A chemical process used to seal test tubes resulted in a fire at Battelle Memorial Institute Wednesday morning.

Columbus firefighters were first called to 505 King Ave. near the Ohio State University campus around 8:20 a.m. Wednesday on a report of a fire in a laboratory involving chemicals in a gas state being used in an experiment, Battalion Chief Steve Martin said.

A second alarm was called almost immediately because of the size of the building and the fact that chemicals were involved, Martin said.

The fire was contained to an area of the lab building and the building will be decontaminated, along with all responding firefighters and their gear.

Further investigation determined the fire started during a chemical process using a torch to seal test tubes, Martin said. The employee working on the process had taken a break, and smoke was reported in the area while he was on break, Martin said.

Battelle released a statement saying no employees were injured in the fire.

“Initial reports indicated that the fire involved an acetylene torch which led to a piece of wood catching fire,” the statement said.

The FDNY descended on the T & T Supply Co. on Willets Point Blvd. in Flushing after an Oct. 9 inspection by the Bureau of Fire Prevention Salvage Yard Task Force uncovered the plethora of tanks, most used to hold propane or acetylene, authorities said.

The business was immediately shuttered as the owners of the company were ordered to hire outside contractors for removal of row after row of the dangerous canisters.

“That’s a disaster waiting to happen,"said a retired city explosives expert with more than 30 years of experience. “I really don’t believe it. Oh my God. How the hell did he get away with it? You know the shrapnel that would have been flying around if God forbid that was a fire? ... If it blew, the tanks would take off like friggin’ torpedoes."

Investigators in Hebei in northern China found that managers of a polyvinyl chloride plant where a deadly accident occurred in November grossly ignored safety. The plant, which produces PVC via the acetylene route popular in China, is run by a subsidiary of ChemChina, one of the country’s largest state-owned chemical firms.
In their 37-page report, officials accused leaders of Hebei Shenghua Chemical Industry of initially hiding their company’s responsibility. Investigators also faulted Shenghua managers for allowing production and safety staff to be absent, sleep on the job, and spend hours playing with their mobile phones.
The accident occurred on Nov. 28 when vinyl chloride gas escaped from a tank and ignited, immediately killing 23 people, most of them in vehicles outside the plant. Another person died later in a hospital. According to the government’s report, over 2,000 m3 of vinyl chloride leaked.
An overly thinned tank seal was the direct cause of the leak, the government investigators found. But the faulty seal would have been easily noticed if Shenghua staff had been regularly inspecting equipment.

Shenghua managers paid almost no attention to safety, according to the report. Line staff had little safety training, standard operating procedures to manage safety did not exist, and managers in charge of safety lacked independence.
Government investigators also faulted the local police force for allowing an illegal parking business to operate next to the plant. The presence of the parking lot made the death toll worse, officials found.

The need for a hazmat response prompted officials to close the east and westbound sides of the road between the Route 125 Bypass and Wilson’s Corner for about two hours, according to the North Andover Police Department.

Four tanks in total fell from the truck, two of which were leaking, officials said.

Video from the scene showed crews loading the tanks into the back of a dump truck for transport.

An apparently accidental explosion at a UPS Freight maintenance facility in Kentucky left at least two people injured Wednesday morning, Lexington fire officials said.

Authorities learned of the blast, at the UPS facility on Blue Sky Parkway in Lexington, around 7:40 a.m. Fire Battalion Chief Jason Wells said crews found “significant structural damage” to the facility when they arrived, with the explosion damaging the roof and walls of the building.

Two people were taken to the hospital for treatment of their injuries, which Wells described as minor burns and concussion-type injuries. They were conscious and talking after the blast, Wells said.

“I believe we’re extremely lucky,” he said.

Six other people who were in the area were taken to the hospital for observation.

A Friday news release from the Air Force Materiel Command’s public affairs office announcing the availability of the report said a private subcontractor’s welder was using the torch to remove a corroded steel beam near the coils containing the chemical, commonly known as R-30.

“Although stable at room temperature and pressure, R-30 can rupture or explode when exposed to heat,” the Materiel Command noted in its news release on the Air Force Ground Accident Investigation Board report.

Methylene chloride is a carcinogen.

On Wednesday, April 25, there was a minor fire in the Unified Arts Building, requiring the immediate evacuation of all students and teachers.
According to Assistant Headmaster Hal Mason, no one was injured, as the teachers and students in career and technology education teacher Elizabeth Brennan’s metalwork and jewelry class followed all proper procedures.
“What happens is that you’ve got this really hot gas burning, which of course is really scary and bright,” Mason said. “It’s a very flammable gas, so you see flames coming out of the hose, and immediately the teachers and the students evacuated the room; they did everything they were supposed to.”
Mason explains that the fire was caused by an overnight buildup of acetylene gas in the tubes connected to a blowtorch used for welding in the class, which had been properly emptied the day before. The buildup of gas caused the nozzle to be knocked off and fire was released from the tube.
The fire from the tubes caused minor charring to the floor, but nothing else caught on fire. According to Mason firefighters were quickly on the scene to extinguish the fire.

BROOKLINE, MA — Brookline fire fighters say they quickly got a fire under control at the Unified Arts building at the High School at 46 Tappen Street.

One girl had burn marks on her sweatshirt but did not need to go to the hospital, according to fire officials.

Fire officials say High School staff evacuated the building quickly after a welding gas tank had a "mini" explosion during a jewelry making class. The 14-year old student was igniting a torch which may not have been properly "purged," and that caused the mini explosion. She dropped the tank and a fire broke out from the leaking welding tank. Staff quickly evacuated that room and the rest of the building as fire fighters made their way to the second floor room just after 11:30 a.m. The fire was out by 11:52 a.m. according to officials.

They did an excellent job evacuating here at the high school, said Deputy Chief Colin O'Connell.

The fire was contained to the jewelry making room. He said there was flame damage to three of the four tanks, he said.

The Circular Quay inferno, which sent plumes of toxic black smoke and caused “fireballs” to fall from the sky, raged through a 10-storey building on a demolition site at about 8.45am. Fire & Rescue NSW extinguished the fire just before 12pm, but it still caused huge delays all day for CBD commuters.
Now, fire investigators have tracked the cause of the blaze to one of the construction workers’ oxyacetylene blowtorches, according to a Fire and Rescue NSW spokesman.
The fire is thought to have erupted on a lower level caused by stray embers from a blow torch cutting through steel. The flames quickly spread up the plastic mesh screening surrounding the scaffolding.

That was the exact reaction the Cornell College Chemistry Club was looking for.

“I think if you tell people there’s going to be an explosion, people are going to come,” Cornell College Chemistry Club president Bethany King said.

The club decided to celebrate National Chemistry Week by using a chemical reaction to explode pumpkins. King said the group used a combination of Hydrogen Peroxide, Calcium Carbide and water. She said that combination produces flammable acetylene gas and calcium hydroxide. After a minute, the group ignited the gas inside the pumpkin, causing a contained explosion.

King said securing faculty approval for the project wasn’t easy.

“I asked a lot of people on campus if I had permission to do it. I had to asks six people in our department, and then Cornell campus security.”

She said the group practiced with smaller explosions, eventually working their way up to the full explosion.

The cause of the fire remains under investigation. However, investigators said Tuesday that damage at the site is estimated at nearly $1.7 million.
...
“While they were inside the structure performing interior fire attack operations, there were several explosions inside the building. They immediately evacuated [the] building,” said Captain Kelly Zane, with Wichita Fire.

KSN learned after firefighters put out the flames, the city-county health department largely takes over the safety of any potential hazmat situation, especially when its effects could impact the general public.

“They’ll work with the company to make sure that it gets properly cleaned up. They actually do air mediation, monitoring downwind when the fire is burning. So, they go as far as to check the water runoff to make sure nothing is in the water,” said Battalion Chief Rich Harris, with the Wichita Fire Department.

The largest concern involving a potential hazmat situation on Monday actually came for firefighters on the scene.

“Propane bottles, Acetylene bottles, Oxygen, so, all those things are considered dangerous to us,” said Chief Harris.

Fire crews were sent at about 3:20 p.m. to Airgas, a distributor of specialty gases in Poca, outside of Charleston. Putnam County emergency management director Frank Chapman said the explosion involved about 50 tanks of acetylene that were at Airgas waiting to be refilled. What caused the tanks to explode wasn't known.

Chad Jones, a firefighter with the Bancroft Volunteer Fire Department, said four cylinders continued to burn Monday evening and that crews were letting them "burn out." They were dousing other tanks with water to keep them from exploding, said Jones, whose station was one of several to respond to the scene.

The tanks were being stored in a bay behind the facility. Jones said after the first tank exploded, "it was like a chain reaction," with fireballs shooting 100 to 150 feet in the air. A nearby business was evacuated, and windows were shattered in the back of the Airgas plant, Jones said.

Police and firefighters found Nicholas Jerde of Kansas City dead after an explosion at Rieke Grading, 8200 Hedge Lane Terrace about 9:30 a.m.

Jerde was pronounced dead at the scene, said Fire Marshal Corey Sands.

According to initial reports, Jerde was using an acetylene torch to free a valve on a vacuum truck when the explosion occurred. It appeared that Jerde ignited the vapors inside the truck. No other injuries were reported.

The car was destroyed but the woman, a nurse from Winchester Hospital, was not hurt.

The bomb squad determined the cause of the blast was an accidental vapor explosion. Investigators said the use of the car's remote unlocking device ignited vapors from plumber's equipment inside the vehicle.

One of the trucks was hauling 250 cylinders of acetylene, a highly combustible chemical, down Jungong Road at 4:30 am, when another truck rear-ended it near the Zhayin Road intersection, according to a report on the news website Xinmin.cn. Two of the cylinders exploded on impact, setting another 34 ablaze, with 26 burning on the truck and two on the road.

The explosion woke up nearby residents, according to the report. Firefighters responded to the report of an explosion and extinguished the fire with foam before 8 am. They also sprayed down the remaining cylinders to cool them so they could be removed from the scene.

One driver suffered a fracture while the other walked away with bruises. Both were sent to the hospital, the report said. Authorities are still investigating the cause of the collision.

NEW CUMBERLAND - The small home at a rural crossroads in central Hancock County is where James Stanley lived and worked for many years. On Thursday, it's where he met a tragic death.

Stanley, who was in his 60s, was working on a project in his driveway with an acetylene torch Thursday afternoon when he was killed by an explosion, officials say.

"It was a bad accident - just one of those freak things that you don't see very often," Hancock County Sheriff Mike White said. "Very unfortunate."

The likely cause of death was trauma and burns from the explosion, according to the sheriff, but officials are still awaiting a ruling from the state medical examiner's office.

White said he knew Stanley as a retired man who still did work on the side with excavating equipment. Several small bulldozers and backhoes sit on his property at the remote intersection of Ballantyne and Shady Glen roads. A lone pine tree stands in his front yard.

Burn marks from the fire that claimed his life can be seen on the ground outside his garage. Police tape no longer surrounds the property, but the state fire marshal's office is continuing its investigation.

"We're not sure exactly what he was doing (with the torch)," White said. "He was cutting something."

Either flames or sparks from the torch ignited a 55-gallon drum that Stanley was standing over while he was working, White said. The drum was sealed and empty, but investigators are conducting tests to see whether vapors were the ignition source.

Deputy Fire Chief Ed Prescott said the fire tore through the 60-foot-by-60-foot, two-story sheet metal building that houses the mid-20th century, 20-cylinder diesel locomotive engine that powers a car shredder. The building contained about 9,000 gallons of diesel fuel as well as containers of acetylene and oxygen, fire officials said.

"Our main concern was that a lot of acetylene tanks and oxygen tanks were compromised and were fueling the fire," he said. "Our guys worked really hard to contain it to where it was and to make sure there was external explosions."

Though the torch and hose ignited instantly, Bloomfield firefighters, arriving within two minutes to the scene at Bloomfield and Watsessing Avenues, were able to contain the fire and extinguish it. The county Hazmat team from Nutley also arrived quickly, said Gehringer. No injuries were reported.

"One of the oxy-acetylene torches [workers] were using ignited," he said. "One of the hoses had a rupture in it. It caused gas to escape."

Used at the processing plant to cut through metal, the oxy-acetylene cylinders are filled with a combination of oxygen and a hydrocarbon fuel called acetylene. There were eight of them in the room at the time of the explosion.

Around 11:11 a.m. Monday, Franklin fire officials and one member from the Greater Cincinnati Hazardous Materials team responded to the company at 501 Shotwell Drive for a leaking tank full of Acetylene — a fuel used for welding, said Capt. Tony Abston said.
Nation Coating Systems is a supplier of thermal spray coaling for industrial application, according to its website.
Abston said about 50 employees were evacuated from various businesses in the industrial park — Nation Coating Systems, M&S Flooring, Caring Partners International, Waytek, Inc. and Olivamed. The workers were relocated to a vacant building nearby until about 1:30 p.m.
Abston said employees of NCS were attempting to unscrew the tank’s lid with a wrench when the liquid started spewing out.
“It’s very explosive,” Abston said. “(Acetylene) chemical is heavier than air and it dissipated.”
Power to the building was shut off by 11:30 a.m. before any officials entered it, Abston said. He added the tank was moved outside and the building’s doors left open to ventilate. No one was injured.

No one was injured in the blaze at the quarry on Penny Creek Road off state Highway 104, said Quilcene Fire Department Chief Robert ‘Mo' Moser.

The 40-foot-by-60-foot wooden shed was fully engulfed in flames when firefighters arrived after receiving a 9-1-1 emergency call at 12:47 p.m., said Moser, whose district was the lead agency fighting the fire.

There were no injuries, said Bill Beezley, spokesman for East Jefferson Fire Rescue, which assisted Quilcene along with Brinnon and Discovery Bay fire departments and Port Ludlow Fire and Rescue.

An investigation into the cause of the fire will begin Wednesday, said Moser, who planned to keep a crew at the site tonight to guard it from interference and watch for flare-ups.

The building contained a 2,000 gallon diesel tank, acetylene tanks and propane tanks.

“We had a couple of small explosions but nothing big,” Moser said.

Firefighters took a defensive tack due to the volume of fire, the potential for collapse and the observed presence of flammable chemicals, which turned out to be a ruptured acetylene tank.

A large fire has broken out at an industrial estate.
Almost 50 firefighters worked throughout yesterday afternoon tackling the blaze in Biggar Road Industrial Estate in Cleland, North Lanarkshire.

They had set up a precautionary 200 metre exclusion zone around the area when they came across cylinders of acetylene, a gas used as a fuel and a chemical building block. The safety zone was later removed.

Strathclyde Fire and Rescue said the fire was in a large industrial unit. No one was injured in the blaze.

Incident commander Gary Wallace said: "This has been a major blaze in a large industrial unit. Additional worries for us has been the 3 acetylene cylinders on site, the roof, which has been confirmed as asbestos, and a 1,000 litre diesel tank. These additional issues required residents to be evacuated from nearby homes. That exclusion zone has now been lifted and residents have been escorted back to their homes."

Several engines and a hazardous materials unit were called out at about 10:30 a.m., Capt. Tracy Carlton said.

The fire department stood by as the leaking acetylene cylinder was allowed to vent off, Carlton said. Depending on what a chemical may be, the best procedure may be to allow the chemical to vent safely into the atmosphere while monitoring it and keeping everyone else away from the location.

No one was hurt in the incident and the normal class schedule resumed after about two hours, officials said.
