and fumes from a gas leak not being expelled through the exhaust fan of the kitchen.

“The content of carbon monoxide that exceeded the content of oxygen caused the three victims to inhale smoke and gas until they passed out,“ he said in a statement today.

He said all the victims who received treatment at the Ampang Hospital were in stable condition and the case was classified as ‘Refer to Other Agencies’, namely to the Hazardous Materials Special Team (Hazmat). - Bernama

Rather than producing the expected blue flames, the stove burned orange. User manuals warn that orange flames from a gas stove mean incomplete combustion, which can cause a dangerous buildup of carbon monoxide. So the flame was promptly extinguished, a window opened, and all-knowing Google consulted for advice.
“Check if the burner needs cleaning,” Google suggested. It did not. “Check if anything is clogging the fuel supply line.” There was not. “Turn off the humidifier.” Wait, what?
An ultrasonic humidifier had recently been installed after a string of dry winter days had this Newscriptster feeling at risk of collapsing into a pile of dust and fingernails. The new gadget, filled with tap water, had been running for about 8 h by the time the pot of rice hit the stove. This seemed significant. But internet commenters were split on how exactly the humidifier was affecting the stove’s flame: Was increased atmospheric water responsible for the color shift, or was the orange color caused by stray minerals forced into the air by the device?
“When you see orange as a chemist in something hot, your mind immediately goes to sodium,” says Mark Jones, a Dow Chemical retiree who offered to help tease out the mystery. Indeed, sodium is present in tap water and produces a distinctive tangerine shade when burned. Calcium, another tap-water mineral, also produces an orange-hued flame. But Jones wasn’t convinced that the concentration of minerals in tap water is high enough to cause this effect. Instead, he suggested that water droplets in the atmosphere could be cooling the flame as they evaporate. And water vapor could displace oxygen, thereby reducing combustion and producing an orange flame.
Definitive research on this subject is tragically lacking. After all, as Jones notes, “Who would pay you to do this?” But a simple test would confirm whether the minerals were a factor. If water droplets alone were responsible, then replacing the tap water in the humidifier with distilled water would result in the same orange flames. The kitchen was vented and re-humidified with distilled water. The flames returned to their usual blue hue.
“I am certainly surprised,” Jones says, citing a 1995 fire extinguishment study and experiments with his own tap water. He agrees that the results exonerated atmospheric water droplets, leaving sodium and calcium as the likely culprits. He suggested a final experiment: adding a pinch of sodium chloride to the distilled water in the humidifier’s reservoir (something Jones warns against doing with other salts). The effect was striking: within moments, the burner’s flames turned bright orange.]]></description>

Meanwhile, 30 people were treated in the parking lot, Sanchez said. Five employees were taken to Jackson Memorial Hospital and three were taken to Mercy Hospital with symptoms of CO poisoning.

The victims are 76-year-old Anne Tyler and her friend, 72-year-old Louis Sanders Sr., of Gettysburg.

Officials said a vehicle was left running in the garage of Tyler's home in the 100 block of Woods Road in Hamilton Township, which sent carbon monoxide fumes throughout the house.

The victims were found Sunday in a bedroom and may have been sleeping when they were overcome, officials said.

The coroner said the victims may have died 14 to 16 hours before their bodies were discovered.

The deaths are being called an accident.

Firefighters said there was no carbon monoxide detector inside the house.

Around 5:30 p.m., firefighters responded to an eight story building at 59 W. Grand for the Level 1 Hazmat due to high carbon monoxide levels. The building is a mixed occupancy building.

According to the Chicago Police Department, three restaurant employees were transported in good condition to Northwestern Memorial Hospital in by the Chicago Fire Department. Two of those people were in serious to critical condition and one was in fair to serious condition.

“Battalion 3, at the moment there is Frontier Airlines with 253 souls on board, and many patients are feeling nausea and vomiting,” a member of the El Paso Fire Department can be heard in a voice recording. “You may also want to make this a dangerous goods without knowing what is causing the disease.”

I hear another firefighter saying, “This could be carbon monoxide. I’m uploading a monitor.”

Several passengers were treated for possible carbon monoxide poisoning. Some on board said they were initially late in Las Vegas as the maintenance crew worked on repairing the fuel line.

Just after 11 p.m. Thursday, Aurora Fire Rescue responded to a medical call at an apartment complex along the 1600 block of N. Hanover Street. While paramedics assessed people, the fire department checked for carbon monoxide levels and found that they were elevated. The people who appeared sickly had symptoms of breathing in carbon monoxide.

Three ambulances, a battalion and a hazmat vehicle were called to the scene after carbon monoxide was detected.

Fire crews evacuated the rest of the building. They then found readings over 1900 parts per million. According to the Centers for Disease Control and Prevention, one-hour exposure to 1,000 to 1,200 ppm would "cause unpleasant but no dangerous symptoms, but that 1,500 to 2,000 ppm might be a dangerous concentration after 1 hour."

The Frontier Airlines flight, containing over 200 passengers, landed about 7:30 p.m. and went to gate B-10. An ABC-7 crew at the scene observed several passengers taken off the plane by stretchers and loaded into ambulances; in all, six people were being treated for possible carbon monoxide poisoning.

The flight originated in Las Vegas and was headed to San Antonio after being delayed 30 minutes for a leaky fuel line to be fixed, according to passengers. Those passengers told ABC-7 that they smelled strong chemical fumes midway through the flight, which prompted the emergency landing in El Paso.

"When they opened the window of the plane, there was just, you could see all the ambulances -- yeah, dozens of ambulances, fire department and everything. Scared us to death. And then they told us, this is a quote, 'if anyone else feels nauseous, sick to your stomach, please come to the front of the plane. Everyone else remain seated,'" passenger Dee Hart told ABC-7.

The Baton Rouge Fire Department said the incident was reported around 10:45 a.m. at the Sterling Place Nursing Home on North Boulevard. The department said at least five people, four residents and an employee, were taken to a hospital for treatment. 

The department said a Hazmat crew determined a boiler vent pipe was leaking carbon monoxide on the fourth floor of the facility. The entire floor was evacuated, and the boiler was shut off.

Initially, firefighters believed an employee was inside the building, firefighters said in a news release.

Both smoke and carbon monoxide detectors were going off in the warehouse at 606 Oneida St. on the city’s southwest side when firefighters arrived at 11:24 a.m., Syracuse Deputy Fire Chief John Kane said.

Firefighters found dangerously high carbon monoxide levels in the building.

The department’s Hazardous Materials Response Team entered the building using “totally-encapsulating chemical protective suits,” Kane said.

The carbon monoxide issue was apparently caused by battery chargers in the building, Kane said.

Negri Sembilan Fire and Rescue Department assistant director of operations Ahmad Mukhlis Mokhtar said rescuers rushed to the scene after receiving a call about the incident at about 9.50am yesterday.

“Preliminary investigations by the Hazardous Materials (Hazmat) Unit found that there were no leaks in the health clinic and the oxygen level in the building was normal.

“It was also found that the four victims had fainted inside the ambulance,” he told reporters here.

When crews arrived, they found multiple people who were unresponsive and a generator in the living area of the home. Hazmat teams were called to check for hazardous materials, Loney said.

A hazmat team detected high levels of carbon monoxide in the home, Loney said, and two of the adults found unresponsive were declared dead at the scene.

Two more, in critical condition, were taken to Vanderbilt University Medical Center for treatment.

In addition to refusing the worker’s request, the employer allegedly threatened to terminate the employee. After going to the hospital, the worker was terminated.

Following an investigation on the employee’s behalf, the U.S. Department of Labor’s Occupational Safety and Health Administration filed a lawsuit alleging that the hotel’s operator, All Seasons Hospitality and Investments LLC and its owner, Tanvir Shahmohd fired the employee for engaging in protected activity, in violation of federal law. 

“All Seasons Hospitality and Investments LLC and Tanvir Shahmohd violated the employee’s rights by terminating them for reporting unsafe working conditions and seeking medical care,” said OSHA Regional Administrator Eric S. Harbin in Dallas. “The U.S. Department of Labor provides protections for workers who exercise their right to raise safety concerns without the fear of retaliation.”

Filed on March 25, 2021, the department’s suit asks the court to order the defendants to comply with the Occupational Safety and Health Act’s anti-retaliation provisions, reinstate the employee, expunge the employee’s personnel record and pay the employee back wages, interest, compensatory and punitive damages and other remedies.

Calgary fire crews received a call just before 6 p.m. Tuesday in the 900 block of 48th Avenue where they were met by a worker saying his co-workers were losing consciousness.

Two other workers were found inside the commercial building exhibiting low levels of consciousness and feeling ill.

Once the injured were removed the hazardous materials response team was called in to investigate.

It was determined that a heater burning a recycled alternate fuel had malfunctioned leading to the release of high levels of carbon monoxide, sulfur dioxide and other gases.]]></description>

After elevated carbon monoxide readings, 36 units were evacuated, authorities said.

The Orlando Police Department said the leak was caused from a burner on a stove that was left on high for more than 24 hours.

The victims were transported to an area hospital for treatment.

The family’s neighbor said the family had cooked tamales on a barbecue pit outside their apartment. When they were done cooking, the family brought the pit inside their apartment while the ashes were still smoking.

High carbon monoxide levels were reported on all three floors of Symphony Chicago West, at 5130 W. Jackson Blvd., around 2:40 p.m.

The Fire Department called a Level 2 Hazmat response, and an EMS Plan 1, which calls five ambulances to the scene.

The ambulances were called to the scene as a precaution. As of 3:30 p.m., the nursing home residents were sheltering in place and no one had been taken to a hospital.

The location was discovered after a group of affected young people flagged down a police car, and several others emerged from a bunker asking for medical attention. The people appeared dizzy and incoherent.

Of the 25 people who were affected, five were in critical but not life-threatening condition and were rushed to a hospital. Many of the patients were young, between the ages of 20 and 30, according to Norwegian daily Aftenposten. Portable generators were suspected to have caused the poisoning.

Two victims were reportedly police officers attending to those affected. “Fortunately, they are all improving. We can say that they are out of danger,” chief physician Fridtjof Heyerdahl at Oslo University Hospital told the publication.

Oslo police said about 200 people were attending the party, with neither the participants nor the organizers realizing using a diesel generator in a confined space could prove dangerous, Oslo police wrote on Twitter. Authorities urged other attendees to call for medical help in case they begin to feel nausea, headaches, or dizziness.

Emergency responders were sent to a business complex in the 10800 block of SW 5th Street just after 2 p.m. after receiving reports about someone experiencing cardiac arrest, according to Tualatin Valley Fire & Rescue.

Police officers found one person who was semi-conscious struggling to leave the building and found a second person unconscious inside on the floor.

Firefighters arrived and got the unconscious person out of the building. The person was rushed to a hospital with life-threatening injuries, fire crews said. The other person was also taken to a local hospital. A third person who was in a different part of the building refused treatment at the scene.

A hazmat team found potentially deadly levels of carbon monoxide inside the affected building, which officials described as a warehouse facility. Carbon monoxide is a colorless, odorless and tasteless gas.

The incident was initially called out as a possible drowning, but was later determined to be a hazmat situation. Mesa fire officials say two family members were drinking in a closed garage with a running car at a home near Lindsay Road and University Drive. Both of them were suffering from carbon monoxide poisoning. Crews began "rapid, advanced life support," and quickly transported a 28-year-old man to the hospital in critical condition.

Hazmat crews were called to the Shetland Park Building around 8:30 Wednesday morning after a woman was found unconscious on the fourth-floor. That woman, along with four others, were taken to the hospital with carbon monoxide poisoning. Three of the victims have been released from the hospital.

Danielle Salvi knew something bad had happened when she walked up to 35 Congress St. Wednesday morning.

Salvi, who works nearby, said there were “A couple of ambulances, tons of fire trucks."

Officials say two of the five victims' CO levels were 40 parts per million, which is just at the dangerous mark. They were taken to Mass General's Hyperbaric Center. Thirty-seven other employees detected positive for carbon monoxide but decided not to be treated. 

The leak was so bad, investigators say one woman was found unconscious.

The victims work for Excelitas Technologies, a company that manufactures equipment for the military. ]]></description>

A building engineer at the school noticed a gas smell at approximately 5:30 a.m., hours before the school, Burnside Elementary Scholastic Academy, was set to open, according to Chicago Police. The Chicago Fire Department was called to the scene.

The leak was due to an issue with the boiler or furnace, said Chicago Fire spokesman Larry Merritt. The school was aired out and turned over to Chicago Public Schools.

The Durham Fire Department said calls came in for the odor and respiratory difficulty at Lofts at Southside on S. Roxboro.

Elevated levels of carbon monoxide were found in tests and the call became a hazmat situation.

Durham fire officials said 40 people were evacuated and two were transported as a precaution.

The issue turned out to be a golf cart that was being charged overnight. The batteries produced an odor which led to the 911 calls for breathing problems, Durham fire officials said.]]></description>

Anthony Conner is president of the Omaha Police Officers Association told 6 News an officer became light headed inside his cruiser for about 10 minutes. Another officer came to check the carbon monoxide detector inside the cruiser. That officer also became light headed and then collapsed. An ambulance then arrived at the scene.

Omaha Police Officer Phil Anson says the HAZMAT team with the Omaha Fire Department then checked out the cruiser but did not find anything unusual. The cruiser was then taken to the maintenance facility to be inspected.

Conner says both officers transported to the hospital had high blood pressure and were treated and released.

6 News reports, "There is a joint safety committee between the Omaha Police Officers Association and Police Management. If there is a safety issue, we make sure to get to the bottom of it," said Conner.

That caused a hazardous materials situation in which dangerous fumes filled the home, and the Fire Department rushed to the scene around 7:45 p.m.

CBS 2’s Eric Cox reported a floor stripper with another kind of chemical was in use, and ventilation was poor because it was in a basement.

The original call happened around seven-thirty Saturday morning at the Quality Inn and Suites. Fire crews were called back multiple times along with Northwest Energy around 2-thirty Saturday afternoon to more calls of carbon monoxide poisoning.

Three men called 911 Saturday morning after they experienced severe symptoms of carbon monoxide exposure, including red skin, vomiting, and loss of motor function.

The men were removed from the room, and the fire department evacuated that wing of the hotel.

The fire department says they went into hazmat operating procedures and located the source of the leak, a boiler in the pool room.]]></description>

A possible carbon monoxide poisoning at a home in Airmont left three people hospitalized.

Ramapo police say a mother and her two daughters were found unconscious inside a home on Bolger Lane. 

They say they received a 911 call from the father after he returned to the house just before 11 a.m. and discovered his family. 

Officials say the victims are a 54-year-old woman, a 16-year-old and 14-year-old. They were all rushed to Westchester Medical Center.

Officials say the noxious odor is believed to be carbon monoxide, but they are not sure from where it originated. It also remains unclear if the home had any carbon monoxide detectors. 

There is no word on the condition of the three people found unconscious inside the home. Officials say they did rescue two dogs inside.]]></description>

According to a news release, firefighters responded at about 5 a.m. to a hazardous material call at the facility located at 2455 Entrada del Sol. 

Officials say two of the employees were unable to move on their own before first responders evacuated them from the building.

A total of six workers were affected by the fumes.

According to the release, firefighters determined that the employees' symptoms were a result of two gas-powered pressure washers that were used inside the enclosed building, which resulted in a buildup of carbon monoxide.

The fire department's hazmat crew cleared the air inside the plant and removed the pressure washers.

Firefighters said the women were treated for possible carbon monoxide exposure, but are expected to recover.

Firefighters said they were called to the home on Creekwater Boulevard around 7 a.m. Monday.

EMS crews responded to a medical call at about 4 p.m. at Venus Nail Spa located at 4801 Overton Ridge Blvd near Hulen Mall.

The person had passed out and was taken to the hospital, MedStar said.

EMS crews noticed an elevated reading on their carbon monoxide detectors while treating the patient. Crews immediately started to evacuate the nail salon and a HAZMAT team was called, the Fort Worth fire department said.

Supt. Damon McDonald stated that fire department crews were called after an unidentified chemical odor was noticed. Fire crews then arrived with sensors to determine the source of the smell and whether or not the resulting odor was harmful. Crews used sensors to confirm the presence of carbon monoxide.
Classes were cancelled and parents were promptly contacted to retrieve students who had made it to the school. According to McDonald, this happened efficiently.
McDonald reported that all levels of carbon monoxide had been eliminated as of early Thursday afternoon and that the elementary building will be open Friday. Classes will begin at the regular starting time.
The unofficial source of the odor is said to have come from a cracked heating element exchange with one of the rooftop units, causing a chemical reaction. The carbon monoxide was believed to have come into the building through the cracked heat exchange overnight. There are presently no individual carbon monoxide sensors in the school building.

Police said that at 7:30 p.m. June 28, officers Tristan Bennett and Joe Pante responded to a residence on Wild Dunes Way to check on the welfare of an individual.

Upon their arrival, the officers made contact with the person who had called police. The individual told the officers he was unable to contact his 78-year-old mother at the residence and did not have a key to get inside.

While checking the residence, the officers were unable to make contact with the resident and they heard an alarm coming from inside. The alarm was later determined to be a carbon monoxide detector, according to police.

The officers forced entry to the home and immediately detected a heavy chemical odor. They found the woman in her bedroom and observed she was barely conscious and her breathing was very shallow.

Police said the officers removed the woman from the home and brought her outside, where she was treated by first aid personnel and transported to a hospital for treatment.

Fire personnel from Jackson Township Volunteer Fire Company No. 1 responded to the scene. Representatives of Southard (Howell) Hazmat, Berkeley Hazmat and New Jersey Natural Gas (NJNG) also responded to the home.

The hazardous materials personnel entered the home and removed numerous containers of household cleaning products from a kitchen counter. Police said many of the cleaning products were open or not sealed properly. Fans were brought in to vent the residence.

NJNG personnel determined there was a high level of carbon monoxide present due to a faulty water heater.

Once the scene was cleared, the police officers were able to continue their investigation and ruled out the possibility of the incident being suspicious. It was determined the chemical odors were caused by the open cleaning products and that the resident had been exposed to high levels of carbon monoxide due to the gas leak on the water heater.

    <dc:creator>dchas</dc:creator><description><![CDATA[High levels of carbon monoxide were found at a Starbucks and Golden Bagel in the 2700 block of Fletcher Parkway around 8 a.m. Sunday morning.
Heartland Fire, La Mesa Police Department, San Diego County Hazmat and San Diego Fire-Rescue all responded to the scene after more than one carbon monoxide alarm went off and people called 911. 
The Starbucks, Golden Bagel and other nearby businesses were briefly evacuated, and one Starbucks employee was taken to the hospital. 
"(The employee) who received medical attention is doing well and the store has since reopened," Starbucks said in a statement to NBC 7.
An exhaust fan above the boiler at Golden Bagel was turned off, causing carbon monoxide to accumulate and spread to the nearby businesses, Heartland Fire said.
The gas itself is not poisonous but can cause problems if it displaces oxygen.

    <dc:creator>dchas</dc:creator><description><![CDATA[A man suffered accidental carbon-monoxide poisoning in his Tolleson home after a car was left running in the garage, Phoenix fire officials said.

The man was found unconscious in a home near 91st Avenue and Lower Buckeye Road after a family member called the Fire Department, Phoenix Fire Department Capt. Larry Subervi said.

The family member came home to find the man unresponsive inside the house, Subervi said.

A Tolleson fire crew called for a hazardous-materials team after rescuing the man, Subervi said.

"Phoenix, Glendale, and Goodyear hazmat teams determined that there were high levels of carbon monoxide from a vehicle that had been left running in the garage," Subervi said.
