Halifax Regional Fire and Emergency's hazmat team responded to The Holiday Inn Express on Parkland Drive around 11 a.m.
According to Halifax Regional Fire and Emergency Chief Kevin Corkum, someone cleaning the pool created a mixture of bleach and muriatic acid, causing a release of caustic gas.
Corkum says five people were treated by paramedics on scene.

A decomposing 30-gallon drum chlorinated dry bleach was spilled at Clearon in South Charleston early Thursday sparking the shelter-in-place, according to county officials.

The order was lifted just before 7 a.m., about 30 minutes after it was issued.

Officials said a light haze and smell of chlorine could be near the spill during the morning hours.]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[HOLLAND, MI (WKZO AM/FM) —  The fire services division was dispatched to the Holland Aquatic Center located at 550 Maple Avenue at 11:57 a.m. on Monday, August 2 for a chemical spill in the building.

Upon arrival, responding units found that the building was being evacuated and at least three people were complaining of some minor respiratory irritation.

These individuals were taken to Holland Hospital.

Firefighters conducted an entry to the building to determine that all occupants were evacuated and to further assess the source of the spill.  It was determined that a small amount of sanitizer, identified as sodium hypochlorite, which is a liquid, was being offloaded into a bulk container in the pump room located inside the building.

Authorities believe that a small amount of the product, which is offloaded by pump, became vaporized into the air allowing it to move throughout building.

There was no spill of free liquid in this space.]]></description>

In one, a resident was overcome by fumes and taken to the hospital for treatment after mixing Simple Green, a powerful cleaner and degreaser, and bleach, the department said.

In the second, a resident accidentally combined towels covered in acetone and bleach, resulting in a chemical reaction and fire. Luckily, according to fire officials, the resident got the burning towels outside and was able to put them out. There was only minor damage to a deck.

Fire officials warn that these chemicals alone, even used for their intended purposes, can be dangerous. Mixing them can cause unexpected and unpredictable results, they said.

    <dc:creator>dchas</dc:creator><description><![CDATA[At around 6pm on Friday evening, emergency services in Ennis responded to a call from a member of the public who reported finding a 30-litre drum in a green area in the Lios Árd estate on the outskirts of the town.

The woman, who had been out walking in the area at the time, noticed the black drum and approached it.

It is understood she got an odour from the drum and called the emergency services believing it to be potentially dangerous.

Units of Clare County Fire and Rescue Service from Ennis station responded along with local Gardaí to what was reported as a ‘hazmat’ or hazardous materials incident.

On arrival, fire crews quickly cordoned off a large section of the green until the drum could be inspected. It was established that it contained Sodium Hypochlorite also known as bleach and considered a ‘dangerous and corrosive’ substance.

Crews arrived at the plant, located at 2076 Memorial Park Drive, around 11:26 a.m. and found that a large tank filled with bleach was leaking, according to Hall County Fire Services spokeswoman Christie Grice. 


“The chemical drained from the tank into an internal contained drain inside the business,” she said. “The business was evacuated, four people were evaluated on the scene for minor injuries. The business was ventilated and the chemical leak has been contained.” 

She said no one was taken to the hospital, and emergency service personnel cleared the scene around 12:20 a.m. 

She said a private hazmat response team was enroute to the plant for cleanup.]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[MUSCATINE — The staff at the Muscatine Transfer Station returned to work and reopened for normal hours Tuesday following a small chemical spill Monday afternoon.

According to reports, the Muscatine Fire Department responded at 12:30 p.m. following the incident, which occurred on the Transfer Station’s tipping floor, where some material had been dumped from a refuse hauler’s load. This material then interacted with another chemical that was also on the floor, creating a toxic gas.

All employees were immediately evacuated from the building as they waited for first responders to arrive. The facility was closed to the public for the rest of the day as a safety precaution, although there was no hazard to anyone outside of the initial spill area and no need for an evacuation outside of the building, as it did not present a danger to the surrounding areas.

The Muscatine Fire Department and Hazmat Response Team arrived at the facility within 10 minutes and quickly confirmed that a gas had been created, detecting a “bleach-like” smell. Looking at the incident report, Assistant Chief Mike Hartman said the likely chemicals involved in the spill were calcium hypochlorite and linen oil.

    <dc:creator>dchas</dc:creator><description><![CDATA[CHARLESTON – The wife of a Putnam County man who died in a chemical plant explosion last December has filed a wrongful death lawsuit against his employer and the owners of the facility.

Tina Gillenwater filed her complaint December 8 in Kanawha Circuit Court against Optima Belle LLC, Chemours Company PC LLC, The Chemours Company, Clearon Corp. and 10 unnamed defendants. She filed the complaint individually and as administratrix of her husband’s estate.

The lawsuit was filed exactly one year after the explosion that killed John Mark Gillenwater II, who worked for Optima as a chemical operator at the company’s Belle location. The 723-acre plant that consists of three buildings is owned by Chemours. Clearon contracted with Optima to perform a process to remove water/moisture from chlorinated dry bleach at the plant.

According to the complaint, December 8, 2020, was the first time Clearon had supplied CDB to Optima in any form, and it was the first time Optima and its employees had ever performed the CDB drying process.

"The Gillenwater tragedy is a prime example of why when chemical manufacturers try something for the first time, it's all hands on deck," attorney Scott Segal, one of the attorneys representing the estate, told The West Virginia Record.

According to the FFD, two cleaning products that were mixed together by an employee at The Lantern at Morning Pointe at 1015 Generations Way resulted in the response. Firefighters were dispatched at 9:30 a.m. for a commercial fire alarm activation. 

Franklin Fire Battalion Chief Joe Polenzani said the alarm was activated by a smoke detector in a resident’s room. Firefighters observed a haze in the room when they arrived, but upon investigation, determined it was caused by hazardous gas.  

Polenzani said the hazardous gas was inadvertently created by an employee who mixed chlorine bleach and ammonia together in order to clean up fecal matter. Residents had already been evacuated to a safe area of the building due to the alarm when firefighters arrived.

Police say a man sealed all the windows and doors of his parents home, then filled the sinks and tubs with bleach and ammonia in an attempt to harm himself and his family.

Officers responded to the home in the 400 block of Dickinson Road in Deptford just after 10 p.m. Saturday for a report of an unconscious 21-year-old man.

Investigators say the man's father found him and called for help.

Arriving police and medics became short of breath and had to leave the home.

The man is now in the hospital in critical condition.

His parents, sister, her boyfriend and the first responders all had to be treated in the hospital.

The Environment Agency said: "We received reports of dead and distressed fish in the River Colne on September 2 and responded immediately. Our incident response officer attended the site and found that the water had been polluted with sodium hypochlorite, killing a number of fish and other aquatic life. Our investigation has confirmed the sodium hypochlorite has stopped flowing into the water. We’re now investigating the circumstances that led to the pollution."

Optima Belle LLC said it is still assessing its terminated operations at the facility to minimize the possibility of another explosion and fire in a letter addressed last week to an environmental inspector supervisor from the West Virginia Department of Environmental Protection. In January, the DEP issued the company a notice of violation for “not operat[ing] its facility to minimize the possibility of an explosion which released hazardous waste constituents to the air and soil.”

The Dec. 8 explosion occurred in an industrial dryer unit as Optima Belle was performing a process to remove water from chlorinated dry bleach, according to a brief incident summary released Wednesday by the U.S. Chemical Safety Board. The CSB report noted that the board is working with Optima Belle to sample and test the chlorinated dry bleach powder involved in the incident.

The DEP announced Dec. 10 that air monitoring data collected by its Division of Air Quality indicated no federal national ambient air quality standards were exceeded during or in the hours following the explosion.

The DEP concluded the fire that burned for two hours after the explosion did not create ongoing emissions to the atmosphere. It based its conclusion on data from an ambient air monitoring station in Charleston, 8 air miles from the Optima Belle facility.

But Terry Fletcher, the department’s acting communications coordinator, said Thursday the Division of Air Quality was still investigating the incident.

Humberside Fire & Rescue Service said crews were called on Christmas Eve to Station Lane in Holme Upon Spalding Moor after a mixture of toilet unblocking chemicals and bleach caused a chemical reaction.

"The female occupier suffered breathing problems due to the fumes," it said. "Casualty taken to hospital by ambulance for precautionary check up. Remainder of household unaffected."

It said the crews wore breathing apparatus and used a fan, and the toilet was flushed to clear the chemicals.

CDB-63 is the product name for a chlorinated dry bleach chemical, sodium dichloroisocyanurate dihydrate.

The chemical was not included in the annual Tier II chemical inventory data sheet that the Optima facility submitted for 2019 and has not been updated since, Messina said. The annual federal Tier II reporting period for the previous year is Jan. 1 to March 1 for hazardous chemicals that exceeded minimum reporting thresholds at facilities in the previous calendar year.

But Optima says that the day of the explosion was the first time it processed sodium dichlorocyanurate dihydrate at the Belle facility.

“At the time of the required report, sodium dichloroisocyanurate dihydrate was not used or processed at the facility,” reads a sheet of frequently asked questions about the facility and its use of the chemical leading up to last week’s explosion. “In fact, it would have been inappropriate to include CDB on the Tier II report since, at the time of the report, sodium dichloroisocyanurate dihydrate was not used at the facility.”

According to company officials, the chemical involved in the explosion had not been used prior to that day.

Optima was providing toll manufacturing services on chlorinated dry bleach, company officials say, also known as sodium dichloroisocyanurate dihydrate.

Sodium dichloroisocyanurate dihydrate is regularly used in home and swimming pools for “chlorination” disinfection purposes.

Company officials say this chemical was not added to the Tier ll Emergency and Hazardous Chemical Inventory, since it was not used or processed at the facility prior to the explosion.

Under Tier II, company officials say the facility is required to provide notification of a change of regulated operations within 30 days after a change in the process occurred. This new chemical had only been on-site for less than two weeks prior to Tuesday.

It reveals more about a chemical that emergency responders have reported was present in the fire that followed.

The document, a safety data sheet from South Charleston-based water treatment chemical manufacturer and distributor Clearon Corp. for CDB-63, a chlorinated dry bleach product, details the many hazards associated with the chemical.

The product, listed as sodium dichloroisocyanurate, might intensify fire, causes severe skin burns and eye damage, should be kept away from combustible materials and not be released into the environment, according to the safety data sheet.

“Sodium dichloroisocyanurate is widely used to disinfect water and water systems,” said Harry O. Finklea, professor emeritus at West Virginia University’s C. Eugene Bennett Department of Chemistry. “It is a solid that is convenient to store and to transport. It reacts rapidly with water to form hypochlorous acid and chlorine gas, both of which kill microorganisms."

“At fire temperatures, it decomposes to form gases that are hazardous to breathe: chlorine, hydrogen chloride and nitrogen oxides,” Finklea noted.

The incident occurred late on 8 December, leading to a shelter-in-place order covering a two mile radius, which was lifted the following day.

‘It was a massive explosion – it blew metal debris out onto the interstate and across the river … hundreds of feet away,’ Kent Carper, president of the local county commission, told a local radio programme the morning after the accident. He said the Optima facility makes a chlorine-based product to clean spas. ‘It was a dry bleach type of chemical, and somehow it interacted with methanol,’ Carper explained.

Carper said it appears that pipework leading down to the site’s river barge terminal blew up. An Optima statement suggests a 4500 litre metal dryer become over-pressurised during operation.

It has been since been confirmed that there was no associated chemical leak, and no danger to the local community. The state Occupational Safety and Health Administration is reportedly investigating the underlying cause, and the federal Chemical Safety and Hazard Investigation Board is expected to initiate its own probe shortly.

The explosion occurred at 10:02 p.m. according to the Kanawha County Commission. Belle Mayor David Fletcher said the fire was "pretty much out" just after midnight. Fletcher said emergency crews were monitoring the plant and waiting for the OK from plant inspectors.

Three people were taken to Charleston Area Medical Center and the other person drove themselves to Montgomery General, according to Kanawha Commission President Kent Carper. The extent of their injuries was not known.

Carper said the incident involved chlorinated dry bleach and methanol, the latter of which was "most likely" the origin of the explosion and acted as an accelerant for the flames.

"It's a very bad explosion and working fire and we're ordering a shelter-in-place in that area," Carper said. "It is an evolving situation."

Fire Lt. Ben Pfeiffer said employees at the Aurora resort reported that chemicals had spilled into some of the drains. A loading dock on the north side of the resort was evacuated as crews cleaned up the hazard.

The chemical was sodium hypochlorite, Aurora Fire Rescue said in a tweet a little after 5 p.m. The dock was turned back to the Gaylord staff.

One person who was feeling ill was taken to the hospital with non-life-threatening injuries. Two others were treated at the scene.

A fire involving 3,600 kilos of sodium hydrochorite, a powdered chemical used to bleach paper and pulp, started in the facility at about 2:10 pm, GlobalNews.ca said, citing information from the Ministry of the Environment and the Fight Against Climate Change.

The chemical can ignite and release toxic fumes if it is contacted by water. Company officials told media that rain likely caused the chemical reaction.

“We saw that there was a little water in the warehouse. That product is in a warehouse which is usually very dry,” Marc Paquin, president of Somavrac Group, said in coverage in The Canadian.

In a Facebook post, the Lower Merion Fire Department said the incident was later classified as a HazMat scene when first responders noted the trailer was leaking Sodium Hypochlorite.

The DPS spokesperson told Patch the incident was turned over to PennDOT around 11:15 and later to The Lewis Group, a company dealing with environmental challenges, for cleanup. No injuries were reported.

The Fire Force chief requested the Disaster Management Authority to grant Rs 2 crore for maintenance works. Meanwhile, the local bodies are in a dilemma after it was decided not to use fire engines for sanitizing.

When sodium hypochlorite solution was diluted in a particular proportion and sprayed using the engine as part of sanitizing, the pressure relief system and nozzle of the vehicle was damaged. Many vehicles turned completely useless.

Crews at Waco Fire Station No. 11, where the department’s hazardous materials unit is housed, have also developed a new spray system for the disinfectant using their air tanks and other common firehouse equipment. Their formula for homemade hypochlorous acid paired with the spray system lets them kill viruses, bacteria and fungus on gear, trucks and high-use firehouse areas in 60 seconds without the need to wipe everything down afterward.

“For lack of a better term, we are the science nerds at Station 11, you know the hazmat crew. Because there is a lot of chemistry that goes into working hazmat calls, we knew we could do something during this time,” said Waco fire Lt. Philip Burnett, who works at Station No. 11. “The mixture is basically water, salt and we have to put vinegar in it to lower the pH level to make it neutral, then basically ‘cook’ it by putting electricity to it to make the acid with a kill-rate higher than bleach.”

The report authors found that, from January to March this year, poison control centers received 45,550 calls related to cleaner and disinfectant exposure. That's up 20% compared with calls over the same period in 2019, according to the report, from the Centers for Disease Control and Prevention.

The report cannot prove for certain that the rise is due to people's cleaning efforts to prevent COVID-19, but it does show "a clear temporal association with increased use of these products," the authors said.

Overall, calls for exposures to bleach, non-alcohol disinfectants and hand sanitizers made up the biggest percentage of the increase from 2019 to 2020, the report found.

The operation started early morning after deploying 15 fire tenders. While, 12 tenders were kept for spraying, three were used to refill the empty tenders.
Chief fire officer Jagdish Prasad Phulwari said, “We had deployed fire tenders having capacities of 4,000 litre and 2,000 litre. Around 1% of sodium hypochlorite was mixed to disinfect area. As Ramganj has narrow lanes, bigger fire tenders were used in main markets and smaller ones in other lanes. Fire tenders with 12,000 litre capacity was used for refilling,” he said. Officials claim, the entire Walled City area, including Ramganj, has been disinfected. “On Wednesday, each tender made at least four rounds to disinfect the area. Even though, chemical has been sprayed in entire city area, the operation will continue and four tenders will be deployed on daily basis,” he said.]]></description>

Combining household products such as bleach with other cleaning products like vinegar, ammonia, or rubbing alcohol can put your health at risk.

According to Cal Fire, mixing bleach with vinegar creates chlorine gas and can cause breathing problems and watery eyes.

Along with improper use, ServPro of North Shasta, Trinity & Greater Tehama Counties says people need to recognize the difference between cleaning and disinfecting.

"Clean the surface before you disinfect it. Before you disinfect it, you might want to read the label on the product to see how long it recommends the product dwell or stand before you wipe it clean or wipe it off, " said ServPro Marketing Manager Laura Burgdorf.

On Thursday, the third day of the disinfection drive by her in Pali Hill, 35 party workers with spraying equipment carrying her photograph were seen going about their work. Among the societies disinfected in Pali Hill were that of lyricist Gulzar, actors Sanjay Dutt, Aamir Khan, and Sohail Khan and the bungalow of actor Dilip Kumar.

The civic body had only a few days back come out with an advisory in which it had warned against private individuals carrying out disinfection work in housing societies or elected representatives getting it done on the request of the residents of their constituencies. The civic body’s mayor had appealed in a special release dated March 31 insisting that the spraying of Sodium Hypochloride is dangerous.

Guam Fire Department's Engine 10 and Medic 10 from the Yigo Fire Station responded to the school and found that workers that were exposed to chemical vapors, said GFD spokesman Firefighter Kevin Reilly stated. 

The unconscious man, who is in his 40s, had a pulse and was breathing, Reilly confirmed. The other two patients, women ages 21 and 54, were conscious and alert but stated feeling ill. According to the two women, they were using bleach to sanitize the boys locker room and were overcome by the vapors due to poor ventilation. According to officials with Lubbock Fire Rescue, just before 10:30 a.m. an employee cleaning a restroom spilled a cleaner and tried to clean that up with bleach. The mixture created a chlorine gas.

Eight units from Lubbock Fire Rescue responded to the scene and HazMat was called as a precautionary measure.

The residents were moved to a safe area until the affected area could be ventilated.

ORG officials said the spilled material was residue from chemical processing with a similar risk as household bleach. The chemical had a high p-h value and had to be washed off to avoid reddening of the skin

ORG said the tanker trailer was over-pressurized and a safety device released six gallons of material.

All lanes of SR 66 between Epworth and Grimm Road are now open.

The spill sent four people to the hospital.

State police say several drivers pulled over after noticing a substance that spilled on their vehicles. They tried to remove it from their windshields. This is what National Poison Centre statistics have found, said the centre's pharmacist (drug and poison information unit) Asdariah Misnan.

Sixty six percent of cases recorded during the 10-year period comprised accidental poisoning involving household chemical products (33%) and medicines (33%).

Among the accidental cases, 46% involved children aged one to nine.

The household chemicals include laundry detergent and bleach, as well as dishwashing liquid, she told Bernama when met at the centre at Universiti Sains Malaysia recently.

Fifty percent of all cases involve children, with the centre receiving calls about such poisoning cases on a daily basis.

    <dc:creator>dchas</dc:creator><description><![CDATA[State prosecutors argue he could have hurt people, but Ron Alleva says he was trying to help. The Anchorage businessman was sentenced Tuesday, after a jury found him guilty of multiple misdemeanor environmental crimes.

In June 2018, Alleva told his employees to sprinkle Zappit, a chlorine bleach powder, on the sidewalk areas along Karluk Street near Bean’s Café and Brother Francis Shelter in downtown Anchorage, an area often occupied with homeless individuals who receive services from both agencies. 

Alleva owns Grubstake Auction Company, located along the same block. He has long expressed concern about a problem he says the homeless have created in the area, including trespassing and other crimes. He was also upset over large amounts of garbage, including syringes, used condoms and fecal matter on and around his property.   

His effort the clean up the area triggered a hazmat response from the Anchorage Fire Department, and the removal of 1,400 pounds of contaminated soil, according to the Attorney General's Office. 

"Alleva's conduct, along with that of Grubstake Auction Co., Inc. and its employees endangered the health and welfare of individuals, putting them at risk of serious physical injury including blindness and even death had the substance been ingested," a previous statement from the attorney general read.  

In April, a jury convicted Alleva of reckless endangerment, pollution, unauthorized pesticide distribution and misuse of a pesticide.

    <dc:creator>dchas</dc:creator><description><![CDATA[BIRMINGHAM, AL - A chemical leak at the Birmingham Water Works Shades Mountain Filter Plant on US 280 that sent more than 50 people to the hospital Feb. 27 may have affected several trees surrounding the plant. Complaints from neighbors who have seen their pine trees turn brown has the BWWB investigating the matter.

"An accidental mix of sodium hypochlorite (which is essentially bleach) and ferric sulfate caused a chlorine off gas at our Shades Mountain Filter Plant," the BWWB said at the time of the leak. "We use these components to treat water as our normal practice, however they are not meant to be mixed together."

Water Works employees and people nearby who breathed in the gas were treated at local hospitals, but no serious injuries were reported.

The Birmingham Fire Department reported that hazmat entry teams closed the valves involved in the contamination, and officials determined that the issue had been resolved without further incident. However, since the leak, a large number of pine trees around the plant have shown signs of discoloration, including in a residential neighborhood and nearby church in Mountain Brook. Initially the BWWB denied any responsibility for the problems with the trees. The BWWB has since backtracked on that denial and said they are investigating the matter.

Harbert after a chemical spill at the Birmingham Water Works Shades Mountain Filter Plant sent 55 people to the hospital Wednesday morning, OSHA officials said.

B.L. Harber is the general contractor for an update project at the plant. The update will increase the water storage capacity of the plant by 600 percent, upgrade the plant’s electrical systems and consolidate pumping operations, according to the company site.

Somehow, two chemicals commonly used to treat water, sodium hypochlorite and ferric sulfate, were accidentally mixed, creating a toxic gas, said Birmingham Fire and Rescue Battalion Chief Sebastian Carrillo.

BWW employees were able to contain the chemical accident that occurred early Wednesday.

In total, approximately 50 to 55 people were transported to area hospitals, but none sustained any life-threatening injuries, according to the BWW. Fourteen contractors working at the site and one Birmingham Water Works employee, were taken to area hospitals immediately after the accident, with additional people taken for precautionary evaluation.

The Grand Forks Fire Department’s team for hazardous materials arrived at 805 Cherry St. at roughly 8:45 p.m., where residents had poured bleach into a plugged floor drain in the basement. The bleach reacted with remnants of a sulfuric acid drain cleaner, creating a chlorine gas that made it hard for residents to breathe.

Fire department personnel cleared the floor drain and flushed it with a large quantity of water before ventilating the home of the chlorine odor, while four residents were taken to the hospital.

Seven Grand Forks Fire Department personnel were on the scene, with assistance from the Grand Forks Police Department and Altru Hospital paramedics.

Deputy Fire Chief Troy Weiland tells NewsChannel 7 they got a call just after 9pm that there was a strange smell coming from a hallway in the Cedar Rail Apartments on South Cedar Street.

Once crews arrived on the scene, they traced the smell to one apartment. Weiland says one person had spilled an acetone-based product and used bleach to try to clean it up, creating the noxious fumes.

Residents of the three-story building were evacuated to another owned by the same company. Four people, two residents and the first two police officers on the scene were sent to the hospital to be checked over. At least one had burning eyes. Weiland says the checkup by the doctors was mostly precautionary.

The Athens-Clarke Country Fire and Emergency Services Department responded to the store at about 6:25 p.m. Tuesday on a report that someone had “maliciously spilled several different cleaners on the floor, said Kyle Hendrix, the county’s fire marshal and assistant chief of Support Services.

“Crews suspected potential ammonia and bleach and evacuated the store,” Hendrix said on Friday.

Exposure to high concentrations of ammonia in air causes burning of the nose, throat and respiratory tract, according to published literature.

An employee and a customer complained of coughing from the fumes, but the fire report did not note if they required medical attention, according to Hendrix.

Dollar General employees were directed to assist police in attempting to identify who might have been responsible, according to Hendrix.

Hazmat team members donned protective suits to enter the store and take air samples and evaluate the size of the spill.

“There were ammonia readings detected and the product was estimated at 4 gallons total,” Hendrix said.

Hazmat team members used containment material to isolate the spill, he said.

The store remained closed until Wednesday when a private cleanup arrived.

However, Hendrix said the company that responded “were not fully prepared or yet equipped to perform cleanup,” Hendrix said.]]></description>

The family who lives there said they called maintenance for some clogged pipes. They believe there was a mixture of something similar to Drano and bleach put down the pipes about a week ago, but the smell had been getting progressively worse.

Maria Alvarado, the daughter of the woman who lives there said her reaction to it was bad.

“Dizzyness headaches, I wanted to pass out. My eyes were turning red like somebody punched me right here. I can still feel the bruise in my eye,” Alvarado said.

Around noon Immokalee fire Control District came out to the area along with North Collier Fire Control & Rescue District and the hazmat team. They checked the entire building and did not find anything.

Officials said they caught the truck early and pulled it over. They said it could have all leaked if they didn’t catch him so soon.

Lanes are back open and a small portion of one northbound lane is closed but no traffic or congestion here.

Chuck Murphy, city utilities director, said workers at the plant noticed a white cloud in a ditch at the plant and notified the fire department.

The chemical, sodium hypochlorite, or household bleach, had entered the sewer system and made its way to the plant. Murphy said between 50 and 55 gallons of the chemical was released.

City Manager James Mavromatis said a drum burst at National Colloid on Adams Street, and the liquid entered a drain.

Murphy said holding tanks used during times of heavy rain were opened and the sewerage containing the sodium hypochlorite was pumped into the tanks. He said that was done to prevent the chemical from entering the secondary treatment section of the plant. Sodium bisulfite was added to the sewerage to remove the chlorine from the water, Murphy said. Chlorine is present in a small amount in sodium hypochlorite.

Mavromatis said National Colloid provided the sodium busulfite to the city.

    <dc:creator>dchas</dc:creator><description><![CDATA[Iredell County officials say they have contained about 455 gallons of chlorine bleach that spilled near the intersection of Nixon Road and Shumaker Drive on Wednesday morning.

A truck operated by Water Guard Incorporated, based in Wilson, was carrying bleach with a 15 percent solution of chlorine when a stress crack in the tank allowed the chemical to spill around 8:45 a.m., according to Matt McAlpin, assistant fire chief for Ebenezer Volunteer Fire Department.

About 300 gallons had been pumped back out of the ground during cleanup, officials say, and a cleanup crew is expected to treat the ground to remove the rest.

TULSA, Oklahoma - A bad mix of chemicals at the Osage Casino in Tulsa caused 13 people to seek medical attention. 
The Tulsa Fire Department and HAZMAT crews worked Sunday afternoon to figure out what went wrong. 

Firefighters said while an employee was cleaning, he accidentally mixed ammonia and bleach, a dangerous combination.

The Tulsa Fire Department said several people complaining of having a scratchy throat, burning eyes and headache caused an Osage Casino employee to call 911. 

By the time firefighters got there, they said four people had already taken themselves to either urgent care or the hospital. ]]></description>

    <dc:creator>dchas</dc:creator><description><![CDATA[GRAND TOWER -- One man is dead and a woman in the hospital after a hazmat situation created by mixing household cleaning chemicals.

According to the Jackson County Sheriff’s Department, officers and later hazmat units responded to a house on 3rd Street in Grand Tower a little after 3:00 p.m. Tuesday where a man wasn’t breathing.

He was pronounced dead on scene from the poisonous gas.

Investigators on scene said the gas was caused after the residents mixed bleach and a commercial drain cleaner in an attempt to clear a drain.

"We arrived, determined there was a chemical hazard due to the mixing of two household cleaning chemicals," said D.W. Presley, an officer for the Carbondale fire department and member of the MABAS 45 hazmat team.

A woman in the house was transported to a local hospital and later flown to St. Louis for further treatment.

Presley said that mixing household cleaners can produce dangerous gasses almost immediately.

"Mixing any household cleaning chemicals can be dangerous," he said. "There are several chemicals that people use and have in their house every day that mixed together can make toxic gasses."

The company Darigold apologizes for the mistake and says the chemicals pose no health risks.

As a precaution, the company is conducting a voluntary withdraw of the product, which is the fat free chocolate milk half pint with the "Best By" date of February 6.

Since yesterday, the company estimates that as many as 16,000 cartons could contain the chemicals, which are...

"A mixture of trace amounts of food grade sanitizer and water," said Sarah Taydas, Director of Communications for Darigold. "It's important to know that this poses no health risks to humans. There actually was no milk in the carton the student had opened."

Taydas clarified that specific product was produced in the Spokane Darigold plant, and that this has never occurred before. The company's top priority continues to be food safety and customer health.

SPOKANE, WA - Spokane Public Schools has confirmed they are pulling all chocolate milk with a "Best By" date of February 6 from all of their schools due to possible contamination.

A Shadle Park High School student noticed a "bleach" taste to the milk on Tuesday, and after an investigation, the decision was made to pull the milk and stop shipment of the rest of the order to other schools.

    <dc:creator>dchas</dc:creator><description><![CDATA[The US Chemical Safety Board on January 3 released its case study titled “Key Lessons for Preventing Inadvertent Mixing During Chemical Unloading Operations,” which examines a mixture of incompatible materials at the MGPI Processing Plant in Atchison KS on October 21, 2016.

The mixture resulted in a chemical release containing chlorine and other compounds that traveled into the community. The CSB’s investigation examines several key issues including the design of chemical transfer equipment, automated and remote shut off systems, and chemical unloading procedures. 

The MGPI facility produces distilled spirits and specialty wheat proteins and starches. The chemical release occurred when sulfuric acid was inadvertently unloaded from a tanker truck into a fixed sodium hypochlorite tank at the plant. The two materials combined to produce chlorine gas and other by-products that sent over 140 individuals, both workers and members of the public, to area hospitals, and resulted in shelter-in-place and evacuation orders for thousands of local residents.

Chairperson Vanessa Allen Sutherland said, “High risk operations, like the delivery and handling of hazardous chemicals, require strict adherence to safety protocols. An inadvertent mixture can result in a chemical reaction with extremely dangerous consequences. Our findings reaffirm the need for facilities to pay careful attention to the design and operation of chemical transfer equipment to prevent similar events.”

    <dc:creator>dchas</dc:creator><description><![CDATA[SAN DIEGO (KGTV) - Children and adults were rushed from an East San Diego County school to the hospital almost two years ago and now some of the former patients are suing the YMCA of San Diego County and an employee over the incident.

More than 80 adults and children were evaluated during the emergency at Rio Seco Elementary school in Santee in October 2015.

Duane McFarland told Team 10 investigator Adam Racusin his son said his skin and mouth were burning and he was having trouble breathing.

“He still gets dizzy every once in a while,” McFarland said. “He still gets short of breath.”

At the time, investigators said the problem was caused by a chemical spill at the YMCA near the school.

According to two lawsuits filed, the YMCA and an employee disposed of hazardous waste by pumping spilled sodium hypochlorite, into a storm drain. It says thereafter attempted to use another chemical, which was in conflict with the labeling, to try and neutralize the spill.

The lawsuit claims the move created a chemical vapor plume that floated over to the school.]]></description>

The Baltimore County fire department’s hazmat unit was called around 8 a.m. to the Maryland Masonic Homes at 300 International Circle in Cockeysville where bleach was mixed with a second chemical inside a laundry room, releasing an odor, officials said.

Two people were transported to an area hospital with minor injuries, said Capt. Tim Rostkowski. He did not have additional information Wednesday morning.

According to Farmington Fire Rescue Capt. Scott Baxter, firefighters and NorthStar EMS responded at roughly 2:44 p.m. after the man cleaning a wall at the business mistakenly mixed bleach with the car soap. The chemical reaction put the man in respiratory distress.

Baxter said that firefighters put the man on bottled oxygen and used a hose line to decontaminate him. They removed his contaminated clothing and put him on the NorthStar ambulance to be taken to Franklin Memorial Hospital to be checked out.

"It was a little bit different," Baxter said of the incident, "but more or less run of the mill once we got there."

Firefighters used the water on their truck to dilute and decontaminate the affected room. No one else was impacted by the chemical reaction.]]></description>

Dispatch received reports of a pressure washer explosion, and injuries around 7:00 a.m. at 2744 Dam Neck Road.

When crews arrived no one was there, but a locked storage unit with the smell of chlorine coming from inside.

After further investigation, officials learned a man mixed chlorine bleach with nitric acid, common in cleaners, which caused an explosion.

The man suffered injuries to his right hand and arm and drove himself to the hospital, but locked the storage unit before leaving.

Hazmat was able to open the storage unit to further investigate.

The spill of 2 or 3 gallons occurred just before 10 a.m. in the westbound lane of Piner Road east of Fulton Road, Santa Rosa Fire Battalion Chief Matt Dahl said. Safety concerns and the need to make room for a hazmat team prompted police to shut down both lanes of traffic between Fulton and Pinercrest Drive until about 12:45 p.m.

The hazmat team, in breathing suits to prevent exposure to liquid or vapor chemicals, used materials to soak up and neutralize the substance, identified as sodium hypochlorite. A city street crew then had to wash down and vacuum the area, Dahl said.

According to documents obtained by the Washington Post, the recruit was ordered by his drill instructor to “perform unauthorized exercises under an upside-down laundry bin on a floor covered in bleach and was required to stay in his wet pants for hours.” The bleach resulted in severe burns that “liquefied” the skin on his buttocks.

The unnamed recruit told another superior about his injuries, but continued to train after he was told he would not be able to graduate with his class if he sought medical attention, the Post reported.

The drill instructor, former Sgt. Jeffrey VanDyke, was convicted at court-martial on numerous allegations, including cruelty and maltreatment, assault and failure to obey a lawful order in the case. He was sentenced in 2014 to a year in prison, but the severity of the hazing carried out by Van Dyke was never disclosed. It’s one of many abuse cases being investigated at the Parris Island boot camp in South Carolina over the past five years, according to the newspaper.

Lt. Jonathan Evans with Myrtle Beach Fire Department says they responded to the call around 11:10 a.m. Monday after a mix of urine-based ammonia and bleach were mixed in a washing machine at the Humane Society and the emission of the noxious gasses affected the person doing the laundry.

“We’re going to be disposing of many of the blankets and things that were in the laundry area to be washed,” says Grand Strand Humane Society Executive Director Suzanne Roman. “And if anyone could donate blankets to us right now, we really really need them, because we will have to throw all of those out. And we are flushing the area out and really making sure that it is safe for the animals and for people to go back in.”

Multiple engines, heavy rescue, and the MBFD hazardous material truck responded to the call. HazMat techs are working to clear the materials, but crews are not expecting any other injuries from the hazardous material, adds Lt. Evans.

The CEO in turn asked neighbors to help him gain zoning permission to legally carry out the work that caused a recent explosion.

Those dueling asks emerged Thursday as neighbors and business owners met with officials from the bleach-making the Welton Street-based New Haven Chlor-Alkali, formerly known as H. Krevit & Co., and city emergency officials at Leeway nursing home to discuss a Dec. 22 explosion that literally shook the ground of the community.

Neighbors learned that the explosion didn’t happen in the old part of the plant that uses a process that requires hazardous chlorine gas to be brought in by rail daily. It occurred in a new wing of the plant that the company touted as safer—a new plant that the company, it now turns out, was not properly permitted to use for manufacturing. It in fact was permitted for use only as a warehouse.

The company is seeking to obtain a special permit to allow for manufacturing at the new plant. That application will be before the Board of Zoning Appeals next month.

“There’s no question that the explosion day-lighted an omitted permit,” said the company’s zoning attorney, Marjorie Shansky said. “I can’t tell what the thinking was at that time when the City Plan Commission approved the site and coastal plan in 2009, and when the city issued an electric bolt permit for over $2 million of electricity for a warehouse. It is a little gray and murky.”]]></description>

500 litres of the liquid, which is basically bleach but about seven per cent stronger than household bleach, spilled but was contained in the storage building.

Firefighters got the call at 3:41 p.m. Tuesday.

They donned Level A hazmat suits to enter the spill area to ensure it was contained and then used absorbal to help with clean up operations.

"Level A is the highest level of hazmat suit that we have but it is just precautionary," said Samantha Hoffmann, Barrie's Public Fire and Life Safety Officer.

    <dc:creator>dchas</dc:creator><description><![CDATA[Workers at the Canadian Keyes Fibre plant in Hantsport were forced from the building Wednesday after two chemicals mixed inside the Prince Street manufacturing facility.

Hantsport deputy fire chief Paul Maynard said that at about 9:30 p.m., hydrogen peroxide and sodium hypochlorite were accidentally mixed together. About 1,500 litres, most of that hydrogen peroxide, was involved.

The reaction was causing a gas to form when Hantsport fire crews arrived on the scene. They called the Kings County hazardous materials team, which is composed of firefighters from Kentville, New Minas and Wolfville, to assess the situation.

“What (mixing of the chemicals) does is cause a reaction that produces oxygen and a lot of heat,” Maynard said at about 12:30 a.m. Thursday. “It's not overly dangerous, but with an oxygen-rich environment, we have to watch for ignition sources.”

According to the Harris County Sheriff’s Office, the spill occurred in the 2400 block of Farrell. The Aldine Fire Department and hazmat unit is heading to the scene.

The Harris County Fire Marshal says that around 10:45 a.m., a flat bed truck carrying containers of various chemicals tipped over. Several chemicals were spilled, including bleach. The construction site where it occurred was evacuated.

Surrounding businesses were told to shelter in place. No residential areas were affected by the spill.
