An explosion at a Seqens chemical plant in Newburyport, Massachusetts, early on Thursday, May 4, killed one worker. The cause of the explosion has not been determined.
The incident is the third serious accident at the plant, which manufactures pharmaceutical chemicals under contract, since 2020 when an explosion damaged a building. A chemical fire occurred at the site in 2021.
The body of Jack O’Keefe, an employee at the company for more than 25 years according to a local news report, was removed from the site late Thursday afternoon. Four other workers on site when the explosion occurred were hospitalized for precautionary reasons, but were uninjured, according to Barry Salt, Newburyport deputy fire chief.
“They had just wrapped up a typical manufacturing procedure that they’d done numerous times daily,” Salt says. “Obviously there was some sort of accident to cause the explosion, but what that was or how that happened is still in the beginning stages of investigation.”
The plant manufactures both active ingredients and intermediates of pharmaceutical chemicals. Chemicals on site at the time of the accident, according to Salt, included acetone, isopropyl alcohol, and methanol.
Salt says air quality has been monitored by the Massachusetts Department of Fire Services beginning 1.5 h after the explosion. Local residents have been informed that there is no risk in the area.
The area of the plant damaged in the explosion has been in operation for more than 20 years. The facility was built in the 1980s. Novacap, a French firm now called Seqens, became the site’s owner when it bought PCI Synthesis in 2018.

Several laboratories and buildings on the University of Delaware’s campus reopened for classes and resumed normal operations on 9 February, following evacuations and then a controlled detonation of ‘a small amount of a shock-sensitive explosive chemical’ inadvertently produced at a research lab on campus the previous day.

The evacuations were in response to a report from the University of Delaware’s Environmental Health and Safety office. The Delaware State Police investigated the materials, removed them from the building and transferred them to a safe outdoor location where they were ‘neutralised’ on the evening of 8 February, the university announced.

‘This was an isolated incident, and there is no threat to campus health or safety,’ the University of Delaware said. ‘No injuries were reported.’ The school declined to release further details of what was exploded and why or what reaction was being run. Chatter on social media suggested that a chemistry graduate student accidentally made triacetone triperoxide, also known as TATP, which is synthesised from acetone, hydrogen peroxide and an acid. A graduate student at the University of Bristol inadvertently made 30–40g of TATP in 2017, requiring a bomb disposal team to dispose of the material.

‘Our safety people did a top-notch job and responded quickly. It was just so sudden and unexpected,’ tweeted Mike Axiotes, an undergraduate student in the University of Delaware’s chemistry department. ‘We were talking about vinyl chloride burn in subgroups as a safety/making reactions count talk and then boom (pun intended) we’re being evacuated 3 hours later.’

A trucker from Illinois was killed Saturday night in a single-vehicle crash on Interstate 80 in the southern Panhandle not far from Sidney. A resulting chemical spill closed the freeway into Sunday.

Cheyenne County Attorney/Coroner Paul Schaub says 42-year old Mohammad Pirzad of Buffalo Grove was pronounced dead at the scene near Potter.

Cheyenne County Sheriff’s deputies responding to reports of the accident found the cab and trailer of a semi blocking both westbound lanes with Pirzad dead inside the cab.

Witnesses told deputies the truck was traveling about 70 mph when it drifted onto the grass shoulder, then continued on the shoulder for a distance before Pirzad turned sharply back onto the pavement.

That caused the tractor and trailer to tip onto the passenger side. The witnesses said they never saw the truck’s brake lights come on.

Another witness told deputies he went up to the windshield of the cab and saw Pirzad on the passenger door. He and another person then broke the windshield to get a better look, then concluded Pirzad was dead.

County Attorney Schaub says the truck was hauling a load of chemicals that spilled in the crash. Those chemicals included acetone, isopropanol alcohol, ethanol, and ammonia. Hazard mitigation and cleanup last well into Saturday.

Drivers are being urged to continue avoiding the Interstate 64/77 split, due to a closure of both westbound lanes near the Washington Street exit after a chemical spill early Tuesday morning.

The West Virginia Department of Transportation, Division of Highways said late Tuesday evening that the westbound lanes will remain closed throughout the night and a detour remains in place.

The lanes were closed after a tractor trailer overturned about 4:45 a.m., spilling acetone, a solvent typically used to break down other materials. Charleston Fire Chief Shawn Wanner said the material is highly flammable.

Emergency crews said that as of Tuesday evening, most of the hazardous material had been cleaned up and a towing service will be towing the tractor trailer away from the area.

However, the acetone based material has dissolved the asphalt down several inches, Charleston Emergency Management Director Chad Jones said.

State engineers will assess the damage when the truck is removed, but the road may not open until sometime Wednesday, depending on the damage found by the engineers.

In one, a resident was overcome by fumes and taken to the hospital for treatment after mixing Simple Green, a powerful cleaner and degreaser, and bleach, the department said.

In the second, a resident accidentally combined towels covered in acetone and bleach, resulting in a chemical reaction and fire. Luckily, according to fire officials, the resident got the burning towels outside and was able to put them out. There was only minor damage to a deck.

Fire officials warn that these chemicals alone, even used for their intended purposes, can be dangerous. Mixing them can cause unexpected and unpredictable results, they said.

The story sounded like it was made for Hollywood: Two chemistry professors in a small Arkansas college town accused of cooking methamphetamine in their school laboratory.

On the night of Oct. 7, 2019, a chemical spill occurred in Room 304 at Henderson State University’s Reynolds Science Center. The next day, a strong odor filled the building, forcing the school to evacuate the building and cancel classes. Weeks later, Terry David Bateman, 47, and Bradley Rowland, 42, were arrested and charged with manufacturing meth and other drug-related offenses.

But since then, the tale hasn’t followed a clear script. Last week, almost two years after his arrest, Bateman was acquitted of all charges by a Clark County Circuit Court jury, despite incriminating testimony from Rowland.

Then, on Friday, Rowland pleaded guilty in the same Arkadelphia courtroom to three felonies — manufacturing meth, drug paraphernalia use, and manufacture of a controlled substance, phenylacetone. Also known as P2P or phenyl-2-propanone, the substance is the penultimate chemical step in a common technique used to manufacture meth.

AFFTON, Mo. (AP) — A firefighter was injured Thursday during a large fire at a chemical plant in eastern Missouri that forced evacuations in nearby neighborhoods.

Fire personnel responded to the scene at the Manor Chemical Plant in Affton, where the fire was reported about 2:45 p.m., authorities said. Witnesses in the area reported hearing several explosions during the fire.

Employees of the plant were evacuated safely and the fire was extinguished by about 5 p.m., said Lt. Matt Coppin of the Metro West Fire Protection District.

The firefighter was taken to a hospital but Coppin said he didn’t know the extent of the injuries or how the firefighter was injured.

The south St. Louis County business is a wholesale chemical distributor but also makes “custom blends” of chemicals for cars, paint, printing, dry cleaning and industrial manufacturing, according to its website. Some of the products Manor Chemicals sells include acetone, rubbing alcohol and other flammable chemicals.

Up to 10 chemicals may have been involved in the fire, Coppin said. Thirteen fire agencies and 62 fire trucks responded to the blaze.

St. Louis County police helped evacuate people within a half-mile of the plant. St. Louis County police said most residents and businesses were allowed to return about 6 p.m.

A fire and several explosions at a chemical factory in Durban, South Africa injured 13 people on December 8. Over 100 workers were evacuated from the factory and two of the injured are reported to have suffered life-threatening injuries.

The incident happened at a factory belonging to Zeta Laboratories in New Germany, outside of Pinetown in Durban and occurred just days after another explosion in the city at an oil refinery operated by Engen.

Firefighters tackled the blaze for several hours before it was extinguished. A lack of fire hydrants in the area had made it difficult for firefighters to tackle the flames until fire trucks arrived at the scene. Paramedics attended the scene at around 11:00 local time and all 13 inured people were taken to local hospitals.

Local officials said that they believe the fire started in the processing area of the factory and quickly spread to 20 gas cylinders which caused an explosion. A number of chemicals were stored on site, including 10 tonnes of sanitisers, acetone, hydrogen peroxide, caustic soda, and petroleum jelly tanks which all helped the fire to spread and caused further explosions.

On receiving information, Devendra Potphode, chief fire officer of Pune Metropolitan Region Development Authority (PMRDA), said, several fire tenders were rushed to the spot at Kusmum Distillation and Refining Private Limited Company to douse the flames.

“The blaze erupted around 10.30 am. Water tankers were rushed to the site and the fire was doused. Cooling operation is underway now,” an official was quoted as saying by news agency PTI.

The incident came to light after a huge sound was heard in the area. Flames and smoke, which were seen billowing from the office premises, triggered a panic-like situation in the area for a while.

The official said that drums containing acetone and ethanol were kept at the plant.

“As the fire spread, those drums caught fire and exploded. A blanket of thick black smoke engulfed the area after the blaze. Nobody was injured in the incident as the plant was shut,” he said.

Deputy Fire Chief Troy Weiland tells NewsChannel 7 they got a call just after 9pm that there was a strange smell coming from a hallway in the Cedar Rail Apartments on South Cedar Street.

Once crews arrived on the scene, they traced the smell to one apartment. Weiland says one person had spilled an acetone-based product and used bleach to try to clean it up, creating the noxious fumes.

Residents of the three-story building were evacuated to another owned by the same company. Four people, two residents and the first two police officers on the scene were sent to the hospital to be checked over. At least one had burning eyes. Weiland says the checkup by the doctors was mostly precautionary.

A TWISTED 44 gallon drum full of dirty chemicals lies on the ground at West Footscray where it flew through the air propelled by a massive explosion three weeks ago.

The mix of grease, oil and acetone residues ooze out of the drum.

It’s a stark reminder of how dangerous and how out of control the factory fire of August 30 was before it raged — contained by still spewing poisonous smoke — for several days.

Weeks after drums filled with chemicals exploded and black smoke forced local residents to close their doors and windows and wait for clear air, those same residents say they’re experiencing bizarre side effects including nose bleeds, headaches and “weeping yellow pus”.

Police say a Hazmat was called in to check on strong odor smelling like Acetone in the area of Connell Drive and Melissa Court, just yards away from Peebles Elementary School.

Homes in the area of W.S.Young were evacuated for about six hours and the streets were blocked off.

People were asked to go to the Gilmore Senior Center and then later to the Killeen Community Center.

Fox 44 spoke with several homeowners, who say they are shocked this was not a big drug bust after someone made a narcotics call.

Officers say that was not the case, but it was a truck that was the culprit.

Ofelia Miramontez, the spokesperson for the Killeen Police Department says, "The smell was emitting from a ruptured fuel tank from a vehicle containing old fuel."

Linda Ament, a resident says, "I smelled the smell like finger nail polish remover and it made me dizzy and sick and nauseated like I wanted to vomit."

The incident occurred due to overheating of a chemical and water mixture as a part of regular chemical formulations. The injured has been identified as Varaprasad, 27, employee of Bio-Chemical Synthetics Private Limited located at Jimkalawadi under Sanath Nagar Police Station limits. SI J Venkat said that a case has been booked and the injured has been shifted to hospital. Biochemical & Synthetic Products Limited is a manufacturer, supplier and exporter of bulk drugs and intermediates. Around 150 workers work in the company. At 6.30 pm on Thursday, a loud blast rocked the unit and soon there was smoke all over. It was later found out that a chemical reactor inside the unit had exploded.

Sources from the company said that six workers were working at the reactor. As a regular process, they mixed acetone with water and were heating the mixture. Usually it has to be heated to a maximum of 75 degrees, but due to overheating, the mixture reacted, leading to the blast. All the workers were out on a tea break and escaped.

The liquid got onto one student's legs and soaked another's feet.
Five employees cleaned up the mess, even though no one knew for sure what it was and whether it was dangerous. They called fellow employee Kim Harrington, their union representative, to let her know what happened.
The spill initially was thought to be all acetone — a solvent used in many nail polish removers that can irritate the nose, throat, eye and skin at moderate to high levels of exposure. The campus did not identify the liquid until the next day, when staff determined that the broken bottles contained not only acetone but another more harmful solvent called dimethylformamide, a known liver toxin that can cause abdominal pain, dizziness, skin problems and nausea.
"No one knew exactly what it was so they cleaned it up the wrong way," Harrington said. "Two students were burned, and our staff was exposed to it. That started us down the road of trying to figure out … how safe is this environment?"

According to the Mid County Fire Protection District, firefighters were dispatched to the house fire on State Route V just before 4 p.m. on Aug. 5. Crews found the fire had begun on the home’s lower floor but had spread to the upper floor and attic. They were able to bring the fire under control within 30 minutes, according to the district.

The home was damaged by smoke and heat as well as fire, but no one was injured.

The Mid County Fire Marshal is investigating, but the district says the fire appears to have been accidental, caused by the unintentional ignition of acetone fumes.

The EPA claims the company’s mismanagement of equipment released chemicals — including acetone and benzene — into a wastewater treatment system. EPA also alleged that Vopak didn’t follow federal regulations for flaring.

As part of the settlement with the EPA, Vopak will install infrared cameras to detect pollution coming from chemical storage tanks at its Deer Park facility in Harris County.

Chemical emissions, such as chemical solvents or car exhaust, can interact with the atmosphere in the presence of sunlight to create ozone, which can be harmful to human health. The Clean Air Act regulates ozone amount present in the atmosphere.

Vopak’s fine will be split between federal government and the state of Texas. The settlement is still subject to final court approval and a 30-day public comment period. Click here for information on how to comment.

Seven years after the deadly chemical fire claiming 124 lives in Nimtoli, parts of densely-populated Old Dhaka still have extremely hazardous stocks of flammable chemicals without any safety measures whatsoever.

“The highly flammable chemicals are stocked and handled in such an unsafe way that any of them may erupt like a volcano anytime, leading to huge fire and devastation,” said Major AKM Shakil Newaz, director of Fire Service and Civil Defence.

All of the surveyed 360 stores, warehouses, factories and shops with the chemicals were found highly hazardous in terms of fire safety, handling of the chemicals and explosion management, according to the findings of a recent fire inspection.

For example, Shakil said, Acetone, a highly flammable chemical, is supposed to be preserved under 23 degrees Celsius for avoiding any explosion. However, it was found left under the sun, exposing it to around 36 degrees Celsius.

All the stores and shops were found lacking even the basic fire safety equipment, like extinguishers, let alone a complete fire fighting system.

Glassers are the surf industry’s version of Clark Kent. While shapers receive praise and name recognition for designing top-selling surfboards, glassers go unnoticed and underappreciated, as they humbly labor away with carcinogenic materials. On Friday, a bastion of those unsung heroes – Global Glassing in Oceanside, CA – was set ablaze from a chemical fire. Reports say the fire caused half-a-million dollars in damage. One employee was in the building at the time, attempting to extinguish the flames, and suffering burns to his face, but opting to drive himself to the hospital.

“Fire crews determined that there was no way to fight the increasing volume of fire inside the involved unit and pulled all personnel outside to begin defensive operations designed to limit the spread to the other businesses in the L-shaped building,” Battalion Chief Pete Lawrence told Fox 5 San Diego.

Surfboard glassing involves using many hazardous and flammable materials, including resin and acetone – which is reported to be the culprit of Friday’s fire. And Global Glassing was a major player in the Southern California surfboard industry, being the last stop for many local shapers and big brands before hitting the store racks. From Channel Islands to Firewire to Slater Designs, the Oceanside glass house had a wide array of popular industry clients.

“I watched from across the street as my place of occupation burned to the ground in what seemed to be an instant,” Global Glassing’s Arin McKinney wrote via Instagram. “It's a horrifying feeling to watch a fire of that magnitude burn beyond our control. Relieved to say that everybody made it out mostly unscathed from what I understand and fortunately I was not directly in harms [sic] way on this go around.”

A chemical compound burst into flames as employees worked with it, sparking a fire that gutted a shop that manufactures surfboards Friday, injuring one person in Oceanside.

The fire at Global Glassing Inc. in the 3200 block of Production Avenue erupted shortly after 11:30 a.m., near the Oceanside Municipal Airport. According to Oceanside fire department officials, it caused an estimated $500,000 in damage.

Crews arrived to find heavy flames issuing from rear roll-up doors at the business, Battalion Chief Pete Lawrence said. Within several minutes, the roof over the burning part of the structure collapsed and the blaze began spreading more rapidly.

“At this point, fire crews determined that there was no way to fight the increasing volume of fire inside the involved unit and pulled all personnel outside to begin defensive operations designed to limit the spread to the other businesses in the L-shaped building,” Lawrence said.

It took crews from Oceanside and several neighboring areas nearly an hour to gain the upper hand on the blaze, which destroyed the unit in which it originated and damaged two others — a second surfboard shop and a smog-testing business.

A Global Glassing employee reported that a crew at the facility was using acetone when it “flashed,” catching the immediate surrounding area on fire, Lawrence said.

Another worker tried in vain to extinguish the flames, suffering burns to his face. He drove himself to a local hospital rather than seek treatment from personnel at the scene. Due to the presence of acetone, resin and other chemicals in the charred building, hazardous materials teams from Camp Pendleton and the city and county of San Diego were called in to evaluate the damaged areas and runoff.

At 12:45 p.m. Mason's fire department was called to the school on a report of a gas leak.

Students and staff members in two classrooms of the health careers area were experiencing coughing and irritation.

The fire department located a one-gallon container of acetone that was left open. Acetone is a colorless liquid commonly used as a cleaning solvent. The evaporation process created the fumes which were then noticed by staff and students.

Everyone was removed from the health careers area and sent to other non-affected areas of the building.

Five faculty members were evaluated for acetone fume inhalation. No students needed medical care. The chemical was removed and the fire department ventilated the area.

Life in a chemistry lab isn’t always fun and games—chemists must also do their chores, including cleaning out used NMR tubes. Although commercial devices are available for the task, they are expensive glassware and typically only clean one tube or a few at a time. To remedy that problem, Thanh Binh Nguyen of the CNRS Institute of Natural Product Chemistry has devised an NMR tube cleaning system that can handle dozens of tubes at once and only requires a small amount of solvent and equipment already at hand in most labs (Org. Process Res. Dev. 2016, DOI: 10.1021/acs.oprd.6b00001). First, Nguyen empties NMR tubes and places them upside down in a beaker containing solvent or cleaning solution. Nguyen then puts the beaker in a vacuum desiccator, which he evacuates and vents with air several times. The liquid rises and falls with each vacuum cycle, cleaning out the tubes. A final rinse with fresh acetone completes the cleaning. Nguyen says he came up with the idea when his research funds were short. “I had to optimize everything—time, chemicals, human power—and here is one of my solutions.” Early responses on Twitter to Nguyen’s OPR&D paper were mixed: Some commenters questioned publishing the work in an industrial process chemistry journal, noting that industrial chemists often consider NMR tubes as a onetime consumable and toss them out. Plus cleaning them creates more lab waste. But most admit it’s a clever idea.

The Western Washington University chemistry building where a lab caught fire is expected to open in time for the start of fall quarter, the school announced Wednesday, Sept. 9.

The accidental fire erupted in a lab on the third floor of the building Aug. 25, with six students and a teacher inside.

It started around 5:15 p.m. after a student disposing a mix of chemicals that included acetone and hexane spilled the liquid on the floor. The vapors found an ignition source and caught fire before the building’s sprinklers extinguished the flames, Bellingham Fire Marshal Jason Napier said days after the fire.

Firefighters who were first to enter the building complained of sore throats and headaches after breathing an unknown gas. They were sent to the hospital and released the same night. The teacher and students in the building, who all evacuated, were not harmed.

The building, Morse Hall, has been closed since, while Belfor — an emergency recovery and restoration contractor — and university workers repair damage. The fire was confined to one room, but the sprinkler system caused considerable damage to scientific equipment and materials on multiple floors.

A chemical fire at Western Washington University’s chemistry building this week was reportedly caused by an accidental spill of flammable liquid.

A student getting ready to dispose of a mix of chemicals that included acetone and hexane accidentally spilled the liquid on the floor Tuesday, Aug. 25, said Bellingham Fire Marshal Jason Napier, who was in charge of investigating the fire’s cause.

“The vapors found an ignition source inside the lab and caused a quick flash-fire,” Napier said. “Then the pool of liquid continued to burn until the sprinklers went off and extinguished it.”

The fire did not appear to have spread to any other chemicals, but a few lab supplies also were burned, Napier said.

No one was injured when the fire took off. There were six students and a professor in the third-floor lab at the time, Bellingham Fire Department reported.

Napier said his investigation did not include looking into what foul-tasting gas may have been breathed in by the first firefighters to enter the building before they put on protective breathing equipment.

Speaking to news channel BFMTV, local Deputy Prosecutor Yves Dubuy said investigations revealed "significant traces of acetone and hydrochloric acid" on the spot and a weapon used for soft air games.
"The assumption of a loss of control over a chemical compound is studied," Dubuy added.
On Saturday afternoon, an explosion hit an abandoned house at Ranchevoux in Bas-en-Basset in central France, killing three teenagers aged 14 to 16.
A fourth victim was seriously wounded, the report added.

The fire alarm at Acton Technologies Inc. on Thompson Street Wednesday summoned nearly 80 Greater Pittston firefighters into action and sent Pittston Area High School students home early.

Classes at Pittston Area, located less than a mile away from Acton Technologies Inc., resumed on Thursday.

The fire was ruled accidental by state police deputy fire marshal Trooper Ron Jarocha, and the investigation determined a static discharge was the cause of the blaze.

Acton Technologies Inc. creates coating material for cookware, and according to state Department of Environmental Protection spokeswoman Colleen Connolly, the storage area that caught fire contained acetone, a chemical used to remove glue-like substances such as nail polish.

It took firefighters three hours to quell the flames. One firefighter was treated by paramedics for minor injuries.

The fire that started in the 19000 block of Poppy Grove Lane in Cypress Friday night was not intense enough to destroy the house, but it did send a 20-year-old woman to the hospital with third-degree burns to half her body.

The culprit, according to fire fighters, was a bottle of nail polish remover.

According to investigators, the woman was using nail polish remover near a candle when the vapors started a flash fire.

The Bellaire Fire Department showed us just how dangerous vapors and fumes from nail polish remover can become.

In this enclosed space, it takes only two minutes for enough vapor to accumulate to cause a flash fire.

This is a long narrow space and may be more than 15 feet deep. Firefighters say they tested the atmosphere in that space and detected chemicals, including acetone. Rescues wore air packs and climbed down into the area to hoist the two victims to safety.

“I think the challenge is getting down to a chemically charged atmosphere, getting some air on the individual so they don’t have to continue to take in all that chemical,” said Battalion Chief James Garrett of the Kansas City Fire Department. “And then strap him up to where we can hoist him up. That’s about what it comes up to.”

A Jewish store owner helped save a man who was seen running down a street in Brooklyn with his clothes on fire following a small explosion from chemicals used for a hookah pipe, the UK’s Daily Mail reported on Wednesday.

The chemical fire broke out shortly before 2 p.m. on Tuesday inside a basement on McDonald Avenue in Brooklyn, across the street from a Jewish seminary. Neighbors said a 22-year-old was drying tobacco leaves using a mixture of acetone and coconut oil for a hookah pipe when fumes from the flammable liquids ignited with a water heater pilot light.

Surveillance cameras on McDonald Avenue caught the man running down the street with his clothes on fire while leaving behind a trail of white smoke, according to the Daily Mail. Witnesses said the man fled his building and raced across the street screaming for help before two people passing by were able to put out the fire.

Alex Mayer, the Jewish owner of a nearby camera shop, told 7Online. ”I hung up the phone and I started chasing him. And I’m like, ‘Drop and roll! Drop and roll!’ I caught up with [him] a half block up, when he finally dropped.” The victim was still burning until a truck driver named Claudio came and sprayed him with a fire extinguisher.

UConn Classes will be back in session on Thursday after a leaking cylinder prompted evacuations on Wednesday.

The Torrey Life Sciences and Pharmacy/Biology buildings were evacuated.

Two students had to be taken to the hospital Wednesday for treatment and evaluation after a lab accident.

UConn spokesperson Stephanie Reitz said the students mistakenly poured acetone into a receptacle that already contained a different chemical. The resulting chemical reaction caused irritation to their eyes, and both students followed procedure and went out of the lab to use the decontamination shower.

The UConn fire department arrived and sealed off the fourth floor of the ITE Building and two classrooms on the third floor. They transported the students to Windham Hospital where they were evaluated and released. Other students who had walked through the puddles of the decontamination shower had their footwear cleaned of any resulting residue.

Later in the day on Wednesday, UConn sent an alert to students that the Torrey Life Sciences Building and the Pharmacy/Biology Building at the Storrs campus were evacuated until further notice.

“The UConn Fire Department is working with a vendor to contain a leak in a helium cylinder,” according to UConn.

The two incidents appeared to be unrelated.

Attorney General Lisa Madigan filed a lawsuit Friday asking a judge to order a review of what caused an explosion earlier this month at a chemical factory that left two workers hurt.

Madigan filed the lawsuit Friday at the request of the Illinois Environmental Protection Agency, seeking an injunction that would order Blue Island Phenol of Alsip to conduct an analysis of the blast and its environmental impact.

The lawsuit also accuses the plant of multiple violations of the Illinois Environmental Protection Act.

The explosion at 11:30 a.m. Dec. 13 rocked the plant at 131st Street and South Homan Avenue, triggering a major emergency response from 40 area fire departments.

Two employees were burned in the explosion and fire, which destroyed a storage facility and damaged other buildings at the plant.

The explosion and fire followed a chemical release in the factory’s cumene unit, which manufactures phenol and acetone. The process to produce the chemicals runs continuously, and the two reactors used to store the chemicals were not damaged, plant manager Bill Moffatt said after the explosion.

BEDFORD HILLS — A cracked drum leaking acetone inside a tractor-trailer closed the Bedford Hills rest area on southbound Interstate 684 Tuesday morning for about 90 minutes, state police said.

The spill inside the Clean Harbors National Transportation truck prompted the driver to pull into the rest area around 11 a.m.

A Westchester County hazardous materials team, Bedford Hills firefighters and state police responded to the rest area. A Clean Harbors team eventually took care of the problem, police said.

Acetone is a solvent. It is often used to make other chemicals and is also part of common products like paint and nail polish remover.

A state police spokesman said the leak consisted of a “minimal amount” and there was never any danger to motorists.

But then this happened at the self-check out counter: "BEEP! Help is required for this item. Please set it aside and continue scanning."

A manager explained: CVS has placed restrictions on the purchase of nail polish removers that contain acetone, limiting the number of bottles a customer can buy.

CVS spokesperson Mike DeAngelis says the ingredient can be used in the illegal manufacture of methamphetamine, much like some cold medications, which are now kept behind the counter. He says CVS will now require a valid ID to purchase nail polish remover. The company has placed additional restrictions on buying the product with other household items that could be used to make meth.

He was removed from life-support early Saturday evening, Sylvie Robitaille, a spokesperson for the Centre Hospitalier de l’Université de Montréal’s Hôtel Dieu burn unit said.
Three others were still said to be in serious condition at the burn unit as a result of the explosion on Thursday involving highly flammable acetone at the Neptune Technologies and Bioressources plant in Sherbrooke.
A total of 21 people, including the three men who died, were injured in the explosion.
On Saturday, officials deemed the Sherbrooke plant too dangerous to enter and said it could take a few days to decide how to safely investigate the site.

SHERBROOKE, Que. -- Rescuers found two bodies in the rubble of a chemical plant gutted by an explosion in Quebec's Eastern Townships on Thursday.

One survivor was pulled from the rubble of the Neptune Technologies and Bioresources plant in Sherbrooke, Que., after a blast rocked the building around 1:30 p.m.

"We had just finished lunch, we were about to go back out when we were told to evacuate," said one employee. "Then it exploded ... the ceiling fell on my head."

Nineteen people were injured, including four who were airlifted to a Montreal hospital with severe burns. Witnesses said some workers were burned from head to toe.

Officials said a fire ignited 15,000 litres of acetone, a common ingredient in cleaning agents and paint thinners.

"The company does extraction with acetone, a highly flammable solvent," former plant employee Pascal Filion told QMI Agency.

"A small spark can cause an explosion, it's like gasoline."

The blast was so powerful that walls collapsed and windows were blown out. By mid-afternoon, firefighters were busy looking for other possible victims, but the unstable state of the building slowed their work.

A store employee had previously told the Observer that employees were working to mount a boar just before the blast.

Investigators said Sonya Graham was cut off by the fire and unable to exit the building. She sought shelter in an adjacent bathroom, authorities said Tuesday.

The case was investigated by the Rowan County’s Emergency Services and Sheriff’s departments.

TAMPA — An overwhelming chemical smell burned the eyes of inspectors Wednesday as they entered a warehouse to check on production of synthetic marijuana, the Hillsborough County Sheriff's Office said.

The inspectors had come to take samples of the substance being processed at 6308 Benjamin Road, Suite 712, north of Tampa International Airport.

The sample would be sent to the Florida Department of Law Enforcement to see if it contains any of the 29 compounds outlawed by the Legislature in its attempts to prevent the possession and sale of synthetic drugs.

But during the inspection, high levels of concentrated acetone caused investigators to call the state Fire Marshal, Environmental Protection Agency and the Occupational Safety and Health Administration to evaluate the safety of the warehouse and its more than a dozen employees.

Specific combinations of chemicals make up the synthetic drugs that are banned in Florida. No arrests were made at the warehouse Wednesday while the case remains under investigation, said Larry McKinnon, a Sheriff's Office spokesman.

Deputies have been inspecting several local distribution centers, as well as local vendors, since Gov. Rick Scott signed the law on March 23 to expand the chemical compounds banned.

Unlike with other drugs, deputies can't perform on-site testing to determine if the compounds are illegal. And as the law changes to ban more chemical combinations, so do the manufacturer recipes to keep their businesses legal.

SANTA FE, N.M. -- Continuing coverage on a suspected meth lab that led to evacuations in Santa Fe last week. It turns out, it wasn't a meth lab at all.
The chemicals that sent two police officers to the hospital were things you probably have in your home.
Captain Aric Wheeler says they've never been in a situation he can remember where chemicals this strong turn out not to be a meth lab.
They were taken into custody, hand cuff, handcuffed and covered in white protective jumpsuits, two jumpsuits. Two tenants at a small apartment complex on Agua Fria were initially suspected of cooking meth.
"We don't try and mess around with these things. Obviously they can be fatal," said Wheeler.
The chemical vapors strong enough to send two cops to the hospital was a common household cleaner, "drano."Drano.
The residents were trying to clean out a clogged drain while next door, neighbors were using acetone to remove floor tile. Together they became a dangerous combination.
"Even though they weren't a meth lab, they still were very toxic in the odors they were emitting," said Wheeler.
The scene Friday afternoon caused quit a commotion. Firefighters and hazmat crews descended on the apartment and even evacuated nearby neighbors. Captain Wheeler stands behind the department's decision.
"We have to take the measures necessary to ensure the public safety," said Wheeler.
Action 7 News tried to talk to neighbors, knocking on door, but were told neighbors were still embarrassed and too angry to talk.
Captain Wheeler says in this case they'd rather be wrong than right, because a meth lab in the middle of Santa Fe is a danger the do not want to deal with.
Wheeler says officers were on high alert because meth labs aren't common in the city, but just that morning they found a mobile meth lab and thought the two location locations might be connected.
The tenants refused to allow police into the home and Wheeler says that was also suspicious because usually when that happens, people have something to hide.
